Job summary
Due to additional investment in to our Children's Speech and Language Therapy (SLT) service, we would like to offer you a fabulous opportunity to come and join one of our Locality SLT teams as Band 4 SLT Assistants (SLTA).
We are looking for creative and enthusiastic SLTAs to join our well established SLT service, and we have sessions available in our community and schools teams (mainstream and specialist). Our posts are across Hertfordshire including in Hemel Hempstead, Watford and Stevenage.
The SLT service in Hertfordshire is long established, and we have a reputation for being forward thinking and committed to ongoing service improvement. We embrace change and are driven by the evidence base, neuro-affirming approaches and functional, communication friendly practices.
Our service is jointly commissioned by health and education, allowing us to provide a seamless pathway of care from early years through to schools, specialist provisions and further education settings. We have excellent joint working relationships with our partners in education and a wide range of stakeholders, and we work in partnership with them to deliver interventions at Universal, Targeted and Specialist levels across Hertfordshire. This successful working relationship has helped contribute towards us being awarded with new investment.

Main duties of the job
The successful applicants will assist SLTs in clinics, mainstream & specialist schools. The role involves delivering individual programmes, group working, developing resources, using IT & undertaking some administrative duties. You will have the opportunity to work with teachers, teaching assistants and health visitors, and to work alongside SLTs and other assistants.
Applicants should have knowledge of normal child development alongside experience of working with children, including those with communication difficulties. Experience of working with, and using augmentative and alternative communication (AAC) is beneficial. We are looking for applicants who are creative, flexible, possessing good organisational and communication skills and with an ability to work on their own initiative.

Main duties of the job
To provide specialist stroke Occupational Therapy assessment, intervention and rehabilitation to patients to ensure improved functional recovery, prompt discharge and to enable the person to remain in their home.
You will provide supervision and support to junior members of the team and student Occupational Therapists and have a passion for delivering evidence based, quality intervention using research, audit and quality improvement projects. The post holder will be flexible to service needs, possess excellent self management skills and administer their caseload efficiently to ensure effective intervention focused on preventing avoidable admissions, rehabilitation and efficient discharge planning, depending on setting.

We are looking to recruit an enthusiastic and committed CAMHS Practitioners to join the Getting Advice Team Pathway within the Single Point of Access function.
This is an exciting time to join our evolving service in the Getting Advice Pathway, where we are committed to improving access to mental health advice and support for children, young people, and their families. This will be a dynamic role where no two days will be the same. This will involve triaging, assessment, and formulation of young people, families, plus signposting and engagement with partner agencies, helping to identify the most appropriate support for each young person at the most appropriate time.

Job summary
Reporting to the Project Director of Major Capital Programmes, you will be responsible for directing and leading the delivery of complex major capital programmes of strategic importance to the Trust to time and within budget.

We are seeking a Band 6 Nurse, Social Worker, or Occupational Therapist to join our Worthing Neighbourhood Mental Health Team as an Acute and Community Transition Practitioner.
This is a pivotal role bridging acute and community mental health services, ensuring clients experience a smooth, well-coordinated transition as they move between settings.
Key Responsibilities:
- Attend all relevant acute and community meetings, including Crisis Team MDT meetings, ward risk huddles/bed meetings, ward rounds, discharge planning meetings, and ATS zoning meetings.
- Act as Lead Practitioner for all new admissions to SPFT acute hospitals and ECR beds who are not already known to the service -- providing active involvement from admission through discharge, and for 4-6 weeks post-discharge. At the end of this period, either discharge to primary care, signpost to other agencies, refer to Pathfinder, or request allocation to long-term support.
- Liaise effectively with carers and families, and maintain strong links with Urgent Care, Havens, and the Staying Well Service.
- Coordinate with team Lead Practitioners supporting clients currently in hospital, facilitating liaison with ward staff as required, including discharge coordinators and clinical bed managers.
- Build and maintain effective relationships with external organisations, including CGL, Adult Social Care, and West Sussex Talking Therapies.
Main duties of the job
As an Acute and Community Transition Practitioner you will be working closely with colleagues in our hospital discharge teams, to ensure that we achieve timely and safe discharges for patients. You will have oversight of all patients identified with complex needs, ensuring that the discharge planning and pathways are being robustly and proactively managed for these individuals.
You will play a key role in supporting our inpatient services in achieving timely discharges, supporting our aspiration to offer care and treatment for all patients within the most appropriate environments.

The department supports several of our band 6 MSK rotations and our MCATS clinicians. You will also be allocates an MSK mentor and participate in the preceptorship programme at the trust. Main duties of the job
Your main duties will include the MSK assessment and treatment of a range of patients from primary and secondary care. Your workload will be varied and include some 1:1 sessions and involvement with some group activities. You will also have an opportunity to shadow the MSK clinical specialists and an MSK career pathway leading to band 6 and 7 rotational opportunities within the organisation including post graduate course funding . There is no on call or 7 day working required in this role.

An exciting opportunity has become available for a part time band 6 Senior Children's Nurse to work within the Bexley and Greenwich Community Children's Nursing Team and integrated Hospital at Home Team.

The team provides a seven day service from 08.00 to 20:30, there is also 24 hour on call provision for children & young people who require end of life care at home.
Main duties of the job
The CCN/HAH is part of an integrated network of services for children in Bexley and Greenwich providing highly skilled, nursing care to children and families with nursing needs in their own homes or community settings.
To provide a high standard of evidenced based nursing care for children and young people with the following conditions: acute and long term conditions, children with disabilities and complex conditions including those requiring palliative and end of life care.
This is a post suitable for a Children's Community Nurse who should have experience of working within a Community Children's Nursing Team.

We are excited to offer a new opportunity for an experienced and passionate midwife to join Frimley Health NHS Foundation Trust as a Pelvic Health Specialist Midwife. We are a traditional GP practice dedicated to delivering safe, high-quality, patient-centred care. We prioritise easy access, effective signposting to additional services and technological innovation to improve efficiency. We are looking to recruit a committed, patient focused Therapeutic Radiographer to join our friendly team here at the Rosemere Cancer Centre in a Bank Capacity. Our department at Lancashire Teaching Hospitals NHS FT is a busy centre providing ISO9001:2015 certificated external beam radiotherapy services for Lancashire and South Cumbria. We are part of a Radiotherapy Operational Delivery Network collaborating with the Christie NHS FT and The Clatterbridge Centre NHS FT to provide high quality radiotherapy across the North West of England.

We have seven matched Elekta linear accelerators all with cone beam imaging capability, two Phillips BORE CT scanners and an Xstrahl Superficial Unit networked by a MOSAIQ patient management system. We employ a wide range of techniques including IGRT, IMRT and VMAT. Radiographer-led daily IGRT is routine for complex and innovative radiotherapy delivery procedures. We are a commissioned provider of Intracranial SRS/T and SABR. Surface guided Radiotherapy has been rolled out across the whole department in the last year and we have installed the first Elekta Harmony Linac in the UK.
Main duties of the job
You will rotate through pre-treatment and treatment delivery allowing you to practice in all aspects of external beam radiotherapy localisation, planning, data prep, treatment delivery and imaging. New graduates are supported by a well-established Preceptorship programme and lifelong learning is central to both Departmental and Trust values.
The Radiotherapy department has successfully integrated role extension into its culture and has implemented Advanced Clinical Practitioner and Consultant Practitioner roles in Therapeutic Radiography. Additional to this there is a strong research ethic with opportunities for active involvement with research at all levels.
You will work flexibly to deliver an excellent service in all Radiotherapy work areas, ensuring efficient throughput of workload and delivery of high standards of technical and patient care whist acting as a role model and patient advocate
