Main duties of the job
We are seeking a reliable enthusiastic Health Care Assistant to join our GP Surgery for maternity cover with fixed term contract till the end of April 2027. This role offers an excellent opportunity to work within a busy primary care setting, supporting the clinical team in delivering high quality care to our patient population and contributing to the effective day-to-day running of our practice.

An exciting opportunity has arisen to join our expanding Frailty Team. We are growing our service to enhance the care we provide to our local community, and were looking for a dedicated professional to become part of this forward-thinking team.
Our established Frailty Team currently includes a two Community Frailty Practitione and a Care Coordinator. A Non-Medical Prescribing qualification is essential for this role, and applicants must have successfully completed this qualification.
The PCN Frailty Team plays a key role in supporting our practices by conducting weekly ward rounds, monitoring new care home residents, and providing high-quality long-term condition management.
In addition, the team delivers a non-urgent housebound service, offering vital support for patients who are unable to attend the surgery but require ongoing management of their long-term conditions.

Main duties of the job
We are looking for an experienced & motivated clinical practitioner to join our supportive & forward-thinking team, delivering high-quality care to housebound patients & residents within care homes.
This is a rewarding & varied role where you will work autonomously within your professional boundaries, undertaking history taking, clinical assessment, diagnosis, treatment, & evaluation of care for vulnerable patients.
You will play a key role in leading long-term condition management for elderly, frail, & housebound patients, helping to improve outcomes & quality of life within the community. Job summary
Severnvale PCN is seeking an enthusiastic and motivated Clinical Pharmacist to join our established team. We are looking for a pharmacist passionate about delivering high-quality patient care and supporting the development of our pharmacy services. Main duties of the job
As a Clinical Pharmacist within Severnvale Primary Care Network, you will work across four GP practices to provide high-quality, patient-centred pharmaceutical care. You will undertake structured medication reviews, support medicines optimisation, and provide clinical consultations for patients with long-term conditions and complex medication needs. You will reconcile medicines following hospital discharge, support safe and effective repeat prescribing processes, and work collaboratively with GPs, practice teams, community pharmacies and other healthcare professionals to improve patient outcomes.
You will play an active role in quality improvement by carrying out clinical audits, supporting prescribing initiatives, implementing evidence-based guidance, and contributing to medicines management projects. Job summary
An exciting opportunity has arisen to join Severnvale Primary Care Network as our PCN Manager on a 13-month maternity cover contract, working 22.5-30 hours per week. We are happy to discuss the exact hours with the successful candidate.
This is a varied and rewarding leadership role, working closely with the Clinical Director, PCN Board and Practice Managers to shape the future of primary care locally. Lead on the planning, mobilisation, implementation and evaluation of new services, transformation programmes and PCN initiatives, ensuring delivery against agreed objectives, timescales and budgets.
Drive continuous improvement across the PCN
Develop and maintain all PCN databases and policies
Carry out any other duties as may reasonably be required by the Clinical Director, PCN Board and its members.
Leadership:
Work with the PCN clinical director and the PCN Board to ensure the delivery of high quality of services, meeting the requirements of the Severnvale patient population.
Develop business cases to secure additional investment into the PCN
Lead on designated work streams as directed by PCN Clinical Director
Work closely with PCN Clinical Director and member Practices, support and build organisational capacity and capability
Work to improve patient and staff experience through supporting the management and facilitation of PCN- wide initiatives
Provide leadership, support and guidance to PCN ARRS positions
Provide strategic leadership for neighbourhood working service transformation and implementation of new NHS initiatives affecting the PCN.
Represent the PCN at locality, Integrated Care Board (ICB) and wider system meetings, building collaborative relationships to influence service development and improve outcomes for patients.
Travel
Regularly undertake travel to variety of sites.
The post holder will be required to support events, meetings or conferences, these may involve travel outside the Severnvale area.

- 5+ years Primary Care experience and experience in General Practice
- Have an understanding and experience of working in the NHS with extensive knowledge and understanding of the roles of NHS organisation and of the primary care sector
- Have an understanding of the national policy direction for primary, community and secondary care as evidenced in the Five Year Forward Review together with good practical and conceptual knowledge of healthcare improvement methods and practices
- Experience of developing primary care provision at scale, for example working with a federation or collaboration of general practices
- Have experience of developing working relationships with local and national stakeholders, demonstrating effective influencing and negotiating skills, training and facilitation skills
- Have experience of managing projects within tight deadlines and implementing complex primary care based initiatives to a successful outcome.
- Have good understanding of the other primary care providers and stakeholders, such as the LMC, OneCare and the ICB.
