Job summary
We are looking to recruit an enthusiastic Health Care Assistant who has a genuine interested in providing safe and effective care for patients at home. The successful candidate must have demonstratable experience, a wide range of clinical skills and will be confident to work without direct supervision. Community nursing is evolving at a fast pace, and you must be able to engage positively with service improvements and change.
If you are looking for flexibility with your working hours to support your work life balance, all flexible working requests will be considered in line with the needs of the service.
Main duties of the job
The role requires the staff member to work as part of a Community Nursing Service to deliver high-quality care to patients in their own homes in accordance with individual care plans. Provide personal and palliative care, ensuring patients are treated with dignity, respect, and compassion while supporting their comfort and wellbeing. Demonstrate the ability to work independently and autonomously within the community setting, effectively manage responsibilities while maintaining a high standard of care. Support the Community Nursing Team by managing the ordering and organisation of clinical supplies and stationery to ensure services operated smoothly. Consistently follow Trust policies, procedures, and professional standards in all aspects of work while maintaining strict adherence to health and safety regulations, infection prevention, and control guidelines. Ensure patient confidentiality is always maintained and information is handled sensitively and responsibly in line with legal, ethical, and organisational requirements. Contribute to a safe, supportive, and professional care environment for patients receiving community-based nursing services.

We are looking for a caring, compassionate and self-motivated individual to join our expanding team and work across our Hospital to Home and Discharge Lounge Transport schemes, where you will make a real difference to the lives of people in our community.
Main duties of the job
You will play a pivotal role in providing excellent support to older people who need it. You will have a flexible approach and be able to adapt to changing circumstances as they arise, as the role may involve working in any of the schemes delivered by SCCCC, described in more detail below.

This service is based in the Discharge Lounge at the Northern General Hospital. It provides transport for patients who do not need ambulance transport but require more support than a taxi can provide. We offer wheelchair-accessible transport and, once home, take time to make sure that the patient is settled and has everything they need.
People who may benefit from the schemes are
Older and vulnerable people who may be frail or socially isolated, have no relatives, live alone or with older carers, and other vulnerable adults.
Older and vulnerable people who may be at risk of avoidable readmission.
Individuals or health/social care professionals who have concerns about home situations and a person's ability to live independently.
How the Schemes Work
Professionals from health and social care teams contact the office to arrange assistance for people being discharged from hospital or who are at home and at risk of readmission. Most requests are for short-term practical help and include, but are not limited to:
Collecting a patient from hospital and transporting them home safely.
Basic shopping.
Fitting temporary key safes.
Delivering and fitting small pieces of equipment, e.g. bed levers, commodes, pressure-relieving cushions and chair raisers.
Collecting clothes and other personal items from home and taking them to the ward.
Short-term feeding of pets left at home.
Picking up the patients key to provide access for a contractor, e.g. for repairs, adaptations or environmental health work.
Assistance with moving items of furniture to facilitate a hospital bed delivery, or to enable greater mobility and thus avoid admission to hospital.
Referrer-led home assessments.
Escorting to hospital appointments.
Welcoming the individual home.
Supporting the individual to follow up any problems associated with the hospital discharge process, for example, contacting district nurses or doctors as required.
Signposting to appropriate community activities and services.
Low-level support in essential areas such as food preparation, shopping, prescription and pension collection, light household cleaning and washing.
Advice and guidance on a healthy lifestyle and safety around the home.

This team provides the opportunity to further develop work within the field of young people's mental health and wellbeing with a focus on development of whole school approaches to mental health and wellbeing and the development of the think family approaches within the schools environment.
The mental health support teams provide CBT, guided self-help, parenting and whole school approaches but also offers support to young people through partnership working with external agencies.
As well as holding excellent people management skills the post holder will be knowledgeable and experienced in mental health problems and will be expected to carry out in-depth mental health assessments including risk assessments where required. Main duties of the job
The role of the Workforce Medical Rota & Compliance Advisor is an integral part of the Medical Rostering Team in supporting the delivery of timely rota creation and e-Rostering across the medical workforce. The post holder will provide comprehensive support to all key staff involved in rostering including colleagues within medical staffing, clinical and operational staff as required. This role requires an ability to fully engage with the Trust clinical workforce to support the understanding and adaptation to, the effective use of e-Rostering tools. The rostering of medical staff and the creation of compliant effective rotas is imperative, without which the Trust cannot function effectively.

We are seeking to recruit two highly motivated, skilled and compassionateClinical, Counselling or Practitioner Psychologists to join our Community Mental Health Team (CMHT) (non-psychosis pathway) based in North Warwickshire (Rugby and Nuneaton).
You will be joining a friendly and vibrant multidisciplinary team that offers great prospects for developing a good range of skills in both direct and indirect clinical work.
We provide a service to individuals presenting with complex and enduring mental health difficulties such as trauma, mood disorders, and co-morbid physical and other mental health difficulties.
The psychological therapies team in the CMHT has a vibrant skill mix including Clinical and Counselling Psychologists, CBT therapists, Assistant Psychologists and Mental Health and Wellbeing Practitioners who offer a range of interventions including CBT, CBT-PD, EMDR, CAT, DBT-informed work, and CFT.
The successful applicants will hold their own caseload. They will receive clinical supervision by an 8B Clinical Psychologist and will have their own supervisory responsibilities for others in the team.
The successful applicant will be supported to develop their skills in all areas. This would include a move from band 7 to band 8A under a structured development process for those applying for a development post.
Main duties of the job
With the support and supervision of a Principal Clinical Psychologist you will providespecialist psychological assessments, formulations, and evidence based psychological interventions (individual and group) to service users accessing the non-psychosis pathway of the Community Mental Health Team presenting with complex and enduring mental health difficulties.
The role also includes joint work with colleagues in the form of team formulation, staff training, support and consultation around psychological issues and reflective practice.
The department has close links with the local D Clin Course and there is ample opportunity to be involved in the supervision of Trainee Clinical Psychologists. We also have links with local MSc courses and other NHS England funded courses and welcome placement opportunities regularly.
You will also be encouraged to be involved inaudit, service development, evaluation and research activities.

This is a cross county post, covering Coventry and North and South Warwickshire, within the community Older Adults Team working with functional mental illness and dementia
This post supports our Hospital at Home Pathway which is a 7 day a week service working between the hours of 08.00-20.00
The focus of this position is:
Triage care home referrals- completing a full review at point of triage, to identify needs fully, respond to urgent needs quicker and improve service response time in both routine and urgent referrals .
Care Home Liaison- building relationships with care homes across trust ensuring better communication, relationships and improving outcomes for service users who have complex needs
Reducing admission to both General and Mental Health Hospital
Better response time to urgent situations- as an in reach nurse to be able to respond within 1-2 working days for urgent contacts which will lead to better outcomes for both services and service users
Reducing impact on Older Adult Community Mental Health clinicians- as an reach nurse being able to support junior staff Nurses with community care home referrals
Main duties of the job
The main functions of the care home in reach nurse is to support patients within the care home setting to try and prevent placement break and reduction in admission to both general and mental health hospitals
In addition the care home in reach nurse will lead on completing Diadems assessments using the Diadem tool to reduce waiting times for patients within care homes who do not have a diagnosis of dementia and support with an increase diagnostic rates within care homes
The care home in reach nurse will provide a quicker response to patients to improve the service response time in both routine and urgent referrals .
The care home in reach nurse will promote collaborative working with care homes across Coventry and Warwickshire, supporting them to meet the needs of Older Adult patients through enhanced communication, modelling of good practice, skills and knowledge.
The care home in reach nurse will deliver strategies and support to enable residents to remain in the care home through crisis avoidance and stabilisation. Reducing placement breakdown and improved patient flow of dementia/ Older Adult patients to care homes. The role reduces the impact on Older Adult Community Mental Health clinicians by providing support with formulation and care planning.

An exciting opportunity has arisen for a Clinical Nurse Specialist to join our Community Specialist Palliative Care Team serving St Helens and Knowsley.
We are seeking a highly motivated and enthusiastic nurse with advanced, evidence-based palliative care skills, a V300 Independent prescribing qualification, and excellent advanced communication abilities.

Breast Care Nurse - 22.50 hours per week (3 days per week)
The post holder will deliver a high standard of specialist nursing care within the Breast Service and will work closely with the Clinical Nurse Specialist (CNS) to ensure excellence in care for all patients within the specialty. The role will involve both unit-based and clinic-based working.
The post holder will support the CNS by providing professional advice to members of the multidisciplinary team and contributing to the development of staff knowledge and skills. This will help ensure the delivery of high-quality patient care and the achievement of agreed specialty performance targets.
Main duties of the job
The post holder will be based within the unit and department, working closely with staff as a positive role model. They will deliver high-quality clinical care and maintain regular liaison with the Clinical Nurse Specialist (CNS) and Lead Nurse to ensure seamless, coordinated patient care.
The role includes management of a defined patient caseload across both the inpatient unit and outpatient department (OPD).
The post holder will attend and contribute to independent, nurse-led pre-operative assessment clinics and, in line with agreed competencies, breast guidelines, and under the remit of the CNS, will obtain consent from patients undergoing breast surgery. This will include providing expert advice and information on breast reconstruction options and facilitating referrals to Plastic Surgery services where appropriate.
The post holder will actively participate in the MDT discharge programme, ensuring patients are discharged safely and appropriately in accordance with agreed criteria, assessments, and outcome measures.
Close liaison with the wider multidisciplinary team and Oncology Services will be required to support coordinated care throughout the patient pathway.

Main duties of the job

The successful candidate will work in the Main X-ray Department of Torbay Hospital. Academic work is largely via distance learning, though there will be attendance at the University a number of times throughout the 3-year training course. The practical training will initially focus on Plain Film, but will include other areas such as CT and MRI.
As a trainee, you will join 2 existing Apprentices and work alongside Student Radiographers who are on the traditional BSc Degree course, also via Exeter University. Practical training is provided by TSDFT Radiographers who have been enthusiastically and successfully supporting such training for many years.

We require an enthusiastic, organised and flexible individual to support patient scheduling and reception duties within the Imaging Business Unit at University Hospitals of Derby and Burton (UHDB) NHS Foundation Trust.
The Imaging Business Unit is a diverse and exciting department with services delivered across 2 acute hospitals at Royal Derby Hospital and Queens Hospital Burton as well as 7 community sites throughout Derbyshire and East Staffordshire.
Responsibilities of this role will include reception duties, administration and clerical support for the scheduling of patients requiring diagnostic tests across a number of different clinical areas within Imaging, e.g. X-Ray, Ultrasound, CT Scan, MRI Scan, Nuclear Medicine, Bone Density.
There are opportunities for personal development within both admin and clerical and clinical teams within the Imaging Team.
Main duties of the job
Applicants must be confident communicators with the ability to liaise with colleagues and patients. Essential qualities are strong organisational/time management skills with the ability to multi-task and work under pressure. Flexibility is required as the post holders will be required to work across UHDB.
Applicants should have demonstrable experience of working in a busy customer-focussed or care environment or have call handling and reception experience. Applicants must be experienced in using IT systems and/or appointment systems although training will be given.
Core skills/qualifications include: educated to English Language and Mathematics at O' level/GCSE level, IT literacy, good written and verbal communication skills, the ability to work as part of a team and individually, with minimal supervision.
