Job summary
Newcastle Sexual Health Team have an exciting opportunity for a passionate and committed Sexual Health Specialist Nurse. We are looking for a qualified sexual health nurse, who is friendly and enthusiastic, with a flexible approach to provide support to the clinical team. This is a permanent full/part-time position, which will predominantly be based within the community sexual health clinic at Newcastle Sexual Health Service in Summerhill. However, there may be times when the employee would need to work at Gateshead Trinity Square Health Centre and Palmers Community Hospital in Jarrow and outreach clinical locations.
We are a friendly, community based, multi-disciplinary team. We are committed to delivering high quality, integrated sexual health care for the local population, our service includes both contraceptive and genito-urinary medicine. The post-holder must be able to maintain a sensitive and non-judgemental approach to support patients with their sexual health needs.

Main duties of the job
The sexual health service is part of the community directorate within South Tyneside and Sunderland Foundation Trust.
The successful candidate will be an experienced Specialist Sexual Health Nurse, and:
Demonstrate they have previous experience working within an integrated sexual health service
Maintain and respect strict confidentiality standards for service users
Maintain a sensitive and non-judgemental approach when working with service users
The post holder will be required to work evening and weekend duties as part of a monthly roster.
The post holder will be required to travel between clinic locations and for meetings as necessary.
The post holder will work effectively as part of the wider sexual health team and demonstrate excellent communication skills.

Main duties of the job

- Safely utilise mechanical and electrical cleaning equipment following appropriate training and to be responsible for cleaning domestic services equipment after use.

Job summary
The integrated care pathway for Chronic Obstructive Pulmonary Disease (COPD) provides a spirometry service across the community and within the chest clinic at the hospital.
The lung function department is part of the chest clinic and is an integral part of supporting diagnosis through a variety of lung function tests.
The post holder will be responsible for carrying out routine lung function investigations to a high standard and in a professional manner whilst working within departmental guidelines, both at Lewisham Hospital and also in the community setting. They will be required to lead on spirometry training, offering support and guidance to practitioners across a variety of healthcare settings and taking an active role in the continuing professional development of their colleagues.
They will be a clinically credible practitioner who can provide effective patient care in a range of clinical settings, having regard for relevant policies. They will be required to work independently at times, and will be responsible for ensuring that they deliver a safe and effective diagnostic service. They will be expected to prioritise patient referrals.
They will be required to help to develop the service, identifying cost and quality improvements, where appropriate.

An exciting opportunity has arisen for competent individuals to join a team of Clinical Support Workers in the busy environment of Cardiology. The unit is responsible for the safe assessment and admission of patients, and ensuring excellent care is provided at all times.

Your Day-to-Day
We are seeking an academically strong candidate to support and enhance the psychological care of patients at the service by assisting the Psychology department with;
- Contributing to direct and indirect assessments, scoring and reporting (verbal / written).
- Adapting assessment methods, intervention materials in accordance with clients communication difficulties, sensory needs or cognitive impairment.
- Clinical/neuropsychologic al formulation and implementing care plans for psychological treatment
- To oversee the gathering and collation of incident data and evidence as part of the functional analysis and formulation.
- Communicating results, formulations and recommendations arising from assessments to the multidisciplinary team.

Our Children and Young Peoples (CYP) Mental Health Support Teams are working with children and young people who are experiencing mild to moderate mental health issues. This includes working with parents/caregivers of Primary School age pupils and working with Secondary schools and colleges to develop the support they offer the whole school community. We are looking for an experienced and motivated Senior Commissioning Officer (Children and Young People) to join the All Age Continuing and Complex Care Team at Central London Community Healthcare (CLCH) NHS Trust.
In this rewarding role, you will help shape and commission high-quality, person-centred services, ensuring children and young people receive the right care, in the right place and at the right time.
Working closely with local authorities, NHS providers and wider system partners, you will support the commissioning, contracting and quality assurance of Children's Continuing Care and other health-funded care packages. You will contribute to service development, effective commissioning processes and the delivery of high-quality services that achieve positive outcomes and value for money.
This is an exciting opportunity to influence service development, support personal health budgets and strengthen integrated working across health and social care.

Main duties of the job
- Support the commissioning, contracting and quality assurance of Children's Continuing Care and other health-funded care packages.
- Develop and monitor contracts, service level agreements and provider performance.
- Work collaboratively with NHS organisations, local authorities, providers and stakeholders to improve services and outcomes.
- Contribute to the development of care pathways, procurement processes and integrated commissioning arrangements.
- Support the delivery and monitoring of personal health budgets and self-directed support.
- Ensure robust governance, funding oversight and effective management of individualised care packages.
- Analyse complex information and provide recommendations to support commissioning decisions and service improvements.
- Represent the CYP Complex Care Team at funding panels, meetings and working groups.
- Support transition planning between children's and adult services in line with national frameworks and best practice.
- Monitor budgets and expenditure, supporting financial planning and value for money.
- Maintain effective relationships with providers and stakeholders and support audits, service reviews and performance monitoring.
- Deputise for senior colleagues as required and contribute to the development of the Complex Care Team.
