Job summary
An excellent opportunity has arisen for a Part Time Band 5 Nurse post within the SACT Outreach Team.
The SACT Outreach Team delivers treatments for Patients closer to home. This is an exciting opportunity to join a dynamic team providing care closer to home.
The Nurse will be required to provide a supportive role within the SACT Outreach sites for nursing, medical & clerical staff whilst also covering the Oak Road treatment Centre if required. Sites are located at St Ann's Little Hulton, New Mills, Dr Kershaw's Hospice, Bury Hospice, St Luke's Hospice & Tameside MacMillan Unit.
You must have driving licence and access to a car for this role, due to role being part time this would not be suitable for sponsorship opportunities.
Main duties of the job
The SACT Outreach Team delivers treatments for Patients closer to home. This is an exciting opportunity to join a dynamic team providing care closer to home.
You will also be required to provide support where required to Oak Road Treatment Centre and staff.
The staff ensure that they are providing a supportive environment to enable others to develop excellent skills and knowledge within this field. Once established in the role, you will be expected to co-ordinate Outreach Clinics, develop your clinical skills within SACT and teach others. You will also manage staff, their personal development needs and management of attendance.
We are looking for a dynamic individual who is able to demonstrate initiative and vision in continuing to move the service forward whilst assisting in implementing service changes and developments with a positive impact of patient care. Encouraging staff engagement and leading by example showing a strong positive role model.

St Lukes Hospice cares for people approaching the end of their life in Harrow and Brent. We aim to give the people we care for the best possible quality of life, every day. Job summary
St Lukes Hospice is seeking an ambitious and compassionate clinical leader to become our Pall24 Lead, providing strategic and operational leadership to a new service supporting patients and families across six London boroughs.
Based within the Integrated Care Co-ordination Hub in Kenton, youll work at the heart of a multidisciplinary system alongside paramedics, NHS 111 clinicians and Frailty Consultants, helping to deliver responsive, high-quality specialist palliative and end-of-life care.
This new and innovative service will cover 6 boroughs of northwest London and will be based in the Integrated Care Co-ordination hub within Kenton giving you access to a dedicated multidisciplinary triage centre that directly connects you with paramedics, NHS 111 and Frailty consultants.
Main duties of the job
As Pall24 Lead, you will provide strategic and operational leadership to the Pall24 team, working closely with the Director of Nursing and Clinical Services, Medical Director, and multidisciplinary colleagues to deliver a safe, high-quality, evidence-based service. You will play a pivotal role in developing integrated care pathways, strengthening partnerships across health and social care, and driving continuous improvement to meet the changing needs of our community.
The successful candidate will have the opportunity to make a tangible impact on patient care and staff experience and service performance while developing their own leadership skills with a supportive senior leadership team.
As part of the requirements of the NWL ICB model of care, this service will provide support for patients and families 24 hours a day. Therefore, the post holder may be expected to work shift patterns including night shifts as part of the service development.

St Lukes Hospice cares for people approaching the end of their life in Harrow and Brent. We aim to give the people we care for the best possible quality of life, every day. Supporting local people and their families is at the heart of what we do focusing on what matters the most to them is at the centre of our care.
Founded by local communities in 1987, we have been providing free expert end of life care and support to local people both in the community and from our beautiful Kenton Grange building, surrounded by tranquil gardens.
Two thirds of our care is provided in the comfort of peoples homes, as we respect that this is where most people would like to be looked after. Our specialist community team of nurses helps people to manage more complex needs whilst our Hospice at Home team supports people to remain in their own homes by providing practical care and support.
Our inpatient unit (IPU) at Kenton Grange cares for people in their final days, providing intensive care to help get symptoms under control or give respite care. Many of the people who are cared for in our IPU go home again.

We are seeking an experienced and motivated Corporate Partnerships Manager to lead the development and growth of our corporate fundraising and trusts and grants income streams. This is an exciting opportunity to secure vital funding that enables us to provide outstanding care and support to patients and families across Sheffield. You will develop and deliver ambitious income generation strategies, build meaningful partnerships with businesses, trusts and foundations, and maximise opportunities to generate sustainable income growth. You will also provide leadership and support to the Corporate Fundraiser and Trusts & Grants Manager, helping to create a high-performing and collaborative team. This role would suit someone with experience in fundraising, business development, account management or partnership management who thrives on relationship building and achieving results.
Main duties of the job
The successful postholder will: Develop and implement corporate fundraising and trusts and grants strategies to achieve income targets. Identify, secure and grow partnerships with local and national businesses, trusts and foundations. Build and maintain strong relationships with corporate supporters and funders through excellent stewardship and engagement. Develop compelling funding proposals, presentations and partnership opportunities. Monitor income performance and provide regular reporting against agreed targets and objectives. Lead and support the Corporate Fundraiser and Trusts & Grants Manager, providing effective supervision, motivation and development. Work collaboratively with colleagues across St Luke's to maximise fundraising opportunities and demonstrate the impact of supporter contributions. Represent St Luke's at meetings, networking events and partnership activities across Sheffield and the surrounding area. The role involves regular travel across Sheffield and the surrounding area, with occasional evening and weekend working. A full UK driving licence and access to a vehicle are essential.

Main duties of the job
The Nursing Associate role involves collaborating with our Registered Nurses to provide holistic care to our patients. This is achieved through personalized care plans and focusing on what matters to the person at the centre of their care.
Our In-Patient Unit team takes great pride in delivering palliative and end-of-life care of the highest standard for patients with complex care needs, alleviating symptoms and preserving their dignity. As a result, the IPU maintains an excellent nurse-to-patient ratio, typically 1:4 to uphold this standard of care.
You will receive a structured orientation program from our dedicated team, supported by the leadership team and practice educators, to further enhance your skills, confidence, and competencies in patient care.
While the position we are recruiting for is challenging, it is also highly rewarding. You will be part of a supportive team working in a positive environment where all our staff and volunteers are valued. Internal rotation to night and weekends will be a requirement of the post.
