Job summary
This role is responsible for the delivery of evidence-based nursing services, working as part of the practice multi-disciplinary team, delivering care within their scope of practice to the entitled patient population.
Working autonomously, the Practice Nurse will be responsible for a number of clinical areas such as public health and screening programmes, including infection prevention and control, chronic disease management, immunisations, health promotion and prevention, well man/woman clinics and cervical screening. The post-holder will be an integral part of the general practice team, as well as supporting the wider practice nurse network and multi-disciplinary team.
In a research capacity, the post holder will support the research teams in clinical trials. This will include assessing, planning, implementing and evaluating research-nursing care and providing support to patients who have been or have the potential to be recruited into a clinical trial programme. This includes ensuring the clinical trial has full approval to proceed, collecting data, interviewing, supporting, monitoring patients, and data entry. Clinical aspects of the role will include phlebotomy, analysis of blood specimens, drug administration and clinical observation. The role includes close liaison with clinicians, data managers, and outpatient and inpatient staff. You will also identify barriers to recruitment to research studies and assist in finding solutions to overcome these.

As a Clinical Reconnect Practitioner you will work autonomously in prison, community and police detention cells, to identify and respond to requests of screening and assessment of people with identified health needs leaving the prison establishment within a 12-week period or less.

Take referrals for service users who are leaving the prison establishment within 12 weeks left to serve or up to 28 days post release. As our new Reconnect Support Time & Recovery Navigator:
- Be part of an extended team working across the Reconnect service
- Liaise with a wide range of services such as the police, probation, courts and legal services as well as both inpatient and community health and social care services
- Be involved in the daily support of children and adults, including those from hard-to-reach communities to engage and navigate through the Criminal Justice System
- Support access to the full scope of identified health needs, including substance misuse (alcohol and drugs), mental and physical health, homelessness, learning disabilities, exploitation, safeguarding and social care issues
- support the health needs leaving the prison establishment within a 12-week period or less.
- The aim of the service is to reduce reoffending by addressing the health and social needs of the service user and ensuring onward referrals are made to ensure continuity of care provision

Main duties of the job
Some of the main duties of this job include:
- Providing a high quality domestic service in a way that is conducive to patient care ensuring legislative and policies are adhered to.
- Follow cleaning specifications and use appropriate equipment and materials.
- Work closely with other staff to ensure a safe and pleasant environment.
- To carry out floor maintenance i.e. mopping & displaying "wet floor" warning cones. Shampoo carpets and corridor mats on a systematic basis.
- Removing waste and clinical waste bags ensuring correct bags and procedures are followed.
- Use chemicals/cleaning agents in accordance with manufacturer's instructions. To only use chemicals which are supplied by the Trust and store under lock and key ensuring COSHH regulations are adhered to.
- Machinery used in a safe manner as per instructions/training.
- Undertake deep clean, clinical clean, admission and infection cleans as required by infection control procedure including removal, laundering, and hanging of curtains.
- To observe the colour coded cleaning system used within the wards, health centres and departments.
- Wear the protective clothing and uniform supplied by the Trust, maintaining a neat and tidy appearance and high standards of personal hygiene.

As our new Speech & Language Therapist, you will:
- Liaise and consult with other Speech and Language Therapists and/or other professions and agencies within mainstream primary and secondary provider services
- To prioritise, organise and manage a highly complex referrals on the basis of risk
- To undertake comprehensive assessment of adults referred using investigative and analytic skills and to formulate individualised interventions based on advanced clinical reasoning and partnership working
- To provide highly specialist report and recommendations to the Police and Court Services
- To provide clinical training, supervision and support to assistants, generic workers and volunteers and others as agreed with the regional and local managers.
- To work within the service multidisciplinary team advising on the management of specific Speech and Language Therapy related clinical issues, to collaborate in the development and co-ordination of team objectives and in the development of care pathways and care protocols related to the specialist area.

Qualifications and skills required:
- Recognised SALT degree
- Registered with the HPCP
- Registered with RCSLT
- Evidence of continuous professional development
- The post holder will need a minimum of 4 years post qualification experience.
- Experience of working with service users, face to face, with a variety of health needs.
- Experience of training other professionals
