Job summary
You'll support patients receiving community-based care, helping them maintain wellbeing and independence through safe, compassionate nursing support. Based within Integrated Community Services, you'll work closely with colleagues across the wider healthcare team to deliver high-quality care and positive outcomes for patients. This role is ideal for a registered nurse who enjoys building relationships, making clinical decisions, and supporting others to develop. You'll have the opportunity to work both independently and as part of a team, contributing to a caring environment where patient safety, dignity and respect are at the heart of everything you do
Main duties of the job
What you'll do:
- Assess, plan, deliver and evaluate nursing care for a group of patients
- Provide safe, person-centred care and respond to patients' changing needs
- Support and guide junior staff, learners and students during their development
- Maintain accurate patient records and contribute to clinical governance activities
- Help coordinate care and support the running of the service during a shift when required
- Registered Nurse qualification (RGN/RN or equivalent)
- Recent experience working in a nursing role
- Strong communication and teamwork skills
- Ability to work independently and make sound clinical decisions
- Commitment to learning, development and delivering evidence-based care

Main duties of the job
MAIN DUTIES AND RESPONSIBILITIES
1. Promote the mental, physical and emotional and social health of individuals, families and groups.
2. Provide a high standard of care to individuals and families.
3. Be aware of the needs of patients and their relatives and have knowledge of statutory and voluntaryservices.
4. Be an effective member of a Nursing Team working within the community directorate .

Job summary
We are pleased to announce, that due to new investment, we are offering an exciting opportunity for a Band 6 Speech and Language Therapist to join our team within NELFT. We are looking for an enthusiastic, innovative and motivated individual to join our friendly department as part of our band 6 rotation scheme on a permanent basis. Rotations include H&N Cancer, Inpatient Rehab, Stroke and Neurology, Voice and Frailty. The post holder will work within the Adult Speech and Language Therapy team and have the opportunity to provide an effective service to patients across NELFT community sites, including the London boroughs of Barking & Dagenham, Havering and Redbridge.
Main duties of the job
The successful candidate will have a great opportunity to gain new skills and experience, and work in an exciting and dynamic team. They will be working with adults experiencing a range of acquired disorders of communication and/or swallowing. They will work closely with other MDT staff to ensure holistic patient care. The ideal candidate must be able to work independently as well as part of a team and have good time management and prioritisation skills. Adult Speech and Language Therapy are a respected and integrated part of the multidisciplinary team, with a positive record of developing innovative and patient centred services. The service runs weekly community videofluoroscopy and FEES clinics, and a monthly joint voice/swallowing clinic with ENT. The team pioneered a student supported Intensive Comprehensive Aphasia Programme and won a national Advancing Healthcare Award for their development of community laryngectomy care. The team is supporting clinical research projects, including the HARP aphasia study, and collaborating with local system partners to improve communication accessibility of services. CPD is actively supported through regular clinical supervision with senior speech and language therapists, appraisals and excellent training opportunities. Main duties of the job
- Provide practical support, guidance and signposting to families and professionals who have concerns about a child or young person's potential neurodivergence.
- Act as a key link between families, schools and partner agencies, helping to coordinate support and build positive, collaborative relationships.
- Support schools to strengthen inclusive practice, improve attendance and reduce barriers to learning for neurodivergent pupils.
- Contribute to multidisciplinary working, including support planning and participation in MDT reviews as this aspect of the service develops.

Job summary
A new and exciting opportunity has arisen for Band 4 Senior Support Workers to join the Intensive Support Function Team for Children and Young People (CYP, ISF).
The successful post holder will work with CYP and families who require intensive support to help support neurodiversity, learning disabilities, challenging behaviours and other health needs.
We are looking for a post holder who is passionate about neurodiversity and learning disabilities, keen to make a difference, highly motivated, enthusiastic and have experience of working with children and young people who are complex in presentation.
The successful candidate will be offered a breadth of opportunities to develop your skills and all additional training will be provided.
When joining the team, you will be provided with an induction into CYP services covering all areas of care as well as shadowing clinical staff during your induction period.
You will have regular supervision and development opportunities to help support your skill set in the area of CYP LD and Neurodiversity.
This position requires you to work 9-5 Monday to Friday office based and some hybrid working. You will be based at our Manor Farm Site in Knowsley, where you will be able to park free of charge. We are looking for enthusiastic, flexible and innovative therapists with a special interest and knowledge of working with children with a range of difficulties, to join the Early Years team. This post will require proficient skills in the assessment and management of pre school children, as well as working with other members of the team to support junior colleges and to continue to develop the service being offered. The appointed therapist will maintain up-to-date knowledge of national guidelines and policies related to service delivery.
Main duties of the job
- To provide a specialist service to pre school children with speech language and communication needs/ Speech disorders/Social communication differences
- To be responsible for providing specialist assessment and diagnosis of children within Early Years settings
- To provide specialist ongoing therapy through individual therapy sessions and groups where appropriate
- To provide advice and training to parents and other professionals around early identification intervention and prevention.

Ealing Adult Speech and Language Therapy Service, are looking for a Band 6 Therapist, to be part of our friendly Community SLT team.
We are a supportive and experienced, specialist community team of a Band 8a, B7's, B6 roles, B5 roles and SLT Assistant's, with a strong commitment to CPD. We offer excellent clinical and professional support and supervision, and access to relevant courses, internally and externally, to support you in developing your knowledge and skills.
The posts will be located within Ealing Community Partners, in the Ealing Adult SLT service based at Clayponds Hospital or at Ealing Day Treatment Centre.
Main duties of the job
You will be managed, supervised and will work closely with SLT Assistants and the wider SLT team, including a Band 8a & 7 team leads.
The successful candidates will ideally have, some knowledge, skill and experience of working with both dysphagia and communication patients as a result of Neurological disorders including stroke, progressive degenerative disorders, care of the elderly and dementia.
The successful candidates will be an active member of the multi-professional team and work to ensure its continued development. Good communication, liaison and time-management skills are required to manage the caseload effectively, in addition to being forward thinking and enthusiastic.

Main duties of the job
We are keen to recruit a confident & compassionate Psychological Therapist with tier 3-4 stepped care therapy expertise to join our team. You will be a clinician with experience of working with complex needs & a real passion for providing support to some of the most vulnerable in society. We want to see someone who will work well with a range of professions, both within the established MDT & also with the wider prison staff, respecting & integrating a range of opinions and approaches to provide the best outcome . A clinical interest in trauma informed approaches & working with personality disorders would be advantageous.

Main duties of the job
- To supervise activities within the Cleanrooms.
- To act as the lead responsible for all preparation activities and staff conduct within the assigned area of supervision.
- Preparation of sterile medicinal products for specific patients in a laminar flow cabinet/isolator/non-sterile extemporaneous preparation area e.g., parenteral nutrition, chemotherapy and non-sterile suspensions, creams, and ointments.
- The above duties involve working in a controlled clean room environment and the preparation and checking of worksheets and labels on the appropriate software (i.e., Chemocare), accurate assembly of raw materials, dosage calculations, accurate measurements using balances, graduated measures, and syringes. These operations require a high degree of manual dexterity, and competencies will be continually assessed.
- Maintain competency to prepare aseptic preparations and to keep up to date with validation processes including the essential training and competencies required to be a named person on the Trusts Intrathecal Chemotherapy register.
- Organise and liaise with the transport department for delivery of medicines off site.

Job summary
The Senior Community Mental Health Practitioner will work as a core member of the Community Mental Health Team to co-ordinate the care of a complex caseload of service users, supervising Band 5 Community Practitioners and Band 3 Community Healthcare Support Workers where appropriate.
Working under the supervision of the Deputy Team Manager/Team Manager the practitioner will formulate care and risk management plans in accordance with identified need and ensure that these are recovery focused and developed in collaboration with the service user and where appropriate their relative and carer.

Main duties of the job
To effectively co-ordinate the care of a complex caseload of service users with severe and enduring mental illness.
To undertake initial assessments where this is clinically appropriate.

To assess risk, develop risk management plans, review and adjust in line with the service user's needs.
To work in collaboration with service users as experts in their own care, promoting the principles of recovery, choice and personalisation, which value and empower the service user.
To use tools which promote user empowerment e.g. Wellness Recovery Action Plans (WRAP), Recovery Star

The Discharge to Assess Team supports people in Care homes/Nursing homes when increasing frailty affects their mental health. Following a Hospital Stay. The team helps prevent avoidable hospital admissions and reduces the need for secondary mental health services.
We work within primary care and closely with Hertfordshire County Council provide early intervention, proactive support, and consistent care. This integrated approach ensures service users receive timely mental and physical health support, helping them stay safely in their preferred place of care working towards effective discharge if appropriate to meet there care needs.
- As a 'wrap around' service you will be linked in with various health and social care professionals to deliver a person centered approach to the service user.
- Candidates must be car driver and have a mode of transport (unless you have a disability as defined by the Equality Act 2010 which prevents you from driving) as a high degree of travel is required. Work will be undertaken in office base, and in care homes around the county.

You must be able to travel to undertake the duties of the post at various locations. WBS fleet vehicles, team minibus and equipment vehicles are provided.We will give you all the technical training you need to perform venepuncture (using needles to take blood), so you don't need a medical background. However, you will need to demonstrate excellent customer service skills and have a friendly and flexible approach which will make donors feel welcome at ease and valued.Work hours are varied and include long days, Bank Holiday and weekend working.There will be a requirement foryou to work in various settings and also on the mobile donation unit.
Main duties of the job
The role will deliver both excellent customer service and direct care to donors throughout the blood donation process. You will be working as part of a multi-skilled team committed to creating a caring environment and excellent customer service experience whilst identifying and meeting the expectation of individual donors so that blood is donated safely and donors are encouraged to return. This will include delivering high standards in customer service, performing venepuncture and driving duties.
Working to agreed clinical and quality standards and procedures to safeguard donor and patient safety.
It will be the requirement of the post holder to develop skills in the areas of venepuncture, donor screening, meeting and greeting, appointment management and reconciliation in addition to core skills.
There will be a requirement for all staff to flex through various teams and blood collection facilities such as blood mobiles, static sites and community clinics covering a large geographical area.

Job summary
We require friendly, positive thinking, organised and motivated individual to work as a Housekeeper within our Older Persons Mental Health Wards here at Gosport War Memorial Hospital.
You will need to be passionate and enthusiastic about patient care putting patients at the heart of everything you do. You will be working in a multi-professional team and will need to be able to think on the spot and be empathetic. You will be passionate about our business, creating a culture of continuous improvement and providing a release of time to care for the Nursing staff.
Experience in cleaning duties and use of chemicals and equipment are essential as you will be working in a demanding environment. Catering and food service experience would be beneficial along with a passion to providing high standards of customer service.
Experience of working or volunteering in a mental health and learning disabilities environment is essential. Also, the ability to provide a positive approach and image, when working with relevant client groups throughout the Trust.
The successful candidate will receive regular supervision and appraisal. A willingness to undertake training relevant to the post is essential.

Main duties of the job
- Support Ward Manager in monitoring standards in service delivery relating to all aspects of housekeeping, catering, and maintenance.
- Oversee the laundry service to the ward, ensuring appropriate levels of clean linen are maintained.
- Liaise with and support the Cleaning Contractor with keeping the ward areas clean to the standard required. Managing and Escalating areas of concern.
- Ensure patients receive support with nutrition and hydration,escalating concerns to Nurse in Charge.
- Ensure the ward is always clean and safe,liaising with Cleaning Contractor.
- Ensure general and specialist equipment are cleaned as per Policy.
- Cleaning of nursing areas and the allocation of bedrooms and necessary movement of property, cleaning of area.
- Ensure equipment is in good working order and report any maintenance issues to necessary department.
- Ensure mattress checks, audits are completed in line with Trust Policy. Monitor rejected linen and ensure necessary process is completed for returns.
- Maintain the safety of patients property and belongings, handling with care and in accordance with Trust Policy.
- Provide support in assisting patients with choice of beverages, nutrition and hydration needs.
- Completion of menu summary sheets ensuring correct information e.g. allergens, special diets is included.
- Receive and welcome visitors.
- Ensure ordering, checking, unpacking of supplies, check stock and rotate as necessary to meet staff and patients needs.
