Job summary
We are seeking an experienced and motivated Practice Nurse with either a level 6 diploma or have a post grad certificate in a long term condition to join our clinical team at Broomfield Park Medical Centre. This role offers an opportunity for a skilled nurse to work within a supportive general practice environment, delivering a broad range of nursing services to our registered patient population in Spon End area of Coventry.
The successful candidate will work alongside GPs and the wider practice team to provide planned and preventative care, support patients with long-term conditions and contribute to improving health outcomes within our community.
This is a varied role requiring clinical expertise, good judgement and the ability to work independently while maintaining effective communication with colleagues and patients. The ideal candidate will be committed to delivering excellent standards of nursing care and supporting the continued development of primary care services.

Main duties of the job
The Practice Nurse will provide a range of clinical services to patients registered with Broomfield Park Medical Centre, working as part of the wider healthcare team to deliver safe and effective care.
The post holder will undertake nursing assessments, develop care plans and support the management of patients with long-term conditions, including diabetes, respiratory disease, cardiovascular disease and hypertension. They will deliver preventative healthcare services including immunisations, cervical screening, NHS health checks and health promotion advice.

The following are the core responsibilities of the Phlebotomist in delivering health services. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels:
a. Obtaining patient consent for the collection of a blood sample
b. Performing venepuncture in accordance with practice protocols
c. Maintaining accurate patient records on the clinical IT system and ensuring specimens are ready for onward transportation
d. The correct labelling, packaging and storing of samples

Senior Clinical Pharmacist - Surgical Specialties ICU and Thoracic
1 WTE (37.5hrs/week) - Fixed-term for10months
An exciting new opportunity has arisen for a highly motivated and enthusiastic clinical pharmacist with excellent clinical and interpersonal skills to support a high-quality service to the Surgical Specialist Division based at Westmoreland Street. The post is full-time, as part of the wider UCLH Pharmacy team.
You will work as full time within the Surgical Specialties multidisciplinary team (MDT), delivering the highest quality service for both inpatients and outpatients, alongside the ICU, Thoracic and Urology consultants, clinical fellows, clinical nurse specialists, dietitians and other healthcare professionals.
UCH at Westmoreland Street offers comprehensive care for a spectrum of Urology and Thoracic services and is split into six sub-groups. The teams work closely together, along with other services such as the Assisted Conception Unit and Uro-radiology. Where possible, you will work to develop and enhance our existing systems, particularly where efficiency and productivity can be improved through our Electronic Health Records System, Epic, and the development of GP Connect.
You will work as part of our broader clinical pharmacy team and contribute to local and national initiatives to develop and enhance the pharmacy service.
Main duties of the job
Your duties will include collaborating closely with the Urology, Thoracic and ICU providing specialist advice to optimise patient care. You will use your non-medical prescribing skills in both the inpatient and outpatient setting to optimise the prescribing of specialist and supportive care medications.
The successful candidate will be joining the largest and most prestigious thoracic & urology department in the UK. We are a highly specialist team, seeing referrals from across the country and internationally. As a centre for excellence and research, you will have an opportunity to contribute to service development and quality improvement projects within urology and thoracic surgery teams.
Additional management and leadership responsibilities for the Surgical Specialties Division include governance, medicines expenditure analysis and reporting, audit and service evaluation, multi-professional education and training, and other strategic projects as required. You will also represent the UCLH Pharmacy Department at directorate and divisional clinical governance meetings

We have an exciting opportunity foramidwife who is passionate about physiological birth and supporting women to birth at home to join our Team of experienced home birth midwives. We are Artemis Home Birth, an established continuity team for women and their families who are planning home births.We provide all our care at home from booking throughout the antenatal, intrapartum and postnatal periods. We have a named consultant obstetrician and are supported by the wider MDT team including our specialist and consultant midwives. You will be a B6 midwife with significant experience of all aspects of midwifery care.You will need to be a confident and competent autonomous practitioner and able to demonstrate understanding, kindness and compassion for a caseload that includes women choosing to birth outside of guidance.You will be supported by the Team at every step of your journey. We'd like you to be NIPE trained and have NLS certification but can support you to gain these if not yet achieved.You will need to be a car owner/driver.
Main duties of the job
Some of the key duties of the job include:
o Fundamentally required to undertake all clinical activities that are required of a midwife
o Responsible for the assessment of care needs, the development, implementation and evaluation of programmes of care in accordance with best practice

Montague Medical Practice is looking to recruit a Practice Nurse for 20 hours per week.

Job summary
We have an exciting opportunity for a Lead Clinical Pharmacist to take on a new leadership roles within our Acute and Emergency village at the Aintree site.
You will provide senior clinical expertise to the Acute and Emergency village, while offering clear direction, organisation, and strategic oversight to the wider team. Stapenhill Medical Centre is currently looking for a receptionist to join our existing Patient Services Team. COAST (Crisis Outreach Acute Support Team) is seeking a Consultant Child & Adolescent Psychiatrist to help lead intensive home treatment and crisis outreach for young people across Mid Sussex. The successful applicant must have previous experience using EMIS in a GP practice setting. Applicants without previous EMIS experience will not be considered.

The post-holder will be responsible for providing an efficient and professional reception and administrative service to patients and the practice team. Duties will include:
Booking, cancelling and rearranging patient appointments via EMIS, telephone, face to face and email
Using EMIS effectively to access and update patient records and undertake relevant administrative tasks
Processing prescription requests in accordance with practice procedures
Handling patient enquiries and requests appropriately and efficiently
Responding to telephone calls, emails and face-to-face enquiries
Liaising with GPs, practice staff, hospitals and other community healthcare providers
Handling requests from patients and GPs and ensuring these are directed to the appropriate member of the team
Signposting patients to appropriate services where required

Job summary
We are looking for an enthusiastic and motivated Pharmacy Technician to join our Primary Care Network (PCN) pharmacy technician team.
This is a varied role supporting the safe, effective and efficient use of medicines across the PCN. Main duties of the job

Main duties of the job
The service we provide is in line with NICE Evidenced Based Treatment for those children and young people. You will provide high quality specialist clinical assessments, including relevant psychiatric and physical investigation to children, young people and their families in a range of settings, and work collaboratively with other members of the team to plan and deliver appropriate individually tailored treatment plans.
You will work as part of a friendly and supportive multidisciplinary team which includes nurses, psychologists, systemic family therapists and child psychotherapists. This post will work alongside other Consultants and Specialty Doctors who work in Sussex CHYPS.
You will also be part of a Trust wide consultant network where there are opportunities to liaise with colleagues in other posts throughout the Trust and develop peer networks. You will also see consultant peers at regular division meetings in the locality and at clinical governance meetings.

Job summary
The STEP team is a stand- alone community service offering support and management of First Episode Psychosis (FEP) to 18-65 year olds for up to 3 years following first episode. You will work as part of a comprehensive MDT including another consultant (0.4), an associate specialist (LTFT), ST (part time as split post with primary care mental health team), advance practitioner, mental health practitioners, psychologists, occupational therapists and admin support. STEP offers early treatment by means of a range of pharmacological, psychological and social interventions to adults who have made a transition to psychosis and are in the early stages of their illnesses. Care is provided according to PIG guidelines for FEP, using the model of care coordination to ensure that patients have access to the best treatment options available. Particular areas of focus are improving social and occupational outcomes in psychosis through vocational work, befriending, and support groups; and working with families to offer psychoeducation, carer support and family interventions.

Main duties of the job
The service we provide is in line with NICE Evidenced Based Treatment for those children and young people. You will provide high quality specialist clinical assessments, including relevant psychiatric and physical investigation to children, young people and their families in a range of settings, and work collaboratively with other members of the team to plan and deliver appropriate individually tailored treatment plans.
