Are you looking for an exciting opportunity to make a positive impact on people's lives through physical rehabilitation when discharged home from hospital and supporting in our In-patient rehabilitation unit? What about an opportunity to support the Ambulance services via an Urgent Care Response referral?
North Bedfordshire is looking for a part time band 6 Occupational Therapist to work within the 7 day service
You need to be responsive, have strong assessment skills with a personalised approach for a wide range of needs/conditions. You will then develop, implement, manage and review the planned recommendations.You will need skills in moving and handling, risk assessing and identifying solutions to reduce risk.
You will work closely alongside all members of the therapy team, as well as the wider multi-disciplinary workforce across Bedfordshire, building close links with professionals and services required to wrap around the needs of each individual to provide a holistic approach.
You will line manage and supervise unqualified staff within the team, supporting them to provide an excellent level of service to the local population. There is also an expectation to support student placements.
All applicants should hold a degree/diploma in Occupational Therapy and be registered with HCPC.

Job summary
The Care Navigator role is a varied and rewarding role, working for the Waterside PCN which incorporates The Red and Green Practice, Forestside Medical Practice and Waterfront and Solent Surgery. As well as working in our Practices you will be visiting patients in their own homes (patients will primarily be over the age of 65).
The post is a supportive role to the health and social care professionals who will take the lead and responsibility for the clinical and social care provided to the patient.
The role of the Care Navigator is pivotal in supporting a self-management approach to care ensuring the patient and carer is at the centre and an active part of the holistic care approach. As part of the Practice teams and wider PCN team, a Care Navigator will work with the voluntary services in the local community and signpost patients to services depending on their needs, liaising with adult services if necessary.
Main duties of the job
To meet with or telephone the patient/carer in a mutually convenient location such as the patients/carers home, hospital, or GP Practice.
To support patients to ensure appropriate referrals are made, identifying clear needs and goals.
Co-ordinate the delivery of care and ensure that the agreed interventions are actioned through onward signposting to the appropriate service.
Explain and help the patient and their carer understand the processes and systems within the NHS and statutory sector. For example, how to refer to the occupational therapy team or adult services for a care needs assessment.
Keep up to date with NHS and community services through proactive networking to ensure individuals are aware.
Devise a strategy with the patient and/or their carer to enable patients to lead more independent lives, reducing their need to engage health and social services.
Act as the coordinator between different agencies involved with the patients/carers to ensure joined up and seamless care.
Enable the patient and their carer to liaise with professionals from secondary and primary care and the wider integrated care team.
Keep up to date well documented notes on the patients medical record ensuring all components reflected on the patient referral are covered.
Attend practice, PCN, and other relevant meetings as required.
Complete all mandatory training and attend any other training opportunities as required.
Participate in 4 monthly check-ins with the Team Leader or Manager.

An opportunity has arisen to join the Pharmacy Homecare Team within the University Hospitals of Derby & Burton (UHDB) Pharmacy High Cost Drugs & Commissioning Team.

Main duties of the job
The candidate will lead, manage and develop the pharmacy operational technical service related to medicines homecare at UHDB. Some of the main duties of the role include:

We are seeking a Pharmacy Dispensing Assistant to join our friendly Pharmacy team at Watford General Hospital.

Working within Surrey and Sussex Integrated Care Board Primary Care Training Hub (primarily for Sussex), the post-holder will act as the point of contact for specific activities and will be responsible for ensuring operational delivery of NHS England Workforce Training and Education requirements and associated primary care workforce programmes and projects across the health and care system.

They will make use of workforce and project data available to understand the needs of the workforce now and in the future to embed recruitment, development and retention strategies that relates to the respective integrated care system (ICS) and Training Hub population health needs.
Main duties of the job
The post-holder will work closely with a range of local stakeholders to support developing the Training Hub agenda within their governance arrangements, enabling existing, and new, workstreams as required by the Surrey & Sussex PCTH programme and activities specific to individual workforce roles.

Main duties of the job
The post is locality team based in Amersham and the Chiltern South Bucks area. If you are a newly qualified therapist you would be given a mainstream school aged caseload for 4 days of your week, alongside 1 day CPD to develop competencies and shadowing for the first year with the aim to develop interests in different parts of the service. Caseload allocations are caped to allow NQTs the ability to acquire necessary competencies within the first year.
This is a jointly commissioned service that will give you the opportunity to use your innovative and dynamic SLT skills to influence the delivery of high quality, evidence based interventions for children and young people in Buckinghamshire.
We use a wide variety of evidence-based therapy approaches with children. Therapy is delivered individually, in groups or flexibly as the child and setting require.

An opportunity has arisen for a Senor Mental Health Nurse to join our multi disciplinary team.
The Crisis Response Team provide interventions to young people at the point of mental health crisis.
The team provide a 24/7 service which allows for flexibility from a 9-5 working pattern as a Senior Mental Health Practitioner, the post holder will provide high standards of evidence based client centred care to children and young people with mental health [CYPMH] problems within the THRIVE Framework for service delivery.

Job summary
The post holder works under indirect supervision of the local anticoagulation lead and is supported by the whole of the clinical and non-clinical anticoagulation team to meet the needs of patients enrolled within the IntraHealth Anticoagulation service
The post holder will:
Provide efficient and safe near patient testing for the anticoagulation service by measuring INRs with point of care equipment
Ensure maintenance of anticoagulation kit and equipment
Be a physical point of contact for patients using the service who will either visit a primary care venue or be visited in their own home
Support and work collaboratively with the AMS clinicians (nurses, pharmacy technicians and pharmacists) and admin team by ensuring all documentation is up to date after each patient encounter. AMS clinicians will use the data generated by the AMS HCA point of care INR testing to provide the other clinical aspects of the service including:
Support to patients, relatives, and other health care professionals involved in the patients care.
Therapeutic adjustments and titration of oral anticoagulants within the Anticoagulant Protocol to ensure patients are kept within their target INR limits.

The post holders will promote independence utilising rehabilitation and enablement interventions, working collaboratively with a vast range of key health and social care teams and services within the community, as well as voluntary and public sector providers, to ensure the successful delivery of the Home Pathway Service in Bromley.

This is an exciting opportunity to develop specialist knowledge in medicines procurement while contributing to the delivery of high-quality pharmacy services across one of the largest NHS Trusts in the country.
As a Pharmacy Technician within the Procurement Team, you will play a key role in supporting the timely and cost-effective supply of medicines across MFT's four hospital sites. Working collaboratively with Pharmacy Technicians, Assistant Technical Officers, Pharmacists, and wider multidisciplinary teams, you will help ensure that patients receive the medicines they need, when they need them. Main duties of the job
Key duties include:
- Undertaking daily medicines ordering activities for North Manchester General Hospital.
- Conducting risk assessments for unlicensed medicines to support the safe and timely procurement of medicines for patient care.
- Supporting the maintenance and updating of national, regional, and local medicines contracts within the pharmacy procurement system.
- Assisting with the management and coordination of medicines recalls and drug alerts.
- Ensuring that off-contract purchasing claims are identified, monitored, and processed in a timely manner.
- Supporting the department's daily stock-taking and stock management processes.

An exciting position has arisen within our Lichfield Mental Health Support Team. We are looking for an experienced & enthusiastic Education Mental Health Practitioner.
Youwill continue to build on the fantastic work carried out within your allocated schools. The role is varied & no two days are the same, you may be setting up and delivering group work & assemblies one day to delivering assessment & interventions the next, as well assupportingschools to develop their whole school approaches to mental health & wellbeing.
You will be skilled & experienced in conducting mental health assessments of children and young people (CYP) presenting with common mental health difficulties such as anxiety and low mood. You will conduct risk assessments & identify the right level of support for CYP you come into contact with.
Alongside the clinical aspect of your role, you will play an integral part in supporting schools with their whole school approaches. There is scope for creativity and opportunities to work alongside CYP & parents/carers to drive the whole school approach agenda.
This is an opportunity to be involved in an established team within the wider CAMHS structure, as a joint initiative between health & education. You will work across the Staffordshire locality as well as supporting other MHSTs across South Staffordshire. You will work deliver parent, staff well-being, 1:1 & group work sessions.
Main duties of the job
MPFT will offer a welcome bonus of 5% of first year salary- at 50% on commencement of employment and 50% following 12 months employment.
- Assess and deliver outcome focused, evidence-based interventions in educational settings for children and young people experiencing mild to moderate mental health difficulties, working at all times in collaboration with and giving respect to the education function of the setting in which the post-holder is deployed.

We are seeking a passionate and dedicated Assistant Therapy Practitioner to join the Home First in the North of Staffordshire. You will be responsible for delivering high quality rehabilitation to patients with a range of physical health conditions including MSK, Stroke, Parkinson's disease, Neurological conditions, Spinal Injuries, Cancer, Post Surgery, General Frailty, Amputees, plus a wide range of other conditions which provides you with a varied and exciting experience within our service.

We are offering a rare opportunity to join our well-established Lighthouse MMHS in Shropshire Telford and Wrekin. We are a Psychology led, values driven team, providing a service for families (including partners) who have experienced birth trauma, pregnancy/neonatal loss, or moderate to severe distress related to fear of pregnancy/childbirth. The team hold a significant amount of expertise, with our clinical lead and several clinicians having also been involved in delivery of the Maternity Review Psychology Service for families who were part of the Ockenden Review.
We are looking for a compassionate, family focused Psychologist with a keen interest in maternal mental health, to work collaboratively with colleagues in providing a service to families experiencing moderate to severe distress related to birth trauma, loss, or fear of pregnancy/childbirth.
The post holder will work closely with specialist midwives, consultants and other mental health services, to offer a holistic service for families
Main duties of the job
- Provision of a Psychology service within the Lighthouse Maternal Mental Health Shropshire, Telford & Wrekin, including complex mental health needs. This is to involve providing specialist psychological assessments, leading to psychological formulations of distress; interpretation & opinion; and forming judgements to inform service provision & providing necessary psychological interventions.
- The post holder will work autonomously within professional guidelines and the overall framework of the service's policies and procedures. In so doing, they will be responsible for adhering to relevant Trust policies and professional ethics & standards. The post holder will be accountable for both professional & clinical actions.
- As a senior clinician, the post holder will develop a field of expertise that is a basis for providing advice to the service within which they work and will contribute to service and project development across the wider Trust area. This to include leading or contributing to training, supervision and governance aspects of the specific models of service delivery.
- The post holder will supervise and support the psychological assessment and therapy provided by more junior Psychologists within Lighthouse and provide peer supervisory support to colleagues as appropriate.

An opportunity has arisen for a Band 6 MRI Radiographer based at West Berkshire Community Hospital (WBCH) in Newbury, part of the Royal Berkshire NHS Foundation Trust. WBCH is a Community Diagnostic Centre (CDC) and forms part of our growing cross-site MRI service.
Due to the expansion of our MRI service, we have vacancies for enthusiastic Band 6 Diagnostic Radiographers with demonstrable post-registration MRI experience. Main duties of the job
