Job summary

We are currently looking to recruit an enthusiastic and motivated Occupational Therapist, to join the Peterborough Community Rehabilitation Team based at Botolph Bridge Community Health Centre, Valley Park Centre, Sugar Way Peterborough PE2 9QB.
The role is suitable for an experienced OT.
This role is based in Peterborough and the surrounding area . You would be working as part of a multidisciplinary team providing high quality rehabilitation to patients with physical disabilities in their home environment.
Main duties of the job
Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence* and have use of a vehicle. You cannot use public transport for this role as this is not a reliable form of transport and will not allow you to meet service needs. Please confirm in your application that you meet the specified criteria.
- We are looking for a self-motivated individual who is able to work as an autonomous practitioner within the Community Rehabilitation team to provide a high standard of care to our patients.
- Your role will include assessing, planning and developing specialist occupational therapist programmes for patients with a wide variety of complex needs.
- You will be responsible for your own caseload, prioritising referrals according to clinical need, signposting appropriately and undertaking assessment of patients with complex and multiple pathologies using, specialist clinical reasoning skills.
- Your role will also include supervision of junior staff and students, overseeing patient intervention and ensuring that Senior Therapy Assistants and Therapy Assistants deliver high quality patient care.

Job summary
We have a unique opportunity for a physical health Therapy Assistant Practitioner to join Intermediate Care Service based in Cambridge. Working alongside our multidisciplinary team, the post holder will play a key role in ensuring that people recover on their discharge from hospital in their own homes or care homes, attaining their goals and progressing towards independence.
The expectation is that you will work autonomously as part of the team and under direction of the qualified therapists. Within the role we offer career development and educational opportunities to build a career within physical rehabilitation services.
We are looking for passionate, supportive, caring individuals, who will be expected to show initiative and be an active part of the team.
Main duties of the job
- To work autonomously under the direction of a registered practitioner within the Intermediate Care Team
- To be responsible for assessing, interpreting, planning and implementing therapy interventions for specific groups of patients
- To use clinical judgement to evaluate patient progress, including the recording of objective and subjective measurements
- To provide advice to patients and carers
- To keep the patient at the centre of care, ensuring patient and carer participation in decision making.
- To plan and prioritise own workload and to provide a high standard of care.
- To work in various community settings, this includes lone working.

Job summary
This is an exciting opportunity to work in the South Yorkshire Service for Deaf People with Mental Health Needs working with BSL users in contact with mental health teams across South Yorkshire.
This part-time post will work alongside secondary mental health teams, Primary Care mental health teams, crisis/home treatment teams and inpatient services to provide specialist assessment and interventions for deaf people who use BSL.
Main duties of the job
The post holder will work in conjunction with secondary and primary care mental health teams (including crisis, Home Treatment teams and inpatient wards) across South Yorkshire (Barnsley, Doncaster, Rotherham and Sheffield) and work into RDaSH, Sheffield Partnership and SWYT NHS Trusts. They will provide specialist mental health assessments which incorporate deaf culture and communication, and provide specialist interventions that meet the deaf person's needs.
They will also have some responsibility for delivering, developing and supporting the equality and inclusion agenda as part of the Deaf mental health service. They will have a good understanding of deaf culture in relation to mental health and/or BSL skills and/or have worked with deaf service users previously in conjunction with the Clinical Nurse Specialist in Mental Health and Deaf People.
The post holder will provide supervision to the Deaf Peer Project Support Worker.

An exciting opportunity has arisen for a Speech and Language Therapist to work in the community CPFT Speech and Language Therapy Team. The post holder will need to be qualified to work independently with patients with dysphagia or have completed a post graduate theory dysphagia course and be working towards completing the relative competencies.

The postholder will be based in the Cambridge/ Huntingdon area of the County but travel across other areas may be required at times.The successful applicant will join a friendly, innovative team of therapists and clinical assistants. The team meet regularly for meetings and the post holder will receive regular line management and clinical supervision.
There may be an opportunity to develop skills in the area of stroke or brain injury or another area of interest to the candidate. We will also aim to secure funding to train the candidate in LSVT.
Main duties of the job
Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence* and have use of a vehicle. The post holder will work independently, providing speech and language therapy input to patients on an outpatient and domiciliary basis and input to the community hospital wards, providing a high standard of quality clinical care.

The team are currently redesigning the pathway for patients with Parkinsons disease and the post holder may be able to participate in this work.

Job summary
We have an exciting opportunity for a Band 5 Higher Assistant Psychologist, to join our enhanced and developing Forensic Psychiatry Community Service.
CPFT are one of six providers of specialist mental health and learning disability care in the East of England, working collaboratively to ensure there is a seamless pathway of care , that is closer to home for the communities that we serve. The Provider Collaborative also works closely with independent sector partners to make improvements to our services and the ways we deliver them.
This enhanced specialist service consists of a multi-disciplinary team, based between Cambridge and Peterborough,. The service works with adults diagnosed with a mental health condition and/or learning disability who present with a risk of significant harm to others, to achieve optimal management of risk and minimise re-offending.
Working closely with the individual, we produce tailored care plans together that is focused around their personal needs and risk. The service adopts a compassionate and trauma informed approach to promote relapse prevention and recovery, so enabling individuals to realise their full potential whilst supporting them to continue to build and live meaningful lives in the community.
Main duties of the job
The post-holder will be supervised to support the development of psychological case formulations and interventions (e.g., CFT, CBT, DBT, Trauma focused therapies, Relapse Prevention) and enhanced risk assessment tools (e.g. HCR20-v 3, SAPROF).
In addition, there are a number of clinically related administration and audit tasks attached to the post, and involvement in service evaluation and research. The post-holder will be required to work independently according to a plan agreed with a qualified psychologist and within the overall framework of the team's policies and procedures.
You will be joining an experienced multidisciplinary team of professionals including psychiatrists, psychologists, nurses, occupational therapists, social workers, support workers and administrators who all share a passion and drive for delivering high quality care and treatment to service users with forensic histories.

Join our renal pharmacy team to support safe and effective medicines management, improve patient care, and ensure continuity of medication supply for renal patients. You'll work closely with multidisciplinary teams, provide training and support to healthcare staff, undertake dispensing duties, and contribute to service development while maintaining the highest professional standards.
You must be an Accredited Checking Pharmacy Technician or working towards it. You will have an enthusiastic approach to education and training, multidisciplinary team working and a passion for frontline patient care. Full training will be provided on medicines reconciliation and other in house procedures.
The working pattern includes a weekly late night until 6:30, 1 in 8 weekends as per the rota and 1 bank holiday per year.
Main duties of the job
As a Renal Pharmacy Technician, you will work closely with the specialist renal pharmacy team to deliver high-quality medicines management services across renal wards, dialysis units, clinics, home therapies and specialist treatment areas. You will support medicines reconciliation, medication supply, patient counselling and compliance with clinical policies, ensuring safe and effective use of medicines for patients with complex renal needs.
The role includes providing ward-based and dispensary services, undertaking accredited final accuracy checking, supporting staff training and development, participating in audits and service improvement projects, and ensuring compliance with governance, quality and CQC standards. You will also collaborate with multidisciplinary teams, respond to patient and staff enquiries, and contribute to the pharmacy rota, including weekends, bank holidays and late-night services.

FCMS has created a home monitoring service for patients with long standing conditions e.g., COPD, Heart Failure, diabetes and hypertension.This will be to remotely monitor patients who are in a Care Home or Semi Housebound to support their GP practice.Patients will initially have an assessment to create a care plan and ensure we have all the information needed for safe admission onto the service.Patients will have access to observation equipment and links to an app to input their readings which will alert a clinician if any problems or readings are out of range.
Main duties of the job
We are looking for forward thinking, motivated clinicians who have a passion for new ways of working whilst providing safe patient care.The clinician will provide support monitoring alerts and may require visiting patients to provide support in their home environment, if necessary, this may be due tech issues or setting up equipment and training when first onboarded.Our aim is to work closely with the local trust, frailty teams, community matrons, heart failure team, GP Practices, Rapid Response, and other teams in the community to provide optimum patient care.

Main duties of the job

Our Pharmacy Team Leader:

We are seeking an experienced Band 5 Occupational Therapist (minimum 18 months post-qualification experience at the point of application) or an existing Band 6 Occupational Therapist to join our expanding service.
Due to the successful growth of occupational therapy within our Prison Integrated Mental Health Teams, we are recruiting a full-time Band 6 Senior Occupational Therapist to work across HMP Full Sutton and HMP Millsike, two neighbouring prisons located on the same site near York. You will work alongside our existing Band 7 Clinical Specialist Occupational Therapist as part of a developing and innovative multidisciplinary team.
The Integrated Mental Health Teams at HMP Full Sutton and HMP Millsike are a partnership between TEWV and Rethink Mental Illness, delivering primary and secondary person-centred, recovery-focused mental health care. As part of a multidisciplinary team, you will provide OT assessment, intervention and care planning for men with complex mental health needs, supporting recovery, wellbeing and successful transitions across custody settings and in preparation for release into the community.
Main duties of the job
The post holder will complete occupational performance assessments and occupational formulations, as part of the wider context of team assessment. In addition, you will use client-centred approaches to co-produce negotiated occupational goals and evaluate these in line with clinical outcome measures.
You will offer clinical advice and support relating to occupational needs with multi-disciplinary professionals and multi-agency partners, contribute to multi-disciplinary team working in line with the service operational policy and Health and Justice Clinical Model and use relevant electronic record systems in the prison setting including Systm One and NOMIS (as required).
The successful postholder will abide by all Trust policies and procedures, be able to respond flexibly and creatively to challenges that arise within the context of a prison setting, contribute to own clinical supervision provided by an experienced Occupational Therapist, line management from the Team Manager, and professional leadership from the OT Lead for Health and Justice Services. You will clinically supervise others where appropriate.

This full-time fixed term post will be based at the Porterbrook Clinic on the same site as the Michael Carlisle Centre in the Nether Edge area of Sheffield. The Sheffield Gender Identity Clinic, also known as the Porterbrook Clinic, is a service for adults who are experiencinggender dysphoria. Our clinic is one of 11 gender identity clinics providing healthcare for adults, commissioned by NHS England.We are a national service and as such, much of our work takes place online via Attend Anywhere NHS platform. Our team is made up of administration staff, medical and nursing staff, and a range of other healthcare professionals, such as psychologists and speech and communication therapists. People who attend our service may be seeking a range of treatment options. We provide support for social transitioning, and medical treatment to decrease gender identity distress.We are proud to provide specialist healthcare to transgender and non-binary people. Main duties of the job
The Assistant Psychologist will support the psychology team and broader service by carrying out psychologically informed work under the supervision of a qualified Clinical Psychologist, including co-facilitating manualised group interventions, 1:1 psychoeducation, assisting with the design and implementation of service development projects (e.g., audits of outcome measures), preparing material for clinical appointments and some clinical admin for the psychology team. The Assistant Psychologist will be an active member of the Multidisciplinary Team.
The Assistant Clinical Psychologist will need to work independently and proactively according to an agreed plan and within the overall framework of the trust's policies and procedures. They would be expected to participate in relevant continuing professional development and attend regular clinical supervision.

Job responsibilities
Clinical
To co-facilitate manualised group intervention(s) and provide 1-1 psychoeducation intervention where appropriate, under the supervision of a Clinical Psychologist.
Where required, to attend and contribute to multi-disciplinary meetings and CPD that supports care delivery.
To have some direct contact with clients who experience significant mental health challenges, concurrent neurodivergence and/or may have experienced abuse or other traumatic events, and where the highest level of interpersonal and communication skills are required.
Teaching, training and supervision
To receive regular management and clinical supervision in accordance with professional practice guidelines.
To develop skills and competencies that assist in the delivery of current duties.
