Clients will be experiencing severe, complex and enduring psychological, emotional and mental health difficulties. The post holder will carry out this process in consultation as appropriate with the client and the family, fellow team members, other professionals, and the referrer.
Main duties of the job
We are looking for a highWe are looking for a highly motivated specialist practitioner to join our CAMHS Service. The successful applicant will be based in a community based service across the communities of North Powys.
Powys CAMHS works within a whole service ethos supporting each other and encouraging individual development and recognition of unique skills. There are opportunities for training and career development. The successful applicant will work with children, young people and their families in clinic and home settings and across a range of emotional and mental health difficulties. As part of the role there will be an expectation of working intensively and flexibility with children to reduce the likelihood of being admitted to hospital as well as when they are discharged.
Powys CAMHS has well established links with partner agencies, neighbouring CAMHS services and educational establishments. Individuals are encouraged to work autonomously with appropriate support and guidance from senior team members.

Job summary
37.5 hours per week (fixed term for two years)
We are pleased to advertise 2 brand new practitioner posts in Barnsley working in the Integrated Recovery Team. The successful candidates will work in a multi agency team and will be integrated into our local substance misuse provider Waythrough. The team will attempt to work with clients in an assertive way who are difficult to engage and have co-occurring substance misuse and mental health issues. The posts are initially on a fixed term 2 year contract but this may be extended.

Main duties of the job
The successful candidates will need to have excellent engagement skills and be prepared to be challenged by often complex clients who historically have been resistant to engaging with treatment. The candidates will need to have exceptional communication skills as liaison with statutory and voluntary agencies will be a key element to the post. An ability to undertake an assessment of needs and risks and complete a thorough care plan is essential.

This is a vital role for the London Ambulance Service, receiving and taking charge of all incoming emergency calls. You are the first point of contact for our callers and patients, some of whom will be distressed, anxious or panicking. Using effective communication, interpersonal and persuasive skills, you will deal with all callers in a professional manner in order to obtain the required essential patient information.
Using the Medical Priority Dispatch System (MPDS), you will obtain an initial assessment of both the patient and the scene in an efficient manner. You will also be responsible for accurately recording essential patient information in a precise and timely manner.

Main duties of the job
You must be able to remain calm in an emergency. When people are panicking, your role is to be clear and assertive where required to ensure the caller remains focused. You will need to be able to follow a script and reassure the caller, but also to be firm where necessary about what they need to do.

Represent the Clinical Effectiveness Team at local, regional and national events as requested, to develop and maintain strong networks with other individuals and organisations.
Support professionals to undertake clinical audit associated with identified quality improvement. Work with colleagues to develop and implement tools and processes that enable a consistent approach.

We are looking for a highly motivated and enthusiastic receptionist/clerical officer to provide reception and administrative support to a busy Sexual Health and Contraception Service for 37.5 hours per week. Working as part of a friendly admin team you will have excellent communication skills and be able to work calmly under pressure using your own initiative, with the ability to prioritise work to meet deadlines.
Essentially, we are looking for a good standard of general education along with previous experience of dealing with patients or customers, and a good level of computer literacy.
We would expect the candidate to be a good team player with a flexible approach to work, providing assistance to your colleagues and cover for sickness and annual leave, helping to ensure an effective administrative service at all times.
The post holder would be based at Lance Burn Health Centre and travel across Greater Manchester as required. The post holder will work Monday to Saturday on a flexible rota basis.
Main duties of the job
Greet all visitors attending the reception area, ascertain their requirements and respond to those requirements accordingly.
Clarify demographics for every patient contact face to face and via telephone and ensure systems are updated regularly.
Enter clinic attendances and outcomes onto INFORM in the identified time scales and to the identified standards
Book appointments,update attendances and action referrals.
Maintain the reception waiting area to an orderly state at all times.
Answer telephone calls courteously and politely, direct enquires appropriately and take messages as necessary, liaise with other departments where appropriate.
Arrange and respond to transport-Interpreter requests as directed. Maintain the confidentiality, safety and security of the reception area reporting any adverse matters to senior staff.
Work flexibly across the service as required.

Main duties of the job
Your primary role being to undertake medicines reconciliation and take drug histories in order to ensure the patients receive the correct medications right from the start of their hospital stay to ensure that there is appropriate usage of Patient's Own medication and to promote one stop dispensing.
You will be involved throughout the patient journey helping the wards to ensure TTOs are dispensed prior to discharge, communicating effectively with our community colleagues when appropriate.
You will be an accredited checking technician (ACPT) and will have regular checking slots in the dispensary. In liaison with the Deputy Chief Pharmacist, Clinical Services and other members of the pharmacy team you will also be involved in the management of medicines on the ward, audit and financial savings projects.
You will participate in late duty, Bank Holiday and weekend rotas and be part of a seven day service.

Job summary
We are looking for an experienced Practice Nurse to join our already experience nursing team. Ideally we are looking for between 28 - 37.5 hours per week, and can be flexible for the right person. The role will include cross site working between The New Surgery and Barnburgh Branch.
Experience in Chronic Disease Management in particular Diabetes/COPD/Asthma is essential, holding a Diploma for some or all of the disease areas is required.
The successful candidate will be responsible for providing comprehensive nursing care to patients including immunisations, cytology, wound care and other clinical duties. They should have excellent communication skills. A good knowledge of QOF is desirable. We would also welcome Practice Nurses with Non Medical Prescribing Certificate and Minor Ailments Certificate to apply.
Main duties of the job
You will provide a comprehensive nursing service to the practice population to meet healthcare needs. You will undertake a variety of activities mainly including managing long term condition reviews for Diabetes, COPD, Asthma, CKD, HF, CHD. Also other nursing duties such as cytology, immunisations, and dressings.

New Surgery and Barnburgh Branch has just over 10,000 patients very supportive and friendly practice. Multiple staff members have been with the practice over 30 years. We have a total compliment of over 30 staff over 2 sites including GP's, Nurse Practitioner's, First Contact Practitioners, Practice Nurses, Nurse Associate, HCA, Phlebotomist and Pharmacist.
We work with and are supported by numerous ARRS staff.

We are seeking an enthusiastic and experienced Band 6 Clinical Nurse Specialist or Allied Health Professional (AHP) with specialist knowledge and experience in respiratory care to join our team at Farnham Hospital.

Job summary
BHFT is developing a specialist multidisciplinary Assertive Outreach (AO) service, aligned with Community Mental Health Services, to provide safe, compassionate and flexible support for people with severe and enduring mental health needs who may struggle to engage with services.
The service will initially operate from two hubs in Reading and Slough, and we are recruiting two Band 6 nurses to support its rollout.
This role is based within the Slough team and is one of several new specialist posts. The successful candidate will also work across the Trust to help develop a consistent AO approach and promote best practice.
Our approach is centred on:
- Working alongside individuals, families, carers and wider support networks to overcome barriers such as trauma, mistrust, communication needs, co-existing conditions and social challenges.
- Delivering proactive, intensive, holistic and flexible care, meeting people where they are and tailoring support to their needs.Promoting fair access to mental health care, particularly for marginalised and underserved communities, while addressing health inequalities
To achieve this, we work with smaller caseloads than traditional Community Mental Health Services, enabling more intensive support. The post holder will be part of the Slough Assertive Outreach Team and work within the service's operational hours. Care will be delivered primarily in community settings, rather than expecting service users to routinely attend health facilities.
Main duties of the job
- Within BHFT's Assertive Outreach Service, the post holder will provide intensive, flexible and proactive support to people with severe and enduring mental health needs who struggle to engage with traditional services. This Band 6 role offers an opportunity to help shape and develop a new service while delivering high-quality care to service users and those who support them.
- Based in Slough, with occasional cross-cover across Berkshire, the post holder will deliver recovery-focused, evidence-based nursing interventions to adults experiencing significant challenges with engagement.
- Working within a shared caseload, the post holder will build and maintain therapeutic relationships with service users, families and carers, using assertive, flexible and person-centred approaches to improve engagement, reduce relapse and crisis, and promote recovery and independence.
- The role involves completing comprehensive assessments, developing and reviewing personalised care plans, and delivering evidence-based interventions tailored to individual needs.
- As a core member of the Assertive Outreach multidisciplinary team, the post holder will contribute specialist mental health nursing expertise to formulation, care planning and decision-making. We are looking for a Advanced Paramedic Practitioner to join our growing team at Aylesford Medical Centre. You will form an integral part of delivering our on-the-day urgent care.

Key Responsibilities: Patient Care / Clinical Practice
Optimise patient care in defined wards/divisions by planning, managing, monitoring, advising and reviewing specialist care plans for patients.
Formulate treatment plans for inpatients, considering their presenting symptoms, medical and psychiatric history (including previous responses to medication), and the evidence base for pharmacological approaches in that condition. These treatment plans could be presented at multidisciplinary meetings for discussion and review, or discussed with individual doctors as required.

Job summary
An exciting opportunity has arisen for an Advanced Specialist Speech and Language Therapist (Band 7) to join our Mental Health services at Rotherham Doncaster and South Humber NHS Foundation Trust.
This is a rare and rewarding opportunity to work at the forefront of specialist mental health speech and language therapy, providing expert clinical input and leadership across the trust. This will initially be 2 days in Phoenix High Dependency Rehab Unit and 3 days in other acute / older peoples mental health wards across the trust. We will work with the post holder to develop a working pattern which means that all of our wards will benefit from speech and language therapy input.
We are seeking a highly skilled and motivated clinician who is passionate about improving outcomes for people with severe and enduring mental health needs, and who is ready to shape practice, influence services and develop others at an advanced level. This will be one of the first roles in Speech and Language Therapy in mental health at RDaSH and we are invested in this being a success. You will be supported through peer networks, cross trust supervision and support from senior clinical leadership including the Chief Allied Health Professional in the trust.
Main duties of the job
As an Advanced Specialist / Clinical Lead Speech and Language Therapist, you will:
- Provide advanced specialist assessment, diagnosis and intervention for adults with highly complex communication and dysphagia needs within mental health settings.
- Act as the clinical lead for speech and language therapy within the MDT, influencing care planning, risk management, discharge and service delivery.
- Manage the most complex and high-risk cases, using advanced clinical reasoning and high levels of professional autonomy.
- Provide expert advice, consultation, supervision and leadership to SLT colleagues, students, assistants and the wider multidisciplinary team.
- Lead and contribute to service development, quality improvement, audit and research, helping shape the future of mental health SLT within the Trust.
- Play a key role in developing communication-enabling environments and promoting parity of esteem for speech and language therapy within mental health services.

Job summary
An exciting opportunity as arisen to work within a Child & Adolescent Mental Health Service, providing CAMHS assessment and therapeutic services to children, adolescents and their families who have been referred from within the existing service who require extra support to promote their social inclusion into the community. Clients will be experiencing severe, complex and enduring psychological, emotional and mental health difficulties. The post holder will carry out this process in consultation as appropriate with the client and the family, fellow team members, other professionals, and the referrer. Powys CAMHS has well established links with partner agencies, neighbouring CAMHS services and educational establishments. Individuals are encouraged to work autonomously with appropriate support and guidance from senior team members.
This post is advertised as a Band 6, however applicants that are undertaking the relevant qualification who do not yet currently hold all the necessary competencies to a Band 6 level are encouraged to apply and, if successful, will be appointed under Annex 21 rules whereby 70% of the top of Band 6 will be payable during the first 12 months, 75% for the second 12 months or until qualification has been completed.

Main duties of the job
We are looking for a highly motivated specialist practitioner to join our CAMHS Service. The successful applicant will be based in a community based service across the communities of Powys. travelling between North and South Powys will be required
- Lead the development of a dedicated pathway to support reintegration into the community, promoting independence, socialinclusion, and improved quality of life.
- Support the development of staff and strengthen the interface between primary and secondary care to ensure coordinated,person-centred care packages.
- Facilitate opportunities for young people to reconnect with their communities through education, volunteering, and socialactivities, including innovative approaches such as peer groups and nature-based interventions.
- Plan and deliver evidence-based clinical interventions for children and young people with serious mental illness and complexpresentations.
- Act as Care Coordinator in line with the Mental Health (Wales) Measure 2010, adopting a recovery-focused, person-centredapproach to co-produce individualised care plans.
- Apply specialist therapeutic skills and NICE-aligned interventions, working autonomously with appropriate supervision while maintaining CAMHS and professional standards.

Do you have a genuine passion for supporting people living with obesity? An exciting opportunity has arisen, for an enthusiastic and motivated band 4 Physical Activity Associate Practitioner, to join our dynamic team within Tier 3 Specialist Weight Management.
The role will involve working with the Tier 3 Specialist Weight Management Multi-Disciplinary Team (MDT) to support patients to lose weight and improve their health and wellbeing. The role will involve assessing and providing tailored physical activity advice and support to patients who access the service. The Tier 3 Specialist Weight Management Physiotherapist will provide clinical support and supervision.
This is a rewarding and challenging post and the postholder should ideally have experience of supporting patients living with obesity and complex health conditions.
Main duties of the job
The role will include identifying, demonstrating, and observing specific, tailored exercises to meet the needs of the patient. Excellent time management skills are an essential part of the role as is the requirement for accurate record keeping within electronic patient records.
The ability to use a range of communication methods and behaviour change approaches are also key attributes required for the role. The postholder should also have a good understanding of the complexity of obesity and the challenges associated with obesity and physical activity

This is an exciting opportunity for a newly qualified Occupational Therapist or an established 5 wanting to gain physical health, inpatient experience, to join our friendly and supportive team at Hillingdon Hospital NHS Trust. The post holder will complete a wide variety of acute and rehab physical rotation alongside preceptorship.
To facilitate your successful transition from student to qualified OT band 5s are supported by senior therapists, and peer support, alongside band 5 meetings and in service training and team meetings. This is a great place to start your career, many of our 6s and 7s have successfully progressed from band 5s within our Trust.
Rotations cover the following specialities:
- Frailty
- Acute Medicine & Surgery
- Orthopaedics
- Acute & Rehab of Older Adults
- Neurology
- The Stroke Unit

Main duties of the job
Main tasks and responsibilities
As part of the multidisciplinary team (MDT), assess the suitability of patients for provision of OT interventions following a referral.
To carry out specific standardised and non-standardised OT assessment(s) within your designated caseload.
To plan and implement appropriate intervention programmes in the hospital, home and community environments according to need.
To ensure ongoing monitoring and evaluation of intervention programmes.
To act as resource and offer support to patient and relatives / carers.
To support the delivery of occupational therapy in other clinical area as appropriate and necessary according to service need.
Appropriately delegate identified tasks to assistant or support staff.
Communication and Partnership Working
To effectively communicate with patient, their relatives and carers, and all other relevant parties involved with the patient's care.
To maintain the role of OT as a member of the multi-disciplinary team.
To communicate and liaise, formally and informally, at all times with the team or appropriate others with regards to OT involvement with all patients in the treatment programme.
To participate in all relevant multi-disciplinary team meetings.
To attend relevant meetings and feedback to appropriate individuals.
To participate in presentations to patients regarding occupational therapy interventions - e.g. through patient education classes as part of Joint Replacement School.
