To work as a clinical nurse specialist as part of the hospital-based specialist palliative care service, providing a 7 day a week nursing service.

We are looking to recruit one Part time (3 days a week, Weds - Fri) Clinical Pharmacist with General Practice experience to join our ever expanding Clinical Pharmacist team. You will join a team of over 40 Clinical Pharmacists and Pharmacy Technicians supported by Lead pharmacists. The role will be based at one or two surgeries in Brompton PCN (in person).
Successful recruits will ideally have:

Main duties of the job

The Primary Care Clinical Pharmacist is a clinical practitioner working as part of the multidisciplinary team to optimise medicines use, improve patient outcomes and support safe, evidence-based prescribing across general practice. The role combines direct patient care with medicines optimisation, chronic disease management and collaborative working across primary care.
1. Clinical Responsibilities
Patient Care
Long-term condition clinics
Run long-term condition clinics for chronic diseases such as atrial fibrillation, asthma, COPD, chronic kidney disease, diabetes, depression, heart failure, hypertension, HRT and womens health, reviewing patients with single or multiple medical problems where medicines optimisation is required.
Make appropriate recommendations to GPs and other members of the practice team to optimise medicines use, and implement changes directly where these fall within the post holder's scope of practice and competence.
Clinical medication reviews (including Structured Medication Reviews)
Undertake clinical medication reviews with patients, producing recommendations for prescribing and monitoring.

Order and interpret relevant diagnostic tests to support clinical decision-making.

Support implementation and ongoing development of repeat prescribing systems and processes.
Manage the repeat prescribing reauthorisation process by reviewing patient requests for repeat prescriptions and reviewing medicines reaching review dates and flagging up those needing a review.
Ensure patients have appropriate monitoring tests in place when required.
Process acute prescription requests in line with practice protocol.
Management of minor and common ailments
Where competent to do so, assess and manage patients presenting with minor or self-limiting ailments, signposting or referring appropriately, to help release GP appointment capacity for patients with more complex needs.
Digital tools, documentation and safety-netting

Unplanned hospital admissions
Review the use of medicines most commonly associated with unplanned hospital admissions and readmissions through audit and individual patient reviews.

Reconcile medicines following discharge from hospitals, intermediate care and care homes, identifying and rectifying unexplained changes.

Undertake clinical audits of prescribing in areas directed by GPs, practice managers or ICB.
Feedback the results and implement changes in conjunction with the practice team.
Implementation of local guidelines and formulary recommendations
Analyse practice prescribing against local ICB criteria (e.g. prescribing dashboards, high cost drugs, and specials) and engage with members of the practice team where prescribing patterns do not align with local ICB policies, guidance, and prescribing standards.
Support cost-effective and evidence-based prescribing in line with ICB formulary guidance and local prescribing policies.
Liaise with hospital colleagues where prescribing needs to be returned to specialists (red drugs) or subject to shared care (amber drugs).
QOF medicines-related targets
Support the practice team(s) in delivering medicines related QOF targets.
ICB Prescribing targets
Contribute to the delivery of ICB prescribing-related targets, including the annual Medicines Optimisation Enhanced Service (MOES) scheme.
Monitor practice performance against MOES indicators, identifying areas of underachievement and supporting action to address these.
Provide regular updates and reporting on progress against MOES and other prescribing targets to the Neighbourhood Pharmacy Lead/Head of Clinical Pharmacy.

This is a part time role of 34.75 hours per week with a working pattern of 4 days as below:

To ensure enhanced patient access to the practice we ask our Care Navigators to work around 1-2 Saturdays shifts per year.
Core shift hours are Monday to Friday, 07:45-13:00 & 13:00-18:30.
You would work across 3 of our sites in Waterlooville on a rota basis (published 4 weeks in advance).

Main duties of the job
As a key member of the Practice team, you will manage patient enquiries by telephone and provide a professional, friendly, and welcoming presence at the Surgery reception desks, acting as a positive ambassador for the Practice at all times.
You will support patients in accessing appointments and appropriate healthcare services, ensuring all enquiries are handled courteously, efficiently, and effectively. The role requires maintaining up-to-date knowledge of Practice services, procedures, events, appointment systems, and booking protocols to provide accurate advice and assistance.
You will work closely with colleagues across the Practice and with external healthcare providers to support the smooth delivery of services, coordinate patient care, and respond appropriately in emergency situations. Through excellent communication and customer service, you will help ensure a positive patient experience and the efficient day-to-day operation of the Practice.

An exciting opportunity has emerged for an experienced Occupational Therapist, Social Worker, or Nurse to join the Newham Early Intervention Service (NEIS). This service supports individuals aged 18 to 65 who are facing their first episode of psychosis. Our goal is to mitigate the effects of psychosis by minimizing the duration of untreated symptoms and engaging clients in a range of biopsychosocial interventions.

Main duties of the job
Be a member of the Early Intervention service being responsible for the delivery of services to people who are residents of Newham with First Episode of Psychosis, as defined by the service referral criteria.
Establish a network of relationships with educational, vocational and leisure facilities, and maintain a database/resource of such organisations. Interface effectively with other community organisations.
Be involved in service evaluation data.
Supervise and co-ordinate the work of trainees/students of your related profession, as appropriate.
Be involved in training/health promotion of First Episode Psychosis as required.

Main duties of the job

- Guide participants through screening appointments, ensuring accurate record-keeping.
- Administer eye drops and accurately check pupil dilation within specified timeframes.

Job summary
The Urgent Care Practitioner will provide autonomous clinical care across NEMS urgent care services. The role is open to suitably qualified UCPs, ECPs and Nurse Practitioners who are independent non-medical prescribers, and is designed to provide a mix of telephone and face-to-face consultations across Integrated Urgent Care Services, urgent treatment centres and other agreed service models.
Main duties of the job
The post holder will be an experienced registered clinician, able to assess, diagnose, treat, refer and discharge patients with undifferentiated urgent care presentations. They will work within their professional registration, agreed scope of practice, NEMS policies and local clinical pathways.
Telephone consultations will usually be undertaken within Integrated Urgent Care Services. Following induction, and where agreed by the service, some telephone consultation work may be undertaken remotely from home, provided that the post holder is meeting agreed KPIs, quality standards, governance requirements and information governance expectations.
Face-to-face work will include assessment and management of patients presenting to NEMS urgent treatment services. The post holder will be expected to recognise serious illness, clinical deterioration, safeguarding concerns, mental health risk and presentations requiring escalation or transfer to another service.
The role will ideally include a mix of telephone and face-to-face work. Multisite working may be agreed, although some NEMS locations provide both from the same base. Job summary
Join Lagans Foundation as a Childrens Play and Health Care Worker and help create meaningful moments for children and young people with health and developmental needs.
Based in Liverpool, you will support children and young people aged 019 within their own homes and communities. This is more than a traditional care role. You will combine high-quality personal care with play, learning, interaction and emotional support, helping each child to build confidence, develop skills and experience fun and enjoyment.
You will build positive and trusting relationships with children and their families, adapting your approach to each childs individual personality, communication and support needs.
You do not need to know everything from day one. We provide a comprehensive induction, training, mentorship, supervision and ongoing development to help you build the confidence and competencies required for the role.
This is a permanent, 16-hour-per-week position, with weekday evening shifts and a six-hour weekend shift on either Saturday or Sunday as part of a rolling rota.
