Job summary
East of England Community Health and Care Trust provides Integrated Contraception & Sexual Health (iCaSH) across Bedford, Cambridgeshire, Milton Keynes, Norfolk, Peterborough and Suffolk.
The service delivers care specifically to meet the needs of its diverse population. The Norfolk service is community based with three main clinics in Norwich, Great Yarmouth and Kings Lynn, providing contraception, sexual health and HIV care and treatment.
The successful candidate will join a team of friendly, highly skilled and experienced medical, nursing and allied health professionals, supported by clinic assistants and administrative staff, all delivering a high standard of care to patients at our Norwich Clinic. You will also work alongside the teams in King's Lynn and Great Yarmouth, you must be prepared to travel.
Working Pattern: Monday to Saturday, including evening until 19:30

Main duties of the job
We are looking for a motivated and enthusiastic Registered Nurse or Midwife with a strong team-working ethic to join our team as Associate Clinical Nurse Specialist: Progressive Post.
You will need to be enthusiastic and passionate about patient centred care, an excellent team player but also be able to work independently when needed. You will also need to be able to work with a diverse range of clients in an innovative sexual health setting.

To support patients in identifying and reducing any risks taken within their sexual behaviours.
To provide assessment and a safe, non-judgemental environment, enabling patients to ask questions and discuss any concerns, providing appropriate signposting and/or referral.
To manage and respond to SRH enquiries from the public and partners/stakeholders, including discussing risk and responding appropriately to those who are anxious or high risk.
To record interventions accurately, concisely, and contemporaneously in patient clinical records, in line with both local and national guidance and policies.
To be competent in the management of young people and vulnerable adults, working within Fraser Guidelines and mental capacity legislation, liaising with the multi-disciplinary team, CCS, and local safeguarding teams/services as appropriate.
Be conversant with the roles of all the staff within the service and with other colleagues internal or external to the organisation.

Work collaboratively with all CCS iCaSH Services to ensure a standardised approach.
Communicate effectively with the multidisciplinary team to promote efficient patient care.
Contribute to effective communication, data, and information management within the team.
Support the correct storage of patients records as per the Trust Records Management Policy.
Liaise with other services as necessary for the co-ordination of patient care and the smooth running of the department.
Ensure that exposure to sensitive, confidential information, is dealt with in line with organisational information governance policies and procedures.
Maintain databases as required by the role, for example the patient electronic record system.
Work under direction to support the process of audit and the improvement of information quality in relation to electronic records held within the department.
Ensure accurate record keeping in line with Trust and national standards. Support timely and accurate data entry and audit for performance and quality reporting.
Maintain a thorough knowledge of and adherence to the full range of policies / procedures and guidelines relevant to iCaSH and the Trust.
Welcome visitors and patients, providing assistance, support and information where required.
Maintain good communication with other staff, patients and visitors including front of house/reception and answering the telephone in a courteous and professional manner.
Actively participate in multidisciplinary training, meetings, and events.
Signpost clients to other services where appropriate.
Ensure that all organisational wide communication is accessed and adhered to accordingly via the nhs.net account.

People Management
Responsible for day-to-day supervision, line management and development of identified staff within the iCaSH service as directed.
Maintain own professional development and requirement to take part in appraisal and supervision process.
Participate in clinical audit, service development and appropriate research projects.
Clinical and Practice Governance
Observe and maintain strict confidentiality with regards to any patient/family/staff/records and information in line with General Data Protection Regulation (GDPR), requirements.
Any data that is taken/shared must be undertaken with regard to the Trust Information Governance (IG) Strategy and Framework and Information Security policy.
The post holder must adhere to the Trust risk assessment and risk management processes.
The post holder must adhere to Trust Infection Prevention and Control policies and procedures.
It is a condition of your employment that you are currently registered with the Nursing and Midwifery Council, and it is your responsibility to maintain your professional registration and any supplementary registrations.
Undertake mandatory training and any other training relevant to the role as required by CCS NHS Trust.

Job summary
As part of the Integrated Community Stroke Service you will be required to participate in the delivery of Speech and Language Therapy, providing specialist assessments and associated care for patients across a defined geographical area. There may be a need to provide countywide cover on occasion.
This is a community-based role that requires frequent travel to various locations throughout the county, including areas that may not be accessible by public transportation.
The successful applicant will have the skills and competencies to provide assessment and rehabilitation through both face-to-face and remote modalities, using approved digital platforms where clinically appropriate. Although the service would ideally benefit from a clinician able to provide face-to-face care, applications are welcomed from suitably qualified and experienced individuals who are able to contribute through remote assessment and rehabilitation delivery.

Main duties of the job
- Support safe discharges from hospital and provide a high standard rehabilitation service to patients referred to the Integrated Community Stroke Service
- Provide specialist assessment, effective treatment, and individualised goal setting of patients in their own home/ place of residence, to promote recovery post stroke.
- Early identification and management of stroke related complications.
- Reduce readmissions due to complications post stroke.
- Provide education and support to formal and informal carers.

Main duties of the job
You will be able to offer a comprehensive service within a busy and often demanding environment so candidates will need to be confident and adaptable to change.
Duties will include: answering the telephone and transferring calls to various departments where appropriate. Welcoming Service Users, Carers, Professionals and Visitors to the Site and dealing with enquiries on the phone or face to face. Checking ID's and handing out security alarms and keys for which training will be given.
Reporting maintenance and cleaning issues to the Help desk and keeping accurate and up to date maintenance logs.
You will also act as a Fire Warden and be responsible for keeping the Reception area clean, tidy and organised.
Other duties would include: Dealing with deliveries of stationery etc., to the building. Booking all Clinical and meeting rooms for the Clinical and Corporate Staff working in the building for which training will be given.
You will work closely with other members of the administration Teams to ensure a smooth and effective Reception Service.

We are looking for enthusiastic psychological therapists / psychologists to work in our Pain Management team as part of an integrated multi-disciplinary service.
Although the main base for the role(s) will be Broomfield Hospital, we have an Pain Service presence at all sites within MSE (Broomfield, Southend, Basildon & Orsett) and this role may involve also working at one other of our hospital sites.
The team works with patients with complex pain management issues, some of whom may also have co-morbid mental health problems.

CYPMHS South Hertfordshire is seeking to recruit a highly skilled and motivated CYP Practitioner with an appropriate professional qualification and relevant experience to join our multi-disciplinary team.

HPFT's Children and Young People's Mental Health Service (CYPMHS - formerly known as CAMHS)has been at the forefront of national developments in CYPMHS, including CAPA and CYP-IAPT, and Hertfordshire is an i-Thrive accelerator service. The HPFT CYPMH service includes four community based 'tier 3' teams, a crisis assessment/treatment team (C-CATT), a DBT service, an Eating Disorder Team, an inpatient unit (Forest House) and a Targeted Team for looked after children. Main duties of the job
This is a community-based role that requires frequent travel to various locations throughout the county including areas that may not be accessible by public transportation. Successful candidates will need to demonstrate how they will manage these travel requirements.
You will be working with your colleagues to manage the referrals coming through to secondary mental health services via triage and initial assessment.
You will be expected to work inpartnership working with GPs, other referring agencies and community mental health teams including, Early Interventions in Psychosis, Home Treatment Team and Talking Therapies.

A fantastic opportunity has arisen for a Senior Mental Health Practitioner within the Taw Core Adult Mental Health Team based at West Pilton Health & Wellbeing Clinic, Barnstaple.

Main duties of the job
The post holder will deliver high quality comprehensive mental health services. The role involves managing a defined caseload with a focus on providing expert assessment and treatment for people with complex mental health difficulties. Liaising with the persons family, carers and partner agencies to provide and balanced and informed approach.
Adopting a recovery co-ordination approach to mutually design a personalised plan of treatment packages that is agreed by the individual and evaluated to evaluate efficacy and patient experience.
We are looking for someone with enthusiasm and energy who wants to make a difference to peoples lives; is creative, innovative and highly motivated. You must have assessment and care planning skills and experience in mental health and multi-disciplinary working is desirable.
You will need to have excellent communication skills, be able to exercise clinical judgement in respect of assessing and managing risk. In addition, you need to be committed to the values and principles of recovery, and able to help deliver a safe, timely, caring and responsive service. Main duties of the job
The post holder will use their personal lived experiences of recovery from mental health challenges to offer empathy and understanding to others, inspiring hope and belief that recovery is possible. The new role of Executive Director of Delivery, a non-voting member of the board, will be the principal strategic adviser to the Chief Executive and Board.
