Job summary
We are looking for an experienced Advanced Clinical Practitioner to join our Ageing Well Team. We would also welcome applications from suitably experienced Enhanced Clinical Practitioners and Paramedics who have the relevant skills, experience and qualifications for the role.
This is an exciting opportunity to work in a developing PCN service supporting older people, including frail, housebound and care home patients across Jurassic Coast Primary Care Network. On a day-to-day basis, the role will involve acute housebound visits and care home ward rounds, providing timely clinical assessment, management plans, prescribing where appropriate, escalation and coordinated care with the wider multidisciplinary team.
We are looking for someone with a strong acute clinical background who is confident working autonomously in a fast-paced, time-sensitive environment. The successful candidate will be flexible, proactive and able to prioritise a varied caseload, while maintaining safe, high-quality and person-centred care.
This role would suit a motivated clinician who enjoys working as part of a supportive multidisciplinary team and is passionate about improving care for older people, supporting admission avoidance, continuity of care and responsive community-based clinical support.
Main duties of the job
The postholder will provide autonomous clinical assessment and management for older people, including frail, housebound and care home patients across the PCN.
Main duties will include undertaking acute housebound visits, completing care home ward rounds, assessing and triaging patients, formulating differential diagnoses and management plans, prescribing where appropriate, and escalating concerns where further input or admission may be required.
The role will involve managing a varied and time-sensitive caseload, recognising deterioration, supporting admission avoidance and working closely with GP practices, care homes, community teams, acute services, social care and other members of the multidisciplinary team.
The postholder will also contribute to clinical leadership, patient safety, clinical governance, service development, audit, quality improvement and shared learning within the Ageing Well Team and wider PCN.

We are looking for an enthusiastic and motivated Pharmacy Technician to join the Jurassic Coast Primary Care Network pharmacy team.
Working across our member practices in Bridport, Beaminster and Lyme Regis, you will support the safe, effective and efficient use of medicines and help improve outcomes for patients. As part of a supportive multidisciplinary team, you will work closely with clinical pharmacists, GPs and other healthcare professionals, combining patient-facing work with medicines optimisation, medication reconciliation, safety audits and prescription management.
This is a varied role offering the opportunity to work independently within appropriate clinical supervision, build strong relationships with patients and colleagues, and contribute to the ongoing development of pharmacy services across the PCN. We are looking for someone who is organised, proactive and able to manage their own workload, with excellent communication skills and a strong attention to detail.
The post is for 30 hours per week and may involve travel between our PCN practices.
Main duties of the job
The Pharmacy Technician will support the delivery of safe and effective medicines management across the PCN. The role will include:
- Undertaking patient-facing medicines optimisation work and supporting patients to use their medicines effectively
- Completing medicines reconciliation following hospital discharge or transfers between care settings
- Supporting medication reviews, including for patients living in care homes
- Carrying out medication safety audits and reviewing the use of high-risk medicines
- Supporting prescription processes, repeat prescribing systems and the Electronic Prescription Service
- Helping to improve medication ordering processes and reduce medicines waste
- Supporting antimicrobial stewardship, shared care protocols and prescribing improvement work
- Contributing to clinical audits, quality improvement initiatives and enhanced services

Job Summary

Working under the supervision of the Lead Clinical Pharmacist, the postholder will undertake a combination of patient-facing, clinical, technical and administrative duties. They will work closely with pharmacists, GPs, practice teams, community pharmacies, care providers and other healthcare professionals to improve medicines safety and patient outcomes.
Main Responsibilities
- Undertake patient-facing and patient-supporting work to help patients understand and use their medicines effectively.
- Use shared decision-making and consultation skills to support patients with their medication.

Job Summary
To be responsible for undertaking a wide range of administrative duties and the provision of administrative support to teams as required. Duties can include but are not limited to, the processing of information (electronic and hard copy) in a timely manner, liaising with multidisciplinary team members and external agencies such as secondary care and community service providers in accordance with current policies, including the use of the electronic referral service (ERS).
Main duties of the job
Primary Responsibilities

- Undertake a broad spectrum of administrative duties
- Process incoming clinical correspondence and documentation, ensuring it is accurately matched, coded and allocated to the appropriate patient record/workflow.
- Type letters, reports and associated documentation as required and within the specified timescales as appropriate for the referral
- Liaise with external agencies, ensuring referrals are processed efficiently
- Process referrals using the electronic referral system (ERS)
- Process requests for information i.e., SAR, insurance/solicitors letters and DVLA forms
- Be aware of duties and responsibilities regarding current legislation and adhere to practice policies and procedures on Safeguarding Adults and Safeguarding Children
- Process incoming and outgoing mail

Ammonite Health Partnership is made up of 3 Practices. These are Tunnel Road Surgery in Beaminster, Pound Piece Surgery in Maiden Newton, and Bridport Medical Centre in Bridport.
Our current team includes: 8 GP partners, 4 Salaried GP, 5 Advanced Clinical Practitioners, 1 Visiting Paramedic, PCN pharmacy team, PCN social prescribing team, with comprehensive PCN based frailty and community care cover, Reception and Administrative teams.
A GP led triage hub contributes to workload management, to reduce workload and maximise clinically relevant face to face consultations. An experienced workflow team minimises document processing with pharmacist input for discharge and other medication management.
Ammonite Health Partnership is a member of Jurassic PCN, covering approximately 60% of the PCN patient population.
