Job summary
We have an exciting opportunity for an enthusiastic and dynamic Clinical Manager to join our dedicated community services team. This is a rewarding leadership role where you will have the opportunity to make a real difference to patient care, service delivery, and staff development.
As a Clinical Manager, you will provide visible leadership and operational oversight, ensuring the delivery of high-quality, safe, and effective care. Working closely with multidisciplinary teams, you will support staff, drive service improvements, and help shape the future of community healthcare services.
The post is 37.5 hours per week Monday - Friday
Main duties of the job
The post requires a strong innovative leader to lead the Hub for Calderdale & Huddersfield Trust Community Healthcare Division in collaboration with the Primary Care Network and Stakeholders.
Calderdale Community Healthcare Division covers five locality Hubs each with a clinical Manager lead.
About us
We employ more than 6,500 staff who deliver compassionate care from our two main hospitals, Calderdale Royal Hospital and Huddersfield Royal Infirmary as well as in community sites, health centres and in patients' homes. We also are incredibly proud to have almost 150 volunteers here at CHFT.
We provide a range of services including urgent and emergency care; medical; surgical; maternity; gynaecology; critical care; children's and young people's services; end of life care and outpatient and diagnostic imaging services.
We provide community health services, including sexual health services in Calderdale from Calderdale Royal and local health centres. These include Todmorden Health Centre and Broad Street Plaza.
We continue to modernise and invest in our health services to build on our strong reputation. Foundation trusts are public leaders in improving quality in health services. They are part of the NHS - yet decisions about what they do and how they do it are driven by independent boards. Boards listen to their Council of Governors and respond to the needs of their members - patients, staff and the local community.
Foundation trusts provide what the health service wants, yet are also free to invest quickly in the changes to the local community needs, in striving to be the best, and in putting their patients first.

About us
Based across two acute sites in Huddersfield and Halifax as well as some community sites, we are one of few NHS organisations recognised as an "investor in people" and works to involve, support and develop staff within the organisation.
Due to our success and reputation in clinical research, we have recently become a 'spoke site' for the NIHR Commercial Research Delivery Centre to improve access to commercial research trials and new treatment options for our patients.

PCN Home Visiting Prescribing Nurse Practitioner
West Leeds Primary Care Network View all jobs
Posted 1 month(s) ago
Reference: W0027-26-0013
Job summary
PLEASE NOTE WEST LEEDS PCN LTD IS NOT AN AGENDA FOR CHANGE EMPLOYER
We have a vacancy at West Leeds PCN for a prescribing Nurse, Paramedic or Nurse Practitioner. This role would particularly suit an Advanced Practitioner with good all round nursing experience as the role itself will be very broad. We need someone who is an experienced Independent Prescriber (ideally 3 years experience), particularly for the Acute Home Visiting element of this role. You would be joining a highly skilled and experienced team with a wealth of peer support available each day.
Main duties of the job
There are two elements to this role:
Annual frailty reviews - these are templated and in depth. We focus mainly on those that are severely frail but we also get the opportunity to work with the moderately frail, changing their health outcome in good time to avoid them from tipping into the severely frail Rockwood score (which you will find immensely satisfying). These are mainly done in the patient's home unless they are able to get into surgery
Acute home visits - you will spend a part of your working week doing these as this services relieves a great deal of pressure on our member practices (particularly the GPs)
Please see the uploaded job specification document for full details
About us
West Leeds PCN is a mature and visionary PCN, boasting a well developed workforce across multiple clinical disciplines. We are fortunate to have our own premises for remote work ("PCN HQ") which means we can work daily in a true MDT style across all of the various clinical teams
Please see our website for more information-West Leeds Primary Care Network.
Please note: West Leeds PCN is not an agenda for change organisation and we deliver our organisations T&Cs and not the T&Cs of NHS Trusts.

Job description
Job responsibilities
Our Frailty Service is a multidisciplinary one for those over 65 years of age who demonstrate two or more of the five frailty indicators (including falls, immobility, incontinence, delirium, medication side effects susceptibility). These patients are comprehensively assessed in their own home (or care home) following which a bespoke patient centred care plan will be created. The intention is to either improve or maintain their level of health and wellbeing and also to enable them to remain as independent as possible in their own residence. The Frailty Team does not carry a caseload; each referral is classed as an intervention. Following review from the Frailty Team the patient care is handed back to their GP practice.
Each patient is assessed for falls prevalence and has their medications reviewed. An independent prescriber would be an advantage for this role but it is not essential as other members of the team can issue medication
You will attend weekly MDT meetings with a consultant geriatrician hosted by our frailty team to discuss the most complex patients who have regular home visits. You will also closely liaise with the wider PCN team as needed in order to deliver a truly holistic healthcare service. The successful candidate will need to be passionate about improving patient health outcomes.
The daily Acute Home Visiting list offers a change of pace and you will encounter such problems as swellings (legs, feet etc), UTIs, chest infections, oedema, infected wounds, skin complaints such as rashes and migraines etc. The provision of this service is vital to our member practices and frees up practice GP time while we fulfil these visits for them
Job description
Job responsibilities
Our Frailty Service is a multidisciplinary one for those over 65 years of age who demonstrate two or more of the five frailty indicators (including falls, immobility, incontinence, delirium, medication side effects susceptibility). The intention is to either improve or maintain their level of health and wellbeing and also to enable them to remain as independent as possible in their own residence. The Frailty Team does not carry a caseload; each referral is classed as an intervention. Following review from the Frailty Team the patient care is handed back to their GP practice.

Reference: 342-BR134-0926
Job summary
Sequoia is a specialist NHS-commissioned service, co-delivered by Avon and Wiltshire Mental Health Partnership (AWP) and Rethink Mental Illness, supporting adults with mild-moderate complex emotional needs (CEN) and personality difficulties across the BNSSG area. Rooted in lived experience and recovery principles, the service provides evidence-based group interventions including Dialectical Behaviour Therapy (DBT)-informed groups, Mentalisation-Based Treatment (MBT)-informed groups, psychoeducation courses, and peer support programmes.
As a Specialist Practitioner and Group Facilitator, you will initially be co-delivering DBT informed skills groups. Recognising the emotional impact of this work, we provide robust supervision (individual and team-based), ongoing CPD opportunities & regular chances to connect socially with colleagues.
Main duties of the job
-
Facilitate DBT informed groups, adapting content to meet service user needs.
-
Support delivery of psychoeducation and peer support courses.
-
Assess and triage referrals, determine service suitability, and plan reviews and next steps with service users.
-
Support safe discharge back to GP care following programme completion.
-
Contribute to crisis planning, safeguarding, and protective interventions.
-
Provide clinical leadership, mentoring, and supervision within the team for our Rethink colleagues.
- Work as part of the duty team dealing with ad hoc situations that arise.
-
Maintain accurate records and meet reporting requirements.
-
Participate in service development, supervision, and training.
