The Dementia Intensive Support Team (DIST) are seeking an ambitious and caring Band 6 Registered Mental Health Practitioners to join our existing team.
The DIST team are proactive and highly skilled at managing risk and supporting service users within CTM who may be experiencing or approaching crisis in their mental health. The service is passionate about providing alternative options to hospital admissions, so you can expect to be undertaking rewarding and highly intensive work.
We looking to recruit a compassionate, dedicated, and conscientious individual, who wants to make a positive difference to adults experiencing a dementia-related crisis.
Staff provide care, treatment, and assessment to patients in their own homes and other places of safety, getting you out and about seeing patients in a range of community settings.

Main duties of the job
The post holder will facilitate rapid assessment, treatment, crisis prevention and intervention along with preventing avaoidable hopsital admissions
Will be responsible for undertaking comprehensive clinical assessments. They will provide specialist clinical advice to other team members on the suitability of various assessents/treatment/support.
Will be clinically resposnible for spporting the delivery of a flexible, responsive local service within the community.
You will work collaboratively with collegues, key partners and those within the local community to develop services to meet the mental health needs of the community.
Assess risk and formulate safety plans accordingly.
Contribute to effective communication with all parts of the mental health service, in particular the Older Adults Community Mental Health Teams (OACMHT).
Establish rapport and maintain supportive professional relationships with clients and their carers, even when there are barriers to acceptance such as lack of insight or difficulties adjusting to complex changes; recognising the stress associated with caring, offering and promoting carer's assessments as required.
Deputise for the team leader as and when required.

We are keen to recruit an enthusiastic, forward-thinking pharmacist to work with the Lead Pharmacist for Lakeside Mental Health Unit to continue to develop the clinical pharmacy service within Local Services. The pharmacist will join a vibrant, expanding team. Building on the excellent pharmacy service with bases in the five main Trust sites, the team includes pharmacists dedicated to the OPMHS teams, CAMHS, perinatal and the Crisis Teams as well as MINT. There is a team focus on growth and development, including Clinical Diplomas, non-medical prescribing qualifications, consultant pharmacist accreditation and writing articles and posters for publication.
Main duties of the job
The successful candidate will provide an outstanding clinical pharmacy service to the service users who are served by the Local Services located in the borough the pharmacist is allocated to.
This service will include:
- Completing Level 2 Medicines Reconciliations
- Clinical screening of medication charts
- Ensuring continuity of supply of medications
- Responding to clinical queries with medicines information and advice
- Providing medicines-related information and advice while taking part in team MDT meetings
The successful candidate will contribute to the clinical governance agenda within the Trust, via audits, QI projects, incident reporting and investigations, writing/updating policies and procedures and contributing to the implementation of the resulting action plans for service improvement.

Main duties of the job
You will participate fully in the out-patient, ward and theatre management of both elective and emergency admissions under the supervision of your named consultant.
You will advise and supervise Junior Staff on clinical matters.
You are second on-call. You will respond appropriately to referrals from your juniors and other departments. You are not on the Trauma Team.
You will attend the Trauma Meeting most weekdays. Every day when on Trauma Week, on-call or the day after on-call. Seven days a week we start with a Trauma Meeting which is part business, part educational. All the admissions from the previous 24 hours are discussed.
You will also perform duties in occasional emergencies and unforeseen circumstances, at the request of the particular consultant, in consultation where practicable with your colleagues both senior and junior.

Role Summary The advanced clinical practitioner (ACP) provides advanced clinical practice as an integrated part of the multidisciplinary team within the Urgent Treatment Centre and Emergency Department. The successful candidate will deal with a wide range of primary care presentations in both adults and children using advanced clinical skills to investigate, diagnose and instigate treatment. The definition of Advanced Clinical Practice as stated by Health Education England (2017) is " delivered by experienced, registered health and care practitioners. This is underpinned by a master's level award or equivalent that encompasses the four pillars of clinical practice, leadership and management, education and research, with demonstration of core capabilities and area specific clinical competence." The ACP has a trust-wide responsibility to provide clinical excellence through the four pillars of advanced clinical practice. The clinical practice pillar accounts for a minimum of 80% of their contracted hours of work and they practice with a significant higher level of responsibility and autonomy which is comparable to that of the level of medical staff providing the same level of service

Job summary
The post holder will be a leader within an integrated multi professional inpatient team that includes registered physiotherapists, occupational therapists and wider therapy team, registered nursing staff, care staff, medical staff and wider trust colleagues. The Operational Manager is responsible for the day to day operational management and leadership of the Community Mental Health Team (CMHT). The Operational Manager is responsible for managing resources to provide quality mental health care to a defined population. The Operational Manager will provide, receive and lead on complex, sensitive, or contentious information where complex skills are needed for operational and clinical situations. They will be required to undertake investigations and support staff to learn from patient safety incidents, complaints and concerns.
Main duties of the job
The Operational Manager is responsible for the leadership, wellbeing and line management of staff, ensuring the delivery of safe, effective care and achievement of key performance standards. Responsibilities include supervision, appraisal, sickness absence and performance management, staff wellbeing, coaching, mentoring and conflict resolution in line with Trust and HR policies.

An exciting opportunity has arisen for enthusiastic outpatient HCSW's to join our existing teams within General Outpatients [GOPD] including GRH, CGH and CDC operational sites. At Birmingham Hospice our teams are committed to improving quality of life for people living with life-limiting conditions, as well as supporting their families and loved ones during one of the most challenging periods they will ever face.
We are seeking a professional and caring Band 6 Clinical Nurse Specialist (CNS), to join our busy community team, based in Selly Park. As a CNS you will work across the city and may be required to travel to our other site in Erdington from time to time. You will provide specialist palliative care nursing services to our patients, families carers and other health professionals. You will be able to analyse complex situations and develop action plans to improve the quality of patient care.
You will actively contribute to multidisciplinary working and will be able to recognise the importance of knowledge and learning to maintain standards in palliative care. You will be comfortable working as an autonomous palliative care practitioner and will be required to manage a caseload of patients within the community setting.

Main duties of the job
The post holder will work as part of a multidisciplinary team in Imaging, liaising with all involved in providing the service, ensuring appropriate imaging, reporting, and patient management. They will provide a high professional and technical standard of ultrasound.
Our Sonographers work within departmental protocols, written in conjunction with national guidelines. The post holder will work alongside colleagues from all modalities involved in the management of patients to ensure the day-to-day workload is managed effectively.
We have a two, purpose-built general ultrasound departments located at Macclesfield and CDC. Macclesfield leads off the Radiology department, three dedicated ultrasound rooms, managed and staffed by Sonographers. A fourth dedicated room for Consultant and Registrar Radiologists within the main radiology department, suitable for interventional and general ultrasound.
The CDC unit, has two newly built, dedicated ultrasound rooms. Located alongside an X-ray room and cardio-respiratory services. Two obstetric ultrasound rooms located in ANC.

Job summary
This is a 10 PA role for a Locum Consultant Dermatology post with activity based mainly at the Friends Dermatology Centre within the Kent and Canterbury Hospital (KCH) in Canterbury, but will also include arequirement to occasionally work Trustwide.
East Kent has a highly regarded, active, established surgical and Mohsmicrographic surgery service that has been developed over many years with leading UK dermatologic surgeons at the helm. An energetic and enthusiastic colleague with an interest in Skin cancer, paediatrics, general dermatology clinic with and occasional Skin surgery.
Main duties of the job
The general responsibilities of the post will be those expected of a senior clinical employee of the Trust.
These include:
- Provision of a first-class clinical service
- Effective leadership to all staff involved in supporting the service
- Conducting clinical practice in accordance with the highest ethical standards
- Co-operation with and maintenance of confidentiality over business plans and developmentstrategies within the Trust
- Support for other clinical specialties and contribution to postgraduate teaching
The appointee will be expected to deliver a service covering all aspects of dermatology, but a special interest to complement those of the existing consultants would be encouraged.
