Job summary
Join our Occupational Therapy team and take a leading role in delivering high-quality rehabilitation to patients with a range of complex and multiple conditions.
As Occupational Therapy Team Lead, you will work as an integral member of the multidisciplinary team on a general rehabilitation ward, providing highly specialised assessment, treatment and expert clinical advice. You will support patients to maximise their independence, achieve their rehabilitation goals and make progress towards the best possible outcomes.
You will also work closely with the OT Lead to develop the service and strengthen integrated working with health and social care partners.
This is an opportunity to use your specialist clinical expertise, support the development of colleagues and contribute to the delivery of evidence-based, person-centred care across Hertfordshire.
You will provide a specialist occupational therapy resource for colleagues across the service, sharing your knowledge and expertise to support high standards of practice.

Main duties of the job
As Occupational Therapy Team Lead, you will:

Work as an active member of the multidisciplinary inpatient rehabilitation team, supporting patients with multiple and complex conditions.

Provide highly specialised occupational therapy assessment and treatment, using evidence-based approaches to support individual rehabilitation goals.

Provide expert occupational therapy advice, guidance and information to health and social care professionals, carers, relatives and other relevant individuals.

Act as a specialist clinical resource for Occupational Therapy colleagues and other healthcare professionals across Hertfordshire.
-
Support the OT Lead with service development and initiatives that strengthen integrated working with partner organisations.

Share specialist knowledge and contribute to the development of occupational therapy practice across the service.

Promote person-centred care and collaborative working to support safe, effective and high-quality rehabilitation.

Contribute to a positive learning environment, supporting colleagues to develop their clinical knowledge and skills.

Job summary
An exciting opportunity has arisen for a Band 6 Occupational Therapist to join the Solihull Community Respiratory Team on a fixed term basis for a period of one year. Working as part of a friendly multidisciplinary team, you will provide specialist assessment, treatment and rehabilitation for patients with respiratory conditions across a variety of settings, including community venues, hospital sites and patients' homes.
You will work flexibly across the service, supporting respiratory outpatients, community services and pulmonary rehabilitation programmes to help patients improve their independence, function and quality of life.
Main duties of the job
*Take delegated responsibility from the Band 7 therapist for managing patients with a range of conditions, providing specialist therapy assessments and developing appropriate treatment plans.

Job summary
These posts are in addition to our existing establishment of 22 middle grade level doctors within the Medical Division at Queen Elizabeth Hospital. The current middle grade establishments consist of 7 IMT3s and 15 Deanery ST4 + in various specialties.
The current general medical on-call cover consists of 2 registrars 0800-2100 daily and overnight.
When not on-call there is flexibility in which specialty each post can be based in with at least 4 months in Acute Medicine.
We have opportunities in Respiratory, Frailty, Acute Medicine and Diabetes and Endocrinology. We can therefore accommodate the preferences of any potential applicant, and offer registrar level training in any of these specialties subject to availability.
The posts currently available are for a years fixed term.
Main duties of the job
The post holder will take part in the middle grade on-call rota, including night shifts and weekends. When on-call one registrar runs the medical take in the emergency department, and one supervises and supports the ward cover medical team. There is a full team of juniors both on the take and covering the ward.
The on-call frequency is approximately 1:10 weekday nights (Mon-Thurs), 1:10 weekend days and 1:10 weekend nights.
When not on-call, the post holder will be able to work in a department of their choosing, subject to availability, with at least 4 months in Acute Medicine. The job plan will depend on which department that is, and be open to discussion, depending on the training needs of the individual and the workload of the department. It is likely this will be a combination of inpatient ward cover and outpatient clinics and/or procedures.

The Pharmacy Department provides a fully comprehensive pharmaceutical service across the Trust and to a number of external community units. We are a large and very busy department with over 180 members of staff including pharmacists, pharmacy technicians, pharmacy assistants, admin and clerical staff and apprentices. There are many varied roles and we all work as a team to provide the different functions and services whilst maintaining a high level of patient care.
Pharmacy Assistants are vital members of the Pharmacy team who work closely with Pharmacists and Pharmacy Technicians. They support colleagues throughout the Trust. Based in Dispensary they are required to work under the supervision of a Pharmacy Technician or Pharmacist.
This is an exciting opportunity to join us as a Band 2 Pharmacy Assistant and develop the required knowledge and skills to become a Band 3 Senior Pharmacy Assistant. Support, training and assessment will enable you to achieve the core and enhanced competencies required for the post.
A level 2 GPHC approved qualification will be undertaken and once qualified you will transition from band 2 to band 3.
Those already holding a level 2 accredited pharmacy qualification will start at Band 3.

Main duties of the job
You will be supporting the supply and dispensing of medication to patients across the hospital, including outpatient clinics. You will have great attention to detail and like working in a fast-paced environment within a supportive team. The post will involve using the pharmacy computer system (CMM) and dispensing robot for which training will be given.
You will also spend some time each week on a ward to top up stocks of medication. If you have had some previous experience working in a hospital or community pharmacy, or a dispensing doctor's practice, this is a great position to further develop your career in pharmacy.
This post is for either full or part-time hours. This can be fixed term or substantive. Full-time hours will be worked Monday to Friday with a shift pattern to cover early starts (08:00) and late finishes (18:00).
Part-time hours can be worked as either full or part days but again will include the requirement to cover some earlies and lates. There will be a requirement to work 1 weekend every 10 weeks with corresponding time off during the week.
There is also a requirement to participate in the bank holiday rota, currently minimum 1 per year. Rotas are subject to change at any time.

Main duties of the job

Main duties of the job
Assessment and management of cases of acute anterior uveitis (an inflammatory eye condition) including clinical examination, diagnosis, management with topical steroid drops, documentation of investigation results including blood tests and x-rays, escalation of complex or non-responsive patients to the uveitis consultants. This will initially be through training under direct supervision within a uveitis consultant-led clinic and within the eye emergency service, with a view to the development of independent nurse clinics for management of straightforward cases.
A major role will be to develop a pathway for patients requiring immunosuppression for the treatment of their inflammatory eye disease. This will involve the education and counselling of patients starting on these medications, as well as pre-treatment screening investigations, initial post-treatment blood monitoring, and handover to general practice for ongoing shared care.
Develop and maintain an up-to-date record of all patients within the uveitis service who are on long-term immunosuppression with either systemic steroids or conventional DMARDs (disease-modifying anti-rheumatic drugs)
Acting as a point of contact for patient queries and providing joined up care by liaising with rheumatology specialist nurses, consultants, GPs, and uveitis consultants to facilitate comprehensive and coordinated management for these patients
Participating in clinical governance, audit and research.

Social prescribing link workers work collaboratively across the health and care system, targeting populations with greatest need and risk of health inequalities. They collaborate with partners to identify gaps in provision and support community offers to be accessible and sustainable.
Social Prescribers manage and prioritise a caseload, according to the needs, priorities and support required by individuals in the caseload. They identify when a persons needs are beyond the scope of the Social Prescribing role and refer them back to other health professionals or organisations.

This unique role offers an outstanding opportunity to combine clinical practice with undergraduate medical education, helping to shape the prescribing knowledge, skills and confidence of future doctors.
Working closely with the Lead for Pharmacology and the Clinical Skills Director, you will play a key role in the delivery and development of the prescribing and clinical pharmacology curriculum within Norwich Medical School. A particular focus of the role will be ensuring that students are well prepared for the Prescribing Safety Assessment (PSA), which all final-year medical students are required to pass prior to beginning Foundation Year 1 training.
Alongside your educational responsibilities, you will practise as a Highly Specialist Clinical Pharmacist within the Emergency Department and Admissions Units at NNUH, supporting the delivery of high-quality acute care services. Job summary
It is an exciting time to join our service with the Trust committed to becoming a designated Major Trauma Centre over the next 5 years and with the opening of a new Norfolk and Norwich Orthopaedic Centre (NaNOC) in July 2024, transforming our elective care provision.
As a result our team is growing and we are seeking a Senior Physiotherapist with experience of musculoskeletal conditions and/managing patients after orthopaedic surgery, to join our acute inpatient Trauma and Orthopaedic team.
Your weekday pattern will include long days (08:00-20:00), early (starting at 08:00) and late (ending at 20:00) shifts and we will require your ability to be flexible to meet service needs.
You will fulfill 7-day working requirements through our Orthopaedic weekend rosters + bank holiday roster.
We are seeking experienced senior Physiotherapists with specialist knowledge and skills in
the acute care of Trauma + Elective Orthopaedic Surgery. You will be involved in assisting clinical lead physiotherapists to develop and transform our services and care pathways, providing clinical leadership to ensure provision of excellent patient care. You will work alongside and gain peer support from other senior staff within the team and department.
A progressive band 5-6 role will be considered for suitable candidates.

About us
We are part of the Wantage PCN in the health centre, working alongside Church Street Practice. The Health Centre has had an extension built very recently offering extra capacity.
Our patient list size is approximately 16,000.

Are you ready to transform MSK care at the frontline of Primary Care?
We are delighted to be recruiting a Advanced Physiotherapist Practitioner (APP) to join our established and forward thinking team on a full-time basis.
In this onsite role, you will work within GP practices as part of a multidisciplinary team, assessing, diagnosing and managing MSK patients at the first point of contact with full autonomy and strong clinical support. Main duties of the job
As an Advanced First Contact Physiotherapist, you will be a qualified autonomous clinical practitioner working in GP practices who can assess, diagnose, treat, and manage musculoskeletal (MSK) problems and undifferentiated conditions, and where appropriate, discharge a person without a medical referral. FCPs working in this role can be accessed directly by self-referral or staff in GP practices can direct patients to them to establish a rapid and accurate diagnosis and management plan to streamline pathways of care.

This is an exciting time to join Acute & Emergency Care at Darlington Memorial Hospital as we continue to improve the process and experience for patients who are admitted to Hospital in an emergency. We are seeking to appoint a Registered General Nurse to join our Acute Medical Unit.

It would place you as a member of a committed, caring and compassionate team, who are supportive and knowledgeable about their specialty.
You will work closely with our experienced and friendly multi disciplinary team to assess, plan, implement and evaluate the medical emergency patients care, providing first line treatment. This rapid plan of care enables 40 percent of admissions to be discharged home from the AMU.
To continue in this excellent work we are looking to recruit an enthusiastic, dynamic and highly motivated nurse to join our team.
Main duties of the job
To assess patients needs and develop, implement and evaluate plans of care.
To organise the delivery of nursing care, deploying staff effectively.
To be aware of, and communicate to team members, changes to patient information and treatment plans, both verbally and in writing.
To adhere to NMC standards of record keeping.
To assist the Ward Manger in setting, monitoring and maintaining standards of care.
To participate in nursing audit and service improvement activities e.g. Essence of Care.
To be aware of resource constraints and the importance of working within budget.
To provide advice and act as a role model to other members of the team.
To be proactive in managing risk and maintaining patient safety.
To supervise the work of nursing support staff, ensuring tasks undertaken have been delegated appropriately.
To contribute to the day-to-day management of the directorate, co-operating with colleagues to provide an appropriate level of nursing service and to address bed management issues.
To participate in the education and training of junior staff
To involve patients/carers in the planning and delivery of care.
