Job summary
This is a role within the Green Paper for Transforming children and young people's mental health programme. The post holder will work within a Mental Health Support Team providing short-term evidence-based interventions to children and young people with mild to moderate anxiety and depression and/or behavioural difficulties and their parent(s)/carer(s).
The post holder will work with children, young people and parents with different cultural backgrounds and ages, using interpreters when necessary and should be committed to equal opportunities.
Main duties of the job
The MHST works with Young people with mild to moderate mental health presentations attending school/education setting in the Hillingdon area.
The purpose of the team is to
- Provide evidence-based interventions for mild to moderate mental health issues.
- Support the senior mental health lead in each education setting to introduce or develop their whole school or college approach.
- Give timely advice to school and college staff, and liaising with external specialist services, to help children and young people to get the right support and stay in education.
Young people have access to the MHST through their education setting. School staff and other potential referrers are able to contact a team of clinical staff to discuss potential referrals.
The MHST will manage referrals, deal with queries, undertake clinical telephone triage and face to face assessment, give advice and signpost young people where appropriate.

Job description
Job responsibilities
The key roles and responsibilities of the Senior Mental Health Practitioner will include:
- Forming strong working relations with a variety of stakeholders
- Working in partnership with schools/colleges and other educational settings.
- Provide training to team members and other professionals/trainees
- Supervising junior staff in the team.
- Providing consultations to education colleagues and support education settings to embed a whole school approach to wellbeing
- Developing and delivering evidence-based individual and group time limited interventions to young people within education settings in accordance with Children and Young People's IAPT principles
- Holding a caseload of CYP with mild-moderate mental health difficulties
- Completion of thorough assessments, risk assessments, care planning and reviews.
- Ensuring that there is accurate and timely documentation to a high standard, and that Routine Outcome Measures are completed.
- Evidencing the outcomes of the MHST service and feeding back this data to key stakeholders
- Providing clinical supervision to Education Mental Health Practitioners and day to day clinical and supervisory support for the Trainee Education Mental Health Practitioners.
- Contributing to group supervision spaces within the team.
- Working as part of a senior team, supporting the Senior Clinical Supervisors and Service Development Leads to expand the service within the Harrow area and improve the quality of the service.
- Supporting and developing the interventions that can be delivered as part of the MHST within the education setting, including SEN settings.
- Appropriately raise and report safeguarding concerns where identified and to support colleagues with this process.

The Dementia Link Service provides targeted care and support for people with dementia and their carers, and provides a link between primary and secondary care teams.
The Dementia Link Worker will provide support and information for people with dementia living in the community in the London Borough of Hammersmith & Fulham, and their carers - linking them with appropriate social and health services.
The role involves on-going review and assessment, informing patients about dementia and liaising with GPs, the West London Health Trust and other services as required.
The post holder should have an ability to communicate effectively in writing and verbally. They should have previous experience of working with, and be empathetic to the needs of, people with dementia, older people or other vulnerable people in the community.

Main duties of the job

Main duties of the job
As a Crisis Care Practitioner, you will provide urgent mental health assessment and intervention for individuals contacting the service in crisis, whether for themselves or others.

- Completing face to face community assessments for patients requiring urgent in person review
- Formulating risk management and crisis plans to support safe and effective patient care

The hospital focuses on innovative care for a challenging and interesting patient group. An exciting opportunity has come up for the Adult Community Nursing Service, Night Nursing. We are currently looking for a skilled person who can do 21 hours a week (1 overnight shift- 11 hours and 2 twilight shifts- 10 hours) to work across the Hounslow locality based at Clayponds Hospital on a Fixed Term Contract for 6 months to cover maternity. The service operates from 19:00 until 07:00 seven days a week. The service has 2 shifts, 19:00 until 12 midnight (Twilight) and 19:00 until 7:00 AM (Overnight).
Please note that the service is currently undergoing service re-design and the operational hours will change by 7th December 2026 from 7pm-7am to 8pm-8am under phase 1 transformation project. The phase 2 (TBC) will be happening shortly after the implantation of phase 1 with the Night Service re-design involving the twilight and the overnight shifts. We are looking for someone who will be very flexible in supporting the new service structure in the future.
The service also delivers urgent community response between 20:00 and 22:00 as part of the service specification and to prevent hospital admission.
Main duties of the job
To carry out clinical duties and be prepared to undertake clinical skills as per needs of the service and specific patient after training and competence assessment with support from an identified mentor for example, venepuncture, Blood Pressure, pulse and temperature recording, urinalysis, blood glucose monitoring, simple wound dressings and non-complex tasks such as continence reviews.
To undertake basic duties as delegated and overseen by the registered nurse and actively report any changes in a patient's condition in a timely way to the registered nurse.
Liaise with other health care and multi-agency workers to provide a multi-disciplinary approach to care.
To communicate with patients at a time and level appropriate to their needs in a manner that promotes positive health.

We are looking to recruit a highly motivated and flexible clinician with excellent clinical reasoning skills to join the Community Recovery Service.
The Community Recovery service is an integrated health and social care service for adults identified with uni and multi-disciplinary needs for recovery. The service will support independence by providing rehabilitation and reablement after an acute illness, injury or change in life circumstances. It aims to restore and maximise function or prevent deterioration in physical, psychological and social functioning through episodes of rehabilitation and reablement.
Therapists provide support and rehabilitation in the home.
The Community Recovery Service provides therapy for various pathways including: Core/ general rehabilitation, Discharge to Assess Reablement (CRS+) Integrated discharge hub at west Middlesex hospital.All health and social care professionals within the Community Recovery Service will have a set of core skills that all staff in the team are able to deliver regardless of profession. This supports the single assessment process, maximises use of resources and minimises duplication.
In addition, team members will retain specialist skills and knowledge which will be utilised as required to support specific clinical pathways. Specialists will also support other team members' work to ensure fulfilment of relevant goals.
Main duties of the job
The successful candidate will act as a specialist clinician, participating as a member of the team while independently managing a specialist clinical caseload.
Manage and prioritise a professional caseload within the interdisciplinary service through efficient workload management, supervision and adjustment of service provision to meet changing priorities and ensure individual patients receive intervention timeously and appropriately.
They will provide high quality comprehensive Physiotherapy assessment and treatment to adults referred to the Hounslow Community Recovery Service.
Develop and deliver an individualised treatment programmes based on a problem solving and advanced clinical reasoning approach to the patients referred and monitor, evaluate and modify treatment to measure progress and ensure effectiveness of intervention using valid outcome measures.
Work closely with the multi-agency team to meet the needs of the patients and enable them to receive a comprehensive service.

Are you a passionate mental health professional looking to make a real difference in primary care? We are seeking a Mental Health Practitioner to join the Mental Health Additional Roles Reimbursement Scheme (MHARRS) within Hounslow.
Working across GP practices and alongside West London NHS Trust's Mental Health Integrated Network Teams (MINT), you will provide holistic mental health assessments, risk assessments, brief interventions, care planning and support for adults with a range of mental health needs. You will play a key role in supporting people with serious and complex mental illness, facilitating access to appropriate services, and promoting recovery through a person-centred approach.
This autonomous role offers the opportunity to work within multidisciplinary teams, contribute to annual SMI reviews, provide advice and training to primary care colleagues, and help shape integrated mental health pathways across Hounslow.

Main duties of the job
Working as an autonomous practitioner with one of the five Hounslow PCNs, 80% of the role is based within the PCN and 20% with the local community mental health service for adults of working age.
In your role you will
o work with patients to support shared decision-making about self-management; facilitate onward access to treatment services; and provide brief psychological interventions, where qualified to do so and where appropriate.
o work closely with other PCN-based roles (e.g. pharmacist and social prescribers) to address the potential range of biopsychosocial needs of patients with mental health problems.
o work as part of the primary care team, operating without the need for formal referral from GPs, including accepting some direct bookings.
o be supported through the local MINT teams by robust clinical governance structures to maintain quality and safety.

An exciting opportunity has arisen for a Band 7 Senior Clinician within the newly developed Mental Health Crisis and Assessment Service (MHCAS) at Lakeside Mental Health Unit. Main duties of the job
The Band 7 MHCAS Senior Clinician will provide senior clinical leadership within the Mental Health Crisis Assessment Service (MHCAS), maintaining a visible frontline presence and supporting the Team Manager in the delivery of a safe, responsive, and high-quality 24-hour crisis service. The post holder will lead on complex assessments, risk management, crisis intervention, patient flow, and clinical decision-making, ensuring evidence-based, recovery-focused, and least restrictive care. Occupational Therapy Assistants work alongside Occupational Therapists to support the delivery of person-centred interventions, helping service users achieve their recovery goals and improve their independence, wellbeing, and physical health.
We are recruiting a Band 4 Occupational Therapy Assistant/Exercise Technician posts within Lakeside Mental Health Unit. Main duties of the job

The main duties of the post-holder will relate to facilitating and promoting research activity within West London Forensic Service (WLFS). This will include identifying, recruiting and supporting patients through the research process as well as providing support as necessary to research active teams and clinicians within the service. The post is based within WLFS, which is a large forensic mental health service providing care to service users across different levels of security (medium and low security as well as the community). Main duties of the job
The role will be based in West London Forensic Service, which is at the St Bernards Hospital site in Southall, Ealing. The role will be a varied one, working with different multi-disciplinary teams, clinicians and service users from across WLFS to support and promote research within the organisation.
You will be working within a forensic mental healthcare environment, with direct patient contact being an integral part of the role. Training and induction will be provided with respect to working in this clinical setting. You will be supervised and managed by the Research Lead for WLFS, but supervision for specific projects may be provided by other clinicians within the service, depending on the study.
You will be one of two Research Assistants for West London Forensic Service. This exciting role will give you a great opportunity to have a positive impact within the organisation and develop your longer term career goals. You will be part of a wider peer group of Research Assistants working across other areas of the Trust.
You should be able to demonstrate effective communication skills, good organisational skills, flexibility, excellent attention to detail, and the ability to work autonomously under appropriate supervision.

Job summary
We are looking for a highly motivated podiatrist, who is passionate about their profession and enjoys making a difference to people's lives. You will be expected to work at an advanced level and should aspire to lead, educate and develop others in the team.
We welcome applications from clinicians currently working at band 6. Ideally you will have at least five years' NHS experience and be confident in all aspects of podiatry care and team working. The service works closely with district nursing teams, tissue viability nurses, and local integrated teams. as well as linking with the wider MDT and PCN networks. We have a direct pathway into the Diabetic Foot Clinic at West Middlesex University Hospital.
Main duties of the job
In our vibrant and multi-cultural community, you will work from well-equipped clinic sites delivering care close to home. You will assess, diagnose, and provide high quality podiatric care to people with complex foot health needs and lower limbs that are at risk. There is emphasis on diabetes foot protection due to the high incidence of diabetes in the population we serve,
You will be responsible for agreeing care plans with patients that support self-management and optimise foot health, in line with service priorities, and you will be expected to manage complex patients at an advanced level.
You will be required to visit housebound patients in their own homes, so you must be able to drive and have access to a car to support their needs.
You will work autonomously as well as alongside others, so you need to be flexible and well organised. You will be responsible for managing other people's workload so need to be great at teamworking and problem solving. You will also need excellent interpersonal and communication skills to interact with our diverse patient population and with various other health care teams and stakeholders.

Main duties of the job

Job summary
An exciting development opportunity for an exceptional candidate, with demonstrable potential, to progress into a Band 8a Advanced Practice First Contact Physiotherapist role. The post is hosted by CNWL and candidates will work within the local PCN. The development programme offers the successful candidate the opportunity, as a Band 7, to successfully complete the associated training package / FCP Portfolio and to work through the competencies for the HEE MSK FCP. Upon successful completion the successful candidate will be promoted to the Band 8a role. There are multiple roles in South Southall and Acton PCN and this is a full time role based either in Southall or Acton area in Ealing.
The first contact physiotherapist will work in the PCN whilst also being part of MSK team. The FCP service in Ealing is well established and provides a high level of training, clinical supervision and support.
An agreed time frame will be agreed during interview for completion. Should the candidate not successfully complete the development package then a role as a substantive, permanent Band 7 Physiotherapist will be offered.

Main duties of the job
The main duties of the job will be to provide clinical expertise, acting as first-contact physiotherapist and making decisions about the best course of action for patients' care in the area of musculoskeletal, rheumatological and pain management. This will include screening and assessment ofpatients with undifferentiated conditions to determine that they meet the criteria for self-management, local MSK or community service criteria or to ensure that those with medical needs are rerouted to the appropriate clinician. This will involve face to face, telephone or virtual contact with patients, without prior contact with their GP, in order to establish a rapidand accurate diagnosis and management plan. Onward referral to community or specialist services may be required.
The successful candidate may need to progress and request investigations to facilitate diagnosis and choice of treatment regime, understanding the information limitations derived from these and the relative sensitivity and specificity of particular tests diagnostic services such as x-rays and bloodtest, and interpret and act on results to aid diagnosis and the management plans of patients.
They will also be expected to deliver programmes of supported patient self-management, in ways that facilitate behavioural change, optimise individuals' physical activity, mobility, fulfilment of personal goals and independence, and that minimise the need for pharmacological interventions

Main duties of the job
