Job summary
The Bognor Assessment & Treatment Service is a community mental health team which supports people living with a severe and enduring mental illness. The MHLP team is part of the Bognor Assessment and Treatment Service and work as liaison between the ATS and GP Surgeries. The post holder will work within GP surgeries and will be the liaison and link between primary care and secondary care services.
We are looking for an experienced Band 6 Mental Health Practitioner (Mental Health Nurser) to work within GP Surgeries to provide up to 10 sessions of psychologically informed or psycho educational interventions to people with moderate mental health problems. The successful applicant will also be responsible for completing initial assessments and liaising with GP's, IAPT services, local agencies and signposting to third sector services.
This post offers the Trust's £2,000 Golden Hello or Relocation Incentive for Band 6 Nurses & Practitioners, (pro rata and subject to terms and conditions; please see attached Protocol Document for full details).

Main duties of the job
Working as an MHLP includes:
- Working autonomously to a defined caseload of clients diagnosed with mild to moderate mental health problems. This will include offering specific interventions such as psycho-education, CBT informed sessions and short term recovery focused interventions
- You will complete initial assessments and work closely with our referrals coordinators, GP's and other agencies.
- Your interventions will be a pre-defined offer of 6-8 sessions.
- You are welcome to co-facilitate groups if you have an interest in this area.

Job summary
Our UTC operates a Digital Front Door, which is a clinically driven digital triage tool designed to improve safety and efficiency across urgent care and is changing the way patients reach and move through urgent care in Greenwich. Our patients use the DFD to check in and complete a short, guided triage, enabling faster prioritisation and smoother patient journeys.
Whats New?
Our clinical system supplier is developing a new appointment book, which for the first time will allow patients to be booked directly into an urgent care appointment rather than simply being given an arrival time or having to sit and wait.
We are redesigning our Directory of Services (DOS) so that appointment slots align to NHS Pathways disposition timescales. A patient with a see a GP within 48 hours disposition can be offered a later appointment, while presentations more likely to need an emergency department can be booked within two hours.
NHS Pathways sorts patients leaving the digital front door into a number of groups, including those who are not urgent and can safely be seen later. Booking to the right slot at the right time helps us manage surge, improve flow and demand, and reduce attributable breaches.
Greenwich Health will be launching a new system where NHS 111 can book patients into a named urgent care appointment.
This role is available on a bank basis. 12-hour shifts finishing no later than 22:00. 7 days/week availability will be required.
Main duties of the job
This is the perfect opportunity to be part of a diverse team at the very front of urgent care in Greenwich, and to help deliver a model that is the first of its kind in South East London.
As a Front Door Navigator you will be the first person a patient meets when they arrive at emergency and urgent care. Working from a tablet at the entrance, you will welcome patients, establish why they have attended, check whether they hold a booked appointment made through NHS 111 or the digital front door, and direct them to the right place without delay.
You will keep people moving away from the reception queue and make sure that patients who are unwell, distressed or vulnerable are brought to the attention of a clinician immediately. This is a non-clinical role, you will not assess, triage or advise on a patients condition, but your judgement about when to escalate is central to keeping patients safe.
You will also play an important part in helping us evaluate the new model, recording how patients arrive and where they are directed, and feeding back to the UTC Lead where the appointment book or the Directory of Services is not working as intended.

They will be working closely with practice and PCN staff to identify, engage with and proactively coordinate personalised care and support planning for the most vulnerable people in our community, including the frail/elderly, people living with severe mental illness or learning disabilities, and those with long-term health conditions.
As well as being linked with individual practices they will work together as a team. This will include sharing learning and best practise both within the team and across the PCN.
Support with patient engagement, which will include ensuring that information is accessible for all and having conversations with patients and carers to increase understanding, alleviate concerns and increase engagement and self-management.
To support people in preparing for or following-up clinical conversations they have with primary care professionals (including health checks) to enable them to be actively involved in managing their care and supported to make choices that are right for them. You will use knowledge of health and social services available in the locality, including those offered by the community and voluntary sector, to link people up with these and help them overcome any barriers they might encounter. Job Summary
As a Care Co-ordinator, you will be an important first point of contact for our patients, providing a friendly, professional and efficient service. You will support patients in accessing the most appropriate service or healthcare professional while ensuring the smooth day-to-day running of the Practice.

This role will be to cover our Inpatient unit, Segregation and Outpatient clinics.

Monday to Sunday: 07:00 - 17:00

Main duties of the job
As a Bank Primary Care Nurse your responsibilities will vary. You will
- Work within a multi-disciplinary team to deliver a nurse led service and prevent hospital admissions
- Undertake medication rounds, dispense controlled medication, attend emergency response situations, assess patients, and undertake reception screening
- Use your skills and knowledge to deliver nurse led clinics to your patient group whilst being supported by a multi-disciplinary team.
- Provide outstanding evidence based care to our patients, seeking out, listening to and acting on their feedback, so that care is personalised and informed by what matters to them.
- Deliver non-judgemental care that makes a real difference
- Inspire excellence by forging strong links with our partners and stakeholders
- Promote best practice, clinical supervision, evidence based care and continuous quality improvement, embedding a culture of shared learning.
- Share knowledge, skills and expertise to ensure safe care, building strong multi-disciplinary teams.

Qualifications required:
- Registered nurse Adult
- Current professional qualification (NMC)
- Evidence of Continued Professional Development
