Job summary
We are looking for an enthusiastic Band 4 Therapy Assistant Practitioner to join our Community Team. You will be working as part Discharge to assess pathways in the community with people who have been discharged out from the acute or community hospitals to their homes / bedded units.
Working as part of an experienced team alongside Physiotherapists, Occupational Therapists, other Therapy Assistant Practitioners, reablement / enablement providers and social care assessors, you will be providing holistic Physiotherapy and Occupational Therapy assessment and interventions in the community, within scope of practice.
You will be assessing patient's independently and as part of a team. The role will involve assessing whether patients abilities in their own home, looking at providing equipment & rehabilitative techniques to enable independence. In addition to supporting & informing of ongoing care needs. You will also provide assessment and rehabilitation plans to improve patient strength and function in their own homes.
As an integral part of the team you will assess, plan, set goals and plan discharge with a personalised approach.
We have a holistic approach and are very focused on preventing de-conditioning.
As a service we are innovative, forward thinking and encourage staff engagement. You will have the opportunity to be involved in service / quality improvement and develop leadership skills, as well as engaging in student placement education.
Main duties of the job
Working as part of the therapy and wider multidisciplinary team, Therapy assistant practitioners use enhanced skills in a specialised area to provide direct high quality and effective therapeutic care and support to patients including those with more complex needs. Within the scope of the role, the post holder will independently (unsupervised) plan and deliver individual or team tasks where the understanding of role boundaries and the importance of supervision and carrying out appropriately delegated tasks and plans is essential.
This role requires understanding, and factual and theoretical knowledge of principles, policies, protocols, procedures, processes, plans and concepts in broad contexts within the area of work or defined group of patients. Although work is guided by standard operating procedures, protocols or systems of work, the post holder must to able make judgements, plan activities, contribute to service development and demonstrate self-development. The post holder may be responsible for supervision of others and delegate appropriate tasks to other non-registered staff.
The post holder will be required to complete the relevant competencies required for the post and participate in 7 day working.

Job summary
The Urology service has links with neighbouring specialist cancer teams including University Hospitals Birmingham and the Wye Valley NHS Trust as part of the Urology Area Network. The Worcestershire Urology Specialist MDT is normally attended by 2-3 Oncologists, 2 Consultant Radiologists, a Pathologist, and all of the key members of the Urology clinical team. Videoconferencing with sub-specialist MDTs in adjacent Centre is also available. Urology services in the West Midlands are currently undergoing a comprehensive review with proposals around Clinical Alliances between providers under consideration. It is an exciting time to join the Urology department within Worcestershire, as we progress through the Trusts Clinical Services strategy. The trust will soon be replacing existing theatre infrastructure with a new 4 operating department complex housing ultraclean-laparoscopic enabled and robotic suites. Novel surgical techniques for treatment of BPH are to be implemented in addition to enhanced community Urology services, delivered in collaboration with our system partners. The ongoing reconfiguration of Surgical Services will deliver co-location of elective pelvis specialties Hospital: Urology,at the Alexandra Urogynaecology, Colorectal Surgery & Gynaecology. The high degree of service interdependency and collaboration between these specialties delivers an opportunity to realise our vision of delivering a Centre of Excellence for Urology & Pelvic surgery.
Main duties of the job
The successful candidate will offer high quality day to day care to Urology and General Surgery patients at Alexandra Hospital Redditch, and deliver a first class clinical service to include activities equivalent to Middle Grade/Registrar level. They will offer high quality day to day care of patients and deliver a first class clinical service to include outpatient/diagnostic clinics, day case theatre procedures, elective theatre activity, inpatient care as required. There may be exciting opportunities to support the initiation of our Robotic Surgery program through tableside assistance with training as required. The post holder(s) will be required to participate in an on call commitment. The post holder(s) will have a continuing responsibility for the care of the patients in their charge. This will include responsibility for undertaking the administrative duties associated with the care of patients and wider administration for the department. The Trust may consider evening working (e.g. three session theatre days) as scheduled activity and may also wish to reach agreement with the individual about the possibility of future scheduling of routine work on a Saturday with other colleagues in the Urology Department. - To ensure that all records are maintained following local Standard Operating Procedures
- To ensure all documentation is scanned, filed, kept up to date and archived where necessary following the legal requirements for the retention of pharmaceutical records
- To ensure that all excluded from tariff medicines are individually costed to each patient on the Pharmacy Ascribe (EMIS) system to provide accurate details required for HCD funding e.g. responsible consultant team
Procurement Administrative Responsibilities

To raise orders for pharmaceutical items including medical gases by using the purchasing system and submit them to authorised personnel for signature.

We have a full time fixed term post for 12 months Band 5 to cover a secondment.

The Ross-On-wye District Nursing Team are a very supportive and dedicated team, we are an incredibly engaging, positive and hard-working team, providing care and support to patients in their own homes.
We work closely with Golden Valley DN Team and often work as a locality where needed.
We work closely with a wide range of multi-disciplinary teams, including GP's, Palliative Care Nurses, Therapy Teams, and Advanced Clinical Practitioners. Ross DN Team also provide a wound clinic service based at the Community Hospital.
Main duties of the job
Our aim is to provide treatment and support to patients in their own homes. Whilst undertaking a holistic nursing assessment, we aim to encourage patients to remain as independent as possible, in order to prevent hospital admissions.
Due to the nature of this role, lone working and the ability to meet the travel requirements of the post are essential.

An exciting opportunity has arisen for a diabetes dietitian to join the dietetic team at Hereford County Hospital. The dietetic team comprises of 16 dietitians and 4 dietetic assistants.
Main duties of the job
The diabetes dietitians work closely with the wider diabetes MDT to provide 1:1 and group education/ promote self-management to patients with all types of diabetes across Herefordshire. Specialist services include antenatal service, insulin pump clinics and young adult clinics.
Post-registration experience in diabetes is desirable for this post however training can be provided. You will be supported in your role by the Band 7 diabetes dietitians.
The successful candidate will have a caseload of face to face outpatient clinics, virtual outpatient clinics, MDTs and structured education groups for all types of diabetes.

The successful Consultant Gastroenterologist will provide high-quality specialist clinical care to patients referred to the Gastroenterology and Hepatology service at Hereford County Hospital.

- Participating in the Gastroenterology Consultant of the Week service, providing senior clinical leadership and decision-making for inpatients.

Supported by a peripatetic RE from The West Midlands Rehab Centre, providing the regional specialist hub to the 12 District Wheelchair services in the West Midlands.
The team has actively developed and expanded over recent years with scope for further service development. The post provides a mix of complex clinical caseload operating through both clinic settings and home visits and have a number of specialist care homes in the area with clients with complex posture. We aim to promote 24 hour postural management and grow good networks for MDT joint working. With proactive links across the acute and community services.
Proactive participation in practice placement education and attendance at PMG is available. We are committed to continuing professional development, in-service training and clinical supervision.
Main duties of the job
The Wheelchair Therapist is responsible for high quality, person-focused assessment prescription of wheelchairs, pressure care, buggies and accessories to clients of all ages (including carers where appropriate) in their own homes, clinic settings and other community environments
Carry out detailed assessment of mobility, posture, seating needs, skin and environment. Collaboratively agree outcomes, prescribe appropriate equipment, fit and adjust to meet client's needs. Providing instruction in safe use, care, and maintenance
Excellent clinical knowledge of 24 hour postural management and pressure mapping. Understanding the properties of the pressure care available to the service
Maintain up-to-date knowledge of product ranges available to wheelchair services. Clinical use, review of product range used within the service for suitability of purpose, safety and durability. Knowledge of prescription, ordering systems, lead times, and the modifications available to the equipment range within the guidelines set by the MHRA
Ensure clients and carers are trained in the safe handling of wheelchairs/accessories. Involving manual handling of wheelchairs and other equipment

We are looking for motivated, compassionate individuals to join our Herefordshire Hospital at Home Team as Band 3 Healthcare Support Workers.
Staff will work shift patterns, including day and night shifts, weekends and bank holidays, in line with service needs.
Main duties of the job
- Assist qualified Nursing staff in a range of patient care duties.
- To undertake a range of delegated nursing tasks, including cannulation and taking blood or urine samples, carrying out basic wound assessments and applying simple dressings, taking routine observations and supporting clients with medication.

Job summary
We have an exciting opportunity for a Therapy Support Worker to support the work of the inpatient therapy team at Leominster Community Hospital.
You will be working with the Occupational & Physiotherapists based on the wards alongside the multi-disciplinary team to promote rehabilitation and the why not home, why not today ethos, enabling the best outcomes achievable. Main duties of the job
Working as part of the therapy and wider multidisciplinary team, therapy support workers use developed skills under the supervision of registered therapists and may also work alone and with other non-registered therapy staff. Therapy support workers are able to make fact-based judgements and suggest treatment progressions to a registered therapist.

They post holder will be responsible for the implementation and adherence to policies to meet the requirements of ISO 15189, reporting on a regular basis to the POCT Manager and departmental Governance meetings.

Main duties of the job
The post holder will work closely with the Senior Pharmacist's for the Surgical Division to provide a comprehensive ward based, clinical service to the Surgical Division specifically the Theatres & Critical Care Directorate and the Women's & Children's Directorate. The role will include taking medicines histories and completing medicines reconciliation, carrying out therapeutic drug monitoring, making therapeutic interventions within scope of practice, supply of medicines, final checking of medicines and counselling patients. Are you looking for a new challenge? The paediatric SALT team at Wye Valley NHS Trust is looking for an enthusiastic, self-motivated and committed Speech and Language Therapist. You will join a friendly & highly committed team of therapists & therapy assistants delivering support to children across rural Herefordshire. Ideally, you will have some knowledge & experience of working with children who present with DLD/ significant & complex speech and language difficulties.
We are looking to recruit a Band 6, full time position (part time applications considered) note: this post would be considered as a developmental role, therefore there is an opportunity for an experienced B5 to work towards a B6 within a set time frame.
The role would include working with pre-school children with significantly delayed/disordered speech and language & supporting children, most with an EHCP, within mainstream schools as part of a school based team.

Main duties of the job

Our working shifts are 08:00 to 16:00 and 12:00 to 20:00, over a 7 day period, including regular weekends. There will be an expectation to work within the North West Locality, though the base for this job is Kington.
Main duties of the job
Our aim is to provide treatment and support to patients in their own homes. Whilst undertaking a holistic nursing assessment, we aim to encourage patients to remain as independent as possible, in order to prevent hospital admissions.
Due to the nature of this role, the ability to meet the travel requirements of the post are essential.

To perform laboratory reception duties including: checking sample quality, ensuring that appropriate samples are received for the tests requested, assigning laboratory numbers to samples and entering requests into the laboratory computer system. Main duties of the job
- Receiving, sorting, numbering and pre-analytical preparation of potentially infectious clinical specimens.
- Ensuring that the correct samples are received and the correct test codes are accurately assigned.
- To answer and make enquiries to/from wards, departments and GP surgeries. Communicate effectively and professionally with other laboratory staff, hospital staff and other service users,
- To maintain confidentiality at all times. This will involve dealing with confidential and sensitive patient information.
- To perform receptionist duties dealing with patients, visitors and other hospital staff.

Job summary
The purpose of this job is to support the provision of an efficient service for the daily operation, testing and commissioning of medical and surgical equipment used within all departments of Wye Valley NHS Trust, as directed by the Medical Engineering and Medical Equipment Library Manager.

To support the Trust in ensuring that service is provided to agreed standards and meets with current legislation and policies.
The requisition of repairs and parts (including stock control) and to check deliveries and repairs against orders.
To be able to effectively communicate technical equipment-related information to clinical staff of all levels.
To reduce the risks involved in using medical equipment, in line with current Health and Safety and other relevant policies.
To actively participate in working parties, audits, and meetings of Medical Engineering and Medical Equipment Library.
To carry out any other duties commensurate with this job title and with the grade and responsibilities of this post.
To comply with the Trust's and the Department's policies and procedures,
e.g. Health and Safety, Equality and Diversity Policy, etc.
To support the functions of the Medical Technicians & Medical Equipment Library technicians in their absence.
To update the Medical Engineering and Medical Equipment Library database.
Assist the manager and the team in running the Medical Engineering and Medical Equipment Library efficiently and cover the Medical Engineering and Medical Equipment Library staff when necessary.
