An exciting opportunity has arisen to join the Bolton Community Practice team. We are a busy GP surgery with four sites across Bolton delivering GP services to a population of over 16,000 patient's. We are an EMIS Web practice.
The practice is seeking an enthusiastic salaried GP for up to 6 sessions per week.
Main duties of the job

In general the post-holder will be expected to undertake all the normal duties and responsibilities associated with a GP working within primary care including:
- Undertaking a variety of duties including surgery consultations, telephone consultations and queries, e-consultations, checking and signing repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion
- Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation
- Assessing the health care needs of patients with undifferentiated and undiagnosed problems
- Screening patients for disease risk factors and early signs of illness
- Developing care plans with patients in line with current practice disease management protocols
- Referring to other care providers as appropriate
- Recording clear and contemporaneous consultation notes to agreed standards

Job Summary:
The post-holder will manage a caseload and deal with a wide range of health needs in a primary care setting, ensuring the highest standards of care for all registered and temporary patients.
Clinical Responsibilities:
- In accordance with the Practice timetable, as agreed, the post-holder will make him/her-self available to undertake a variety of duties including surgery consultations, telephone consultations and queries, visiting patients at home, checking and signing repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion
- Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation
- Understand and apply legal policy that supports the identification of vulnerable and abused children and adults, being aware of statutory child/vulnerable adult health procedure and local guidance, including working within policies regarding family violence, vulnerable adults, substance abuse and addictive behaviour, and refer as appropriate
- Assessing the health care needs of patients with undifferentiated and undiagnosed problems
- Screening patients for disease risk factors and early signs of illness
- In consultation with patients and in line with current Practice disease management protocols, developing care plans for health
- Providing counselling and health education
- Admitting or discharging patients to and from the caseload and referring to other care providers as appropriate
- Recording clear and contemporaneous consultation notes to agreed standards
- Collecting data for audit purposes
- Compiling and issuing computer-generated acute and repeat prescriptions (avoiding hand-written prescriptions whenever possible)
- Prescribing in accordance with the Practice prescribing formulary (or generically) whenever this is clinically appropriate

Job summary
The post-holder is a Pharmacy Technician and is an integral part of the general practice team, working within their professional boundaries.
This role will work under the supervision of the Senior Clinical Pharmacist to ensure the safe, accurate and timely supply of prescribed medication to patients.
The Pharmacy Technician will provide technical and administrative support to the clinicians.
The Pharmacy Technician will support the practice clinical teams with queries regarding medication from patients.
The Pharmacy Technician will complete audits regarding patient safety in the prescribing of medication and present the information for sharing with the clinical team.
The Pharmacy Technician supports public health and screening programmes including immunisations and vaccination programmes whilst participating in quality improvement initiatives to enhance patient outcomes and reduce health inequalities.

Main duties of the job
Primary Key Responsibilities
- Support medication reviews and medicines reconciliation for new care home patients
- Support initiatives for antimicrobial stewardship to reduce inappropriate antibiotic prescribing
- Assist in the delivery of medicines optimisation and management incentive schemes and patient safety audits
- Support the implementations of national prescribing policies and guidance within GP practices, care homes and other primary care settings.
- Work with the multi-disciplinary team to ensure efficient medicines optimisation, including implementing efficient ordering and return processes, and reducing wastage.
- Support practice reception team in sorting and streaming general prescription requests.
- Support medicines optimisation systems with a range of services to get the best value from medicines by encouraging and implementing electronic prescriptions, safe repeat prescribing systems, and timely monitoring and management of high-risk medicines.
- Handle prescription queries and requests directly
- Be an integral part of the general practice team
- Demonstrate initiative and be creative in finding solutions to problems
