Job summary
We are redesigning our community services model and implementing a new Intermediate Care Team and therefore an exciting opportunity has arisen to appoint General Therapy Assistant Practitioners to help deliver an urgent community response services and will play a crucial role in developing the service, providing essential support and care to individuals, with a focus on promoting independence, dignity, and well-being.
The objective is to optimise safe and timely discharge from an inpatient stay for patients that need some form of continuing care or prevent admission with interventions that can be managed at home.
This is an exciting opportunity to become part of the Hospital@home therapy team. Working closely with our multidisciplinary team to provide meaningful, high quality care and interventions for our patients with the service aimed at reducing length of stay, improving admission avoidance and reducing avoidable readmissions through partnership working with community based services. Whilst this post is community based, there may be opportunity to work in clinics, where required. You will be able to support the assessment, intervention and discharge planning of people with a range of functional difficulties within the GTAP role. Patient management will include responsibility for the delivery of care including liaison with patients, family, members of the MDT and external agencies.
Main duties of the job
Supporting the registered Occupational Therapists and Physiotherapists within the Intermediate Care Service in the delivery of intervention for patients in their own homes and community.
Carry a delegated caseload of non-complex patients to deliver interventions prescribed by and under supervision of an Occupational Therapist or Physiotherapist.
To prioritise own work and carry out duties, as prescribed by the Occupational Therapists and Physiotherapists and AHP Lead. Will work independently within Occupational Therapy and Physiotherapy standard operating procedures referring to qualified Occupational Therapist and Physiotherapist where necessary.
To work as part of the Intermediate Care Service multidisciplinary team to undertake specific clinical and administrative tasks, liaise with key stakeholders such as Local Authorities and the third sector.
Support the AHP staff by assisting with all modalities of therapeutic care focused towards expediting hospital discharge and maintaining the patient within their own home and the prevention of avoidable hospital admission.
To undertake environmental assessment, deliver and fit assistive equipment in the patient's home.
Participate in service assessment, audit, and data collection to demonstrate effective outcomes of AHP intervention.
Promote independence, health, and wellbeing.

Job summary
A new and exciting opportunity has arisen in the inpatient mental health Speech and Language Therapy (SLT) team in Sheffield. We are delighted to be expanding our small team to include an assistant post.
SLTs working in the mental health team help service users on inpatient wards to manage swallowing difficulties (dysphagia) and communication needs. We support people to understand and express themselves, so they can meaningfully participate in treatment decisions and therapeutic activities on the wards. We support staff training in dysphagia awareness and communication adaptations. As an SLT assistant, you will be supporting our SLTs to deliver a range of interventions which will include directly working with service users and indirect interventions such as developing accessible resources. On the job training will be provided to ensure you are appropriately skilled to deliver interventions within the remit of the role.
The successful candidate will work across all working-age adult wards, including a Psychiatric Intensive Care Unit (PICU), as well as older adult wards and 2 nursing homes. Wards are located over 3 sites (Michael Carlisle Centre, Longley Centre and Grenoside Grange); nursing homes are situated in Chapeltown and Norton.

Job summary
The post holder is to assist the Operations Chief Technician and Senior Technicians in the day to day running of the Sterile Production Unit, the Non-Sterile Manufacturing Department, the Aseptic units and the training of less- experienced staff working in the units. The department is split into four teams: Sterile Batches, Chemotherapy, Non-Sterile and CIVAS. The post holder will be a member of one of these and will rotate between teams as required. It is the post-holder's responsibility to ensure that all products made are manufactured in accordance with the principles of Good Manufacturing Practice, COSHH and Health and Safety Directives.
The post-holder will be expected to deputise for the Senior Technician in each area in his / her absence.
The post-holder is expected to develop and maintain personal expertise in all technical aspects of manufacture and extemporaneous preparation undertaken within the production unit at The Royal Free Hospital, in particular aseptic preparation. To this end, they will be responsible for ensuring their own accreditations are completed and re-validated within the agreed time frames.
Main duties of the job
To responsibly manufacture products in accordance to Standard Operating Procedures and the principles of Good Manufacturing Practice.To liaise with ward staff and pharmacists, on a daily basis, to ascertain their CIVAS and cytotoxic requirements and to prepare doses, as necessary.To be responsible for making sure that the differential pressures in the rooms and on the safety equipment to be used are within the correct limits and facilities and equipment are thoroughly cleaned before and after compounding of preparation. To report to the Chief or Senior Technician if facilities are out of specification and to prevent other staff from using the room or equipment until it is back in specification.To ensure all equipment is cleaned and fit for purpose, and appropriate records made.To accurately calculate the total quantity of each ingredient required to manufacture products.To allot suitable batch numbers and assemble the required drugs and equipment accurately and to check that all ingredients and equipment are fit for use.To carry out a sanitisation process on all drugs and equipment being passed into the "clean" rooms and Grade A facilities, using appropriate agents as specified in Standard Operating Procedures.

Job summary
An exciting opportunity has arisen for a 0.6wte / 3 day per week Band 7 Clinical / Counselling Psychologist or High Intensity CBT Therapist to join our friendly HIV psychology team comprising of clinical psychologists and CBT therapist.
The post-holder will work will support our creative, vibrant multi-disciplinary clinical team (consultants, doctors, nurses, research nurses and pharmacists) to deliver integrated care.
The successful applicant will assist in the provision of offering assessments and interventions to people living with HIV within the Ian Charleson Day Centre, the first open-access HIV clinic in the UK, which is based at Royal Free Hospital.
You will be joining a team of clinical and counselling Psychologists with a particular expertise in health psychology for HIV populations. You will have access to supervision (clinical or cognitive-behavioural supervision).
You will have a clear expression of interest in working within health psychology and a desirable interest in working with people living with, affected by HIV. You will need to be adaptable, confident and able to prioritise a combination of diverse demands on your time and be able to work autonomously.
Main duties of the job
To provide a qualified psychological therapy to patients of the HIV service including psychological assessments and intervention for common mental health problems (depression, anxiety, sleep, stress). The postholder will primarily provide psychological assessment and therapy for issues that fall outside of the HIV condition but may have an effect on the HIV condition and management.
To provide advice and consultation on clients' psychological care to non- psychologist colleagues and working autonomously within professional guidelines and the overall framework of the team's policies and procedures.
The post holder may offer clinical supervision to trainee clinical / counselling psychologists. The postholder will work on-site on Mondays and offer remote telephone and video appointments on the second day.

You will also utilise contextual safeguarding principles, considering the influence of peer groups, communities, education settings, online spaces, and wider networks on a young person's safety and wellbeing. Using professional curiosity and evidence-informed practice, you will assess emerging risks, respond to changing circumstances, and make confident decisions that balance empowerment, independence, and safety.
You will work collaboratively with schools, colleges, health professionals, housing services, and other agencies to create a consistent and joined-up approach that supports the young person holistically.
The role is varied and dynamic, requiring you to use your professional judgement to assess and manage risk, respond to emerging needs, and advocate for young people as they navigate key transitions. You will be a consistent presence in their lives, meeting them where they are and building trusting, supportive, and stable relationships. You will offer encouragement and help them develop the practical, emotional, and social skills they need for a successful transition to adulthood.

What will you be doing?
As a Social Worker within our Leaving Care Team, you will play a pivotal role in supporting young people aged 1618 as they prepare for adulthood. You will undertake comprehensive assessments and take the lead on developing, reviewing, and coordinating Pathway Plans that are forward-looking, aspirational, and firmly grounded in each young persons individual strengths and needs.
A key part of your role will be to champion effective transitions work. You will ensure that young people experience a smooth and well-supported transition from Childrens Services into Adult Services where eligible, building strong working relationships with Adult Social Care teams, health services, education providers, housing partners, and community organisations. Your focus will be on creating a seamless, coordinated approach that removes barriers, anticipates needs early, and supports each young person to move confidently along their pathway to independence.
You will maintain consistent and meaningful contact with the young people you supporthelping them understand their options, navigate key decisions, and access the full range of support available through our Local Offer for Care Leavers. Your case recording will be accurate, timely, and reflective of high-quality social work practice, ensuring clarity of planning and decision-making across all agencies involved.

Job summary
You will work closely with the inpatient wards in the Bay and HBTT to support patients be safely discharged to their own homes or appropriate accommodation. You will work to support patients be discharged from hospital and engage with HBTT. You will be focused on developing community links preventing hospital admission and will ensure the smooth transition for service users between specialist and mainstream services and other teams.
Main duties of the job
You will be a Registered Mental Health Nurse, Approved Social Worker, Occupational Therapist or Dip SW with evidence of professional development. You should have some post qualifying experience and should have experience of working with people who have a severe mental illness within an acute environment. Some experience of working within a community setting may also be beneficial.
We can offer you supervision, training and support. You will be part of a team and be included in the developments moving the service forward.

An exciting opportunity has become available for an experienced cancer pharmacist to lead with support the cancer pharmacy at the Royal Free London NHS Trust with the support of the Consultant Pharmacist-Cancer Role.
This post will be responsible for service based at Barnet Hospital and Chase Farm Hospital.
The successful candidate will lead the team further developing the role of the pharmacist within the clinic setting and providing excellent patient care and outcomes within the cancer speciality. They will also be responsible for a cancer pharmacy team and for delivering the safe and cost-effective use of systemic anti-cancer therapy (SACT)
The ideal candidate will be a highly experienced cancer pharmacist who is looking for the next step up to a leadership role with a proven track record in managing a team and a service. Experience of working in cancer clinics would also be beneficial.
Main duties of the job
To lead and harmonise (with the support of the Consultant Pharmacist-Cancer) the pharmacy cancer services team. Cancer services are provided across all Royal Free London site namely the royal free hospital, Barnet hospital, Finchley Memorial Hospital and Chase farm hospital. This post holder will have specific site responsibilities for Barnet Hospital and Chase Farm Hospitals.
To deliver, develop and evaluate clinical pharmacy services to the oncology, haematology, palliative care specialities in accordance with objectives set by the Clinical Lead Pharmacist, Cancer and the Deputy Chief Pharmacist.
Working closely with the multidisciplinary team, Service Line Leads and Divisional Managers, the post holder will develop initiatives and guidelines to ensure safe and cost-effective use of medicines and chemotherapy in this area, and across the interface between primary and secondary care.
The post holder will be jointly responsible for all aspects of the management of high cost drugs for cancer with the principal pharmacist based at RFH site, and the supply of medication via homecare where appropriate.
The post holder will be a key member of the Chemotherapy Operational Group (COG) jointly with the Principal Pharmacist Cancer Services based at the RFH and with the Consultant Pharmacist.

Job summary
The Radiological Physics and Radiation Safety group provides medical physics services to departments using x-ray radiation, with the aim of ensuring the safe and optimal use of ionising radiation in all diagnostic and interventional radiology procedures. The post holder will contribute to many aspects of the Group's work which includes performance testing of x-ray imaging and associated equipment both on and off-site, data analysis, investigating faults, optimisation, and patient dosimetry.
Thepost holder will also be involved in radiation safety audits and surveys, and risk assessment. The post holder will be involved in computer programming and software development related to the work of the team. They will also liaise closely with the Radiology PACS team and the trust IT team and be directly involved in the routine testing of radiology reporting workstations.
Main duties of the job
The post would suit someone with a physics background who can demonstrate personal initiative, flexibility, team working skills and an enthusiasm for the application of science to the clinical environment. The post holder will be required to liaise closely with the Radiology PACS and trust IT teams and also have the skills to be involved in computer programming and software development related to the work of the team.

Job summary
BHFT is developing a specialist multidisciplinary Assertive Outreach (AO) service, aligned with Community Mental Health Services, to provide safe, compassionate and flexible support for people with severe and enduring mental health needs who may struggle to engage with services.
The service will initially operate from two hubs in Reading and Slough, and we are recruiting two Band 6 nurses to support its rollout.
This role is based within the Reading team and is one of several new specialist posts. The successful candidate will also work across the Trust to help develop a consistent AO approach and promote best practice.
Our approach is centred on:
- Working alongside individuals, families, carers and wider support networks to overcome barriers such as trauma, mistrust, communication needs, co-existing conditions and social challenges.
- Delivering proactive, intensive, holistic and flexible care, meeting people where they are and tailoring support to their needs.
- Promoting fair access to mental health care, particularly for marginalised and underserved communities, while addressing health inequalities.
To achieve this, we work with smaller caseloads than traditional Community Mental Health Services, enabling more intensive support. The post holder will be part of the Reading Assertive Outreach Team and work within the service's operational hours. Care will be delivered primarily in community settings, rather than expecting service users to routinely attend health facilities.
Main duties of the job
- Within BHFT's Assertive Outreach Service, the post holder will provide intensive, flexible and proactive support to people with severe and enduring mental health needs who struggle to engage with traditional services. This Band 6 role offers an opportunity to help shape and develop a new service while delivering high-quality care to service users and those who support them.
- Based in Reading, with occasional cross-cover across Berkshire, the post holder will deliver recovery-focused, evidence-based nursing interventions to adults experiencing significant challenges with engagement.
- Working within a shared caseload, the post holder will build and maintain therapeutic relationships with service users, families and carers, using assertive, flexible and person-centred approaches to improve engagement, reduce relapse and crisis, and promote recovery and independence.
- The role involves completing comprehensive assessments, developing and reviewing personalised care plans, and delivering evidence-based interventions tailored to individual needs.
- As a core member of the Assertive Outreach multidisciplinary team, the post holder will contribute specialist mental health nursing expertise to formulation, care planning and decision-making. They will work collaboratively with colleagues and partner agencies, applying evidence-based practice and reflective working to support stability, recovery and improved quality of life for service users.

Main duties of the job
You will have the opportunity to;
Offer assessments for children, young people and families
create formulations to aid an understanding of the child's needs
utilise evidence based, goal focused and effective interventions which lead to a positive discharge from our service.
We offer individual, family and group interventions which may be time limited or longer-term therapies according to need.

Job summary
We are recruiting relief/bank Approved Mental Health Professionals to support the work of our 24 hour AMHP Service.
This is an opportunity for an experienced and dedicated person to join our countywide Approved Mental Health Professionals (AMHP) service as a Relief AMHP.
The role is only open to those who are already qualified as an AMHP.
As a Relief AMHP joining the 24/7 AMHP Service, you will undertake work on a rota basis, coordinating and completing Mental Health Act assessments as well as responding to urgent social care requests out of hours, in accordance with the relevant legislation and under the guidance of the management team.
These roles will be primarily home based with travel (as required) across the county and to the Lincoln Office. As an organisation we value the wellbeing of our employees and recognise the importance of work/life balance.
Standard shifts are 13 hours long, starting at either 08:00 or 20:00, though there is flexibility for variance in this for relief shifts.

Job summary
The Avenue Surgery is seeking an enthusiastic and forward-thinking clinical pharmacist to join our like minded team. The post holder is a pharmacist, who acts within their professional boundaries, supporting and working alongside the pharmacy team within the practice. In this role they will be supported by the senior GP partner prescribing lead.
The post holder will work as part of a multi-disciplinary team in a patient-facing role. The post holder will take responsibility for areas of chronic disease management within the practice and undertake clinical medication reviews to proactively manage patients with complex polypharmacy.
The post holder will provide primary support to general practice staff with regards to prescription and medication queries. They will help support the repeat prescription system, deal with acute prescription requests, and medicines reconciliation on transfer of care and systems for safer prescribing, providing expertise in clinical medicines advice while address in both public and social care needs of patients in the practice.

Job summary
This is an exciting and rewarding opportunity to help make a real difference to Older Adults in acute Older Adult functional and dementia inpatient care in North Wales.
This senior post is to provide psychological input and oversight in acute care Older Adult psychology services in the in BCUHB Central area.
Core aims will be to provide psychological leadership for the continued development and delivery of a biopsychosocial MDT for acute Older Adult functional and dementia care, MDT interventions within older adult dementia care inpatient settings, and to lead the provision of expert advice, consultation, training and supervision in psychological approaches within these settings with the acute care pathway in mind.
The post holder will provide psychological interventions, behavioural and neuropsychological assessment, staff training, support and supervision within the acute dementia care pathway in Conwy and Denbighshire. The post holder will also be required to provide direct expert psychological clinical input to service users and families.
There is the potential for this post to be combined with the concurrently advertised Adult Mental Health Acute Care Principal Clinical Psychologist posts. Please enquire if interested in this.
Main duties of the job
To be an active and integrated member of the inpatient multidisciplinary mental health team.
To provide a highly specialised clinical psychology service to clients of the sector, providing specialised psychological assessment and therapy to clients, and consultation to staff.
To work autonomously in accordance with The British Psychological Society Code of Conduct, Ethical Principles and Guidelines 2004, and Health and Care Professions Council.
To plan and provide individual, and group interventions, both directly and via the guidance of MDT members.
To carry a specialist clinical psychology caseload of adult clients who present with complex mental health presentations and in an inpatient setting, including offering specialist neuropsychological assessments, formulation, individual and group interventions.
To provide effective clinical leadership by demonstrating and promoting excellence in the provision of highly specialist assessment, formulation, evidence-based intervention and offer advice and consultation in complex clinical cases to other health professionals.
To act as a highly specialist resource to the wider professional community.

We are seeking an experienced Trusts and Foundations Manager to lead the delivery of our trusts and foundations fundraising programme, securing income from charitable trusts, foundations and grant-making organisations to support hospice services, projects and developments
This role is responsible for achieving agreed income targets, developing a sustainable funding pipeline worth up to £2M and ensuring funding opportunities align with the needs of EACH's services and the priorities of funders. The postholder will manage a portfolio of existing and prospective funders, oversee the development of funding applications, proposals and impact reports, and lead the identification, cultivation, stewardship and renewal of funding opportunities
As part of this role, you will line manage, coach and support the Senior Trusts and Foundations Fundraiser, working collaboratively with colleagues across Fundraising, Care and Finance to develop funding opportunities, monitor performance and contribute to the delivery of the wider fundraising strategy

Job summary
We are looking for an experienced Physiotherapy Assistant to join our specialist 7-bed bariatric rehabilitation service at Ribble Court Care Home.
Working alongside our registered Physiotherapist, you will support the delivery of individual rehabilitation programmes designed to improve residents mobility, strength, function and independence, with the aim of enabling them to return home following their period of rehabilitation.
This is a hands-on role working with a small, defined caseload of seven bariatric residents with complex rehabilitation needs.
Main duties of the job
About the role and Responsibilities:
- deliver rehabilitation and exercise programmes under the instruction of the Physiotherapist and in close collaboration with the nursing and care team;
- support residents with mobility, strengthening and functional activity;
- encourage and motivate residents to work towards their individual rehabilitation goals;
- observe, record and report residents progress or changes in presentation;
What are we looking for?
- Previous experience in a physiotherapy, rehabilitation or similar role is essential.
- Experience supporting people with complex mobility and rehabilitation needs.
- Bariatric rehabilitation experience would be advantageous, but is not essential.
- A relevant qualification, such as Sports Therapy, Physiotherapy or Rehabilitation, would be desirable.
- A positive, person-centred approach and the ability to engage and motivate residents who may face physical, psychological or emotional barriers to rehabilitation.
- Good communication, and confidence using digital care records system
