THE OPPORTUNITY
This role will be hosted by South East Leeds GP Group and will be based in the LS25 LS26 Primary Care Network (PCN).
LS25/26 PCN has a patient population of circa 75,000 across 7 practices:
Garforth Medical Centre
Gibson Lane Practice
Kippax Hall Surgery
Lofthouse Surgery
Moorfield House Surgery
Nova Scotia Medical Centre
Oulton Medical Centre
Applicants must be available to work on Wednesdays as this is an essential requirement for the role.
Main duties of the job
Our Care Coordinators will play an important role within the PCN to reduce health inequalities and support meeting our PCN and practice targets. They will be working closely with practice and PCN staff to identify, engage with and proactively coordinate personalised care and support planning for the most vulnerable people in our community, including the frail/elderly, people living with severe mental illness or learning disabilities, and those with long-term health conditions.
As well as being linked with individual practices they will work together as a team. This will include sharing learning and best practise both within the team and across the PCN.
Support with patient engagement, which will include ensuring that information is accessible for all and having conversations with patients and carers to increase understanding, alleviate concerns and increase engagement and self-management.
To support people in preparing for or following-up clinical conversations they have with primary care professionals (including health checks) to enable them to be actively involved in managing their care and supported to make choices that are right for them. You will use knowledge of health and social services available in the locality, including those offered by the community and voluntary sector, to link people up with these and help them overcome any barriers they might encounter. The aim is to help people improve their quality of life and avoid unplanned hospital admissions.

Provide administrative support for the Leg Club Service by accurately updating and maintaining patient records with information received from Leg Club appointments and interventions.
Support the delivery of GLP-1 Weight Management Clinics, including contacting patients, arranging appointments, recording heights and weights, coordinating blood tests, maintaining accurate records, and supporting ongoing patient monitoring.

Job summary
Blackpool Teaching Hospitals is an acute hospital with many community supporting functions across the Fylde Coast and wider Lancashire. Blackpool Hospital sits within Blackpool itself close to the picturesque Stanley Park and within walking distance of the town centre. Blackpool has a highly transient population due to being a very busy seaside resort, which makes this role both unique and challenging for the right candidate.
It is an exciting time to become part of the Blackpool Teaching Hospitals Family as we embark on a transformational change journey in the way which we provide services to our patients and their families across our Integrated Care System.
Main duties of the job
There are two wards within the acute hospital setting dedicated to Care of The Older Person, however the Trust will also have several outliers across other medical wards in the hospital. We also have a rehabilitation hospital based in the seaside resort of Lytham St. Annes. The Division boasts a revolutionary Community Frailty Service as part of our out of hospital services, which provides support to our frail local population in their homes and supports our Frailty Virtual Ward. The Trust has recently seen the introduction of a dedicated Acute Frailty Unit which is supported by both acute, community and social care multi-disciplinary teams. Care of the Older Person services include Parkinson's Disease, Ortho geriatrics and Older Adult Mental Health in-reach. The Trust has seen significant developments in its Emergency Department and Medical Assessment Centre (MAC).

In addition, we have a variety of other clinical support teams who we work alongside through an MDT approach to care ensuring the best possible outcomes for our patients. We are looking for an experienced Emergency Practitioner who is either a Registered Nurse, Extended scope Physiotherapist or Paramedic - who can Independently assess, investigate, diagnose, and treat patients in our Minor Injury Unit.
Working in our Minor injury Unit, which will be moving to Gate 24, across 7 days of the week. Shift patterns 12 hours, and between 07.00 and 23.00.

37.5 hours per weekWe are looking for highly motivated nurses to work in our busy and challenging Critical Care environment and our enhanced surgical recovery area.
Our critical care department not only has 33 commissioned critical care beds for both level 2 and 3 patients we have also been able to create an exciting 5 bed enhanced surgical recovery unit This enhanced surgical recovery area cares for level 1.5 elective-post op patients. Our dedicated critical care team is also responsible for:
- Caring for critically ill patients in the hospital
- Caring for patients after major operations
- Providing first responder support to all unstable patients in the hospital.
The Royal site is also the regional specialist centre for infectious diseases, acute pancreatitis, hepatobiliary surgery, upper gastrointestinal surgery, and thrombocytopenic purpura. Critical care also provides a critical care outreach service, follow up clinics, psychological support for patients, relatives, and staff.
We have a dedicated Practice Educator team, who provide extra support and in-house training. Further CPD opportunities include the Critical Care course. Successful applicants are permitted to work 'long days' on completion of a minimum of 60% of their Step 1 competencies. Prior to this, the postholder will be expected to complete a combination of early, late and night shifts.
Main duties of the job
The postholder will work as a member of the multi-disciplinary team under the supervision of the nurse with continuing responsibility of the unit. Key responsibilities will include the development, implementation and evaluation of programmes of care.The postholder will always work within the NMC's Code of Professional Conduct.
