Team Administrator Care Coordination Centre - Band 2

Bromley Healthcare are currently looking for administrators to join their Care Coordination Centre (CCC) who can work 22.5 - 37.5 hours per week.
The CCC currently provides administration support for 35 community based clinical services, processing referrals, answering the telephone, responding to emails, booking appointments, dealing with patient enquiries and completing other elements of administration as and when required.
Proposed Interview Date: Friday 25th September
Main duties of the job
Based in the Care Coordination Centre (CCC,) the Team Administrator will be responsible for providing a high level of customer service and safe and efficient administrative support to our Health Visiting 0-19 clinical services in our CCC Tri-borough Team.
They will be required to liaise with parents/ carers, healthcare professionals and other colleagues on a frequent basis dealing with a wide range of standard administrative tasks including booking appointments and answering general enquiries via the telephone and email. The Team Administrator will work with their team to achieve key performance indicators (KPIs) and service level agreements (SLA's.)
The CCC is open 7 days a week, 8am to 10pm, 365 days a year including Bank Holidays. Our Triborough administrators work on a fixed 4-week roster and work shifts that cover 8am-6pm, i.e.: 8-4pm, 9-5pm, 10-6pm.Please refer to the job description and person specification for more information should you wish to apply. The post holder will be the OT Lead for Education and Health Care (EHC) assessments, with responsibility for completing assessments, providing reports and attending EHC panels along with oversight and quality assurance of EHC assessment provision across the service.
The post holder will develop, plan and provide specialist Occupational Therapy assessments, interventions and advice for children with functional difficulties, working closely with the multidisciplinary team, wider CYP service, school staff and families/carers. The postholder will work autonomously, seeing children in a variety of community healthcare and educational settings, including clients' homes, clinics, schools and nurseries.

This role is responsible for providing administrative support to the Advanced Neurodevelopmental Practitioners (Advanced ND Practitioners) and Specialist Registrars (SPRs), to ensure smooth running of the services and continuing high standards of patient care. We are currently looking for a Medical Admin Team Administrator to work in our Children and Young People's Directorate.

Bromley Hospital at Home is an integrated service delivered through Bromley Healthcare and One Bromley partners for avoiding acute hospital attendances and admissions while also supporting patients' early discharge from acute hospital beds.
The service provides diagnosis, treatment and monitoring of patients in their usual place of residence using both face to face and virtual care in the London Borough of Bromley. Operating 7 days per week 8am to 8pm. Referrals are accepted to the service, from the Acute Trust - Princess Royal University Hospital (PRUH) & Bromley Healthcare Urgent Community Response - Rapid Response Team.
One Bromley, the place-based partnership of health, care and third sector organisations in Bromley has significant ambitions for the service.

We are seeking an enthusiastic, flexible and competent Dental Nurse to join our Special Care Dental Service. We are based across 5 sites in Bromley, Bexley and Greenwich. Even though the successful candidate would be ideally based at 1 or 2 sites on a regular basis but may on occasions be asked to cover for colleagues at different sites. We would expect you to make your own travel arrangements to meet the needs of the job. A valid driving licence and access to a car is essential for this position.
The successful applicants must have an appropriate qualification in Dental Nursing and be registered with the General Dental Council. Excellent communication skills, compassion, empathy, good organisational skills, IT skills and being a team player are integral to this post. Previous experience of working in CDS is desirable but we would be happy to train someone with the right aptitude and skills.
Main duties of the job

We run our General Anaesthetic sessions from PRUH in Orpington, we would expect the post holder to part take in these session when required. We aim to provide high quality dental care for our special needs patients and children who are referred into our service.
We carry out domiciliary visits to meet the needs of the patients in their own settings- their own homes, care/residential homes and sometimes hospitals. We also provide treatment under conscious sedation. Any previous experience of conscious sedation will be desirable.

The Special Care Dental Service is looking for an experienced and caring clinician with GDC specialist registration in Special Care dentistry, to join our team of Dentists. Your main area of responsibility will be to provide support and advice, including treatment planning/case discussions on complex paediatric patients with/for colleagues working within the service.
As part of the NHS England Special Care Dental Care pathway, you will be a team player, supporting the head of the service with guideline updates in relation to Special Care dentistry, acting as safeguarding lead, and providing staff training as required by the Clinical Director.

The Specialist in Paediatric Dentistry is a senior clinician within the Community Dental Service, responsible for providing high-quality, evidence-based dental care for children and young people with complex oral health needs, behavioural challenges, medical conditions, disabilities and dental trauma. Childrens Community Nurse - band 6 Part time 23.75 hrs per week The post-holder will provide high-quality, specialist community nursing care to children and young people with acute, chronic, complex and palliative health needs. The Living Better, Living Well Service in Bromley is expanding to Bexley and Greenwich and we are looking for an enthusiastic and creative Therapy Assistant who enjoys working independently and as part of a team. In this role you will have the opportunity to complete assessments, interventions and reviews to adults residents in Bromley, Bexley and Greenwich who have multiple symptoms including fatigue, breathlessness and brain fog.
We are looking for someone who has some experience working as a support worker and would like a role working with people who may have both physical and mental health symptoms that are affecting their ability to function, work and enjoy life as they used to. Main duties of the job
To be a member of the service providing assessments, treatments and reviews within a high quality service to the residents of Bromley, Bexley and Greenwich. The post holder will hold a small caseload and be responsible for progressing simple rehabilitation programmes. Providing basic equipment provision, walking aid provision, referring on to external support services and reviewing and discharging patients with overall supervision from a registered member of staff.
The Therapy Assistant will support all aspects of service delivery through virtual and face to face means, in both group and 1:1 settings in the community, clinics or people's own homes.
