Job summary
As part of the Integrated Community Stroke Service you will be required to participate in the delivery of Speech and Language Therapy, providing specialist assessments and associated care for patients across a defined geographical area. There may be a need to provide countywide cover on occasion.
This is a community-based role that requires frequent travel to various locations throughout the county, including areas that may not be accessible by public transportation.
The successful applicant will have the skills and competencies to provide assessment and rehabilitation through both face-to-face and remote modalities, using approved digital platforms where clinically appropriate. Although the service would ideally benefit from a clinician able to provide face-to-face care, applications are welcomed from suitably qualified and experienced individuals who are able to contribute through remote assessment and rehabilitation delivery.

Main duties of the job
- Support safe discharges from hospital and provide a high standard rehabilitation service to patients referred to the Integrated Community Stroke Service
- Provide specialist assessment, effective treatment, and individualised goal setting of patients in their own home/ place of residence, to promote recovery post stroke.
- Early identification and management of stroke related complications.
- Reduce readmissions due to complications post stroke.
- Provide education and support to formal and informal carers.
- Promote the development of self management skills following stroke.
- Provide advice and signposting to appropriate services

Job summary
As part of the multidisciplinary West Community Learning Disabilities Health Team, you will be managing a caseload of adults over 18 with learning disabilities, who are referred to the service.
This post is based within the West community team but the service works together with the East community team and will be part of the physiotherapy team across the county. There may be possibility of flexibility on where this post can be located and please speak to the lead physiotherapist about this possibility.
You will provide physiotherapy assessment and interventions using core physiotherapy skills for individuals with a learning disability. This includes monitoring their health outcomes and progress through review of their care plan.
You will have or be supported to develop skills in community based physiotherapy and understand the health needs of people with learning disabilities. You will provide advice and training regarding learning disability physiotherapy issues and interventions. You will have a key role in working with service users and a wide range of health professionals and other partners and agencies to reduce health inequalities and promote access to mainstream health services, enabling people to be as independent and involved as possible.

Main duties of the job
You will carry out specialist assessment of those with complex needs under the supervision of senior staff, including those with complex postures and physical health needs and will also be supervised to deliver specialist interventions such as postural management, aquatic therapy and rebound therapy as well as assessing and prescribing specialist equipment (eg postural management and specialist mobility aids) and providing training in its use.

We are looking for a clinician with a passion for delivering high quality evidenced based care to patients with diabetes and complex foot pathology.
You will play a key role in the prevention, assessment and management of diabetic foot complications working as part of the multidisciplinary team to reduce the risk of infection, ulceration, and amputation. This post will be responsible for community care planning of podiatry patients and will be required to provide cover and input into a multidisciplinary clinic (MDT) as part of the foot protection team. Multidisciplinary working will be an important aspect of this role to ensure best outcomes for the patient. Main duties of the job
The post holder is a key member of the Facilities team. They will authorise annual leave of Porters, complete digital rosters, raise purchase orders, deal with the Portering Service vehicles, Medical Cylinder deliveries and stock checks, etc. as well as deal with daily operational issues. They will also be required to provide support in the development of service improvements and assist with or undertake specific tasks/projects as required.
They will participate in the local induction of new staff members and record accordingly. Act as a mentor for operational staff, providing in-house training and assessing competence where appropriate. Maintain records of training and development.
Undertake staff appraisals for operational staff. Assess any training or development requirements highlighted during the assessments and report to the Facilities Management Team. Assist in all elements of management of staff, e.g. sickness absence meetings, recruitment, Respect & Resolution.
Monitor the conduct of staff under area of responsibility during the course of their duties in line with BCUHB policies and procedures, advising staff when their conduct falls outside of expected standards and escalating if improvements are not achieved. Regularly use computer software to produce rotas, Personal Development Reviews, etc. and to ensure all records are accurate, up to date and comply with the Data Protection Act.

An exciting opportunity has arisen for an experienced and motivated pharmacist to join the Swansea Bay University Health Board Pharmacy Team as a Band 8a Specialist Pharmacist supporting inpatient cancer services at Singleton Hospital.
The successful post holder will deliver clinical pharmacy services to cancer inpatients, providing expert pharmaceutical care for patients receiving systemic anti-cancer therapy (SACT), and other complex treatments. Working as part of the multidisciplinary team, the post holder will undertake medicines reconciliation, clinical review, medicines optimisation, discharge planning, specialist medicines information and support the safe delivery of inpatient cancer treatments.
The Cancer Pharmacy Team is a highly specialised, supportive and innovative service dedicated to delivering safe, effective and patient-centred care. The post holder will also contribute to service development, workforce education, clinical governance and quality improvement.
Applicants must be registered with the GPhC and have substantial post-registration clinical experience. Previous oncology or haematology experience is desirable but not essential, and applications are welcomed from pharmacists with strong clinical skills wishing to develop a specialist career in cancer pharmacy. A comprehensive induction and ongoing development programme will be provided.
Successful candidates will be appointed to the role of Advanced Pharmacist (Band 8a).
Main duties of the job
As a Specialist Cancer Pharmacist, you will play a key role in delivering and developing a high-quality clinical pharmacy service to inpatient haematology and oncology wards. Working as an integral member of the multidisciplinary team, you will provide expert pharmaceutical care to patients with complex conditions, supporting the safe and effective use of high-risk medicines, optimising treatment, and contributing to improved patient outcomes throughout their inpatient stay.
The role will involve undertaking comprehensive medicines optimisation, clinical review, medicines reconciliation, therapeutic monitoring, prescribing where appropriate, and supporting timely discharge planning, including the management of complex discharge prescriptions. You will work closely with medical, nursing and allied health professional colleagues to ensure the highest standards of patient care for individuals receiving systemic anti-cancer therapies and supportive treatments.

Main duties of the job
Working closely with our Clinical Lead for Spinal Cord Injury Speech and Language Therapist, you will play a key role in delivering specialist assessment, diagnosis and intervention for adults with complex communication, swallowing and voice needs.
Your work will span a variety of clinical environments, including:
- Specialist spinal injury services
- High Dependency Unit (HDU)
- General medical wards
- Outpatient voice clinics
- Neurological / general outpatient clinics
- Community settings
In addition, you will contribute to our expanding neuromuscular pathway and be actively involved in the ongoing development of a specialist speech and language therapy service within this well-established and nationally recognised MDT.
This role offers excellent opportunities to develop expertise in managing patients with highly complex presentations while working as part of a supportive and innovative multidisciplinary team.
The current suggested work plan includes; 1 session of outpatient voice clinic/week, 1 session of neuro/general outpatient clinic/week, 4 sessions on the wards across the hospital, 2 sessions within the neuromuscular service and 2 sessions of domiciliary community work. Total of 10 sessions per week or 5 days per week.

We are looking for a qualified nurse with experience in Radiology to be the Lead Radiology Nurse in a progressively expanding service within the Radiology Centre. Previous Radiology/scrub experience is required. Experience in nursing governance and leading a team is beneficial. Experience in any of the following specialities such as high dependency care, acute medicine, surgery, theatre and day investigation are advantageous.

To be responsible for assessing individual patient care needs and developing, implementing and evaluating plans of care for a variety of patients with complex medical histories in accordance with agreed policies and professional guidelines.

Main duties of the job

This is a really exciting time to join UCR as the Wirral CAS launched in October 2025 and accepts lower acuity referrals directly from NWAS to offer an enhanced triage and utilise community services, where appropriate to support patients to remain at home.
The multi-skilled team of therapists, nurses, social care and health care assistants provide urgent assessments, care and treatment for people living in Wirral. People are assessed within two hours and personalised support is in place within two days (usually 24 hours).
The team provide urgent assessments within two hours for patients who experience any of the following:
- Fall (in or outside of their home or place of residence)
- Reduced mobility
- Confusion / delirium
- Worsening of dementia
- Urgent catheter care
- Diabetes deterioration

During these hours the HCA with support the clinical nurse (visiting in twos) in responding to urgent specialist palliative response visits, these visits will aim to take place within a 2-hour timeframe.
The post holder will demonstrate a level of skill and understanding which enables them to undertake specific tasks delegated by the nurse in charge and in accordance with relevant procedures, guidelines and policies. They will undertake both routine and specially identified tasks for which they have been trained and assessed as competent.
Main Duties and Responsibilities
Key areas of work
Within the patients place of residence the HCA will assist the Registered Nurse in the assessment, planning, implementation and evaluation of nursing care.
Carry out high standard of direct patient care alongside team colleagues
Report verbally (including hand-over) to appropriate team members
Document any care given in patient held notes and/or relevant systems e.g. SystmOne
Contribute to multidisciplinary discussions.
Be sensitive to the spiritual, psychological and social needs of the patients and their families.
To supervise and support all nursing assistants and to assist in the induction of all new staff.
Contribute to the learning environment e.g. teaching on the in-house training days and training new members of staff
Undertaken training for clinical skills relevant to post and maintain up-to-date practice.
To take emergency action alongside the registered Nurse as necessary to safeguard the patients and carers and report any concerns to the nurse in charge as soon as possible.
To maintain accurate documentation alongside the registered Nurse of care given and to report any changes in condition or circumstances of the patient to the relevant key worker and the nurse in charge on duty in a timely manner.
To participate in audit activities.
Be aware of and undertake infection control measures to minimise the risks of infection.
To perform administrator duties is required.
Attend mandatory and in service training as identified and required.
At all times to represent the organisation in a professional and caring manner maintaining professional boundaries in relationships with patients and their families or carers.
To be part of a rota to provide a 7-day service, 365 days per year including weekend working.
To work flexibly and willing to travel across the city and organisation.

Identify learning and development needs and produce a personal development plan in conjunction with your line manager / supervisor.
Achieve and maintain competence in mandatory and statutory training associated with the role.
Complete documentation of care in line with the Trust Standards for Documentation.
Following the completion of training undertake a range of physical / clinical observations appropriate to the area of practice. Achieve and maintain competence in using a range of equipment to undertake these, record the outcome and report and escalate exceptions to the registered nurse who delegated the tasks to you.
Measuring and recording may include:
Blood pressure (using electronic equipment)
Pulse rate and rhythm (manually and using electronic equipment)
Temperature (all routes)
Height and weight
Blood glucose monitoring
Urine testing / Urinalysis
Respiratory rate
Oxygen saturation
Accurately recording and documentation of vital signs monitoring if appropriate for the patient.
Simple wound dressings in line with a plan of care and infection prevention and control policy and procedures.
Last offices, alongside the registered Nurse in line with the relevant policies and procedures.
Administration of eye drops/ear drops/nose drops in line with policies and procedures.
Assessment and monitoring of Skin care/pressure area care, alongside the registered Nurse in line with policies and procedures.
Assessment and Promotion of continence and the management of incontinence, alongside the registered Nurse, in line with policies and procedures

Obtain verbal consent from the patient before any clinical intervention is undertaken, where this is not given report this to a registered nurse.
Participate in discussions about patient care and departmental issues at hand over and departmental meetings.
Observe and report to the registered nurse all changes in a patients physical and psychological condition.
Support families and carers following a patient bereavement or when bad news has been shared
