The key principles of Wye Valley Trust are to improve the health and well-being of the people we serve in Herefordshire and surrounding areas. We have an exciting opportunity for the right candidate to join our wonderful District Nursing Team.
The Ross-On-wye District Nursing Team are a very supportive and dedicated team, we are an incredibly engaging, positive and hard-working team, providing care and support to patients in their own homes.
We work closely with Golden Valley DN Team and often work as a locality where needed.
We work closely with a wide range of multi-disciplinary teams, including GP's, Palliative Care Nurses, Therapy Teams, and Advanced Clinical Practitioners. Ross DN Team also provide a wound clinic service based at the Community Hospital.
Main duties of the job
Our aim is to provide treatment and support to patients in their own homes. Whilst undertaking a holistic nursing assessment, we aim to encourage patients to remain as independent as possible, in order to prevent hospital admissions.
Due to the nature of this role, lone working and the ability to meet the travel requirements of the post are essential.
Main duties of this role include, but are not limited to;
- Palliative care
- Wound care
- Managing end of life symptoms and providing support for relatives/carers- Managing long term conditions in a community environment- CVAD and drain management- Chemotherapy disconnect- Holistic assessments- Post operative care following discharge- Wound clinic service- Venous blood samples

Job summary
The purpose of this job is to provide secretarial support to the Specialty Team in Obstetrics and Gynaecology by typing letters and reports, dealing with telephone queries and ensuring appropriate diagnostic investigations are arranged; working on your own initiative using knowledge, skills and experience to manage and prioritise workload independently inaccordance with operational standards and protocols.
Main duties of the job
? To type clinic letters, correspondence, reports from digital dictation for the Consultant,Middle Grade and junior medical staff in adherence with standard turnaround times.? To manage own work ensuring all clinic letters are typed in chronological order.? To ensure 3-signed correspondence is checked and dispatched daily.? Forwarding letters/reports by either: - post, fax, telephone or e-mail as required.? To ensure any dictated requests are actioned appropriately and to notify the relevantmedical secretary to maintain communication, a good service and efficient records.? Answering departmental phone calls in a courteous and professional manner.? To review data quality reports as required and update in Administration systems asnecessary.? Maintain an up-to-date knowledge of Departmental procedures.? To be able to provide cover for Band 4 medical secretary in times of annualleave/absence.? To maintain confidentiality at all times.? To ensure awareness and understanding of the data protection act and other national and local NHS guidelines/policies as required.

Our School Nursing team is seeking a Registered Nurse to support the delivery of the Healthy Child Programme 5-19 years to the children, young people and families of Herefordshire.

- To assist in the recruitment and selection of staff into the team and to participate in the induction and orientation programmes for new staff members, this may include preceptorship support for newly qualified staff nurses.

Complete holistic occupational therapy assessments within patients' own homes following hospital discharge. Assess functional abilities, activities of daily living, cognition, mobility, environmental factors and risks. Recommend and arrange appropriate equipment, adaptations and support services.Work closely with hospital teams, primary care, community services, social care and voluntary sector partners to ensure seamless transitions of care. Support the prevention of avoidable hospital admissions and readmissions.Manage a varied caseload, prioritising effectively within a dynamic community setting. Due to maternity leave an exciting opportunity has become available for a Specialist Band 6 Occupational Therapist to join our inpatient Stroke team, based here at Hereford County Hospital.

The team is part of the stroke pathway which links with the community stroke team.
We provide hyperacute/acute stroke care for people presenting with symptoms of stroke, as well as providing inpatient rehabilitation prior to discharge from hospital.

You will be able to work at a fast pace to ensure timely assessments / interventions & contribute to MDT decision making.

We are looking for a ward clerk to join our friendly and supportive team on Ashgrove ward. Ashgrove is a busy 24 bedded Frailty ward.
The successful candidate will be working directly with all members of theMultidisciplinary team of nurses, doctors, physiotherapists and other professionals.
You will be one of the first faces our patients and relatives will see when they first walk into our hospital, and you will be dealing with queries face-to-face, on the phone and via email. For this reason you will need excellent verbal and written commutation skills, you will need to be able to multitask and prioritise whilst remaining calm under pressure keeping a professional approach at all times.
Main duties of the job
- To maintain effective communication with ward staff, external departments within the Trust and key stakeholders outside the Trust, utilizing telephone, fax machine and email as appropriate.
- The post holder will arrange appointments, transfers etc, on the request of the nursing and medical staff, liaising with other wards, departments and outside agencies as necessary.

An exciting opportunity has arisen in the RMC for an RMC Clinic Clerk.

We are seeking an experienced and innovative Band 7 Occupational Therapist to join our Children's Community Occupational Therapy team.
As a Band 7 Children's Occupational Therapist you will be responsible for providing a specialist and comprehensive Occupational Therapy service for children 0-18 years and their families, who have a Herefordshire GP.

To agree child focused aims, goals and objectives for all OT intervention.
To design individual therapeutic treatment plans, for children to be carried out either by you or delegated to Occupational Therapy support workers, families, carers or school staff. Job summary
We are looking for a passionate and dynamic Middle Grade Doctor to join our friendly team at Hereford County Hospital. This is a full time (12 sessions/PAs) hospital based post on the senior resident doctor Rota (tier 2/middle grade) to fill one of our specialty doctor posts.The current rota consists of 1 Paediatric deanery ST trainee and 5 specialty doctor posts. This will be expanding to 1 Paediatric deanery ST trainee and 7 specialty doctor posts. We are looking for a Doctor with a minimum of 12 months NHS experience.
Main duties of the job
The job entails responsibilities in acute Paediatrics and neonatal ward work, outpatient clinics, assessment and treatment of patients in the Paediatric assessment unit and sharing with consultants the supervision and teaching of resident doctors. The job involved a combination of normal working day shifts, long day and night shifts.
There will be opportunities for in -service training, development of special interests or participation in the CSER Programme. Wye Valley NHS trust believes in investing in our staff to develop local consultants and leaders of the future and have an active education department providing local training. We support SAS doctors by investing in local teaching programs for generic professional capabilities such as human factors, leadership development and quality improvement training. Additionally we offer a consultant supervisor to all our SAS doctors in Paediatrics. The trust has 2 SAS tutors who advocate for this group of doctors and link with the wider SAS network in HEE.

Job summary
This is an exciting opportunity for an Associate Specialist in Endoscopy to join a high-performing, JAG-accredited endoscopy service delivering diagnostic and therapeutic procedures across Wye Valley NHS Trust's two-site model. Working within a multidisciplinary team of Gastroenterologists, Upper and Lower GI Surgeons, Clinical Endoscopists, specialist nurses and decontamination technicians, the post holder will contribute to the delivery of safe, effective and patient-centred care for a population of 235,000 across Herefordshire and Powys.
Main duties of the job
The role is central to supporting the Trust's commitment to high-quality endoscopy, with the expectation of undertaking five independent scoping lists per week, covering both upper GI endoscopy and lower GI endoscopy. The post holder will also play an active role in supporting the department's training culture, including participation in the JETS training programme, and will contribute to the clinical supervision and development of Clinical Endoscopists. As part of ongoing service expansion, the successful candidate will be expected to undertake further training in Transnasal Endoscopy (TNE) with appropriate support and mentorship from senior endoscopists.

*** INTERNAL APPLICANTS ONLY*** Clinical Nurse Specialist Breast - Band 6 Job overview An exciting opportunity has arisen for a Clinical Nurse Specialist to join the Breast Care Nursing Team at Wye Valley NHS Trust This post will enable the successful applicant to join a well-established, enthusiastic and motivated clinical team. There will be the opportunity to engage in further training and development to support the individual's educational needs within the role. The post holder will work alongside the Breast Care Nursing Team to provide a quality holistic specialist service for patients with Breast Cancer from diagnosis throughout the treatment pathway acting as a keyworker. Hours per week: 37.5 to be worked in a flexible manner to optimize patient care. The post holder will work as a member of the Breast multi-disciplinary team within the secondary care setting. The post holder will support patients from diagnosis through the treatment pathway.
Main duties of the job
Duties will include managing a caseload of patients throughout their cancer pathway, providing expert sensitive care and communication. This will include supporting patients and their families at diagnosis and helping them to decide on surgical options when a choice such as reconstruction is given. The ability to liaise with patients, carers, consultants, GP's and related therapists is essential; Supporting awareness and health promotion and support for patients with suspected Breast Cancer. The post holder will work closely with the Breast Nursing team providing cover for clinical commitments such as MDT meetings, Consultant clinic sessions, Nurse led clinics and telephone and email advice. As a member of the Breast multi-disciplinary team, you will be expected to have or be willing to undertake advanced communication skills training. This will enhance skills necessary in order to facilitate clinics and provide holistic assessment and complex psychological support to our cancer patients and their carers. An understanding of the recovery package for Breast Cancer patients, living with and beyond cancer and the specialist knowledge to manage our telephone triage service.
