Job summary
The Royal Surrey Cancer Centre is looking for an empathetic and motivated individual to join our Teenage and Young Adult (TYA) Cancer Service as Youth Coordinator.This is a job share where the post holder will be expected to have a strong understanding of person-centred care, with the ability to build supportive relationships with young people aged 16-30 facing cancer.

Main duties of the job
To work as part of the Teenage and Young Adult (TYA) cancer service to provide age-appropriate activities and support to facilitate cognitive, social and emotional development of young people aged 16-30 years. The role will involve working in a TYA-specific clinical areas at The Royal Surrey Foundation Trust Hospital, and some non-hospital settings e.g. external peer support activities

Job responsibilities
1. Establish effective relationships with teenagers and young adults, using age-appropriate approaches and communication skills to identify individuals interests, strengths, areas of concern/need and help them to participate in positive activity while in hospital.2. Develop a programme of 1:1 and small-group diversionary and supportive activities and enable young people to participate as appropriate to their physical health and personal preferences.3. Use motivational techniques to engage young people in their cancer treatment plan, follow up schedule and self-care. Assist colleagues in effective communication with individual young people to achieve this aim.4. Contribute to information-giving and preparation of young people undergoing medical procedures and accompany them to provide non-clinical support as required. 5. Collaborate with colleagues to identify health and wellbeing issues for the patient group and for individuals, and to provide young people with access to age-appropriate awareness and health promotion materials.6. Assist in providing ward environments in which young people feel safe, welcomed, respected and encouraged, and that facilitate supportive social interaction between patients.7. Ensure recreational, craft and other non-medical equipment used by TYAs is maintained for safety and cleaned in line with relevant policies e.g., infection prevention, food hygiene.8. Co-ordinate agreed therapeutic hospital-based activities provided by internal or external agencies e.g. art therapy, music workshops, and liaise with the complementary therapy service.9. Supervise unqualified staff such as students and volunteers.10. Encourage teenagers and young adults in hospital to maintain their personal routines, independence, education/work and personal interests. Help them keep up their links with family, friends, school or work.11. Attend the relevant ward and multidisciplinary meetings to contribute to coordinated assessment and support, report on work being undertaken with patients, raise any concerns and seek advice.12. Contribute to transition programs for teenagers in the childrens service, working under guidance of clinical staff to help young people develop their understanding of their illness, treatment and self-care needs and supporting successful transfer from child to adult services.13. On the request of clinical colleagues, contribute to the support available for young people attending follow up clinics to identify their concerns and signposting them to sources of appropriate information, advice and support.
Participatory Activities14. Provide opportunities for social interaction, creative expression and informal support with other teenagers and young people with cancer including Look Good, Feel Better, and accompanying young people as required e.g. Ellen MacArthur Sailing Trips and Royal Albert Hall events15. Organize and lead social and support activities out of hospital for teenagers and young adults with cancer. Provide supported opportunities for parents, children, friends, partners and siblings in the TYA age group to participate as appropriate.16. Identify, organize and promote opportunities for teenagers and young adults with cancer to engage in activities that encourage positive contribution and personal development for example activity weekends and opportunities.17. Collaborate with colleagues to develop age-appropriate methods for gaining feedback from young people and supporting their input into developing services. Report on Involvement activities to the appropriate management teams and share the outcomes with patients/families.18. Co-ordinate and contribute to the development of communications technology to facilitate the opportunity for therapeutic support encouraging social interaction and explore options for using internet for patient involvement and feedback.
Record Keeping and Administration19. Maintain records of patient contacts and activities provided. Ensure personal information or observations about individual young people are appropriately shared with colleagues to support multidisciplinary care.20. Record and report all concerns relating to needs assessment or safeguarding children or vulnerable young adults to Lead Nurse and or appropriate department for support.21. Record any untoward incidents using Trust reporting systems.22. Undertake and record structured risk assessments for all organized activities in and out of the hospital liaising with the Lead Nurse and other colleagues as appropriate to manage risk.23. Ensure all necessary consents and medical information requirements are obtained for activities and maintain these records safely.24. Keep records of the referrals, contacts, activity/interventions and outcomes for patients accessing the service and be able to analyse the data to help inform practice25. Plan for and utilize TYA funds effectively to ensure maximum benefit from expenditure.26. Provide agreed information on request to patients/families who wish to make financial or other donations to support TYA cancer services. 27. Ensure records of recreational and technological equipment maintenance are kept according to Trust requirements.
Networking28. Develop strong and collaborative professional links with the hospital teams, working particularly closely with representatives and colleagues from other organizations such as the Young Lives versus Cancer, including on joint activities and projects where appropriate.29. Act as a contact for Teenage Cancer Trust and other organizations regarding patient participation, and liaise with TYA team to ensure each patient has made an informed choice to participate, with signed consents as required. Support individuals throughout these activities.30. Act as point of contact for Teenage Cancer Trust and other organizations regarding trips, treats, holidays and other opportunities. Liaise with TYA team regarding any such activities as appropriate.31. Be an active member of forums, and any other local TYA service development groups as appropriate, taking a lead on projects related to the role, agreed with TYA lead Nurse.
Personal and Professional Development32. Adhere to TYA service guidelines concerning the maintenance of personal and professional boundaries.33. Maintain appropriate professional contact with patients, whether face-to-face or via online systems, telephone, or text.34. Ensure the responsibilities for patient confidentiality are maintained in all contacts with young people, families, friends and any other individuals or organizations.35. Maintain own learning through personal study/reading and attending agreed study days or conferences. Undertake mandatory and other specific training required to fulfil the role and responsibilities.36. Participate in research by outside agencies regarding teenage cancer and adolescent issues.37. Be responsible and proactive in maintaining personal physical safety and emotional wellbeing, utilizing supervision and support systems on offer via the Hospital Trust and others establishments.38. To work in collaboration with other trusts, designated TYA hospitals and Principle treatment Centres39. To develop and engage young people in Patient Public Involvement forums, ensuring their voices are integral to service improvements and developments.40. To manage and facilitate social media platforms within the TYA service.41. To support TYAs through the delivery of psycho social interventions, working alongside TYA well-being specialist, acting as a bridge for young people in being enabled to access psychological support.42. To develop support groups.43. To develop and maintain existing projects designed to meet the needs of the young people.44. To supervise agencies delivering workshops to the TYAs off and onsite.45. To participate in research process by outside agencies regarding TYA issues.46. To support TYA CNSs through collaborative working to meet the needs of the TYAs.47. To support TYAs in their transitions through their cancer trajectory including post treatment and or end of life.

Main duties of the job
The EPR Training Coordinators will provide high quality administrative, project and co-ordination support for the EPR Programme Training.
The post holder will have responsibility for setting up training tracks, tracking the learning progress, and reporting on training compliance from LMS. The post holder will ensure effective training organisation and support efficient in-person and virtual training delivery using their initiative and administrative, event management, and organisational skills. They will bring their excellent communication skills to support engagement with our stakeholders.

Part of this role also includes the processing of samples in Clinical chemistry at Sheffield Children's Hospital:
- To receive, sort and label laboratory specimens as required.
- To be responsible for the accurate data entry into the Laboratory Computer I.T. System.
- Pre-analytical processing of samples to include the receipt, preparation and processing.
- Store, retrieve and dispose of biological material according to the relevant procedures.

The aim of the SYBP Phlebotomy services is to provide parity of phlebotomy duties and responsibilities on all sites where phlebotomy services are under the remit of the laboratory. As such, all members of Phlebotomy staff will be trained to a standard that grants a 'training passport' so that they can be deployed on any SYBP site to perform all duties.

Main duties of the job
The successful applicant will be expected to work as part of the Dental team, preparing and monitoring patients for and during their treatment, providing pre and post-operative instructions.

Job summary
An exciting opportunity has arisen for a one-year Fixed term contract for a Genetic Technologist with good interpersonal skills and a committed team member to join the Clinical Genomics department.Cytogenetics is part of the Clinical Genomics Department bringing together cytogenetics, translational science and molecular diagnostics and is one of the few specialised clinical genomics laboratories of cancer in Europe. The laboratory provides a diagnostic cytogenetics service for haematological malignancy and solid tumours for several NHS Trusts and Cancer Networks, as well as participating in clinical trials.
The major focus of the role will be in performing analysis of acquired genetic aberrations by G-band karyotyping and FISH. Attention to detail is essential and the post holder must be able to work to high standards, following defined protocols and assist in the production of accurate & high quality analytical results for the diagnosis and treatment of patients.
The successful applicant will have a degree in a relevant science subject and have experience working in a genomics laboratory.
Main duties of the job
Provide technical support for the scientific teams within the Cytogenetics Laboratory and, if necessary, in related Pathology or research disciplines.
Be responsible for carrying out G-banded karyotype analysis and FISH analysis with minimal supervision. The range of procedures required will be subject to development and the post-holder will be expected to work within all areas of the department.
Maintain a satisfactory level of competence, provide cover for absence of other staff, and assist in the training of other staff and visitors.
Undertake clerical and other support duties when required.
The post holder will report to the Laboratory's Section Heads and will be subject to the supervision and direction of the scientific staff. The post holder will be expected to maintain a high level of accuracy and reliability, and collaborate with other members of the laboratory and other departments to achieve the maximum efficiency and quality of the service provided.
The post holder will be expected to undertake an appropriate share of the department's general diagnostic workload, and may be required to work in different Sections of the laboratory according to the needs of the department. They will maintain long periods of a high degree of analytical accuracy during microscopy.

We have an opportunity for a patient focused individual to join our Medical Records and Release of Information Service working at both our Chelsea and Sutton sites. The Out of Hours District Nursing Service cover a large geographical area, which covers Wrexham and Flintshire and other border locations. The team are based in Wrexham. The service is a 7 - day service working between 18.00 - 08.00 each day.
The aim of the service is to provide a timely respond to patients, families and other healthcare professionals to avoid unnecessary hospital admissions, provide advice and support when other help is unavailable.

An exciting opportunity has arisen for an experienced HCSW to join the home therapies peritoneal dialysis team. This progressive, passionate team strive to support patients to self manage their peritoneal dialysis treatment at home. The role will involve management of patients attending clinics in settings, as well as home visits across Dorset and Somerset. This role would suit someone who is dedicated to providing truly holistic care to their patients.

Main duties of the job
Teaching patients to independently manage their peritoneal dialysis across the geographical area of Dorset and Somerset.

Be comfortable working as a team, but also be able to manage time effectively when working independently.
Driving licence and confidence in driving across the geographical caseload patch is essential, as involves home visits.

Main duties of the job
As part of the Healthy Child Programme, Specialist Community Public Health Nurses and Family Public Health Nurses work within a defined locality or within our Single Point of Access, assessing need, delegating work within the team, implementing planned programmes of care and evaluating care delivered.
The role is to provide a child centred, inclusive, holistic and accessible Public Health programme to children, their families and communities, establishing links with the Early Childhood and Family Service, Early Years Settings, Early Help, other Health providers, communities and agencies (Education, School Services, Health and Voluntary organisations).
To promote the physical and emotional well-being of children, and young people, supporting them to establish good health now and in the future. To promote early intervention, health promotion, advocacy, empowerment, inclusion and the reduction of health inequalities to ensure children are supported to reach their full potential.

Job summary
The post holder will work in the Islington Urgent Community Response Team (IUCRT) as part of the multi-disciplinary team working across the Rapid Response and Hospital at Home pathways, supporting hospital discharges and admission avoidance. The post holder will primarily be working in Islington borough but may be required to see patients in other NCL boroughs. Key partners are Whittington Health and North Middlesex University Hospital Emergency Departments, assessment units, acute wards, community teams including district nurses and long term condition teams, London Ambulance Service and GP's within the Boroughs of Islington and Haringey
Main duties of the job
Provide specialist, high quality, expert nursing/paramedic care and support for patients who are suitable for Urgent Community Response (Rapid Response and Hospital at Home) care in liaison with the multidisciplinary teams (MDT) through assessment and care planning.
- Initiate agreed range of diagnostic tests.
- Refer to other disciplines/professions when required.
- Support patients to self-manage, monitor their condition and make informed choices.
- Act as a named nurse for designated patients within the service
- Ensure that nursing documentation is maintained to required standards and to be a competent user of and participate in the ongoing development of the Trust's planned Electronic Patient Record
- Promote the development of evidenced based practice.
- Participate in clinical supervision to agreed standard.
- Ensure high standards of care are given to all patients.
- Advise patients on the promotion of health and the prevention of illness.
- Be responsible for gaining intravenous access and administration of prescribed medication for patients requiring short-term antibiotics and for monitoring their care.
- Collaborate with medical colleagues in ensuring that appropriate investigations and procedures are performed to complete the patients' treatments and that patient and their families are fully informed and understand the purpose for these.
