You will work alongside experienced multidisciplinary teams, managing your own clinical caseload while developing specialist skills in assessment, intervention and recovery-focused practice.

Main duties of the job
You will manage a varied clinical caseload of adults aged 18-65 with a range of mental health conditions, delivering high-quality, person-centred occupational therapy and care coordination interventions that promote recovery, independence and meaningful participation in daily life.
Your key responsibilities will include:o Managing a defined caseload, undertaking comprehensive occupational therapy assessments and developing evidence-based, client-centred intervention plans using sound clinical reasoning.o Completing initial assessments, contributing to risk assessments, crisis plans and care plans, and participating in ongoing care coordination where appropriate.o Supporting service users and their carers to develop personalised goals and implementing trauma informed approaches that maximise occupational performance and social inclusion.o Planning and facilitating safe, timely discharge and transfer of care to primary care, community services and other appropriate agencies.o Working closely with partner organisations to ensure continuity of care and effective transitions between services.o Providing clinical supervision, support and guidance to junior occupational therapists, occupational therapy assistants and support staff, contributing to their professional development.o Participating in service evaluation, clinical audit and quality improvement initiatives, using evidence-based practice to support ongoing service development.

If you have previous experience in PR and wish to develop further skills as a Band 5 physiotherapist this is a fantastic opportunity. We provide adequate training to support your development in this role. Candidate will need to have experience in core areas and valid HCPC to apply for this role. This role is very popular. JOB SUMMARY:
- Provide a high standard of physiotherapy service to patients referred to services in Ealing Community Partners Pulmonary Rehabilitation Service.
- Participate in the Service development Projects
- Provide sound and appropriate clinical advice on the management of patients, carers and other health care professionals.
- Work without direct supervision.
Main duties of the job

- To carry out assessment and treatments of a range of respiratory patient who may have other comorbidities , with muskeloskeletal, medical and neurological conditions.

- To carry out assessment and treatment of patients referred for group classes in the community sites and out patient clinics.

About us

Ealing Dietetics provides dietetic services across community settings, including Clayponds Hospital, a community rehabilitation hospital that supports patients requiring inpatient rehabilitation, nutritional support, and multidisciplinary care to promote recovery and independence. Dietitians work closely with medical, nursing, therapy and catering teams to ensure patients receive appropriate nutritional assessment, treatment and monitoring throughout their admission.
The Nutrition Assistant Practitioner plays an important role across both the Ealing Community Service and Clayponds Hospital. Working under the supervision of registered Dietitians, the post holder supports nutritional screening, monitoring food and fluid intake, providing first-line dietary advice, facilitating group education sessions, collecting clinical data, and assisting with the implementation of nutritional care plans. The role contributes to improving patient outcomes, promoting self-management, and ensuring timely access to dietetic support across inpatient and community pathways.
Main duties of the job
To assist in obtaining dietary information from patients and following up care plans as recommended by the clinical dietitian
Observe patients for signs of diet-related complications
Offer support to the Adult and Paediatric Dietitians, nurses, health care assistants in identifying nutrition-related issues

We are seeking an enthusiastic Specialist Clinical Psychologist keen to develop a career in neuropsychology, to provide 12 months' parental leave cover.
This post offers an excellent foundation for developing specialist neuropsychology skills within the friendly and supportive Clinical Health Psychology and Neuropsychology Department at Charing Cross Hospital, part of Imperial College Healthcare NHS Trust and including the Northwest London Regional Neurosciences Centre.
The role includes specialist neuropsychological assessment and intervention within our outpatient service, which receives referrals from a wide range of neurology and neurosurgery clinics including cognitive disorders and dementia, traumatic brain injury, movement disorders, epilepsy, multiple sclerosis, neuro-oncology, infectious diseases and general neurology. There are also opportunities to contribute to exciting service development initiatives. Weekly supervision is provided by a senior Clinical Neuropsychologist.
Main duties of the job
The post holder will provide comprehensive neuropsychological assessment and formulation for patients presenting with a wide range of neurological disorders referred from specialist neurology and neurosurgery clinics. The role requires experience of neuropsychological assessment, a high level of attention to detail and an ability to assimilate information from a range of sources to arrive at a neuropsychological formulation of a patient's current difficulties and presentation.
The post holder will also contribute to the delivery of a specialist intervention pathway providing psychological interventions for patients presenting with mental health difficulties associated with Parkinson's disease, as well as supporting the development of new evidence-based intervention pathways for other groups of neurological patients.
In addition to direct clinical assessment and intervention, the post provides opportunities for involvement in service development, staff consultation and training, and research and audit.

We are looking for an enthusiastic professional who can help the Community Independence Service (CIS) make Hammersmith & Fulham (H&F) a great place to grow old. Join us and be part of a committed and caring team as we deliver high quality patient care.
Your role as a Health Care Assistant (HCA) within the CIS Home First Team will involve providing a rapid response care service for people who need urgent help with primary acute physical health illness in their own homes, often immediately after a hospital admission. Please note that this role is not an inpatient ward based or mental health service, and the vast majority of our patients need support with functional activities.
As a HCA you will be expected to provide support to our patients in a wide range of community and patient home environments, working autonomously and collaboratively in supporting the integrated multidisciplinary team in the delivery of excellent patient care. You will be responsible for the holistic, comprehensive care of complex community patients, delivering clinical care delegated by a registered professional and contributing to the evaluation of person centred care plans.
You will have excellent communication skills, IT skills, and the ability to be adaptable, flexible and reliable as changes to your caseload and patient visits may happen on a daily basis.
The service is operational 7 days a week 8am to 8pm. Hours of work for HCA may vary between these hours.
Main duties of the job
Key Responsibilities:
Carry out day to day care of patients within their own home environment, such as personal care, medication prompting, toileting and meal preparation.
Carry out safe moving and handling of patients with the use of equipment prescribed by Occupational Therapists and Physiotherapists, for example rollator frame, sara steady, re-turn and hoist.
Review and adjust basic equipment that has been prescribed and installed by Occupational Therapists or Physiotherapists, for example mobility aids or toilet frames.
To be able to take observations (for example blood pressure and temperature) after completing the appropriate competency, NEWS2 scoring and early identification and escalation of the deteriorating patient.
Participate and contribute to audits, service development and projects associated with improving the quality of service of CIS.

The CEN Service is a specialist Tier 3 community service supporting adults experiencing complex emotional needs, including emotional instability, relationship difficulties and difficulties managing distress.

This is an ideal post for an experienced Band 3 Rehabilitation Assistant looking to further develop their neurological rehabilitation skills or someone already working within a health or social care setting looking for a new challenge.

Main duties of the job
The Hounslow Community Neuro Rehab Team (CNRT) is a client-centred, integrated multi-disciplinary team comprising of an Advanced Clinical Practitioner, Physiotherapists, Occupational Therapists, a Neuro Psychologist, a Pharmacist, a Specialist Nurse practitioner, an Occupational Therapy Assistant and Rehab assistants.
The Neuro-rehab team works across both intensive short-term rehab and longer-term rehab pathways. There are opportunities to develop skills in rapid assessment and facilitating discharge from hospital, long term condition management and specialist neuro-rehab.
This role provides the opportunity to work across both rehabilitation pathways within the service, with both Occupational Therapists and Physiotherapists. This role involves providing therapy for patients within their own home environments as well as the local community. There will be expectations for lone working and independent monitoring of patients progress with timely feedback to the MDT.

Main duties of the job
This role will support access to the Individual Placement and Support (IPS) model across the organisation, whilst contributing to the ongoing development, sustainability, and quality assurance of the Peer Employment Specialist (PES) workforce.
Alongside the Trustwide Lived Experience Peer Support Lead and the Employment Services Manager, they will support, develop, and strengthen the Peer Employment Specialist workforce and have responsibility for the recruitment, professional peer supervision and skills development of this group. In addition, they will undertake joint working and provide consultation in relation to best practice of the Peer IPS blended model.
In all aspects of this role, the post holder will utilise their lived experience of the role employment has played in their recovery, to inspire the hope and belief that people with mental health difficulties can thrive in employment.
The post holder will also occasionally support the Recovery and Wellbeing College employment-focused offer across the Trust, supporting borough engagement, co-delivery opportunities, and equitable access to employment-related learning opportunities.

The post holder will assume the role of Unit/Site Coordinator at least four times a month. They will provide unit management support and coordinate patient flow (bed management) outside of regular hours. Additionally, the Unit Coordinator will offer senior nursing support, professional leadership, and guidance in all clinical areas within the unit, available 24 hours a day, 7 days a week.
We are seeking an individual with excellent communication skills, capable of engaging effectively with service users, their carers, and other professionals.

Job summary
We are seeking a passionate Clinical, Forensic or Counselling Psychologist to join the inpatient team at West London Forensic Service (WLFS). The post is 1.0 WTE for 1-year fixed term and will be based on a male low secure complex rehabilitation ward. It can be appointed at Band 7 or Band 8a depending on experience and with responsibilities adjusted accordingly. For Band 7, this will be a preceptorship towards an 8a role.
The successful candidate will be responsible for the provision of a highly developed and comprehensive specialist psychology service to culturally diverse service-users with enduring mental health difficulties and offending histories.
We provide a comprehensive induction and ongoing training and development, with opportunities to develop specialist skills and receive specialist supervision (e.g. EMDR and schema therapy), in addition to regular supervision. There are excellent staff benefits including a generous annual leave allowance and an on site gym (with membership at just £10 per month).
Main duties of the job
You will be responsible for providing a high quality psychology service on a low secure complex rehabilitation ward. As a core member of the MDT, you will be responsible for developing and implementing psychologically informed treatments for male service users who present with enduring mental disorders (both mental illness and personality disorder) and an offending history. As such you will be able to utilise varied skills in assessment, formulation and evidence-based intervention, tailored to individual needs.
As well as individual ward-based work, you will contribute to the cross-ward group therapy programme. You will also advise on risk assessment and risk management, and support psychological practice within the team through consultation and training.
In addition to direct clinical work, the role may involve staff supervision, service development and audit, and collaborative working within the psychological therapies department. There are also opportunities for research within WLFS.

Main duties of the job
To provide high quality, skilled nursing care and to assist the Community Nursing Team Manager in the on-going planning, implementation, and evaluation of the holistic needs of the patient/client, family, and/or carer.
Deliver high quality nursing care to housebound patients with complex needs to enable them to remain in their own homes. Working closely with out of hours service providers, specialist nurses, allied health and social care professionals to meet the needs of the patients without unnecessary attendance in A&E or be admitted to hospital.
This will involve working independently but as part of the Community Nursing Team covering a particular geographical area. This will include first assessments and reviews, planning, implementation, monitoring, and evaluation of individual programmes of care. The Dementia Link Service provides targeted care and support for people with dementia and their carers, and provides a link between primary and secondary care teams.
The Dementia Link Worker will provide support and information for people with dementia living in the community in the London Borough of Hammersmith & Fulham, and their carers - linking them with appropriate social and health services.
The role involves on-going review and assessment, informing patients about dementia and liaising with GPs, the West London Health Trust and other services as required.
The post holder should have an ability to communicate effectively in writing and verbally. They should have previous experience of working with, and be empathetic to the needs of, people with dementia, older people or other vulnerable people in the community.

Main duties of the job

The West London NHS Trust (WLT) Mental Health Single Point of Access (MH SPA) and NHS111 Option 2 for Mental Health (NHS111*2) provides 24/7 access to mental health services, every day of the year. Both services support people in a mental health crisis and provide a telephone support line for service users, their families and wider support networks. This includes a triage, signposting and referral pathway to appropriate onward care, if indicated. Main duties of the job
As a Crisis Care Practitioner, you will provide urgent mental health assessment and intervention for individuals contacting the service in crisis, whether for themselves or others.

- Managing complex and high risk triage calls, undertaking robust mental health and risk assessments in time-sensitive situations
- Completing face to face community assessments for patients requiring urgent in person review
- Formulating risk management and crisis plans to support safe and effective patient care

An exciting opportunity has come up for the Adult Community Nursing Service, Night Nursing. We are currently looking for a skilled person who can do 21 hours a week (1 overnight shift- 11 hours and 2 twilight shifts- 10 hours) to work across the Hounslow locality based at Clayponds Hospital on a Fixed Term Contract for 6 months to cover maternity. The service operates from 19:00 until 07:00 seven days a week. The service has 2 shifts, 19:00 until 12 midnight (Twilight) and 19:00 until 7:00 AM (Overnight).
Please note that the service is currently undergoing service re-design and the operational hours will change by 7th December 2026 from 7pm-7am to 8pm-8am under phase 1 transformation project. The phase 2 (TBC) will be happening shortly after the implantation of phase 1 with the Night Service re-design involving the twilight and the overnight shifts. We are looking for someone who will be very flexible in supporting the new service structure in the future.
The service also delivers urgent community response between 20:00 and 22:00 as part of the service specification and to prevent hospital admission.
Main duties of the job
To carry out clinical duties and be prepared to undertake clinical skills as per needs of the service and specific patient after training and competence assessment with support from an identified mentor for example, venepuncture, Blood Pressure, pulse and temperature recording, urinalysis, blood glucose monitoring, simple wound dressings and non-complex tasks such as continence reviews.
To undertake basic duties as delegated and overseen by the registered nurse and actively report any changes in a patient's condition in a timely way to the registered nurse.
Liaise with other health care and multi-agency workers to provide a multi-disciplinary approach to care.
To communicate with patients at a time and level appropriate to their needs in a manner that promotes positive health.

We are looking to recruit a highly motivated and flexible clinician with excellent clinical reasoning skills to join the Community Recovery Service.
The Community Recovery service is an integrated health and social care service for adults identified with uni and multi-disciplinary needs for recovery. The service will support independence by providing rehabilitation and reablement after an acute illness, injury or change in life circumstances. It aims to restore and maximise function or prevent deterioration in physical, psychological and social functioning through episodes of rehabilitation and reablement.
Therapists provide support and rehabilitation in the home.
The Community Recovery Service provides therapy for various pathways including: Core/ general rehabilitation, Discharge to Assess Reablement (CRS+) Integrated discharge hub at west Middlesex hospital.All health and social care professionals within the Community Recovery Service will have a set of core skills that all staff in the team are able to deliver regardless of profession. This supports the single assessment process, maximises use of resources and minimises duplication.
In addition, team members will retain specialist skills and knowledge which will be utilised as required to support specific clinical pathways. Specialists will also support other team members' work to ensure fulfilment of relevant goals.
Main duties of the job
The successful candidate will act as a specialist clinician, participating as a member of the team while independently managing a specialist clinical caseload.
Manage and prioritise a professional caseload within the interdisciplinary service through efficient workload management, supervision and adjustment of service provision to meet changing priorities and ensure individual patients receive intervention timeously and appropriately.
They will provide high quality comprehensive Physiotherapy assessment and treatment to adults referred to the Hounslow Community Recovery Service.
Develop and deliver an individualised treatment programmes based on a problem solving and advanced clinical reasoning approach to the patients referred and monitor, evaluate and modify treatment to measure progress and ensure effectiveness of intervention using valid outcome measures.

Job summary
The Mental Health Single Point of Access is at the centre of urgent and crisis mental health care across Ealing, Hammersmith & Fulham and Hounslow. Alongside our 24/7 SPA, the team also delivers NHS 111*2 mental health provision and Mental Health Urgent Community Response, creating a unique service where senior clinicians can work across telephone triage, urgent response, crisis pathways and system-wide decision-making.
We are looking for an experienced and dynamic Band 7 Senior Clinician who enjoys challenge, autonomy and leading from the front. You will make complex clinical decisions, support staff in real time and help shape how people access the right care at the right time.
This is a role for someone who wants more than a traditional clinical post. You will influence pathways, lead practice across a fast-moving service and play a key role in the continued development of urgent and crisis mental health care
Main duties of the job
As a Band 7 Senior Clinician, you will be one of the key clinical leaders within SPA, NHS 111*2 and MH UCR. You will provide senior oversight of complex referrals, telephone triage, urgent and crisis presentations and professional-to-professional contacts, supporting the team to make safe, confident and proportionate decisions.
You will lead the service clinically and operationally on shift, providing immediate support with risk, escalation and complex pathway decisions while maintaining oversight of service pressures and patient flow.
You will supervise and develop staff, contribute to quality improvement, audit and performance and work closely with CATT, primary care, ambulance services, police and wider partners. You will have the opportunity to influence practice, develop new ways of working and help shape a service that sits at the heart of urgent mental health care across three boroughs.
