Betsi Cadwaladr University Health Board has an opportunity for an enthusiastic and innovative Community Nursery Nurse to join our Health Visiting Service in Wrexham, North Wales . We are looking for a self-motivated team player who has excellent communication, organisational and interpersonal skills.
Main duties of the job
The post holder will facilitate one to one, group and community work that aims to address Health inequalities in partnership with the wider multi-skilled team. He/she will take a key role in delivering parent support, health promotion and programmes of care. He/she will provide a service to families as identified in the needs assessment carried out by the Health Visitor and undertake planned packages of support through implementing and evaluating planned programmes of care. In addition, the post holder will contribute to the public health role in line with government public health initiatives.
The Health Board offers an Induction Course and ongoing Clinical Supervision and support from its Safeguarding Children Team and opportunities for in house training.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

Job summary
The Care Plus Group Patient and Events Medical Services has two discrete elements.
The safe transportation of medical well patients for the sole purpose of transporting individuals for discharge from hospital, or from community facilities for example, care home or home to hospice or attendance for Out-Patients Appointment. Provides a community response provision to falls that are criteria levels 1 and 2.
The second element is to provide event management services providing both First Aid support or Medical services for a range of events, including festivals, concerts sporting event and other community events.
We have a full team with a vast range of experience in successfully delivering medical cover at events across the country and our team ranges from First Responders up to Doctors and Practitioners.
The service also supports with the delivery of training in relation to first response emergency care for internal and external participants.
Main duties of the job

An opportunity has arisen to join CPG in the role of Patient Transport Driver/First Responder for our Event and Patient Transport service.
You will:
Be responsible for the safe transportation of patients/ service users between designated sites including Hospital, Hospice, Home, Care home, ensuring all Health and Safety regulations and Road Traffic legislation are adhered to in relation to transportation and vehicle safety including using advanced driving skills under blue light for emergency response.
- Attend accidents and sudden illness as first responder providing initial first aid and lifesaving treatment and to assist a qualified practitioner in the on-going management of such incident this includes moving and handling of individuals on to stretchers and within wheelchairs.
In return you will be part of a developing service in line with the wider emergency and urgent care agenda, have access to training and a flexible approach to work. You will receive ongoing support and development.

Cygnet Hospital Bierley is a 62-bed hospital in Bradford, providing a range of acute and low secure mental health services for men and women across four wards. The hospital works within a recovery-orientated and trauma-informed model of care. Its multidisciplinary teams support individuals experiencing acute, medium-term and longer-term mental health needs, including people with complex presentations and behaviours that may challenge.
The purpose-built hospital offers a range of therapeutic and social facilities, including an activities of daily living kitchen, gym, IT classroom, sensory room, multipurpose art room and enclosed outdoor courtyards.
Bronte Ward is a 15-bed low secure service providing assessment, treatment and rehabilitation for women with complex and enduring mental health needs, some of whom may have a dual diagnosis or a history of involvement with the criminal justice system.
The service supports women who may present with risks including aggression or self-harm and can also support individuals with a secondary diagnosis of personality disorder.
Care is recovery-focused, collaborative and person-centred. The team works with each woman to understand and reduce risk while developing the skills, confidence and occupational identity required to progress towards greater independence and the least restrictive environment.

Job summary
An exciting opportunity has arisen for suitably qualified and experienced candidates who are registered with the NMC within our new Specialist Outreach Psychosis Service within the Borough of Ealing. Ealing is mobilising these new services bringing together the existing Community Rehabilitation teams alongside new investment to develop an assertive outreach pathway for service users with complex psychosis who need an assertive approach to engagement as part of their clinical presentation to provide care, treatment and support.
Successful candidates will show knowledge and experience of the clinical processes and treatments that support the care of people experiencing psychosis with complex needs. They will also demonstrate experience of working within a multi-disciplinary team setting; taking responsibility for assessing need and setting up robust care plans to support service user recovery. They will be able to take a flexible, robust and persistent approach to working alongside service users and their support networks to deliver care and get the greatest benefit from treatment.
Experience of working with complex mental health needs and high risk clients is essential as well as strong skills in risk assessment, care planning and crisis management. Experience of community based and outreach working is also essential.
Main duties of the job
- To provide comprehensive, culturally appropriate, evidence-based assessments to establish client's needs; level of support and type of interventions required to facilitate their engagement and recovery.
- To manage a small caseload of service users with complex, severe and enduring mental health needs; using evidence based and recovery principles to assess, plan, implement and evaluate treatment and support for clients in our Boroughs.
- To act as a named worker for service users within the context of the Care Programme Approach within core hours of Monday to Friday 9-5, but with some flexibility as required by the needs of the service users within the caseload.
- Undertake assessment and activities within a multi-disciplinary framework, in order to identify the needs of service users, help them to access relevant support and treatment and to arrange and provide appropriate packages of care within the available budget.
- To present cases to the relevant placement panels for in-borough and out-of-borough placements, both for initial funding requests and reviews.
- To work proactively with service providers to draw up resettlement plans and carry out regular reviews for clients to assess their progress towards identified goals.

An exciting opportunity has arisen for existing physiotherapy staff and new physiotherapy graduates to join our innovative and pro-active Band 5 rotational scheme team rotating across the four sites of the H.G.S. component of the UHB Foundation Trust
The successful candidate will progress through the core specialities of MSK, Neurology and respiratory rotations, working at Good Hope, Heartlands, Solihull Acute and Solihull Community Services and will spend four months in each location consolidating their knowledge and skills working under the supervision of mentors.
The Trust is supportive of staff development and well-being, with teaching and mentoring and 1:1 supervisions being provided across all areas and teams. Additional preceptorship training and support is available for all new registrants. There is the opportunity for peer support and band 5 champions have a direct link to therapy management.
A flexible and organised approach to working is required as Therapy services are committed to seven day working and on call and some shift patterns are based on extended hours.

The service is a regional specialised mental health service for deaf people from 18 years onwards. This is a tertiary mental health service for Deaf and Deafblind people 18 years and older, who have a range of mental health issues. Our target group are British Sign Language users, but we do support other deaf non-BSL users. The service works in partnership with local secondary care mental health teams, offering specialist assessment, treatment, and advice. The secondary care team retain care coordination/lead professional responsibility and we work in close partnership in order to deliver optimal care in a culturally appropriate manner.
We are looking for a dedicated, enthusiastic individual, with specialist clinical experience. The post holder will be able to work as part of a multi-disciplinary team and have the skills and knowledge to develop the delivery of Speech and Language Therapy within this service whilst providing excellent, high-quality input.
Main duties of the job
The post holder will work with a small caseload to ensure that communication and emotional literacy needs have been thoroughly assessed and that clients have a comprehensive care plan to meet their individual needs. The aim of the interventions will be to support the client's independence in their ability to manage their mental health and to also support them/their secondary care team to communicate effectively so that they can deliver optimised care. This may involve therapeutic intervention to assist in skill and knowledge building regarding communicating their mental health needs or working with the secondary care team to adapt care plans.
The post holder will need to be confident in providing high quality, evidence-based assessments and interventions and have an understanding and experience of working with people with a range of complex issues from a variety of settings and environments. Liaison with community teams, other organisations, families and carers will be a significant part of this role.
The post holder will possess excellent communication and organisational skills and the ability to work independently. The post holder will support the continued professional development of the team and advise specifically around communication and emotional literacy.
Regular support and supervision will be available and professional supervision will be provided by a speech and language therapist.

Main duties of the job
Individual to support the ward with serving meals and drinks to patients on a busy ward environment throughout the day and supporting staff to monitor nutrition and hydration. Individual will be responsible to cleaning and monitor kitchen areas on the ward and and making sure fridge temperatures are taken and recorded accurately. Supporting ward staff to maintain patient safety; ensuring stock ordering and replenishment.
