Main duties of the job
The successful candidate will provide high-quality, patient-centred care to our registered and temporary patient population and will manage a varied and interesting clinical caseload.
Responsibilities include:
Face-to-face surgery consultations
Telephone and remote consultations
Home visits where clinically appropriate
Reviewing and signing repeat prescriptions
Managing clinical correspondence, referrals, results and associated paperwork in a timely manner
Making autonomous clinical decisions regarding presenting problems
Assessing and managing patients with undifferentiated and undiagnosed conditions
Identifying disease risk factors and early signs of illness
Developing personalised care plans in partnership with patients
Referring patients appropriately to community and secondary care services
Working collaboratively with the wider multidisciplinary team

Job responsibilities
Job summary:
The post-holder will manage a caseload and deal with a wide range of health needs in a primary care setting, ensuring the highest standards of care for all registered and temporary patients.
Clinical responsibilities:
In accordance with the practice timetable, as agreed, the post-holder will make themselves available to undertake a variety of duties, including surgery consultations, telephone consultations and queries, visiting patients at home, checking and signing repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion
Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation
Assessing the health care needs of patients with undifferentiated and undiagnosed problems
Screening patients for disease risk factors and early signs of illness
Developing care plans for health in consultation with patients and in line with current practice disease management protocols
Providing counselling and health education
Admitting or discharging patients to and from the caseload and referring to other care providers as appropriate
Recording clear and contemporaneous consultation notes to agreed standards
Collecting data for audit purposes
Compiling and issuing computer-generated acute and repeat prescriptions (avoiding hand-written prescriptions whenever possible)
Prescribing in accordance with the practice prescribing formulary (or generically) whenever this is clinically appropriate
In general, the post-holder will be expected to undertake all the normal duties and responsibilities associated with a GP working within primary care.
Other responsibilities within the organisation:
Awareness of and compliance with all relevant practice policies/guidelines, e.g. prescribing, confidentiality, data protection, health and safety
A commitment to life-long learning and audit to ensure evidence-based best practice
Contributing to evaluation/audit and clinical standard setting within the organisation
Contributing to the development of computer-based patient records
Contributing to the summarising of patient records and read-coding patient data
Attending training and events organised by the practice or other agencies, where appropriate.

In the course of seeking treatment, patients entrust us with, or allow us to gather, sensitive information in relation to their health and other matters. They do so in confidence and have the right to expect that staff will respect their privacy and act appropriately
In the performance of the duties outlined in this job description, the post-holder may have access to confidential information relating to patients and their carers, practice staff and other healthcare workers. They may also have access to information relating to the practice as a business organisation. All such information from any source is to be regarded as strictly confidential
Information relating to patients, carers, colleagues, other healthcare workers or the business of the practice may only be divulged to authorised persons in accordance with the practice policies and procedures relating to confidentiality and the protection of personal and sensitive data.
Health & safety:
The post-holder will implement and lead on a full range of promotion and management of their own and others health and safety and infection control as defined in the practice health & safety policy, the practice health & safety manual, and the practice infection control policy and published procedures. This will include (but will not be limited to):
Using personal security systems within the workplace according to practice guidelines
Awareness of national standards of infection control and cleanliness and regulatory / contractual / professional requirements, and good practice guidelines
Providing advice on the correct and safe management of the specimens process, including collection, labelling, handling, use of correct and clean containers, storage and transport arrangements
Correct personal use of Personal Protective Equipment (PPE) and ensuring correct use of PPE by others, advising on appropriate circumstances for use by clinicians, staff and patients.
Management of the full range of infection control procedures in both routine and extraordinary circumstances (e.g. pandemic or individual infectious circumstances)
Hand hygiene standards for self and others
Managing directly all incidents of accidental exposure
Management and advice relating to infection control and clinically based patient care protocols, and implementation of those protocols across the practice
Active observation of current working practices across the practice in relation to infection control, cleanliness and related activities, ensuring that procedures are followed and weaknesses/training needs are identified, escalating issues as appropriate to the responsible person
Identifying the risks involved in work activities and undertaking such activities in a way that manages those risks across clinical and patient process
Making effective use of training to update knowledge and skills, and initiate and manage the training of others across the full range of infection control and patient processes
Monitoring practice facilities and equipment in relation to infection control, ensuring that proper use is made of hand-cleansing facilities, wipes etc., and that these are sufficient to ensure a good clinical working environment. Lack of facilities to be escalated as appropriate to the responsible manager
Safe management of sharps use, storage and disposal
Maintenance of own clean working environment
Using appropriate infection control procedures, maintaining work areas in a tidy, clean and sterile and safe way, free from hazards. Initiation of remedial / corrective action where needed or escalation to responsible management
Actively identifying, reporting, and correcting health and safety hazards and infection hazards immediately when recognised
Keeping own work areas and general/patient areas generally clean, sterile, identifying issues and hazards/risks in relation to other work areas within the business, and assuming responsibility in the maintenance of general standards of cleanliness across the business in consultation (where appropriate) with responsible managers
Undertaking periodic infection control training (minimum twice annually)
Correct waste and instrument management, including handling, segregation, and container use
Maintenance of sterile environments
Demonstrate due regard for safeguarding and promoting the welfare of children.

Job summary
The Trust is seeking a consultant psychiatrist to join Walton CMHT based at Norris Green Hub, Falklands Approach, L11. The vacancy has arisen as a result of the previous substantive post holder retiring. This post is one of three CMHT general adult consultant psychiatrist posts (the other 2 are also 1 WTE each) within Norris Green hub.
Norris Green Hub serves a catchment area of around 120,000, covering the Mid to North areas of Liverpool for adults of working age. The current post encompasses the Walton catchment areas. This is a demographically mixed area overall, but with some areas of social deprivation and some drug and alcohol- related difficulties in an urban environment.
The post holder will carry no responsibility for inpatients.
The staff at Norris Green Hub are organised in three distinct CMHTs. This CMHT consists of:
1 whole time equivalent (WTE) consultant psychiatrist (this post)
1 WTE higher / core Psychiatric Trainee.
0.5 WTE dedicated medical secretary, Band 4 and a full time band 3 shared with the other 2 teams.
1 Band 7 team leader
5 WTE community psychiatric nurses (band 6 and band 5)

Other duties

Work programme
It is envisaged that the post holder will work 10 programmed activities over 5 days. The Trust is seeking for a part time Consultant in Psychiatry (0.6WTE) in North Mersey Crisis Resolution Home Treatment Team (CRHT) covering Liverpool & Sefton. Job summary
As a Consultant Old Age Psychiatrist working for Mersey Care NHS Foundation Trust you will be responsible for ensuring that our service users' needs are met through the delivery of high quality care.
As a leader of the older adults outpatient multi-disciplinary team at Hartley Hospital, you will work in a modern mental health facility for older persons, providing clinical care to service users who have been referred for specialist cognitive assessment as an outpatient and for assessment and management of functional mental disorders. You will work closely with a wide range of clinical and professional disciplines in order to deliver holistic care by completing assessments of new referrals to the team, making formulations and managing their treatment in order to optimise the quality of life of the service users under your caseload.
Main duties of the job
The post holder will be the Responsible Clinician for patients in their caseload within the older adults Community Mental Health Team. There are allocated GP surgeries for each caseload within the service and these are allocated to ensure equity in terms of number of referrals. In line with New Ways of Working, the post holder will provide a consultative, supervisory and supportive role to the multidisciplinary team's work.
They will take an active part in the clinical work of the team and a lead role in the development of the service. The post holder will be expected to provide leadership to the Team whose role is to provide assessment, management and support to older adults with organic and functional mental disorders. Practitioners within the team will support access to services in a timely manner, as well as ensure arrangements are in place to enhance smooth pathways between primary care and secondary services.
The post holder will be enabled to work in a consultative manner, being accessible to team members, GP and primary care staff, patients and carers, for the provision of expert advice and interventions in a timely manner. The post holder will operate within multi-disciplinary integrated and co-located teams, which including Health staff and Social services staff. The post holder will be expected to be part of the allocation procedure within the team and be responsive and flexible to the needs of the team.

The Greenwich Training Hub ARRS Support Officer will provide dedicated operational, workforce development, programme management and administrative support to the ARRS Lead and Greenwich Training Hub. An exciting opportunity has arisen for a Band 3 Clinical Support Worker within Frailty Same Day Emergency Care (FIT SDEC).We are looking for an enthusiastic and motivated individual who is keen to take on a nursing support role within a busy and fast paced Frailty SDEC unit.

You will have the opportunity to develop your clinical skills and receive structured appraisal, support, mentorship and training.
As a Clinical Support Worker you will:

- Perform clinical skills ie. venepuncture, ECG and cannulation
- Record patient information.
- Provide basic patient care and document that care in the patient records.

Neonatal Services at UHMBT are provided from both Furness General Hospital and Royal Lancaster Infirmary sites and are part of the North West Neonatal Operational Delivery Network.
The Special Care Unit at Furness General Hospital is integrated into the South Lakes Birth Centre and has a cot capacity of 4. We provide care to babies and their families from 32 weeks gestation but also provide stabilisation for sick and preterm babies born below 32 weeks, in preparation for transfer by the regional transport team.
The Local Neonatal Unit at the Royal Lancaster Infirmary (RLI) has 1 intensive care cot, 2 High Dependency cots and 7 Special Care cots, caring for babies from 28 weeks gestation and provide stabilisation for sick and preterm babies born below 28 weeks in preparation for transfer by the regional transport team
Both sites have a well developed transitional care service which enables babies and their mothers to stay together when requiring additional support.

Main duties of the job
The post holder will be a Registered Children's Nurse, Registered Adult Nurse. If not already completed, you will be expected to complete the Neonatal Qualified in Speciality course.
You will be based on the Special Care Unit at Furness General Hospital as your main place of work, however you may be required to cover the RLI Local Neonatal Unit at the Royal Lancaster Infirmary.
The post holder will work as part of the multidisciplinary team in the assessment, planning and implementation of care to the neonate, through the delivery of evidence based practice and be responsible for accurate record keeping.

This post is designed for doctors pursuing the Certificate of Eligibility for Specialist Registration (CESR) in Community Sexual and Reproductive Health or HIV and Sexual Health providing a structured programme of supervised clinical practice, leadership, governance, education and service development. We are seeking candidates with a realistic chance of achieving specialist registration within 18 months, based on clinical experience already gained. The successful candidate will spend 60% of their time (6 PAs) working within the multidisciplinary TransPlus service and 40% of their time (4 PAs) undertaking activities specifically designed to support acquisition of CESR evidence in their specialty.
The post offers a unique opportunity to develop advanced expertise in gender medicine within one of the UK's leading specialist gender services, while simultaneously progressing towards CESR through structured clinical exposure, educational supervision, audit, research, quality improvement, leadership and teaching opportunities.
Main duties of the job
The Fellow will provide care for trans and non-binary people accessing gender health, sexual health and reproductive healthcare services. They will work alongside consultants, specialty doctors, resident doctors, nurses, psychologists and allied health professionals to deliver holistic, evidence-based care and contribute to service innovation and clinical excellence.
The fellow will work alongside junior and senior members of staff to help manage those with gender dysphoria and sexual health needs. They will assist with the prescribing of hormone therapy, management of results, and liaison with GPs and other associated health professionals. They will be responsible for the formulation of treatment plans for those diagnosed with gender dysphoria. The service refers for the range of services described in the NHS England service specification and agrees and co- ordinates treatment plans specifically tailored to address each service-user's goals and clinical needs.
These services include:
1. An assessment, diagnostic, and person-specific treatment planning service, involving the client in every aspect of their care
2. Endorsement for surgical gender affirming surgeries
3. Referral for facial depilation
4. Referral for speech and language therapy
5. Advice and advocacy to support bio-psycho-social adjustment

Job summary
We are seeking an experienced and highly organised GP Receptionist to join our busy and friendly practice team.
The successful candidate will have previous experience in a GP practice or NHS healthcare setting and demonstrate excellent administrative, communication, and organisational skills. Experience handling referrals, managing clinical systems, and working within NHS pathways is essential.
Main duties of the job
The post holder will be primarily responsible for carrying out reception duties, including:
Managing incoming calls and patient enquiries
Booking GP appointments and signposting patients appropriately
Front-of-house reception cover
Processing registrations and general administrative tasks
Prescription management
Managing waiting lists
Liaising with GPs and other healthcare staff
Supporting data collection, reporting, and audit requirements
Supporting management when required
About us
We are a two-partner GMS (General Medical Services) teaching practice with a steadily growing patient list of approximately 4,370 patients. We pride ourselves on delivering excellent clinical care with a strong ethos of service to our local community. This commitment is reflected in consistently high QOF achievement, a low level of complaints, and regular positive feedback from patients. We were rated GOOD in all areas at our most recent CQC inspection (08.06.22).
In addition, we also deliver the Nottinghamshire Community Dermatology Service. The service is commissioned by the local Integrated Care Board and is currently expanding further into Nottinghamshire. The dermatology service operates from within our GP surgery and provides high-quality, community-based specialist care.
We are also a recognised teaching practice, supporting medical students from the University of Nottingham, PA students from Sheffield Hallam University, and De Montfort University, Leicester. Personal development and ongoing training for the entire practice team remain a longstanding priority.

Main duties of the job
The purpose of this job will be to support the day-to-day activities of the department in co-operation with the rest of the surgical staffing and the successful candidate will participate in all aspects of the unit's clinical work including clinics, operating sessions, and ward rounds.
The successful candidate will also be required to contribute to the emergency general surgical workload of the team.
The post would be ideally suited to a Senior House Officer looking to gain specialist General Surgery experience, and you would be expected to participate in all the other on-going activities of the department including audit, research projects and to attend various departmental meetings and teaching sessions (MDTs, Audit meetings, M&M, ward, and formal teaching).
Within the hours contracted, the successful candidate is expected to be flexible in the exact hours worked each day.
All Medical and Dental Staff employed by the Trust are expected to comply with hospital health and safety policies, clinical governance, and other risk management policies.

It has been agreed between the professions and the Department that while juniors accept that they will perform such duties, the Secretary of State stresses that additional commitments arising under the sub-section are exceptional and in particular that juniors should not be required to undertake work of this kind for prolonged periods or on a regular basis.
