Job summary
Working under the delegated authority of a Registered Nurse, the post holder will compliment and form a key part of the Multi-Disciplinary healthcare services, providing high standards of individualised, non-judgemental care for patients and supporting the operational requirements of delivering Prison Healthcare across Primary care, Mental Health and Substance misuse services to support the delivery of holistic care packages within our integrated services.The post holder will undertake delegated duties covering the patient journey from reception of prisoners into prison through to their discharge through the gate. This includes reception screening, providing agreed care packages, undertaking appropriate physical health observations, appropriate clinical duties, undertakingclinics such as Phlebotomy clinics, ECG Clinics and NHS health checks, sexual health and health promotion to improve mental, physical and social needs.The post holderwill also be expected to work as part of the emergency response team as required.The post holder will have the skills to support patients showing low level mental ill health and withdrawal from illicit substances by providing support in accordance with agreed care plans.The post holder will be expected to operate in line with our shared commitment to our values; Working Together, Integrity, Respect and Recognition, Compassion and Innovation.
Main duties of the job
Patient careo The post holder will support, implement and provide high standards of care to patients under the supervision of a qualified nurse/practitionero Develop appropriate and professional relationships with service users, which will promote effective communications and recoveryo Ensure consent is obtained at every interventiono Promote patient engagement and personal responsibility to manage theirhealth providing the necessary support to do soo To support the development of appropriate care plans and contribute to the review and evaluationo To carry response radio and respond to emergencies with qualified nurseundertaking relevant interventions as directedo Deliver 1:1 intervention as agreed within care packageo Support patients presenting to the service in crisis ensuring appropriate onward referrals and flag immediate riskso To have a good understanding of System1 and complete electronicdocumentation using the agreed read codes where appropriateo To record, use and store information in accordance with Caldicott and GDPR

Main duties of the job
As a Lead Nurse, your role encompasses a wide range of responsibilities and leadership such as:
Shift coordination - overseeing staffing levels and production of rotas.
Participating in audits
Management of junior members of staff
Supporting multi-disciplinary team's such as:
o Diabetic retinopathy screening
o AAA screening
o Ultrasounds and X-Rays
Medication rounds and dispensing
Emergency response situations
Patient assessments
Nurse-led clinics, such as:
o NHS Health Checks
o Immunisations and Vaccinations
o NHS Bowel Screening
o Phlebotomy clinics
o Wound and Tissue Viability assessments.
Assessing new patients who have just arrived at the establishment for the first time.
Complete clinical assessments
Triaging the acutely unwell patients

Main duties of the job

We are seeking an enthusiastic, motivated and experienced Practice Nurse to join our friendly and supportive multidisciplinary team at Tieve Tara Medical Centre.
This is an exciting opportunity for a registered nurse with strong primary care experience to play a key role in delivering high-quality patient-centred care. The successful candidate will provide a comprehensive range of nursing services including chronic disease management, health promotion, screening, immunisation programmes and preventative healthcare.
Working as part of an integrated healthcare team, you will contribute to improving health outcomes, supporting self-management and delivering evidence-based care to a diverse patient population.
Main duties of the job
About the Role
As a Band 6 Practice Nurse, you will:
Deliver high-quality nursing care within a primary care setting.
Undertake assessment, treatment, screening and health promotion activities.
Lead and support the management of long-term conditions including diabetes, asthma, COPD, hypertension and cardiovascular disease.
Provide cervical screening services and contraception provision (including emergency contraception).
Deliver childhood, routine and travel immunisation programmes.
Undertake a range of clinical procedures including venepuncture, ECGs, wound care, urinalysis and spirometry.
Empower patients to improve their health through education, prevention and self-management support.
Participate in clinical governance, quality improvement, audit and service development activities.
Support students and junior colleagues within the practice environment.

Job summary
As a member of Spectrum Community Health the post holder will be responsible for delivering a pharmacy service at HMP Humber in line with GPhC standards and recommendations outlined in "A pharmacy service for prisoners" and 'Safer Prescribing in Prisons'. This will involve the overall organisation, planning and co-ordination of all pharmacy services on-site. The post holder will work with the Medicines Management Pharmacist, other Spectrum Specialist Prison Pharmacists, Lead GP, other GPs and the prisons to ensure prison pharmacy services are of the highest quality and successfully integrated into Spectrum Community Health. In line with the well-being model of healthcare the post holder, through a partnership approach, will also help patients to optimise their medicines by tailoring medicines and support to the individual patient needs. They will also identify unmet pharmaceutical need, and assist in improving access and service efficiency including a reduction in wasted medicines.

To strategically plan, organise, co-ordinate staff and deliver pharmacy services at HMP Humber. To ensure that pharmaceutical care is consistently delivered in accordance with the Medicines Act, GPhC guidance, Commissioner service specification and in line with recommendations outlined in "A pharmacy service for prisoners" and "Safer Prescribing for Prisoners".
In addition, the post-holder will be responsible for encouraging the development of better medicines management understanding throughout the wider healthcare team and Prison Team and promote good practice in line with therapeutic developments in all prescribing decisions. This will involve assisting the Prison Health Care team in achieving NSF targets, NICE implementation, medicines management initiatives and the introduction of electronic prescribing.

We are looking for a salaried GP to join our MDT consisting of a variety of allied healthcare professionals (mental health workers, pharmacists, ANPs etc.) to deliver a primary care clinic within HMP Forest Bank. A session consists of 8 appointments (15 mins long) and are face to face. Presentations are as with community primary care as we are aiming to deliver 'equivalence of care' within our clinics. No prior prison experience is required as you will given the training required for the role although prior experience is preferable. Are you the right fit for this role? The GP most suited to this role is someone passionate about reducing health inequalities, empathetic, and enjoys working within an MDT. You will need leadership qualities and ready to explore a different, lesser known but more paced form of primary care.
Main duties of the job
GP's are required to provide reception cover from 5pm-9pm Monday to Friday (to be discussed).
The post holder will provide face-to-face clinical services including the medical component of prison reception assessment care, appointed, unscheduled and emergency care services to inpatient and segregation unit. Clinical services will include the following core responsibilities: Full clinical assessment of patients; Ability to formulate differential diagnosis and management plans; Pharmacological assessment and prescribing; Request and interpretation of clinical investigations; Appropriate referral on and follow up to other specialist areas and secondary care services; Information and advice and facilitation of informed choice for ongoing treatment and care; Medical assessment and monitoring of chronic diseases in accordance with NICE guidance and local clinical policy; Appropriate end of life care under clinical supervision from palliative care specialist services where needed. All GPs are expected to have an up to date working knowledge of the underpinning evidence base and clinical capabilities needed to formulate robust care plans to meet the full range of complex health needs of vulnerable groups with an emphasis on the substance misuse and offender health delivered in the secure environment setting. Psychiatric assessment, diagnosis and management of prisoners with serious mental ill health will be the responsibility of the specialist mental health provider.

Main duties of the job

Job summary
An exciting opportunity has arisen for an experienced and motivated healthcare professional to join the senior healthcare team at HMP Humber as Clinical Team Manager.

Job summary
Spectrum is recruiting a Substance Misuse specialist to lead our dedicated nursing team in Health and Justice.
To take forward our service, we're looking for a nurse with excellent leadership skills and the ability to build relationships with a wide range of stakeholders - from pharmacists and Heads of Healthcare to NHS providers and services.
You'll be an affective and visible leader, providing robust clinical substance misuse support and operational guidance to all staff on matters relating to physical health and wellbeing within the prison setting. You'll manage our skilled substance misuse team and be passionate about patient care - with advanced nursing knowledge, an understanding of NICE guidelines and a desire to be involved with key decision-making.

Main duties of the job
- The daily management of the Substance Misuse Service, including co-ordination of patient care and the provision of strong clinical leadership.
- This includes being involved with shift planning, taking appropriate action to adjust staffing levels and to prioritise the workload in accordance with patient need, service demand and the availability of staff.
- The post holder will be involved with assessing, diagnosing, providing advice and administering care to patients.
- You will also be expected to demonstrate knowledge, skills and understanding of health issues and treatment options.
