Job summary
The Children's Rapid Response Service have an exciting opportunity as it continues to expand the team. The Children's Rapid Response Service works collaboratively with the Community Children's Nursing & Palliative Care Team: It is the team's primary aim to prevent and reduce hospital attendances, admissions and length of stay for children already known to the Community Children's Nursing & Palliative Team.
The emphasis of this service is to improve the quality of life for children and families with life limiting, palliative or complex healthcare needs, offering them greater choice around their chosen place of care.
Are you an enthusiastic, dynamic, flexible and motivated experienced Nurse or Nurse Practitioner, with a strong knowledge of Children's Nursing? Clinical expertise and good communication skills are required as well as excellent time management and prioritisation skills which are essential for this role. Main duties of the job
We are looking for experienced children's nurses that have extensive experience in assessment with children with complex health care needs, Ideally the suitable candidate would hold, or be willing to work towards obtaining the independent Non-Medical Prescribing qualification / Assessment Course.
You must have the ability to prioritise your workload using advanced assessment skills with the ability to provide a symptom management plan of care to children/young people during times of acute deterioration or illness.

- Work within the community (e.g. within clients own home) as a lone practitioner managing situations of risk with telephone support. Seek support and advice with cases from more experienced colleagues when faced with matters outside sphere of responsibility, knowledge or skills.
- To provide a calm and supportive approach to the management and delivery of sensitive and complex information, providing both practical and emotional support, utilising flexibility and empathy in difficult situations.
- Use effective communication techniques communicate to the multidisciplinary team to ensure the child and family referred to Childrens Rapid Response receive the best possible care.
- Undertake risk assessments prior to service delivery within the home setting to ensure staff safety.
- Work in partnership with children, families and carers to provide optimum individual outcome acknowledging children and familys personal preferences, spiritual and religious beliefs.
- Formulate individualised management and treatment plans using sound knowledge and evidence based practice. Plans should be in response to sound clinical reasoning and include expected signs of deterioration.
- To provide timely, effective symptom management, resulting from a comprehensive history taking utilising independent non medical prescribing, GPs or hospital team, completing the symptom flow charts to monitor the childs changing condition/deterioration.
- Provide and promote strategies/detailed plans of care to parents and carers empowering care that will prevent inappropriate admissions and reduce length of hospital stay.
- Ensure assessment, planning and delivery of holistic care is of the highest standard utilising an agreed nursing model.
- Liaise and work in collaboration with acute services, primary healthcare team and other agencies when planning and implementing programmes of care facilitating early discharge and preventing admissions.
- Advise the Team Leader, Clinical Lead or Service Manager of any circumstances which give cause for concern in relation to patient care and/or any concerns raised by the primary healthcare team or partner agencies.
- Maintain clear, concise and contemporaneous records adhering to BCHC record keeping policies.
- Provide a flexible service to accommodate the changing needs of children and families.
Management
- In the absence of the Team Leader/Clinical Lead provide leadership to ensure the effective service delivery.
- Responsible for accepting referrals and discharging patients from own caseload.
- Responsible for reassessing children, ensuring regular formal review of the caseload in conjunction with the Team Leader, ensuring these are documented and evidenced as per record keeping guidelines.
- Ensure standards are maintained with both clinical quality and governance being central to the function and delivery of the service.
- Accountable for advice and guidance to patients, families, carers and other team members.
- Act as an effective role model for other team members, students and professionals.
- Participate in research and audit relevant to the service in conjunction with the Team Leader/ Clinical Lead.
- Appropriately manage the service resources e.g. equipment, staffing, supplies in conjunction with the Team Leader/Service Manager.
- Develop policies, procedures and guidelines in conjunction with the wider multidisciplinary team.

- Act as a link Nurse and take clinical lead in areas of expertise. Attend courses and meetings as required to fulfil the link role as delegated by the Team Leader.
- Participate in reflective practice/clinical supervision, critical incident analysis and debriefings.

The Ross-On-wye District Nursing Team are a very supportive and dedicated team, we are an incredibly engaging, positive and hard-working team, providing care and support to patients in their own homes.
We work closely with Golden Valley DN Team and often work as a locality where needed.
We work closely with a wide range of multi-disciplinary teams, including GP's, Palliative Care Nurses, Therapy Teams, and Advanced Clinical Practitioners. Ross DN Team also provide a wound clinic service based at the Community Hospital.
Main duties of the job
Our aim is to provide treatment and support to patients in their own homes. Whilst undertaking a holistic nursing assessment, we aim to encourage patients to remain as independent as possible, in order to prevent hospital admissions.
Due to the nature of this role, lone working and the ability to meet the travel requirements of the post are essential.

Job summary
The Training Programme Director for O&G Ultrasound Co-ordinator will coordinate delivery of the basic and intermediate ultrasound modules.
They will be a member of the School Board and be expected to attend School Board Meetings. They will work with ultrasound trainers in the region to coordinate delivery of the ultrasound curriculum and assessments to support the School Operational Programme.

Main duties of the job

Main duties of the job
- To plan and deliver a highly specialist psychology service in Mindworks Community Hub North and to oversee its clinical governance.
- To be responsible for the professional leadership and continued development of psychologists within the Mindworks Community Hub North.
- To demonstrate highly specialist expertise and specialist psychological skills such as specialist psychological assessments, formulation, therapeutic interventions and consultation to support children and young people with complex mental health needs
- To lead in the supervision and contribute to Individual Performance Review/Appraisal of psychology staff, as required.
- To ensure that the healthcare needs of people using services are communicated to and work in partnership with service commissioners and other healthcare providers for the best outcomes.
- To advocate and represent the service.
- To be responsible for the development and implementation of appropriate policies, protocols and care pathways.
- Contribute to appropriate staff recruitment and contribute to the wider strategic plans regarding psychology recruitment.

Join our expert Radiotherapy Team as a Senior Review Radiographer, based at Velindre at Nevill Hall Radiotherapy Unit, Abergavenny. Main duties of the job
To organise and lead on radiographer led review clinics, including assessing and managing treatment side effects, and advising patients and carers on how to manage these side effects. To include prescribing medications where appropriate utilising the current prescribing guidelines.
To lead on information and support for radiotherapy patients and carers, providing a holistic approach to the emotional, practical, social and information needs, utilising a variety of communication media.
To be part of the multidisciplinary team involved in the total care of patients undergoing radiotherapy and chemotherapy treatment and highly specialist treatments e.g. SRS and SBRT

To take responsibility for training and supervising more junior review radiographers, rotational radiographers, support staff and students.

As a Cancer Navigator you will have direct contact with cancer patients, family and external healthcare professionals over the phone; a pivotal role in helping to ensure that all cancer patients of the Trust receive high quality empathetic care and support. We have a part time position available for a Catering Assistant to join our team at Wansbeck General Hospital.
This is a part-time position for 21 hrs per week
Hours of work for this post will be flexible whilst rotas are under review but will usually be worked as 3 x 7hr shifts per week between 7am - 7pm.
Please note the successful applicant will be required to work a share of weekend and bank holiday dates, for which an enhanced rate is payable.
As part of this role, you will be required to maintain the standards of cleanliness as specified throughout the work areas, by means of approved methods.
The role also involves serving in our staff and visitor restaurant and taking cash and card payments using our EPOS till system. It is important that the successful candidate is comfortable and confident to speak and interact with our customers.
Please note that it is a requirement of Northumbria Healthcare Facilities Management that all successful applicants pay for their own DBS certificate, if a DBS check is required for the post. The method of payment is a salary deduction from your first monthly pay.

An exciting opportunity for an experienced Physiotherapist, with an interest in Obstetrics, Gynaecology, Urology and Colorectal conditions has arisen, to join our friendly team. This post is to provide a highly skilled and specialised physiotherapy service in predominantly an outpatient setting. You will be responsible for providing a service to women and men with bladder, bowel, and pelvic floor disorders as well as providing an obstetric service which will include the provision of classes. You should be a member of the Chartered Society of Physiotherapy with HCPC registration and have at least 2 years post graduate NHS experience as a Physiotherapist. The successful applicant will need to demonstrate appropriate CPD activity.

Main duties of the job
- Complete the daily shift plan in the agreed time scale, ensuring cleanliness and hygiene standards are met in line with New NHS Cleaning Guidelines.
- Complete weekly departmental performance monitoring, to achieve and maintain the Trust's own standards.
- Maintain a patient focused service across clinical areas.
- Undertake general cleaning and other related domestic tasks to a high standard, within a designated area and managing own task schedules.
- Maintain a high standard of provision of other Hotel Services related tasks.
- Work as part of the Hotel Services team in conjunction with ward managers on all aspects of ward cleaning and hygiene.
