These posts are available from 4 November 2026. Start dates after this time will also be considered. Applicants are required to have fully completed the FRCA or equivalent qualification and have a minimum of 5 years' experience in anaesthesia and intensive care in a structured training program.
The clinical duties would cover the Stage 2 and 3 (sub-specialties Airway and Neuro-anaesthesia available) curriculum of the Royal College of Anaesthesia (RCOA). You will undertake a recognised MSc in Advanced Practice and develop the capabilities outlined within the NHS England Multi-professional Framework for Advanced Practice in England (2025) across the four pillars of advanced practice: Clinical Practice, Leadership and Management, Education, and Research. Throughout your training, you will receive structured clinical and educational supervision from the Advanced Practice leadership team and designated medical supervisors.
On successful completion of the MSc advanced practice programme, demonstration of the required advanced practice capabilities, achievement of all specialty-specific competencies, and successful presentation of your portfolio through the Trust's Advanced Practice governance and expert review process, you will progress to a substantive Band 8a Advanced Practitioner role.

This post would suit trainees wishing to pursue a career in transplantation, general surgery or urology. It provides an excellent opportunity to expand knowledge in pre/post-operative management of surgical patients in a busy transplant environment and to develop basic general surgical/vascular/urological technical skills. Those wishing to improve their academic portfolio will be supported to do so.
The post will incorporate ward and on-call commitments as well as theatre and outpatient commitments. There will be an opportunity to work on Audit or a Quality Improvement project. The post will therefore suit individuals who wish to develop these areas. The postholder will be supervised by a Transplant Surgery Consultant as well as the Transplant medical team with whom they will be working closely.
Main duties of the job
Clinical:
The post holder will, along with other junior colleagues be responsible for:
a. Diagnosis and treatment of patients of the Guy's & St Thomas' NHS Foundation Trust in such hospitals, health centres or clinics or other premises as required
b. Continuing clinical responsibility for the patients in your charge, allowing for all proper delegation to, and training of, your staff.
Proposed timetable:
The exact time table would be determined after discussion with Clinical Lead for Transplant to consider the educational needs of the post holder along with those of other junior surgical colleagues. However broadly speaking it would involve the following:
- Participation in Consultant led ward rounds
- Participation in at least one transplant or surgical clinic a week
- Attendance at 2 in-patient theatre sessions per week
- 1 session per week for research / quality improvement / audit
Clinical sessions will involve pre- and post-operative assessment of dialysis access, transplant urology and general surgery patients.
On call commitment:
The post holder will undertake a 1 in 10 (or higher) on call rota. This will be resident on call. This will involve covering dialysis and transplant patients based at Guy's & St Thomas sites and may involve travel for organ retrievals. Please note the banding of the rota is subject to change and will be confirmed at the interview stage.
About us
Department Information
The Department of Nephrology and Transplantation at Guy's is one of the largest in the UK, providing kidney care to South East London, neighbouring areas of Kent and the Channel Islands. In addition, we receive a significant number of complex referrals from a much wider geographical area. We provide a comprehensive range of services for this population, from management of mild kidney disease through to acute kidney injury (AKI) and the full range of modalities of renal replacement therapy. We perform around 300 transplants each year, have the largest living kidney donor programme in the country and are a supra-regional centre for both combined kidney/pancreas and pancreas alone transplantation. We have the country's largest antibody incompatible transplant programme. We performed the first robotic transplants in the UK and have world-class expertise in vascular access and transplant urology.
We offer a full range of peritoneal and haemodialysis therapies both at home and through a network of satellite units. We are nationally recognised for quality of care, innovation in clinical practice and service delivery, and for our close functional and geographical integration with the research programmes of both the MRC Transplantation Centre and the Comprehensive Biomedical Research Centre.

Reference: E0281-26-0154
Job summary
This is an exciting opportunity for an experienced and compassionate Diabetes Specialist Nurse to join Modality East Surrey Medical Practice and help shape the continuing development of specialist diabetes care within primary and community care.
We support approximately 45,000 to 46,000 patients across our sites in Horley, Smallfield and Townhill in Caterham. You will join an established multidisciplinary team and work closely with GPs, nurses, healthcare practitioners, clinical pharmacists, healthcare assistants, community services and Consultant Endocrinologists.
Our existing Diabetes Specialist Nurse is moving on to pursue a different venture, creating an opportunity for someone new to build on the strong foundations already established while bringing their own expertise and ideas to the service.
The role combines autonomous specialist practice with clinical leadership, education and service improvement. You will support adults with Type 1, Type 2 and complex diabetes, including patients requiring treatment intensification, insulin management, diabetes technology or coordinated multidisciplinary care.
Previous general-practice experience would be advantageous but is not essential. We welcome applications from experienced diabetes nurses working in community, integrated or secondary-care diabetes services. Appropriate induction, supervision and development will be provided to support a successful transition into primary care.
Main duties of the job
The Diabetes Specialist Nurse will manage an appropriate specialist caseload and provide expert assessment, treatment, care planning and follow-up for patients whose needs extend beyond routine long-term-condition review.
You will:
- Assess and manage adults with Type 1, Type 2 and complex diabetes within your professional competence and agreed clinical pathways.
- Develop clear, personalised and evidence-based management plans, with appropriate monitoring, follow-up, safety-netting and escalation.
- Initiate, optimise and monitor diabetes treatments within your competence and prescribing arrangements.
- Support insulin initiation, dose adjustment and complex insulin management.
- Review and support the appropriate use of contemporary diabetes treatments, including GLP-1-based therapies and SGLT-2 inhibitors.
- Interpret capillary glucose profiles, flash glucose monitoring and Continuous Glucose Monitoring data.
- Run or contribute to specialist clinics involving CGM, complex insulin management and diabetic-foot assessment.
- Support patients using insulin-pump therapy within demonstrated competence and agreed shared-care pathways.
- Recognise and respond appropriately to hypoglycaemia, hyperglycaemia, sick-day risks and potential diabetic emergencies.
