Job summary
Wye Valley NHS Trust has an exciting opportunity for an experienced physiotherapists to join our community based Neighbourhood Teams across Herefordshire on a permanent basis. The post holder will work out of the Leominster Community Office base.
You will be working alongside other Physiotherapists, Occupational Therapists - registered and non-registered, District Nurses, ACPs and alongside Home First reablement service and care agencies, completing home visits and offering physiotherapy assessments and treatment plans to patients. The role includes community rehabilitation, discharge to assess and urgent response.
Our community teams, we strive to invest in your development and offer regular training, team meetings, shared learning and regular supervision sessions. We aim to support the Trusts focus of providing care closer to home.

Main duties of the job
Working as part of the therapy and wider multidisciplinary team, Band 6 senior therapists use specialist skills in a variety of areas to provide direct high quality and effective therapeutic care and support to patients. This post is based out in the Community.
Within the scope of the role, the post holder will independently plan, complete specialist assessment and deliver interventions. They will be responsible for independently managing their own patient caseload, demonstrating clinical autonomy, specialist clinical reasoning, prioritisation and manage competing demands. Supervision is clear and direct from senior therapy staff and includes formal training, supervised sessions, advice and support.
The post holder will initiate and contribute to service development, demonstrate self-development and will be responsible for supervision of others and delegate appropriate tasks to registered and non-registered staff. The band 6 senior therapist will be required to participate in 7 day working and for physiotherapists to join the on call rota once competencies have been achieved.

Job summary
An exciting opportunity has arisen for an experienced and highly skilled clinician to join the Single Point of Access (SPA) team as a Band 6 Therapy Clinical Decision Maker. The post holder will provide clinical assessment, triage, decision-making and leadership for referrals with identified therapy needs, working within a complex and fast-paced service. The role will be 2 days, 8-6 (Monday to Friday), based at the Community Response Hub.
The Single Point of Access provides a centralised route into community services, ensuring patients are assessed and directed to the most appropriate service based on their individual needs. The Therapy Clinical Decision maker will play a key role in delivering safe, responsive and effective clinical triage, supporting timely access to community-based care, to prevent avoidable deterioration, hospital attendance and admission.
This is a telephone-based clinical role, requiring the post holder to undertake comprehensive clinical triage and assessment remotely. The post holder will use clinical reasoning and professional judgement to identify clinical risk, determine urgency and make autonomous decisions regarding the most appropriate pathway of care.
This opportunity would suit an experienced registered clinician with strong assessment and decision-making skills who is confident managing clinical risk remotely and is motivated to contribute to the development of an integrated, responsive and patient-centred Single Point of Access service.
Main duties of the job
Undertake comprehensive, predominantly telephone-based clinical triage and assessment of patients identified as having therapy needs, with a wide range of acute, complex and changing health and care needs.
Complete triage and/or screening of urgent and more routine therapy referrals, filtering referrals for UCR and Neighbourhood team, identifying the most appropriate onward service.
Work alongside a Senior Triage Clinician, HUB GP being part of a MDT clinical decision-making team. Using clinical reasoning, professional judgement and evidence-based practice to determine clinical urgency, level of risk and the most appropriate course of action.
Recognise signs of acute deterioration, red flags and safeguarding concerns, taking immediate and appropriate action as required.
Work collaboratively across organisational and professional boundaries, liaising with community teams, primary care, ambulance services, acute providers, social care and other system partners to identify and coordinate the most appropriate pathway of care.
Contribute to the development, implementation and review of clinical pathways, triage frameworks, protocols, standard operating procedures and service guidance.
Participate in clinical audit, quality improvement, service evaluation and other clinical governance activities, using findings to support improvements in the quality, safety and effectiveness of the SPA service

Job summary
- To enable WVT to successfully role out E-Rostering to different areas and staff groups across the Trust, the post will primarily focus on the introduction of medical staff rostering, implementing a systematic rollout and to embed into the appropriate areas as agreed by the E-Rostering Programme Board
- To support with the basic configuration, maintain and develop the E-Rostering utility across clinical and non-clinical services within the Trust
- To facilitate and monitor progress of the system, ensuring delivery value for money as identified in Key Performance Indicators.
- To provide both face to face training to a larger group or 1:1 in the use of the E-Rostering system to staff who use the system which will ensure that full benefits are realised.

Main duties of the job
To assist in the basic configuration, setup, maintenance and development of the Trust E-Rostering system.
To work closely with service managers and staff to ensure that the rostering system is used appropriately, efficiently and effectively
To assist in the development of a comprehensive plan to support both long term and short term goals across the organisation around electronic rostering
Run reports from the E-Rostering system to enable management overview of the use of the system and analyse detailed results and escalate where required
Support where appropriate that relevant policies and procedures are followed with the support of Senior E-Rostering Team and contributes to the development/update of any relevant policies
Provide sensitive and contentious information to support the need to implement electronic rostering. Where there are potential barriers to acceptance use interpersonal, influencing, negotiating and communication barriers to overcome barriers
To work effectively with team members to ensure that the rostering system reflects information held in ESR.
Use ESR to maintain the integrity of both systems ensuring that any anomalies are fed back to workforce information colleagues and the payroll team

Garway is a 24 bedded frailty unit.
As a B6 junior sister/ charge nurse you will be responsible for promoting and implementing practice conducive to the development of high quality patient care by providing professional, clinical and managerial leadership. Main duties of the job
Garway are looking for an experienced B6 nurse to support the Ward Sister with the day to day management of the frailty unit . The successful candidate will be expected to take on the management and daily oversight of Ward in the absence of the Ward Sister.
As a junior sister/ charge nurse you will have responsibility for monitoring standards and quality in the development, implementation and evaluation of patient care. You will monitor and support staff training and education, risk management reporting at ward level and budget control including allocation of resources.
A strong clinical background in acute general medicine is essential to this role and knowledge of caring for patients with impaired cognition issues, to support the nursing team and ensure our patients receive the right standard of care.

An exciting opportunity has arisen for an experienced and highly skilled clinician to join the Community Referral Hub (CRH) team as a Band 7 Senior Triage Clinician. This senior clinical role combines expert assessment, triage, decision-making and clinical leadership within a fast-paced service.
The CRH provides a centralised route into community services, supporting timely access to care and helping prevent avoidable deterioration, hospital attendance and admission. The post holder will undertake predominantly telephone-based clinical assessment and triage to identify risk, determine urgency and direct patients to the most appropriate care pathway.
Working autonomously, the Senior Triage Clinician will manage referrals for patients with a wide range of needs, including increasing acuity, complexity, frailty and multiple health and social care requirements. This is an exciting time to join the Surgical Division, which has already made significant productivity progress and theatre utilisation, with a new theatre opening soon providing further capacity. The purpose of this job is to undertake routine and specialist respiratory function testing in a variety of clinical settings. Interpret results and provide accurate reports. To plan, undertake and perform interpretation on a range of diagnostic assessments and treatment interventions. Main duties of the job

An exciting opportunity has arisen to join the Dermatology Department as a Surgical Booking Coordinator. The post is 32 hours per week on a permanent basis. We are looking for someone who has a professional approach to work and colleagues, has the ability to prioritise and organise their own workload and has excellent communication skills.
Main duties of the job
The purpose of this job is to be responsible for providing a coordinator role for booking skin surgery in the Dermatology Department. The role involves working for Consultants and their team and supervising junior secretarial/clerical staff. This includes managing the inpatient worklist and booking TCI (to come in) dates for patients to attend for their surgical procedures. It involves arranging surgical sessions, ensuring nursing staff are available and closely monitoring the waiting lists for both urgent suspected cancer, urgent and routine patients. There needs to be flexibility in working throughout the Division as service needs require.

We have an exciting opportunity for an Operational Support Officer to join the Surgical Division.

The role is imperative to support the smooth running of the Directorate services and will mainly compromise of creating and monitoring service rotas. Main duties of the job
Administrative Responsibilities
- Undertake routine monitoring of EPR and RTT; data quality reports system to ensure waiting times; booking targets are met and that patients will not breach the in-patient and outpatient targets and escalate areas of concern to the Directorate support team.
- Monitor Patient Tracker List investigate outliers and escalate issues as appropriate.
- Input and produce data in support of the administration management of the Directorate e.g.: ESR; EPR; rotas; trackers; Datix, Integra; Shared drives
- Support project work as delegated e.g.: coordinating authorised waiting list initiatives; EPR training
- Undertake administrative tasks and storage in support of the Directorate Team e.g.; arrange meetings; take minutes; typing of policy / SOP; completion of action logs; manage diaries
- Develop and maintain robust Directorate and Speciality systems that capture and enable timely access and accurate monitoring of relevant information.

We are seeking an experienced and innovative Band 7 Occupational Therapist to join our Children's Community Occupational Therapy team.

Main duties of the job
This role is a mix of having your own clinical caseload and providing leadership and management of the Occupational Therapists and support staff within the team to facilitate the development and transformation work.
Whilst working as an autonomous practitioner in the community, within the county of Herefordshire, plan and organise own caseload according to the needs of the children and the service. To provide highly specialist clinical support and supervision to the team.
To use evidence based standardised and non-standardised assessment tools with children, including observations in the most appropriate settings for the child/young person, in order to establish levels of functional ability to inform your practice and intervention.
To agree child focused aims, goals and objectives for all OT intervention.
To design individual therapeutic treatment plans, for children to be carried out either by you or delegated to Occupational Therapy support workers, families, carers or school staff. These interventions will take place in the most relevant environment for the child and their family - the home environment, therapy suite or schools.

We are seeking an experienced and innovative Band 7 Occupational Therapist to join our existing community Frailty Clinical Specialist Occupational Therapists working in our East PCN.
The post-holder will work as part of our multi-disciplinary PCN Wellbeing Team, which includes two other Frailty Occupational Therapists, Social Prescribers, a Health and Wellbeing Coach and Safeguarding Care Coordinators.
This role is an extended scope, advanced practice role working in the community with people in their own homes. The purpose of the role is to provide advanced Occupational Therapy clinical assessments, interventions and planning. This includes advanced care planning discussions. Liaising closely with the MDT including the GPs at the surgeries to facilitate and support these assessments. To proactively manage frailty and ageing well in the community, reducing un-necessary hospital admissions and supporting individuals to achieve the best possible quality of life.
