Beaumond House

Role Purpose
To work with the Beaumond House team to maintain the standard of excellence to ensure that the philosophy and values of Beaumond House Hospice Care are met.
A major part of this role is being responsible for planning, co-ordinating, delivering and evaluating person-centred, evidence-based palliative care to patients, their families and carers. The RN will ensure the smooth running of the in-patient service, work with the multi-disciplinary team and other professionals and agencies and contribute to the development of the staff and the service.
Main Duties
- To assess, plan, implement and evaluate patient care applying a holistic personalised approach and continually review and update care plans and documentation.
- To ensure that the dignity, safety, and confidentiality of all patients are respected at all times and that all patients receive the highest possible standard of physical, psychological, and spiritual care.
- To ensure documentation is accurate legible and appropriate and complies with NMC and CQC requirements and participate in audits to ensure standards of documentation are maintained. To comply with safe use, storage, and destruction of medication. To personally develop, deliver and maintain competence in clinical nursing skills including symptom control and end of life care for patients with cancer or other life-limiting conditions.
- To take responsibility for your own personal development and maintain professional registration with the NMC. To demonstrate and record reflective practice, seek out clinical supervision, review learning needs with Director of Care & Quality (DoCQ) or Clinical Nurse Lead & Educator (CNLE) at individual review/appraisal and attend training as arranged. To build resilience and support networks for reflecting on practice, striving to improve personal performance and emotional wellbeing.
- To communicate effectively and sensitively with patients, carers, staff and wider health care team and other agencies, often about difficult and sensitive topics and ensure confidentiality is maintained in all channels of communication and in line with the confidentiality policy.
- To contribute to current improvement initiatives in end-of-life care planning involving patients, relatives, carers, other health care team members.
- To provide bereavement support for carers and identify and refer people who require higher level support.
- To ensure appropriate and timely communication and referrals are made to other members of the multi-disciplinary team and other professionals and agencies.
- To liaise and work with the wider multi-disciplinary team to ensure good working relationships are built and maintained, attending, and feeding back from external meetings, e.g. GSF, MDT.
- To lead a team of Health Care Assistants and Volunteers in providing patient-centred evidence-based palliative care to patients with life-limiting conditions who are receiving care and support from Beaumond House Hospice Care.
- To delegate nursing duties appropriately to care staff and contribute to their training and supervision in line with the NMC requirements and Beaumond House policies and procedures.
- To act as role model, supervisor/mentor to Health Care Assistants and other care staff members, participating in recruitment and selection, promoting an environment of continual learning, contributing to training programmes and assisting in induction and on-going development.
- To monitor levels of staffing in relation to patient dependency and ensure staffing and skill mix enables safe delivery of the care service, liaising with DoCQ/CNLE when necessary.
- To provide advice and support for patients and carers over the 24-hour period.

Job summary
Hospice at Home Care Assistant - Community based
To provide individual high-quality care for patients in their own homes, including appropriate support for families, in line with hospice policy and agreed care plans. Care may include supporting patients while symptoms are assessed and brought under control, caring for the dying, supportive psychosocial and spiritual care in line with any agreed plan, enabling patients independence and supporting discharge home.
Working under the direction of a registered nurse and as part of the Beaumond House team and the End-of-Life Care Together Integrated Hub you will help maintain the standard of excellence, so the philosophy and values of Beaumond House Hospice Care are met.
Main duties of the job
Patient Care
Provide personal care in line with the patients care plan under RN supervision, supporting toileting, washing, eating and drinking while maintaining dignity and privacy.
After competency sign off, undertake independent visits to deliver delegated care with clear escalation routes.
Support patients with medication after completing medicines support training, including prompting, assisting and, where authorised, administering prescribed medication within your competence and under RN direction.
Prepare and serve simple meals, snacks and drinks following food hygiene training and hospice policies.
Provide care after death, respecting cultural,spiritualand religious needs.
After training, carry out verification of expected death in the community in line with hospice policy.
Ensure safe custody and accurate recording of patient property.
Always maintain patient safety, delivering care only within your training and competence.
Communication & Family Support
Communicate sensitively with patients and families, adapting to preferred communication methods and supporting those with communication difficulties or sensory loss.
Respond to queries in a friendly, respectful manner, directing people to theappropriateprofessionalwhen needed andmaintainingconfidentiality
Support families with practical next steps after death, including liaising with funeral directors.

As a registered charity, we provide four in patient beds, and Hospice at Home services for people with palliative and end of life needs.
