Job summary
The Jubilee Street Practice Ltd is looking for an organised, proactive and patient-focused Care Coordinator to join our team. The job is being advertised at 22 hours per week. Working arrangements can be agreed with the successful candidate and in line with Working Time Regulations.
This is a key role in helping patients receive the right care at the right time. Much of good general practice happens before and after the consultation: identifying who needs review, contacting patients, helping them understand why they are being recalled, coordinating appointments, tracking follow-up and making sure actions do not get missed.
You will support the practice with proactive recall, planned care, long-term condition reviews, NIS, LES, QOF and local contractual work. You will work closely with GPs, nurses, HCAs, pharmacists, social prescribers, Patient Assistants, administrative colleagues and external services to ensure patient follow-up is clear, safe and timely.
This role would suit someone who is highly organised, confident using digital systems, comfortable speaking with patients, and able to work independently while staying closely connected to the wider team.
It is highly recommended that candidates download the Job Advert, Job Description and Job Specification supplied within the Supporting Information section for this role.
Candidates that are successfully shortlisted for this role will be invited to interview between 13 and 16 October, 2026.
Main duties of the job
The Care Coordinator will support the practice in delivering proactive, coordinated and well-tracked patient care.
The role will focus on recall activity, planned care coordination, patient engagement, NIS, LES, QOF and local contractual work, EMIS searches, trackers, reporting systems, external service links and safe escalation of concerns.
The post-holder will work within a non-clinical role, supporting patients to access appropriate care and ensuring clinical questions, safeguarding concerns or risks are escalated to the appropriate clinician or manager.
Please see the full Job Description and Job Specification within the Supporting Information section for this role, to understand the main duties of the role.
About us
The Jubilee Street Practice Ltd is a GMS, twice-rated CQC Outstanding practice in Tower Hamlets with a reputation for ambition, innovation and high-quality care. We are proud of our clinical standards, but we are just as proud of our culture that promotes curiosity, is values-led and committed to learning. Watch our introduction video here: Welcome to The Jubilee Street Practice
Our work has been recognised in national and international improvement spaces, including IHI and BMJ Conference in Oslo in 2026. We are continuing to develop new models of access, digital working, patient feedback, staff wellbeing, governance and neighbourhood care. We are a practice that tests ideas, learns quickly, shares openly and takes improvement seriously.
We are not a practice that sees Outstanding as a badge on the wall. We see it as a responsibility to our local community and our staff.
We are also a practice with a strong internal culture. We invest in staff, protect learning time, encourage leadership at all levels, and create space for people to contribute beyond the narrow boundaries of their job title.
Our population is diverse, our work is complex, and our ambition is high. We are looking for an individual who wants to be part of that.

Job summary
Our UTC operates a Digital Front Door, which is a clinically driven digital triage tool designed to improve safety and efficiency across urgent care and is changing the way patients reach and move through urgent care in Greenwich. Our patients use the DFD to check in and complete a short, guided triage, enabling faster prioritisation and smoother patient journeys.
Whats New?
Our clinical system supplier is developing a new appointment book, which for the first time will allow patients to be booked directly into an urgent care appointment rather than simply being given an arrival time or having to sit and wait.
We are redesigning our Directory of Services (DOS) so that appointment slots align to NHS Pathways disposition timescales. A patient with a see a GP within 48 hours disposition can be offered a later appointment, while presentations more likely to need an emergency department can be booked within two hours.
NHS Pathways sorts patients leaving the digital front door into a number of groups, including those who are not urgent and can safely be seen later. Booking to the right slot at the right time helps us manage surge, improve flow and demand, and reduce attributable breaches.
Greenwich Health will be launching a new system where NHS 111 can book patients into a named urgent care appointment.
This role is available on a Full Time or Part Time (24 to 40 hours/week) basis. 12-hour shifts finishing no later than 22:00. 7 days/week availability will be required.
Main duties of the job
This is the perfect opportunity to be part of a diverse team at the very front of urgent care in Greenwich, and to help deliver a model that is the first of its kind in South East London.
As a Front Door Navigator you will be the first person a patient meets when they arrive at emergency and urgent care. Working from a tablet at the entrance, you will welcome patients, establish why they have attended, check whether they hold a booked appointment made through NHS 111 or the digital front door, and direct them to the right place without delay.
You will keep people moving away from the reception queue and make sure that patients who are unwell, distressed or vulnerable are brought to the attention of a clinician immediately. This is a non-clinical role, you will not assess, triage or advise on a patients condition, but your judgement about when to escalate is central to keeping patients safe.
You will also play an important part in helping us evaluate the new model, recording how patients arrive and where they are directed, and feeding back to the UTC Lead where the appointment book or the Directory of Services is not working as intended.
This is a genuine opportunity to be part of something new and exciting in South East London, with a real opportunity for personal growth and to influence Urgent Care in your area. The service is 24/7, 365 days per year which means a range of shifts will be available.
About us
Greenwich Health is the GP Federation for the Greenwich Borough and have a not-for-profit ethos. This means we reinvest all our surplus into developing our staff, our organisation and to support our GP Practices in being sustainable for the future. We are committed to providing quality of care to our patients whilst building resilience for General Practice and providing services and programmes of work to deliver care at scale to the Greenwich Population. This role will be part of an exciting opportunity to work with leading specialists in the creation of an innovative Urgent Treatment Centre.

Job summary
The Shrewsbury and Telford Hospital NHS Trust is recruiting a Trainee Critical Care Outreach Practitioner (CCOP) to join the Critical Care Outreach Team (CCOT) during the expansion to a 24/7 Trust-wide service. This role is ideal for a compassionate, motivated individual committed to improving outcomes for acutely deteriorating patients.
The CCOT provides early recognition, assessment, and management of deteriorating patients across the Trust. As a Trainee CCOP, you will deliver patient-centred care, assess and manage critically ill patients, provide autonomous specialist advice, and work collaboratively with the multidisciplinary team to support escalation decisions and facilitate timely admission to critical care.
The role includes follow-up assessments of patients discharged from critical care, participation in the cardiac arrest team, and providing clinical leadership and support in the day-to-day operation of the outreach service.
The successful candidate will undertake a structured academic and clinical training programme aligned with the National Outreach Forum CCOP Framework, including an Enhanced Clinical Practitioner (ECP) Apprenticeship and non-medical prescribing qualification. Successful completion of all academic and clinical competencies within the required timeframe will lead to progression to a substantive Band 7 Critical Care Outreach Nurse Practitioner post.
Informal visits and discussions about the role, service development, and the Trust are welcomed.
Main duties of the job
The Critical Care Outreach Practitioner will work autonomously within all specialities across The Shrewsbury and Telford Hospitals NHS Trust and provide expert clinical practice and leadership on a 24-hour shift rotation that covers 365 days per year including bank holidays, providing expert care and intervention for the acutely ill, deteriorating and critically unwell ward patients across the Trust.
Perform comprehensive patient assessments based on history taking and physical examination of the acutely unwell adult patients to arrive at a differential diagnosis. Undertake appropriate investigations and initiate, evaluate, and modify a range of interventions to prevent patient deterioration.
Assist and facilitate in the timely admission to the critical care area and involve the appropriate members of the multidisciplinary team in the decision-making process when determining escalation plans to prevent further deterioration or inappropriate interventions.
Respond to cardio-respiratory arrest and medical emergency calls with the skills to lead in emergency situations. Debrief teams if required and assist in the appropriate aftercare of the patient.
Enable timely discharges for patients from the Critical Care Unit by supporting their continuing recovery and rehabilitation within the ward environment.
Act as an independent non-medical prescriber.
About us
Our Poppy's Promise Commitment
We believe that compassionate care sits at the heart of everything we do. Every member of our team -- clinical or non-clinical -- plays a vital role in ensuring that patients, families, and colleagues are treated with dignity, empathy, and respect at all times.
We expect all staff to demonstrate compassion not only through their words, but through their actions: by listening attentively, communicating clearly and kindly, and recognising individual needs, feelings, and circumstances of those in our care.
Effective communication is central to safe and high-quality healthcare. We value colleagues who take the time to understand others, explain information in a meaningful way, and respond to concerns with patience and understanding.
By joining our team, you are committing to uphold these values -- ensuring that every patient feels heard, respected, and valued, and that compassion is embedded in every interaction, every day.
