We are searching for a Practitioner Psychologist to join the Older Adult Mental Health Services (OAMHS) Psychology Team within Hywel Dda University Health Board.
We are looking for an enthusiastic, compassionate and motivated Practitioner who is committed to delivering evidence-based psychological care and who is keen to contribute to the continued growth and innovation of psychology services for older adults.

To be considered for this role, you must be a HCPC-registered Practitioner Psychologist and hold a doctorate-level qualification in Clinical Psychology, Counselling Psychology or an equivalent recognised qualification.

Main duties of the job
As part of a dedicated multidisciplinary team, you will provide specialist psychological assessment, formulation, intervention and consultation for older adults experiencing complex mental health difficulties across a range of secondary care settings. The role offers the opportunity to work collaboratively with colleagues across services, contributing to the delivery of high-quality, person-centred care.
In addition to clinical work, you will play an active role in service development, consultation, supervision, teaching and training, as well as participating in audit, research and quality improvement initiatives that support the ongoing development of psychological services within OAMHS.

We are seeking a dynamic and motivated person with a level 3 qualification in a health or related subject and experience of working with pregnant women and families or community groups, in a health or voluntary sector capacity.
The role will work with both the public health team and maternity teams providing a holistic, behavioural change approach to caring for pregnant women and their families, providing a range of early support and information to promote a healthy pregnancy and targeting work to reduce health inequalities faced by families in your care. You will work with the maternity tobacco dependency team and public health team to provide behavioural support and treatment to promote changes to support a healthier pregnancy. In addition, you will care for women in priority groups who experience poorer outcomes in maternity and healthcare, includingthose who are living with complex social needs and thoseandwomen from ethnic minority backgrounds.

To work with a caseload of women and families during pregnancy and the early postnatal period to promote the best start in life for every child. You will be based in both the hospital and community setting, providing support in Family Hubs, family homes and community bases.
To support the wider 'Best Start in Life' programme of work within the public health and wider maternity team.
To support and manage prioritisation of a caseload in the maternity Best Start in Life pathways to ensure safe, well organized and evidenced based care is provided to all that access the service.
You will provide support in a range of settings as required for the delivery of smoking cessation services and the enhanced continuity or carer pathway, this may include work with people by phone, by video conference, or face-to-face.
To support the delivery of the service key performance and reporting indicators for tobacco dependency and enhanced continuity of carer pathways.

Main duties of the job
Duties for the post holder will be on the inpatient wards, Children's emergency department, CYPAU and Tropical Bay. The rota involves daytime and night duties, compliant with the European Working Time and Deanery Directives. Full induction to operations, duties,responsibilities and expectations will be provided.
There is a Consultant led handover followed by a consultant led ward round every morning on both inpatient wards and the successful candidate will be responsible for taking an active role in managing the patients along with the rest of the team. At any time the immediate clinical back-up from a middle grade doctor (Registrar) and our supportive nursing team will be available.

We are pleased to announce an exciting 12 month secondment opportunity for a Band 5 Nurse on Rowan lea ward, Cross Lane Hospital.
This secondment would suit an experienced nurse wishing to return to or gain experience in MHSOP nursing. It is an opportunity to develop skills and knowledge whilst working alongside and shadowing an experienced nursing team.
Rowan Lea is a 20 bedded mixed gender assessment and treatment ward for older people, with both organic and functional illnesses. We work collaboratively with patients and carers to develop individual well-being treatment plans. We have a broad multi-disciplinary inpatient team that includes nurses, medical staff, a physical health care practitioner, physiotherapy and occupational therapy. There is local access to nutrition and dietetic staff within MHSOP. We work in partnership with local Community Teams.

The post holder will need to hold a first level qualification in mental health nursing and have excellent clinical knowledge and experience of mental health and the Mental Health Measure 2010. Excellent communication, listening and organisational skills are essential along with MDT working, in a challenging and changeable environment.
The successful applicant will need to demonstrate sound clinical knowledge and awareness of current clinical practice in mental health, have excellent clinical assessment skills and have sound knowledge of safeguarding procedures. The post holder will be expected to manage a caseload of patients with mental health issues, subject to CTP and will work under the leadership of a Band 7 nurse.

Planning and Design

The post holder will be guided by established processes but will also be required to act on their own initiative with advice from the Guardian where needed.
Improvement and Monitoring
Analyse data and information received and identify trends of high-risk Exception Reporting across Departments.
Monitor the system database on a daily basis for all Exception Reports logged.
Escalate any Immediate Safety Concerns generated via an Exception Report to the Guardian.
Monitor deadlines for the review process of Exception Reports by Educational Supervisors and proactively seek resolution within required response times.
Work with the Guardian to identify where there are areas of concern and support the development of action plans and reports in relation to this.
Communications
Deal with all day-to-day correspondence on Exception Reporting - initiating appropriate responses in order to provide relevant staff with the required information.
Liaise with Resident Doctors, Educational Supervisors, Rota Co-ordinators and Service Managers to ensure submission of claim forms or via the roster for payment arising from Exception Reporting are processed within contractual timescales.

Band 3 Clinical Support Worker - part time - 22.5 hours per week
The band 3 will cover the Neurology Service at the CDC
The post holder will be flexible across CDC/CIC to cover service need.
You will work with indirect supervision under the Registered Nurse; assisting with nursing care and providing information and assistance to the patient, their relatives and carers.
