Post: Ultrasound Sonographer (General & MSK)

Main duties of the job
Main Tasks & Responsibilities
The post-holder will work in the Community setting supporting the team with carrying out agreed tasks:
- To perform and have expertise in a range of highly specialised Non-Obstetric Ultrasound procedures/techniques in line with BMUS guidelines
- To perform scans, provide accurate, reproducible reports. The reports must be completed within the timescales agreed by the Commissioners & Head of Diagnostics.
- To recognise the significance of investigation findings/diagnoses and act on these accordingly
- To liaise closely with the Lead Sonographer & Consultant Radiologist and seek guidance/advice when required.
- Keep up to date with all relevant policies and guidance e.g. NICE
- To contribute accordingly to the maintenance of standards and quality in service development of the Non-Obstetric Ultrasound service provided, including active involvement in the training of related staff, whist also maintaining and further developing ones own professionalism and expert skills and knowledge.
- Participate in the implementation and on-going delivery of the relevant service.
- Adhere to policies and procedures of the company and keep up to date with procedures and changes within the role.
- To build and maintain professional relationships with host practices and secondary care clinicians.

FCMS has created a home monitoring service for patients with long standing conditions e.g., COPD, Heart Failure, diabetes and hypertension.This will be to remotely monitor patients who are in a Care Home or Semi Housebound to support their GP practice.Patients will initially have an assessment to create a care plan and ensure we have all the information needed for safe admission onto the service.Patients will have access to observation equipment and links to an app to input their readings which will alert a clinician if any problems or readings are out of range.
Main duties of the job
We are looking for forward thinking, motivated clinicians who have a passion for new ways of working whilst providing safe patient care.The clinician will provide support monitoring alerts and may require visiting patients to provide support in their home environment, if necessary, this may be due tech issues or setting up equipment and training when first onboarded.Our aim is to work closely with the local trust, frailty teams, community matrons, heart failure team, GP Practices, Rapid Response, and other teams in the community to provide optimum patient care.

Day to Day Duties to include, but not exhaustive:
- To provide remote advice for patients with long standing conditions linked to why they are being remotely monitored by the Luscii
- To support the patient by linking in with their GP Practice

Welcome to FCMS!
Compass Medical Practice is a GP practice for those who are difficult to manage in mainstream practice. Patients are either consulted via telephone/ video link or face to face. When a face-to-face appointment is required, a risk assessment is undertaken to assess what support the clinician requires. Consultations are supported with the use of patient liaison officers and consultations are carried out in our mobile clinic van or in an agreed prebooked venue.
Compass Medical Practice also provides medical cover to temporary patients residing at Thornton House. These are short term patients that are residing at Thornton house for either rehab, rest or recouperation. There is a weekly ward round and multidisciplinary team meeting. Compass Medical practice also offers medical support throughout the week, outside of the ward round, during opening hours.

We are looking for a Health care assistant to join the team on a bank basis, covering all the services within Inclusion Health.
Main duties of the job
Compass Medical Practice is a GP practice for those who are difficult to manage in mainstream practice. Patients are either consulted via telephone/ video link or face to face. When a face-to-face appointment is required, a risk assessment is undertaken to assess what support the clinician requires. Consultations are supported with the use of patient liaison officers and consultations are carried out in our mobile clinic van or in an agreed prebooked venue.
Compass Medical Practice also provides medical cover to temporary patients residing at Thornton House. These are short term patients that are residing at Thornton house for either rehab, rest or recouperation. There is a weekly ward round and multidisciplinary team meeting. Compass Medical practice also offers medical support throughout the week, outside of the ward round, during opening hours.
Our homeless health service is nurse led and is operates on a Monday, Tuesday, Thursday and Friday from the Salvation Army in central Blackpool. The nursing team take a multidisciplinary approach, often working with wider services to meet the needs of a complex patient cohort who are facing multiple disadvantage.
The targeted demand management service offers a wound care service in Fleetwood on weekly basis, and a health care assistant led health check service every two weeks.

To provide a professional and high-quality driving service for the GP Out of Hours Service
To provide a high-quality reception service at the Primary Care Reception
To be able to follow strict procedures and protocols at all times
When necessary, to be able to complete stocktakes of medical equipment
Using a Patient Record system experience of Adastra would be an advantage
To complete routine administration checks when working overnight

Compass Medical Practice is a GP practice for those who are difficult to manage in mainstream practice. Patients are either consulted via telephone/ video link or face to face. When a face-to-face appointment is required, a risk assessment is undertaken to assess what support the clinician requires. Consultations are supported with the use of patient liaison officers and consultations are carried out in our mobile clinic van or in an agreed prebooked venue.
Compass Medical Practice also provides medical cover to temporary patients residing at Thornton House. These are short term patients that are residing at Thornton house for either rehab, rest or recouperation. There is a weekly ward round and multidisciplinary team meeting. Compass Medical practice also offers medical support throughout the week, outside of the ward round, during opening hours.
Our homeless health service is nurse led and is operates on a Monday, Tuesday, Thursday and Friday from the Salvation Army in central Blackpool. The nursing team take a multidisciplinary approach, often working with wider services to meet the needs of a complex patient cohort who are facing multiple disadvantage.
The Targeted demand management (TDM) service supports people across Blackpool to address clinical and non-clinical factors increasing their risk of an unplanned hospitalisation.
Key aims of TDM are to provide clinical and non-clinical interventions that meet the needs of individuals identified in Blackpool with the following respiratory or cardiovascular long-term conditions. The TDM service, through reducing individual risk, will play a role in relieving pressure on acute services, through preventing health crises.

Compass Medical Practice is a GP practice for those who are difficult to manage in mainstream practice. Patients are either consulted via telephone/ video link or face to face. When a face-to-face appointment is required, a risk assessment is undertaken to assess what support the clinician requires. Consultations are supported with the use of patient liaison officers and consultations are carried out in our mobile clinic van or in an agreed prebooked venue.
Compass Medical Practice also provides medical cover to temporary patients residing at Thornton House. These are short term patients that are residing at Thornton house for either rehab, rest or recouperation. There is a weekly ward round and multidisciplinary team meeting. Compass Medical practice also offers medical support throughout the week, outside of the ward round, during opening hours.
Our homeless health service is nurse led and is operates on a Monday, Tuesday, Thursday and Friday from the Salvation Army in central Blackpool. The nursing team take a multidisciplinary approach, often working with wider services to meet the needs of a complex patient cohort who are facing multiple disadvantage.
The Targeted demand management (TDM) service supports people across Blackpool to address clinical and non-clinical factors increasing their risk of an unplanned hospitalisation.
Key aims of TDM are to provide clinical and non-clinical interventions that meet the needs of individuals identified in Blackpool with the following respiratory or cardiovascular long-term conditions. The TDM service, through reducing individual risk, will play a role in relieving pressure on acute services, through preventing health crises.
Day to Day Duties to include, but not exhaustive:
To provide proactive reviews and support for patients with CVD and / or COPD in line with clinical competence and local pathways
To initiate prescribed medication and amend medication when required.
Undertake holistic health assessments, identifying physical, mental and social needs
Deliver health prevention and health promotion interventions including lifestyle advice (alcohol, smoking, diet and physical activity), weight management and wellbeing support, Opportunistic screening and vaccination advice
Identify unmet needs and escalating health concerns, taking appropriate actions
Deliver care within the mobile clinical unit adapting consultations to a community-based environment
Support people who present to the mobile unit by providing assessment and advise where appropriate, signposting and referring to the most suitable service when out the scope of practice and offering reassurance, health information and ongoing support
Promote self-management and empower patients to engage with health services
Accurately record all activity, assessments and outcomes on EMIS using appropriate clinical coding and templates

We are looking for a Registered Nurse (Wound Care) to join our Wound Care Service. The service is spread across clinics in Fleetwood, Freckleton and St Annes.
Pay: £15.88 - £19.33 per hour depending on experience
Hours: 22.5 hours per week permanent post
Accountable to: Lead Wound Care Nurse
Reports to: Clinical Manager for Lancashire Place Based Care
Main duties of the job
The post holder will undertake wound assessments and treatment for patients with simple wounds. Patients who will be referred to this service will be the following types:
Post operative including suture and staple removal
Post trauma skin tears, self-harm
Acute wounds that require follow up
Cavity Wounds
Burns/scalds that require follow up
Patient education including nutrition in relation to wound healing and scar management
There is an expectation to refer to the wider community team if required, support Senior clinicians and HCAs and attend meetings when required. The post holder will help promote the service and implement change as requested from the ICB.
Exclusion Criteria: Those patients who are: Not registered with a GP in Fylde and Wyre CCG (i.e., reside outside area) House bound Undergoing or requiring Vac Therapy patients Undergoing or requiring Larvae Therapy Doppler patients

We fully support our health advisors and do require that they pass assessments at regular stages of their training course. The course consists of an intensive training programme right at the start of your journey with us. Following this, we will train you on a programme called NHS Pathways, this requires approximately 150 hours of training to be able to use the software in a safe, effective, and efficient manner alongside any other recognised training for development. This is covered in a 2-week course where you need to be available every day, Monday to Friday. There are two exams during this period with a 70% pass rate. Failure to pass both exams will mean you sadly can not continue your employment.
When you have completed the training course, we require approximately 75 hours of shadowing to run straight after the course has finished. Overview of the Role: As a Receptionist / Driver you will behelping to deliver a safe and quality Out Of Hours GP service where you will also act as a driver to clinicians when home visits are required.
Main duties of the job
This is a Patient facing role where you will be representing FCMS at the OOH reception desk, greeting and assisting patients where necessary including other reception duties in line with the operational duties of the service. You will also be providing a professional and high-quality driving service for the GP Out of Hours Service to enable our Clinicians to complete home visits when required.
