Job summary
Thank you for your interest in this Band 6 Mental Health Practitioner position within our Ealing Community Rehabilitation Service (CRS). This is a full-time and permanent position and attracts 15% Outer London weighting. We welcome applications from Registered Mental Health/LD Nurses (registered with the NMC) and qualified Social Workers (registered with Social Work England) who are interested in working in complex psychosis in the community.
Mental health practitioners play a key role in dissolving the barriers that can exist between mental health and physical health, social care, voluntary and community social enterprise organisations and local communities, and primary and secondary care, to deliver integrated, personalised, place-based and well-coordinated care for adults - moving towards a 'whole system' of mental health care, which will provide timely access to support, care and treatment.
We need qualified, experienced, skilled and professionally registered practitioners, to support people with mental health needs to access mental health care when and where they need it, and to be able to move through the system easily.
What we are looking for are highly motivated and enthusiastic practitioners with a passion for psychosocial rehabilitation who are excited by service development. You will need experience at band 5 (or equivalent) and of working in the community to be able to step up to this band 6 role.
Main duties of the job
Depending on your professional background, this role may include clinical assessment and evidence-based interventions, coordination of mental and physical healthcare, relapse prevention and recovery-focused support, alongside the psycho-social interventions that help people manage their problems and move towards individualised recovery on their own terms - surrounded by their families, carers and social networks, and supported in their local community.
You will be an autonomous practitioner who will carry continuing responsibility for a defined caseload of service users experiencing complex psychosis.
You will support individuals to be able to contribute to and be participants in the communities that sustain them. A key part of the role will be liaison with Local Authority and ICB colleagues to ensure that any out-of-area and supported accommodation placements are suitable for the needs of the service user and that people are supported in the least restrictive environment possible.
Please consider applying if you are passionate about supporting people to live as independently as possible and are committed to a strength based and recovery focused approach to supporting people with complex psychosis.

Avon & Wiltshire Mental Health Partnership NHS Trust View all jobs
Posted 17 day(s) ago
Reference: 342-SS119-0926
Job summary
Please note that this role covers Devon and Cornwall. The Weston Super Mare address is an admin base and regular travel there is NOT required.
The Southwest Op Courage Veterans Mental Health Service operates across the entire Southwest and has an admin base in Weston-Super-Mare. The service, one of six national NHS England commissioned Op Courage services, is delivered by Avon and Wiltshire Mental Health Partnership NHS Trust. This role will cover Devon and Cornwall so the postholder will need to live within this area; however, on occasions, it may be necessary to work outside your normal area to support service delivery.
Op Courage provides a complete care pathway and is delivered by a Multi-Disciplinary Team. The pathway starts with a comprehensive needs-led conversation and is followed by evidence-based interventions/treatment for a wide range of need and complexity, from the early signs of mental health problems through to complex mental health difficulties and psychological trauma.
The service is delivered via a hybrid model which includes using clinical spaces, hot-desks and homeworking. Clinical appointments are delivered face to face or virtually, so some lone working will be required. The postholder will also be expected to attend face-to-face team meetings and Trust training which will be within the Op Courage footprint but may be outside their usual working area.
Main duties of the job
- To contribute to the achievement of corporate and Strategic objectives by leading effective operational management of the Team under your responsibility.
- To take management responsibility for the operational delivery of services across your nominated responsibility and to ensure safe and effective services for service users.
- To liaise at a local level with Service Users, Carers, Staff and Partners.
- To be responsible for the delivery of Health and Safety responsibilities as required by statute, regulation, legal obligation and Trust policies.
- To ensure full compliance of all staff within the area of operational management, to support the clinical delivery of safe and affective services, incident reporting, serious incident review and complaints.
- To contribute to and take specific leadership responsibility for service redesign promoting the development of practice in line with national policies and evidence base.
- To ensure involvement of the area of operational management in service planning, delivery, choice, and governance.
- To undertake investigations of serious incidents, complaints, disciplinary and grievances across the LDU when required.
- To ensure that meaningful staff involvement takes place within the area managed so that commitment and ownership are maximised and staff have the opportunity, where possible, to influence service provision.
