We wish to recruit an Advanced Nurse Practitioner to join Amersham Vale Practice in Deptford, Lewisham.
We are a16,000 patient training practice based in the Waldron Health Centre, a modern, spacious facility in the heart of Deptford, North Lewisham. Deptford is a dynamic, diverse, culturally rich and rapidly changing neighbourhood with pockets of deprivation alongside new developments. We are situated directly opposite New Cross station and within walking distance of Deptford DLR, and Deptford station.
Our practice values kindness, approachability, friendliness and compassion, alongside high standards of clinical care. Our working environment is supportive and collaborative, with twice weekly tea breaks for all staff. We are a training practice and have a diverse clinical team including Advanced Nurse Practitioners and a Clinical Pharmacist. We very much work as a team and encourage and value everyone's contributions
We run a dedicated clinic for the homeless, and a service for drugs and alcohol addiction. We also have large numbers of students in our population and we have a close link with Goldsmiths University. Main duties of the job
We are a Gold standard practice for the LGBTQ+ community
You will be part of the ANP team which includes 2 other ANPs.
We are diversifying the way we work. A significant part of the role involves managing our duty desk and triage service. The successful clinician will assess online requests, prioritise clinical need and decide the safest and most appropriate care for each patient. This may include remote or face-to-face management, signposting to other services or escalation to urgent or secondary care. The role requires confident independent decision making, safe prescribing, clear documentation, effective safety netting and appropriate use of available clinical pathways and specialist advice. You will have your own designated clinics to call patients or see them face to face to address more complex health needs and also conduct regular home visits.
You should be able to see all patients including children, adults and patients with mental health challenges

Clinical Nursing Practice
The post holder will provide clinical care within a team consisting of general practitioners, advanced healthcare professionals (ANPs, clinical pharmacist), practice nurses and Healthcare assistants. Patient contact will be mainly via triage, on-call duties, face to face clinics and home visits
The post holder should be able to see all patients that present including children, adults and those experiencing mental health challenges
Carry out clinical nursing practice at an advanced level using expert clinical skills, critical judgements & knowledge to deliver appropriate and safe care.
Delivery of care in line with NSF, NICE guidelines and evidence-based care
Triage and treat patients, performing specialist assessment of their needs and make referrals as necessary to other members of the primary and secondary health care teams as appropriate.
- Co-ordinate the planning and delivery of care ensuring patients and carers/relatives are involved.
- Prescribing and reviewing medication that is appropriate to patient needs and in accordance with latest evidence-based practice, national and practice protocols and within the scope of qualified practice.
- Diagnose and manage acute and chronic conditions, including drug and non-drug-based treatment methods into a management plan.
- Request diagnostic investigations and interpret within agreed protocols e.g. x-ray, blood tests, scans, etc.

Job Summary:
The Paramedic Practitioner (PP) will act autonomously within their professional scope of practice providing care for patients face to face, telephone consultations and home visits.

Job Summary:
The Paramedic Practitioner (PP) will act autonomously within their professional scope of practice providing care for patients face to face, telephone consultations and home visits.
The post-holder will operate within their own sphere of competence and agreed acceptable limits of practice to provide expert professional clinical advice to patients, carers and colleagues, ensuring clinical safety and excellence.
The Paramedic Practitioner will need to prioritise and triage the needs of patients accordingly, making any necessary referrals for investigations in the appropriate manner.
