Job summary
The core purpose of the MDT co-ordinator is to provide consistent patient centred focus throughout the pathway of care, by providing a point of contact along the administration pathway for the Multi-Disciplinary Teams (MDTs). This will ensure coordination of the patient journey, especially for patients that need to move between cancer units and cancer centres. will be responsible for tracking the patients along their care pathway and identifying potential breaches of cancer waiting times and targets. The role will ensure that all relevant patients are discussed at MDT meetings and all supporting clinical information is made available. Also accurate prescriptive data collection and recording to enable effective patient tracking of all suspected and positively diagnosed cancer patients The post holder will be responsible for the coordination and organisation of the multi-disciplinary team meetings and will attend these meetings obtaining and recording relevant information. They will collect, record and report cancer information as required to meet both national and local requirements.
Main duties of the job
1) To provide administrative facilitation to the MDT meetings, including preparation, organising, and circulating lists of patients for discussion, and ensuring MDT run as smoothly as possible to reach their full potential in discussing all cancer patients.
2) To ensure that all patients are listed for MDT meeting once positive investigation/s or histology reports received and recorded on Somerset Cancer Registry To attend weekly MDT meetings and input live information on to the Somerset Cancer Register including the recording of the outcome of each discussion and any actions to be taken.
3) To use this information for minimum data set collection and cancer waiting times To ensure that information relating to patients referred from Tertiary Centres for treatment or discussion is fed back to the referring Trust in a timely manner, to include MDT outcomes and plan for patient To ensure that the Cancer Services Tracking Tool and Somerset database reflect the outcomes of MDT discussion.
4) Working closely with the Cancer Pathway Tracker where applicable (site dependant). To proactively track patients to ensure their pathway through the system is smooth and efficient and achieves all relevant cancer waiting time targets.
5) To ensure that inter-departmental and inter-Trust referrals are actioned and closely monitored, and information fed back to referring teams or organisations as appropriate and within monitored timescales

This includes the recording of a cancellation reason in the event of an MDT meeting being cancelled To work with the Clinical Nurse Specialist (s) and in collaboration with the relevant support services to ensure that tests, appointments and treatment plans are arranged as agreed at the MDT meeting To ensure that the CNS (GP) communications forms are processed within 24hours of completion by CNS and all relevant information is updated on Somerset Cancer Registry To organise and minute an annual MDT operational business meeting. To liaise with Research and Development to establish systems for identifying and recording of patients for possible entry into clinical trials.
1.2 MONITORING OF WAITING TIMES
Working closely with the Cancer Pathway Tracker where applicable (site dependant). To proactively track patients to ensure their pathway through the system is smooth and efficient and achieves all relevant cancer waiting time targets. This will include liaising with the various departments to facilitate the timely booking of diagnostic test and treatments To ensure all patients progress is accurately recorded and updated in a timely manner on the Somerset Cancer Register, managing their entire pathway, including post 62 day care Maintain robust data collection systems ensuring that clinical and non- clinical information, covering all aspects of the patients cancer journey is collected in a timely fashion and entered onto the cancer database and site specific tracking tools as applicable. This will, as a minimum, meet the requirements for the Minimum Data Set (MDS) and monitoring of cancer waiting times To ensure that all new patients treatments are booked within target times and that any potential breaches of waiting time standards are clearly escalated to the clinical team and immediate line manager, within cancer services, in a timely manner To contribute to the validation of the monthly cancer waiting times submission, using relevant data sources, to ensure data is accurate and national cancer minimum data sets are complete To liaise regularly with the members of the MDT and Divisional specialties to complete breach analysis reports, identifying delays in treatment and locating missing data as necessary To participate in the weekly cancer Patient Tracking List meeting (PTL), to report on progress of patients through their pathway of care and against waiting time targets and complete any remedial actions that may result from the meeting To contribute to the analysis of breaches and redesign of cancer pathways as necessary
1.3 COORDINATION OF MDT REFERRALS
To ensure that inter-departmental and inter-Trust referrals are actioned and closely monitored, and information fed back to referring teams or organisations as appropriate and within monitored timescales To ensure correct referral processes are followed and are supported with accurate and timely documentation
.4 PEER REVIEW or equivalent AND SERVICE DEVELOPMENT
To work with members of the MDT and Cancer Services Team to collate information as required in support of the Peer Review or equivalent process. This may include attendance records, collating data and information and supporting the annual MDT business meeting To work with members of the MDT and the Cancer Services Team in the development of patient pathways and continual improvement of the service for patients To contribute to the development and implementation of improvements in the efficiency and accuracy of MDT administrative processes To participate in the collection of accurate cancer data and information, working closely with the MDTs and Cancer Information & Data Analyst to improve collection & reporting processes with particular reference to COSD,SACT and other National Cancer Audits.
To support and contribute to audit projects, both national and local, as undertaken by the MDT by maintaining robust data collection systems, ensuring that both clinical and non-clinical information covering all aspects of the patients journey is collected and entered on to the Trusts data base in an accurate and timely manner

Consultant Anaesthetist with an interest in Chronic Pain based at Darlington Memorial Hospital (DMH), County Durham and Darlington NHS Foundation Trust. DMH intensive care admits approximately 580 patient per year with a mix of surgical and medical take.
DMH has an 11 bed combined ICU and high-dependency care located near the theatre suite. This unit is used flexibly between level 2 and 3 beds. Three consultant sessions are allocated on each weekday to provide 15 sessions over the week with a rota based on continuity of care.
At DMH site surgery is provided within 11 operating theatres covering Community Dental, ENT (Day case), General Surgery (including major colorectal surgery, breast surgery and bariatric surgery), Orthopaedic and Trauma Surgery, Obstetric and Gynaecological Surgery, Ophthalmology and Urology (Day case). There is a Consultant-led pre-assessment service with weekly clinics including CPEX testing. Anaesthesia for CT scanning and MRI services is available.
The majotiry of intensive care admissions are emergency surgical patients and Monday to Thursday we have a PACU to provide Level 1 care of elevtice patients under the support of the surgical and anaesthetic teams.
There is an obstetric department, located on the 6th floor has its own theatre with dedicated elective lists in the main theatre suite. An anaesthetic nurse provides assistance for all theatre cases.
Medical department consisits of in house acute internal medicine, cardiology, respiratory (including 6 bedded ARU), haematology, endocrinology, gastroenterology and care of the elderly. Outpatient dermatology, rheumatology and psychiatry will provide virtual and face to face consultations. Palliative care provide face to face support with a team of consutlant and specialist nurses.
Consultant led RaCI plus nurse led IV, renal outreach and Acute Intervention Teams (critical care inreach) support our work. Weekend work is completed as part of the out of hours commitment with 12 hour shifts. The majority of specialist support is provided via in house referrals with otherwise good relations with our tertiary referral hospitals. Renal and neurology specialities based at James Cook University Hospital provide in house reviews. Our consultant group form a supportive team and recent appointments include a job share with James Cook Hospital and with Acute Internal Medicine. We have an exciting development opportunity for a suitably qualified and experienced professional who would like to extend their scope of practice to an advanced level.
Where required, the post holder will be expected to undertake a four-year training programme under Annex 21 (Band 8a) working towards achieving a level 7 qualification in advanced practice and local consultant sign-off as an EM:ACP.

Job summary
The Acute Medical Unit at NNUH is set across three wards currently with a combined bed base of 84 beds plus 9 trolleys. Much like other Acute Medical Unit, we receive patients directly from GP's and the Emergency Department and we ensure all emergency admission patients receive seamless, high-quality care at the point of admission. We see around 20,000 emergency admissions each year.
Where we differ from other acute units is that NNUH has spent a lot of time in the development of Emergency Care which has led to the expansion of the services offered by the Acute Medical Unit. This includes Same Day Emergency Care (SDEC), Priority Assessment Unit (PAU) acute medical short stay beds, and advanced care for critically ill patients.
Main duties of the job
The Assistant Practitioner will function as part of the Multidisciplinary Team within the Same Day Emergency Care Unit and the Acute Medical Unit, to provide a seamless service to patients under the remote supervision of a Registered Nurse/Practitioner.
The post holder will continue to develop and undertake a range of delegated tasks and delegated authority and will report to a Registered Practitioner. The Assistant Practitioner will provide a high standard of general/specific care for an identified client group and will be proficient and competent to work across professional disciplines.
The post holder will be able to deliver elements of health and social care and undertake clinical work in domains that have previously only been within the remit of registered professionals. He or she would also have the underpinning knowledge and assessed level of competence to undertake such as role - locally assessed following academic instruction. It is anticipated that this role will continue to develop through the acquisition of further skills, knowledge and competencies to be determined within the clinical teams, with focus on clients need.

The Locality E-Rostering Advisor will provide dedicated locality-facing support, training and expertise to enable managers and teams to use the Trust's e-rostering system confidently, consistently and effectively. The post holder will act as a visible and accessible point of contact for designated localities, working closely with operational managers, roster creators and system users to understand local challenges, provide hands-on support, deliver system training, and embed good rostering practice.
The post holder will support the effective deployment of staff resources by analysing roster performance, identifying opportunities for improvement and working collaboratively with locality teams to improve roster efficiency, workforce utilisation and compliance. The role will promote continuous improvement through coaching, practical training, relationship based support and data driven decision making.
The role will promote best practice in roster creation and management, supporting services to maximise workforce resources through a solution focused and customer centred approach.
Main duties of the job
Develop and maintain effective relationships with managers, roster creators and system users, acting as the key locality contact for e-rostering support, guidance and problem resolution. Provide operational support to ensure rosters are accurate, compliant and aligned to workforce establishments, supporting service changes and workforce initiatives.
Plan, deliver and evaluate e-rostering training through workshops, one-to-one coaching, floor-walking and refresher sessions, creating user guidance and identifying opportunities to improve system adoption and competency.

This is an exciting opportunity to join the Buckinghamshire and Milton Keynes Bowel Cancer Screening centre as a Programme Clerk, supporting the early diagnosis and prevention of cancer across the county.
The programme works across Milton Keynes University Hospital NHS Foundation Trust (MKUH) and Buckinghamshire Healthcare NHS Trust (BHT). This post will be based at Milton Keynes University Hospital, providing on-site administrative support for the programme. This will include, but is not limited to, handling incoming and outgoing post, preparing letters and packs for patients, communicating with patients, and working alongside clinical staff. It is an expectation that the successful post-holder will be able to travel to MKUH and BHT sites as required.
The successful candidate will be able to handle the demands of a busy NHS service. They will be self-motivated and able to work independently, as well as in a team of clinical and non-clinical staff. The post holder will be required to abstract, analyse and accurately code clinical information from patient records (paper and electronic) across all specialities using ICD-10 and OPCS-4 Classifications in accordance with National Standards and the Trust's Policy and Procedures guidelines. The role requires the successful candidate to produce consistent, accurate, complete and timely coded clinical data, for use by the Trust's Finance Department among others and to meet increasingly demanding deadlines whilst maintaining accurate coding at all times.
Our source documentation for coding is a mix of electronic and some paper documents, with an emphasis on moving towards a fully electronic patient record.

Our Forensic HCPS are responsible for assessing and addressing the health needs of detained persons, as well as conducting forensic medical examinations gathering evidence for use in the criminal justice system.
Main duties of the job
You will be conducting clinical assessments, identifying, and implementing appropriate interventions and collecting forensic samples. Providing advice and guidance to ensure appropriate care is maintained and keeping detailed and accurate records to ensure the health, safety and welfare of detained persons held in police custody.

As a key member of the Estates and Facilities Division, youll be part of a highly responsive team providing 24/7 support across the Trusts sites, including Wonford, Heavitree and Mardon House.
Your responsibilities will include:
- Responding to incidents involving theft, assault, anti-social behaviour, and other security concerns
- Supporting clinical teams with patients experiencing confusion, delirium, dementia or post-operative delirium
- Adapting your approach to ensure all interactions with patients are handled with empathy, professionalism and care
- Using de-escalation techniques as a first approach, backed up by accredited physical intervention training
- Writing accurate and timely incident reports and statements to support investigations or police involvement
- Monitoring and operating CCTV systems and supporting the Trusts car parking team as required
Youll be working an average 37.5 hours per week on a 4-week rolling rota with 12 hour day and night shifts, with enhancements for nights, weekends and bank holidays. These enhancements typically amount to an additional £5,000 per annum pro rata on top of the basic salary.

An exciting opportunity has arisen for an enthusiastic and drive team player to join the rapidly growing pharmacy team at the James Cook university Hospital. You will be working within the busy pharmacy dispensary team assisting in the provision of an effective and timely service to the patients and staff of South Tees Hospitals NHS Foundation Trust under the leadership of the Senior Pharmacy Technician dispensary manager.
The successful candidate will work full time hours 08.30-17:00 Monday - Friday.
Weekend working once every 5 weeks Saturday or Sunday (9.00-17.00), reimbursed with a full weekday off the following week.
Would be expected to contribute to bank holiday working rota.
Main duties of the job
The role consists of supporting with running of the pharmacy dispensary including covering reception, using the pharmacy tracking system, bagging medication ready for deliveries, putting away stocks into the pharmacy robot and medication shelves, filing prescriptions and paperwork, expiry date checking and other general house keeping duties.
On completion of training, you will be dispensing a wide variety of prescription types including controlled drugs, prescription only medicines and unlicensed drugs for the inpatients of JCUH and the community hospitals.

We have an exciting opportunity for a candidate to join the team within the IV Suite at the Wrexham Maelor Hospital. The Clerical Officer role is a 18.75 hr post working alongside 2 other team members on a rolling 2 week rota. This is to provide clerical support to the department between the hours of 8.30am-4.40pm Monday through to Friday.
The Clerical Officer(s) will provide a Front of house service, greeting patients on their arrival to the department and supporting the patient journey runs as smoothly as possible. This will include arranging appointments and processing the outcomes of the outpatient clinics, ensuring patient records are updated accurately, receiving and dispatching of patient case notes, along with other duties to ensure that the department meets all the patient's needs.
You should have excellent IT, communication and clerical skills, be well organised with a meticulous approach to work and attention to detail. You will be proactive and able to manage your own working time effectively. You must have a comprehensive working knowledge of Microsoft Office packages. High standards of professionalism and a desire to learn will be essential.
Welsh language Skills are essential for this post.
Main duties of the job
To ensure confidentiality is maintained at all times.
Provide a friendly and comprehensive reception service to all staff and patients visiting the department.
Undertake efficient reception duties e.g. answering the phone and receiving people at the reception desk.
To deal efficiently and courteously with colleagues and all visitors at all times as per departmental standards.
Visitors to the department include, staff from other areas, company representatives, visiting medical staff and patient relatives.
Deal courteously with telephone calls and enquires and relay messages in an efficient manner as service standard, processing urgent referrals.
Assist with general clerical duties, including distributing incoming mail.
To be responsible for accurately inputting data onto relevant databases and to collate data for the purpose of clinical audit.
To collate and distribute clinic lists effectively, liaising with secretaries and other administration staff. Provide an excellent communication system between relevant personnel, regarding any alterations or additions to clinic lists. This may include the booking of emergencies.
Print and distribute clinic lists in a timely manner.
Liaise with the nursing and medical teams within the department, to help maintain patient flow.

We are looking for a well-organised, motivated and enthusiastic nurse to join our team. Ward 24 is a dynamic, fast paced unit consisting of 29 Inpatient beds and 8 Paediatric Assessment beds. Within the ward footprint is an established High Dependancy Unit which has recently been given Level 2 status.
The role of Band 6 sister requires a pro active, forward thinking nurse. The successful candidate will help further develop the team, whilst delivering exceptional levels of care and family support.
