About us
Sutton Primary Care Networks is the collaborative vehicle for the four Sutton PCNs and the former Sutton GP Federation, working together as a single entity. We are a Community Interest Company that aims to reinvest surpluses back into general practice and primary care for the benefit of our practices and the people who form the communities we serve.
Sutton has four PCNs whose boundaries are based on historical relationships and organisational groupings. There are some similarities in residents' health and care needs that are unique to a particular PCN, and many more that are shared across all four PCNs.
Our four PCNs represent all 21 GP surgeries in Sutton. Their collective footprints cover the London Borough of Sutton and its residents, who can access and benefit from the services and improvements offered by Primary Care Networks.
Primary Care Networks (PCNs) are a key part of the NHS Long Term Plan, with all general practices being required to be in a network by June 2019, and CCGs/Integrated Care Boards (ICBs) being required to commit recurrent funding to develop and maintain them.
The networks will have expanded neighbourhood teams comprising a range of staff, including GPs, pharmacists, district nurses, community geriatricians, dementia workers, and Allied Health Professionals such as physiotherapists and podiatrists/chiropodists, alongside social care and the voluntary sector.

Make a Difference Where It Matters Most
Lagans Foundation is a specialist childrens charity providing home health care, respite and support to families of children and young people aged 019 with complex health needs.
Our charity was founded from lived experience of caring for a child with complex congenital heart defects and feeding difficulties, and from understanding just how difficult it can be for families to find the right support once they are at home.
Today, our carers work alongside families across the North West, providing individualised care and respite to children and young people with a wide range of complex and high-dependency health needs.
For some families, having a skilled carer they trust means a parent can rest, spend time with another child, have a meal, take a shower or simply have some time knowing that their child is being safely and compassionately cared for.
For the child or young person, our role is to understand their individual needs, their communication, their routines, their preferences and the clinical care that keeps them safe and well supported.
No two children, families or shifts are the same.
Were Looking for an Advanced Carer
We are now looking for an experienced Advanced Childrens Play & Health Care Worker Carer Level 3 to join our specialist care team.
This is not an entry-level care position.
You will already have experience supporting people with complex or high-dependency health needs and have the confidence and clinical competencies required to provide more advanced care.
You will work in children's own homes and communities, providing safe, compassionate and individualised support while giving parents and unpaid carers valuable respite.
Depending on the individual child, this could include complex respiratory support, enteral or parenteral nutrition, seizure management, medication, personal care, positioning, communication support and other specialist interventions.
Just as importantly, you will get to know the individual child and family and understand what good care means for them.

- Work confidently within individual care plans, risk assessments and clinical protocols.
- Deliver advanced clinical interventions within your assessed competencies.
- Recognise changes or deterioration in a child's condition and escalate concerns appropriately.
- Administer medication safely in accordance with care plans, training and policy.
- Provide personal care while maintaining dignity, comfort, privacy and respect.

- Understand each child's individual way of communicating and respond to their preferences and needs.
- Build professional, trusting relationships with children, parents and families.
- Provide families with confidence that their child is receiving safe and compassionate care while they take a much-needed break.
- Maintain accurate and timely care and clinical records.
- Support and mentor other carers in clinical practice.
- Share your experience and help less experienced colleagues develop their confidence and competence.
- Work closely with Area Care Team Leaders, our Clinical Lead and other health and care professionals.
- Identify and escalate clinical, safeguarding or care concerns promptly.
- Maintain your professional development, mandatory training and clinical competencies.

- An understanding of professional boundaries and when to seek clinical support
- Excellent communication skills with children, families, colleagues and professionals
- A full UK driving licence and access to your own vehicle with business-use insurance

What Makes Care at Lagans Different?
Technical competence is essential but it isn't everything.
When you enter a family's home as a Lagans carer, you are being trusted with someone incredibly important to them.
We therefore look for carers who understand that excellent complex care is also about dignity, consistency, patience, communication and trust.
Some children we support may communicate verbally and be active within their community. Others may communicate non-verbally, have very limited mobility or require significant clinical intervention throughout their care.
Good care looks different for every child.
Your job is to understand the person you are supporting, work safely within their individual care plan and help provide the family with reliable, compassionate support.
Our Values
At Lagans Foundation, we expect our team to demonstrate our values in the way they care for families and work with one another:
Passion Taking pride in providing excellent care and making a meaningful difference to families.

Daytime Induction and Competency Shifts
Although this particular waking-night package does not routinely involve personal care or play-based support, these remain part of the wider Childrens Play and Health Care Worker role and competency framework.
During your induction, training and competency period, you will therefore be required to undertake daytime shifts. These shifts will enable you to:
- Get to know the child and build a trusting relationship.
- Develop positive rapport with the family.
- Understand the childs individual routines, communication and support needs.
- Complete required observations and competency assessments.
- Develop and be assessed in relevant personal care and play-based support competencies.
- Complete the required BiPAP and ventilation training and competency assessment.

Documentation and Administration
You will:
- Complete accurate visit records and documentation following each shift.
- Accurately record relevant observations and support provided throughout the night.
- Complete accident, incident and safeguarding documentation where required.
- Accurately log in and out of visits in accordance with the rota and organisational procedures.

You will be expected to:
- Complete Lagans Foundations comprehensive induction programme.
- Complete all required online and face-to-face mandatory training.

- Take responsibility for continuing professional development and maintaining the knowledge and skills required for the role.
- Work within the limits of your training and assessed competencies at all times.
Professional Responsibilities
You will:
- Maintain high standards of professionalism, reliability and timekeeping.
- Deliver all hours of care assigned to you on the agreed rota.
- Maintain confidentiality and appropriate professional boundaries.
- Follow safeguarding, health and safety and care standards.
- Demonstrate adaptability, resilience and effective communication.
- Promote equality, diversity and inclusive practice.
- Represent Lagans Foundation positively when working with children, families, colleagues and other professionals.
- Contribute to maintaining and continually improving standards of care.
Working Pattern
This is a permanent waking-night position with a regular shift of 10:30pm6:30am.
The working pattern alternates between:
- Two waking-night shifts one week
- Three waking-night shifts the following week
This equates to an average of approximately 20 hours per week.
You must be able to reliably commit to the required rota and remain awake, alert and responsive throughout every waking-night shift.
Daytime shifts will also be required during induction, training and competency assessment before you progress to independent waking-night support.
Our Values
Everyone at Lagans Foundation is expected to demonstrate our four core values:
Passion Deliver heartfelt care and strive to make a positive difference every day.

The role will support delivery of the Trusts elective access standards, with a particular focus on 18-week Referral to Treatment performance, waiting list management, pathway validation, staff training and data quality improvement.
A key part of the role will be providing leadership and day-to-day management for the corporate RTT validation and training team. You will be responsible for planning and overseeing routine validation activity, monitoring progress against agreed validation schedules, supporting staff development, managing workloads, and ensuring the team delivers a high-quality service. You will review waiting list statistics, cancellation data, diagnostic reports and other elective care information, using tools such as Excel to filter, analyse and present data in a clear and user-friendly format. You will produce reports, action plans and updates that support operational decision-making, performance oversight and data quality improvement. The role will also involve working with the Trust Information Team to develop weekly reports on cancellations, diagnostic issues and breach reasons.
You will support governance and assurance by participating in audits of waiting lists, reviewing compliance with the Elective Access Policy and RTT rules, and communicating audit findings to relevant services. You will provide regular reports highlighting risks, themes, data quality issues and opportunities for improvement. The post holder will also provide an important link with data quality and EPR groups, ensuring that relevant issues are escalated, feedback is provided, and agreed actions are taken forward.
The role also includes project management responsibilities. You will support the delivery of elective access and service improvement projects to agreed timescales and quality standards. This will include maintaining project plans, tracking progress, documenting issues, completing assigned tasks, and providing accurate written and verbal updates. You may also work with specialist teams, including the EPR team, where projects involve changes to systems, processes or training requirements.
Communication and relationship-building are central to the role. You will need to work confidently with a wide range of staff across the Trust, including operational service managers, elective access teams, information colleagues, patient contact centre staff, outpatient reception teams, nursing staff, ward staff, medical secretaries and clinicians. You will be expected to communicate complex information clearly, adapt your approach for different audiences, and support collaborative working across divisions and services.
