Specific duties and responsibilities include:
The assessment of adopters and the matching and placements of children for adoption:
Undertaking assessments of prospective adopters and presenting assessments to the Adoption Panel
Provide support to prospective adopters during and after approval, including through the LAC reviewing process.
Contributing to the development and training of adopters.
Assessment of adoption support needs giving due consideration to the wishes and opinions of the child and the adoptive family, the nature of the problem that has called for adoption support:
To draw up support plan and implement, with the child and adoptive family, an appropriate action plan to come to terms with or ameliorate the identified problems.
To convene and chair case specific and other liaison/multiagency meetings as appropriate.
All children in need procedures are completed in a timely manner.

Providing an efficient homefinding service that meets the needs of children needing adoptive placements within prescribed timescales:
To provide consultation to Social workers who are placing children for adoption
To work jointly with social workers to assess appropriate matches for children needing adoptive placements.
To support and promote the development of a dedicated adoption team identity and shared objectives within a changing and developing environment which include:
To contribute to the timeliness of the Prospective Adopter Reports (PARs), Adoption Support Assessments and Non Agency Adoptions
The team continues to provide and develop the adoption support core offer as outlined in the adoption passport
Looked after children and adoption regulations requirements are completed within timescales.
Provision of birth records counseling and supporting adopted adults to access birth records.
Participate in the teams duty rota
Model and provide good equalities practice. Value diversity within the team and in contact with other professionals, children, young people and families.
Operate within the County policy and regularly updates him/herself with social work methods, new legislation and procedures, including Childrens Act 2004 and Every Child Matters.

We have an exciting opportunity for two part time Reception Administrators to join our reception team within our Gloucester Hub at Pullman Place. There are two permanent positions available. Both positions are part-time, 24 hours per week. (8.30am to 5pm). The working pattern is either Monday, Tues & Wednesday, or Monday, Thurs & Friday.
The Gloucester Hub accommodates approximately 270 staff, with 19 assessment rooms, 3 clinics, 6 large meeting rooms and over 18 teams working together.

We are looking for a motivated clinician to join our South Gloucestershire CAMHS locality team based at Kingswood Hub and Patchway Hubs (ideally at least one day will be based in Patchway). Main duties of the job
The role will include taking minutes at the team meeting, processing referrals and completing administration tasks for the Team Manager and Clinicians. You will be completing administration tasks on RiO, our patient computer system, as well as using other systems for example E-Roster, ESR and Centros Integra, training will be offered.

We are looking for a High Intensity CBT Therapist to join our Young Mind's Matter team.
This role will encompass:
Delivering evidence-based intervention for children and young people in education and community settings with moderate mental health problems where it is felt that a multidisciplinary team approach is not required.

The Physiotherapy Teamconsists of practitioners with specialist experience of working with Adults with a Learning Disability whose needs are unable to be met by mainstream services with reasonable adjustments. The team supports individuals, families and carers to understand an individual's physiotherapy needs and identify person centred treatment plans to be implemented into an individual physiotherapy plan.
Service hours: Monday to Friday 9am to 5pm

Job summary
SPA is looking for a Community Specialist Practitioner who shares its vision to progress remote clinical decision making within the community and provide a highly effective Urgent Community Response.
SPA have been delivering a more effective urgent response, promoting, and championing a "Home is Best" ethos. We are working closely with system partners to deliver new ways of working such as Frailty-ACE and direct pathway from SWAST to Sirona. This role provides a key component to these services and will work onsite with Brisdoc at Osprey Court.
We are utilising our Community Specialist Practitioners to provide remote clinical decision making and using their skills to best support patients. We are looking for a partner to support us in continuing to change the way we work to meet new demands and within this role there will be opportunities to develop your leadership skills, share your knowledge and passion for community practice, whilst also having opportunity to help shape and develop the service and your role within.
Main duties of the job
- Provide remote urgent assessment, treatment, and diagnosis of clinically deteriorating service users.
- Triage and evaluate clinical information from examination and history taking and initiate appropriate treatment and/or referral.
- Signpost appropriately to other services in the identified healthcare pathway, such as specialist services.
- Provide a service that focuses on achieving and demonstrating overall improvements in health care for people who are complex and frail, with long term conditions who may present with urgent care needs.
- Provide robust clinical leadership to ensure high quality harm free care.
- To promote and champion use of digital technology in remote clinical assessment and decision making.
- Work collaboratively with the other CSPs
- Be responsible for the provision of relevant clinical training within the team which may include;
- Supporting team performance in key identified areas including frailty and the assessment and management of long term conditions.
- Support the development of advanced clinical practice within the team through the provision of education and clinical supervision.
- Facilitate local and organisational audits, clinical investigations and RCAs relevant to clinical practice.
- Work as an autonomous practitioner, actively using and promoting shared decision making and empowerment of service users, ensuring they have comprehensive, individualised care plans.
