About the role
Riverside is looking for a motivated and compassionate Practice Nurse to provide high-quality, evidence-based nursing care to our practice population.
You will assess, plan, deliver and evaluate care, working independently and alongside GPs and the wider practice team. Youll help maintain clear communication between clinical, administrative and community teams, and provide a welcoming, professional service to patients and colleagues.
This is a varied role in a supportive team. The postholder will contribute to the effective running of the treatment room, health promotion, clinical audit and service development. Duties will be undertaken within the nurses competence, agreed protocols and guidelines, and the NMC Code.
Main duties of the job
Main responsibilities
The role may include:
- Providing nursing assessments, treatment, screening, health education and advice.
- Wound care
- Blood pressure monitoring, venepuncture, ECGs and spirometry.
- Cervical screening, immunisations and routine injections, where appropriately trained and authorised.
- Well-person, NHS Health Check, new patient and travel health assessments.
- Supporting family planning and sexual health services
- Supporting chronic disease prevention and management, including for diabetes, hypertension, COPD, asthma and cardiovascular disease. QOF, LTC and HAA assessments.
- Minor illness and injury care, within competence and agreed pathways.
- INR monitoring and dosing using INRStar N3, where trained and authorised.
- Home visits for housebound patients, including routine checks and vaccination clinics.
- Referrals, including to podiatry, and ensuring specimens are processed appropriately.
- Maintaining accurate, timely clinical records, including appropriate read coding and QOF-related information.
- Helping to maintain a safe, well-equipped treatment room, including infection prevention, equipment checks, vaccine stock and cold-chain procedures.
- Working with the Practice Manager and nursing team to review protocols and maintain appropriate stock and equipment.
- Attending relevant practice meetings, training and protected learning events.

We are a supportive, multidisciplinary practice led by three GP partners and supported by salaried GPs, Advanced Nurse Practitioners (ANPs), nurses and Healthcare Assistants (HCAs). Our wider team includes Additional Roles Reimbursement Scheme (ARRS) staff, including physiotherapists, Mental Health Practitioners (MHPs), paramedics and Advanced Clinical Practitioners (ACPs).

Principal Responsibilities
To provide assessment, treatment, screening and health education services and advice, working in accordance with set protocols, guidelines and NMC Code of Conduct.
Provide nursing service/treatments to patients independently or in participation with general practitioners with reference to aforesaid agreed protocols and guidelines.

Evaluating and adapting care using evidence based practice, in consultation with colleagues as a forum for team education, development and audit.

Ensuring all specimens are checked and sent to laboratory as required
You will be fully conversant in QOF, read coding and uphold strategies to provide a holistic combined healthcare for the patient and demonstrate a willingness to participate in research/audit within the practice team.
You will be aware of DES/LES policies and apply and produce, alongside the Practice Manager, protocols that reflect the guidelines that are workable in the practice.
Participate in the education of students of all disciplines.
To be responsible for the efficient running of the treatment room and equipment and to ensure the clinical areas are at all times safe for staff and patients.
Adhere to strict Infection Control in line with guidelines and policies.
Advise patients and relatives/carers on general health care and ailments with referral to GPs as necessary.
Administrative responsibilities
To maintain accurate, comprehensive records of all consultations and treatments in the patients notes, both written and computerised, awareness of QOF, read codes and alerts needed for standardised care.
All associated nursing administration e.g. Vaccine forms, Travel Health Questionnaires, protocol review and assisting Reception with queries.
To access and file microbiology/cytology results.
Manage clinics effectively and contact patients to attend.
To take personal responsibility for own personal/professional development, providing sufficient evidence of the achievement of set objectives through individual performance and the review process.
Ensure accurate completion of all necessary documentation associated with patient health care and registration with the practice.
Help with collection of statistical information required for QOF & GPQC.
Attend and participate in practice and staff meetings as appropriate.
To take prompt action to defuse situations of complaints and be equipped with the knowledge to advise patients and relatives/carers about the complaints procedure.
Maintain appearance and behaviour in keeping with a member of the nursing profession and in accordance with codes of conduct.
Support and co-ordinate health promotion campaigns, i.e. flu, no smoking, healthy lifestyles, etc.
Maintain effective liaison with other agencies and staff concerned with patient care and with other disciplines within the practice, with appropriate regard to confidentiality.
Treatment Room supplies and equipment
To have working knowledge of equipment used in the surgery. Ensuring that any faulty equipment is taken out of service for repair and reported to the nursing team for information and Practice Administrator for action.
To be aware of Infection Control and updated policies.
In participation with the Practice Manager to ensure the maintenance of equipment and stock levels in relation to patient care.
To recommend medication changes/modifications in accordance with agreed protocols/guidelines and current regulations.
To regularly update knowledge and information through use of medical journals, training, courses, etc.
To maintain the cold chain and stock levels of vaccines ordered, used and needed in the vaccine fridge/fridges and to delegate the daily recordings to other appropriately trained staff members.
To be responsible for the resuscitation equipment and expiry dates on emergency trolley.
Checking all equipment returned from patients after loan, i.e. BP monitors and nebulisers.
Any other duties as reasonably requested by the Doctors, or Practice Manager, providing these duties are within the scope of the role and that the individual is trained and capable of such duty.
The role of the Practice Nurse is an evolving one and it is anticipated that the post holder will be responsible and reasonably flexible in the interpretation of the above job description.

The Haldon Unit is a specialist 10-bedded inpatient eating disorders service within Devon Partnership NHS Trust, providing assessment, treatment and recovery-focused care for adults experiencing severe eating disorders. The post holder will assist with therapeutic meal times and various ward activities.

Main duties of the job
Nursing Assistants play a vital role within the ward's structured daily programme. A significant part of the role involves supporting patients during six supervised dining room sessions each day, working alongside nursing colleagues to provide consistent therapeutic support before, during and after meals. Staff are involved in monitoring and documenting care, contributing to individual care plans, maintaining therapeutic relationships, and supporting patients through periods of distress or increased risk. - Provide substance misuse clinical leadership and management to the registered nurses and care teams.

Job summary
Applicants must be a qualified and registered Education Mental Health Practitioner (EMHP) or a qualified and registered Children and Young People Wellbeing Practitioner (CWP) with experience of working with in CYP Mental Health Services in the provision of both individual and group treatments.
Mental health professionals with post graduate experience of working with young people with emotional and mental health issues and with experience of delivering evidenced-based CBT interventions for mild to moderate mental health issues such as anxiety, low mood and behavioural problems to CYP both individually and in groups may also apply.
MK MHST works with Young people with mild to moderate mental health presentations attending school.
The purpose of the team is to
- Provide evidence-based interventions for mild to moderate mental health issues.
- Support the senior mental health lead in each education setting to introduce or develop their whole school or college approach.
- Give timely advice to school and college staff, and liaising with external specialist services, to help children and young people to get the right support and stay in education.
Main duties of the job
You will be part of an established Mental Health Support Team (MHST) created to improve access and availability of mental health and wellbeing support for children and young people and to deliver evidence based low intensity interventions into schools enabling life long aspirations. You will work alongside other EMHPs and CYPWPs, Senior Wellbeing Practitioners and Team Leaders.
The post holder will undertake assessments of children and young people and deliver evidence based low intensity group and individual interventions to children and young people with mild to moderate anxiety and depression and/or behavioural difficulties and their families and carers. You will also provide training and consultation to schools and other agencies about mental health in children and young people and support schools in identifying specific areas of need as part of the whole school approach to mental health.
Please note this post is based at Milton Keynes Headquarters which is currently situated on the hospital site. However a consultation is currently underway with staff regarding moving the Headquarters to the Open University within Milton Keynes in the new year. If this moves goes ahead as proposed the successful candidate would be based at the Open University site.

Milton Keynes Mental Health Support Teams are looking for qualified registered Mental Health Practitioners to join the established service.

MK MHST works with Young people with mild to moderate mental health presentations attending school.
The purpose of the team is to
- Provide evidence-based interventions for mild to moderate mental health issues.
- Support the senior mental health lead in each education setting to introduce or develop their whole school or college approach.
- Give timely advice to school and college staff, and liaising with external specialist services, to help children and young people to get the right support and stay in education.
Main duties of the job
You will be part of a Mental Health Support Team (MHST) created to increase access and availability of mental health and wellbeing support for children and young people and to play a key part in a team of people delivering low intensity interventions into schools and other settings in MK. You will work alongside EMHPs, CWPs, Senior Wellbeing Practitioners, Team Leaders.
You will undertake assessments of children and young people and deliver evidence based low intensity group and individual interventions to children and young people with mild to moderate anxiety and depression and/or behavioural difficulties and their families and carers. You are expected to work in schools closely with school staff to support CYP, their families and staff to foster a whole school approach to mental well-being.
This role requires flexibility and adaptability and above all, a desire to work with children and young people with mental health difficulties within an educational context.
There is professional development opportunity for applicants who have not yet completed the PG Certificate in Supervision for Children and Young People's Services or Senior Wellbeing Practitioner training. Other service specific training in a range of evidence based therapeutic modalities is also available.

Ealing Community Partners has an exciting opportunity for an experienced and highly motivated SLT to join a team of Paediatric SLT Clinical Leads, working with the Children's Services Manager to manage a large, diverse and dynamic Paediatric SLT service. This team is part of Ealing Children's Services which hosts most of the community health services for the borough. The post holder will join the leadership team and also work closely with our partners in the voluntary, education and social care sectors.
This post is a permanent position and would suit a progressive clinician who would like to further develop their leadership and management experience within a supportive team.
The post holder will work together with the other SLT Clinical Service leads to develop the three strands of the service:-
- core (health funded initial/EHCP assessment, dysphagia pathways and universal/targeted offer)
- buy in (working in mainstream schools at a universal, targeted and/or specialist level and SLT provision in EHCPs in special schools)
- Local Authority contract (universal, tribunal and SLT provision in EHCPs in mainstream schools).
Ealing Paediatric SLT Service sits within Ealing Community Partners which was created in 2019 and hosted by West London NHS Trust in partnership with CNWL NHS Foundation Trust.
Main duties of the job
You will be joining a dynamic team of SLT Clinical Service Leads. Together, you will focus on the operational management and clinical delivery of a high quality SLT service for children and young people at a Universal, Targeted and Specialist level, across a range of community sites and educational settings.
You will jointly plan services for children and young people with SEND with stakeholders such as Early Start SEND, as well as working closely with the Local Authority to roll out Ealing's local SEND reforms plan (including Expert at Hands) and Best Start in Life initiative.
You will be involved in ongoing service transformation including the review of a revised core service offer and the commencement of a new Local Authority contract.

Job summary
CNWL Brent and Harrow Mental Health Support Teams are recruiting for an exciting opportunity to join one of these teams as a CYP_PT CBT Therapy Trainee. This is part of the NHS England (NHSE) funded Programme in partnership with Anna Freud, and Kings College London and UCL.
The post-holder will be training on the Children & Young People Mental Health (CYP-MH/CYP IAPT) Therapy Course and carrying out clinical work in our service to complement their studies and use their newly developed skills. We are seeking highly motivated and enthusiastic mental health practitioners to work and train within our service.
The training post will equip the post-holder to provide interventions for children, young people or families using evidence-based intervention consistent with the course they are attending.
This is a fixed-term appointment for the period specified. Completion of the contract does not, by itself, guarantee renewal or further or permanent employment.
Main duties of the job
The successful candidates would be working within CNWL Mental Health Support Team - Brent or Harrow.
The MHST works with young people age 5 - 18 with mild to moderate mental health presentations attending school/education settings.
The purpose of the team is to:
- Provide evidence-based interventions for mild to moderate mental health issues.
- Support the senior mental health lead in each education setting to introduce or develop their whole school or college approach.
- Give timely advice to school and college staff, and liaising with external specialist services, to help children and young people to get the right support and stay in education.
The role will involve clinical work in school settings as appropriate to meet the requirements for the training course.
The post holder would attend MHST team meetings, group and 1:1 supervision and be part of the team.

Oak Tree Ward in Hillingdon Part of CNWL is looking for compassionate and enthusiastic Band 5 RMN to work on our Older Adults Mental Health In-patient ward based at The Woodland Centre. We can boast excellent learning opportunities and further development both in physical and mental health conditions.
We were also shortlisted for the 'coveted Nursing Times Team of the Year award in 2021'.
We care for older persons with enduring mental health conditions, long-term physical health conditions. Our dynamic MDT comprises of Medical Doctor; Nurses; OT's; Psychologies; Wellbeing support officers and collaboration with outside Agencies. This provides optimum opportunities for both newly qualified and experience RMN's with interest in older adult mental health services.
Main duties of the job
Our Band 5 nurses work closely with the people who use our services, their families, friends and carers and play an important role in contributing in how people experience our services. Keeping the people who using our services as safe as possible through the use of sound clinical skills, effective risk assessments and care planning.

Ensuring that the people using our services have a valuable experience by respecting, empowering and working in partnership with people and their families during the recovery process.

Main duties of the job
As a Senior Mental Health Practitioner, your responsibilities will vary. You will
- Responsible for assessment and ongoing management of our patients mental health needs.
- Work collaboratively with the multi-disciplinary team as an autonomous practitioner managing a caseload of patients, assessing their physical, social and psychological needs to develop a holistic health care plan.
- Contribute to our nursing strategy and vision to; Provide outstanding evidence based care to our patients, seeking out, listening to and acting on their feedback, so that care is personalised and informed by what matters to them.

Main duties of the job
As a Senior Substance Misuse Nurse, your responsibilities will vary. You will
- Work as part of a multi-disciplinary team, prioritising patient care to develop individualised packages of care through comprehensive assessment and recovery planning
- Deliver a range of treatment options including clinical and psychosocial interventions in both a group and 1-1 setting with recovery at the centre of our healthcare wellbeing model
- Be able to carry out comprehensive assessments, initiate and review individual recovery plans and confidently communicate the full range of treatment options available to service users.
- Monitor therapeutic responses to medication and potential side effects and liaise with prescribers. You will work with individuals, on a 1-1 basis, providing advice and information
- Support in group work delivery and liaise with prescribers and Recovery Practitioners on clinical interventions. You will occasionally be required to support dispensing
- Have an understanding and commitment to recovery and a person centred approach.
