Job summary
Mayflower Medical Group is looking to recruit a Practice Nurse to work 37.5 hours per week. The surgery has 4 branch sites in addition to our main site. You will be given a base site but occasional travel across to the branch site may be needed.
The surgery has a team of Practice Nurses, NA's and HCAs who work alongside our team of GPs, ANPs and PCN colleagues to provide excellent patient care.
We are looking for applicants with experience in Chronic Disease Management to provide effective Long Term condition management to our 34,000 patients.
The organisation is also committed to providing and supporting staff development via internal and external training.
Main duties of the job
Provide nursing care including wound management, vaccinations, phlebotomy and chronic disease management. Conduct health assessments and cervical screenings. Implement treatment plans for acute and chronic conditions in collaboration with GPs and other healthcare professionals.
Provide patient education on health management, disease prevention and lifestyle changes, and participate in public health and community initiatives promoting wellness.
Manage chronic diseases including diabetes, asthma and COPD in line with clinical protocols and registers. Regularly review and monitor patients, adjusting care plans as required.
Administer vaccinations in accordance with national immunisation schedules, ensuring follow-up and accurate records.
Maintain accurate, detailed and confidential patient records in line with GDPR requirements. Use SystmOne effectively to record, manage and retrieve patient information.
Work collaboratively with GPs, Lead Nurses and the wider multidisciplinary team to provide coordinated, high-quality care. Participate in continuous professional development to maintain and update clinical knowledge.
Contribute to clinical audits and quality improvement activities, using patient and staff feedback to enhance practice and outcomes. Maintain high standards of infection prevention and control, including appropriate PPE use and hand hygiene.

Job summary
An outstanding individual is sought to fulfil the role of Training Programme Director for Core Surgery for London.
The role of Training Programme Director is to work with and support the Postgraduate Dean in leading the delivery of a wide range of functions within their specialty, aligned to the NHS England mandate. The Programme Director will ensure that both national standards, determined by the Royal College and also guidelines and standards established within Core Surgery are met.
The successful applicant will work closely with the Head of School, other Core Surgery TPDs, LaSE team including; PGMDE Business Manager, Postgraduate Dean, and other key departments including Quality.
They will work with the Postgraduate Dean to produce doctors who will strive for the highest standards of patient-centred care and commit themselves to a lifetime of professional development.
Main duties of the job
The Programme Director will ensure that both national standards, determined by the Royal College, and guidelines and standards established within Core Surgery are met. Responsibilities will include:
- To oversee the Specialty's Regional Training Programme
- Specialty-specific matters and trainee/trainer concerns
- Recruitment to training posts and programmes
- Postgraduate programme management, including assessments, progression, rotations, support and remediation, OOP management, trainee management careers support, less than full-time training, inter-deanery transfer, academic training and other related work-streams.
The ideal candidate will be an experienced NHS Consultant with a thorough understanding of Core Surgery training across the region and who will also have strong leadership and communication skills and be able to work effectively as a member of a multi-professional team.

Job summary
We are looking for a dedicated, skilled Band 6 Radiographer to work within our team at Liverpool University Hospitals NHS Foundation Trust based at Aintree University Hospital and the North Liverpool Community sites (St Chads, Litherland town hall and Kenilworth Road).

The post-holder will deliver consultant psychiatric input to inpatients admitted to the ward per the agreed Acute Service Operational Policy. In addition, the post holder will provide clinical leadership and be actively involved in service improvement activities.
This vacancy will come with an additional acuity supplement (equivalent to 1PA) for successful applicants.

Main duties of the job
The Consultant Psychiatrist will be expected to work effectively with colleagues from other professional disciplines and deliver services consistent with the Trust policy for the Care Programme Approach. The post-holder is also expected to liaise effectively with the other general and specialist teams operating in Surrey to ensure effective handover of care when patients transfer between teams as part of the patient pathway.

This is a full-time post for a Specialty Doctor with significant experience in General and Upper GI Surgery. An exciting opportunity exists for enthusiastic and ambitious individuals to join and support the surgical team. The successful applicant will be expected to work within the Upper GI subspecialty team. The Surgical Department maintains a busy full shift emergency and general surgical commitment.
It is expected that this post will provide you with good experience in looking after acute and elective general surgical adult patients.
During the Consultants' on-call week, there will be daily Post-Take Ward Rounds and no routine clinical activities.
Almost half of the surgical admissions to the hospital are emergencies. This will provide the post holder with ample experience in the management of the acute abdomen, head injury, and common general surgical emergencies. There is excellent back up for managing critically ill patients from consultant anaesthetists and intensivists. They provide help with resuscitation and transfer of patients with major head injuries to neurosurgical units.
It is expected that you will become familiar with the pre/peri- and post-operative management of patients in the general surgical specialties and will acquire the practical skills appropriate to your level of training to support the general surgery department.

Main duties of the job

Main duties of the job
Kingston Home Treatment Team (KHTT) is a multidisciplinary, community based mental health team that aims to provide a safe and effective home based assessment and treatment service as an alternative to in-patient admission, and for early discharge from the ward in the least disruptive and restrictive environment. It works in conjunction with, and is integrated with, other local service elements. The workload is team-based and team members do not carry a specific caseload.
The Multidisciplinary team is made up of Nurses (RMN), Recovery Support Workers (RSW), Occupational Therapist (OT), Team Administrator, Peer Support Worker (PSW), Advanced Clinical Practitioner (ACP), Line Manager, Consultant Psychiatrist, Trainee Doctors and Nursing Students.
The Team is available to provide care and intervention from 08:30 to 22:30, 365 days of the year for people who are experiencing a mental health crisis of a nature or degree that would require admission in the absence of the HTT.

Sutton CAMHS are looking for an experienced and enthusiastic Systemic Family Psychotherapist to join our multidisciplinary team.
You will provide highly specialist systemic assessment and therapeutic interventions to children, young people and their families/carers, working collaboratively with colleagues to develop systemic practice across the service.

Main duties of the job
Provide highly specialised clinical services and advice within specialist area. At the time of advert exact details of speciality are being agreed by the senior pharmacy team but the details will be discussed at interview.
Provide a personal clinical and professional contribution to the clinical pharmacy service e.g. by participating on medical and other ward rounds, in clinics, counselling patients, prescribing. Clinical pharmacy describes how pharmacists use their expertise and experience to make sure that patients get the best out of their medicines. It involves proactive and direct patient care which reduces clinical and financial risk i.e. maximising clinical outcomes, minimising patient morbidity and risk of error and maximising cost effectiveness.

We recently merged our Health and Social Care teams to form an Integrated Team, offering a new model of care and innovative ways of working.The primary focus of the therapy team is to provide short-term rehabilitation in the community, supporting timely hospital discharge and preventing unnecessary admissions. This is achieved through tailored exercise programmes, equipment provision, and functional interventions that promote independence. We visit patients in their own homes, care homes, and nursing homes, as well as offering rehabilitation within the gym at Aldeburgh Community Hospital. We also run clinics, such as GLINT (Geriatric clinic) and community OP clinic.
Our team works closely with local GP practices and benefits from a wealth of experience and knowledge among its members.We are looking for a motivated and committed Physiotherapist who is passionate about delivering high-quality, patient-centred care in a community setting.

Main duties of the job
- To be an active member of a multi-disciplinary, integrated community team.
