Job summary
Baddeley Green Surgery is looking for a Practice Nurse experienced in Primary Care, which is essential to the role, to be responsible for the delivery of practice nursing services, working as part of the practices multidisciplinary team who provide care within their scope of practice to the patient population. The Practice Nurse will be responsible for a number of clinical areas such as Infection Control, Immunisation Lead, Health Promotion, Chronic Disease Management, Health Prevention, Well Woman and Well Man clinics, leading on QoF and QiF as well as supporting the management team in the reviewing of clinical policy and procedure.
Main duties of the job
Develop, implement and embed health promotion and well-being programmes Implement and evaluate individual treatment plans for chronic disease patients Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects on patients health Provide routine nursing care to patients as required in accordance with clinical-based evidence, NICE, NSF and other legislative frameworks and guidelines Provide wound care (ulcer/Doppler etc.) to patients Provide travel vaccination and advisory services Request pathology services as necessary Process pathology results as required Urinalysis Provide chronic disease management, delivering patient care as necessary, referring patients to secondary/specialist care as required INR Star -Anti Maintain accurate clinical records in conjunction with extant legislation Ensure read codes are used effectively Maintain chronic disease registers Chaperone patients where necessary
About us
Baddeley Green Surgery consists of a Senior Partner, two salaried GP's, one long term locum GP, ANP, Practice Nurse and two HCSWs. The practice has a growing population with approximately 6,100 patients at present.
The practice has a compliment of Managerial, Administrative and Secretarial staff in order to provide an efficient and effective service to all our patients.

Job responsibilities
Develop, implement and embed health promotion and well-being programmes
Implement and evaluate individual treatment plans for chronic disease patients
Ensure QoF & QiF adherence to meet targets
