Job summary
We are looking to recruit a Band 4 exercise technician to work with the countywide Pulmonary Rehabilitation Service. This is a part time 0.6 WTE. Days are fixed: Tuesday, Thursday and Friday.
The Pulmonary Rehabilitation team provide a comprehensive exercise and education programme across Worcestershire. The successful candidate would be working predominantly within the Redditch programme, although some cross county cover may be needed at times.
A full induction programme would be given to any successful candidate.
Main duties of the job
- To assess, plan, treat and review the exercise needs of patients referred for Pulmonary Rehabilitation with an underlying chronic respiratory disease, providing high quality, evidence-based exercise interventions
- To assess service users suitability for exercise, adhering to guidelines
- To carry out maximal functional exercise testing with patients with chronic respiratory disease pre and post PR; monitoring and assessing oxygen levels, BP, heart rate, Pre and post PR.
- To facilitate patients to set personal SMART goals and screen for anxiety and depression (referring on where necessary) and quality of life measures using questionnaires.
- To contribute in the educational support of patients, carers and MDT teams on all aspects of Pulmonary Rehab.
- To assist specialist practitioners in the planning, co-ordination and delivery of care to patients receiving Pulmonary Rehabilitation
- To manage a daily caseload of patients working independently without direct supervision
- To ensure maintenance of pulmonary rehabilitation databases to enable on-going service review

Main duties of the job
To act as a prescription clerk, dealing with receiving requests from patients and/or representatives and allied professionals for prescriptions to fulfil medication requirements. Requests will be channelled via various sources ( e-mail, on-line). Ensuring patients are up to date with reviews and assisting and directing patients in booking the appropriate appointments to ensure their care is ongoing and that their health is reviewed at the appropriate time and with the appropriate clinical follow-up. The Older Adult Community Mental Health team operates from Benet Building, Belmont, Hereford with patient contact being primarily in the community or via outpatient clinics. Inpatients wards are located in the Stonebow Unit, Hereford.
You will be delivering ECT alongside other consultants and join 1:11 on-call rota. The overall sessional pay with on-call commitments will be 11 PAs plus 3% plus 1%. You will have a dedicated office and secretary.

Main duties of the job
Main duties of the role include:

Job summary
The post holder is a Clinical Pharmacist within Worcester City PCN, working across general practice teams to support the safe, effective and cost-efficient use of medicines. The role focuses on improving patient outcomes through medicines optimisation, delivering local prescribing priorities, including the Quality Prescribing and Medicines Service (QPMS), and supporting national targets such as the Quality and Outcomes Framework (QOF).
Working as part of a multidisciplinary team, the pharmacist provides expert advice on prescribing and medication queries, undertakes medicines reconciliation following transfers of care, and promotes safer prescribing practices. Responsibilities include conducting structured medication reviews, particularly for high-risk patients and those with complex medication regimes, and supporting chronic disease management through patient clinics.

- Substantial experience working in primary care/general practice, with a good understanding of the general practice environment, systems and multidisciplinary working.
- Minimum of 2 years post graduate experience in pharmacy as demonstrated within a practice portfolio.
Desirable
- Member of the Royal Pharmaceutical Society (RPS).
- Independent Prescribing qualification with demonstrated prescribing portfolio.
- Experience of working with QOF, QPMS, DES, LES or other primary care prescribing and medicines optimisation initiatives.
