Main duties of the job
We provide overnight short breaks for children and young adults (5-17) with complex learning disabilities, health and care needs. Our 'whole child' approach ensures empathetic, holistic care, fostering stability, belonging, and positive well-being. We champion diversity, promote equality, and partner with families and professionals to deliver tailored care for all. As a Waking Nights Care Support Officer, you will provide waking night support for the children and young people, where no two nights are the same. You'll work a ten-hour night shift (specific hours to be determined). The ideal candidate should be flexible for planned annual leave and short-notice sick leave coverage. Experience of caring for children and young people is essential, ideally you will have a background in supporting children and young people with complex health, care and learning needs. An NCFE qualification in Health and Social Care in residential childcare at level 3 is desirable but not essential, as this can be undertaken as part of the role. A strong understanding of confidentiality, trust, child-centred care and support and effective safeguarding is a must. This role is designed to support Digital Transformation within the Southport and Formby Primary Care Network (PCN).
It will involve working centrally at our main Southport office suite, as well as visits to the fourteen practices within Southport and Formby.
The post will support several core functions:
- Digital transformation: Support the PCN and its practices in utilising digital solutions such as online consultation systems and digitised workflows to improve efficiency and service delivery.
- PCN services: Enable PCN services to digitise workflows and utilise data more effectively to better understand and optimise their services.

Job summary
We are seeking an enthusiastic and patient-focused clinician to join Kent Community Health NHS Foundation Trust Dental Services as a Dental Officer on a full-time maternity cover basis. Based at Aylesham Health Centre, you will provide high-quality dental care for children and adults with additional physical, medical, behavioural, or mental health needs across a variety of community settings. The role includes delivering Tier 2 dental care, supporting patients requiring sedation or general anaesthesia, and providing treatment in clinics, prisons, mobile dental units, and patients' homes when required. Main duties of the job
o To provide tier 2 level dental care for paediatric and special care patients who have been referred into the service and remote triaging for dental patients in pain and patients on the waiting list as directed by the clinical director.
o To provide dental treatment under inhalation sedation (intravenous sedation if required by service need) and general anaesthetic
o To provide dental treatment at HMP prison sites, mobile dental units and domiciliary care as required by service need
o Involve patients in their care and treatment. Offer information, support and motivation to patients regarding oral health, diet and smoking
o To work with other healthcare professionals and stakeholders to provide a seamless care package for KCHFT dental patients
o Monitor the application of clinical governance practice including dental nurse standards, infection control and cross infection

Main duties of the job
We are looking for experienced, dynamic, motivated and compassionate Staff Nurses to join our teams on L03, L04, L06, J54 and J81 who can ensure the consistent delivery of high-quality patient centred care to the patients nursed within our critical care units.
There may be an opportunity for successful candidates to undertake a rotation across the 5 critical care units gaining vast experience from across the speciality. The successful candidates will be expected to work clinically within the multi-disciplinary team, maintain high standards of practice and deliver outstanding critical care.
All applicants are required to have current UK NMC registration, with evidence of registrant clinical practice in the UK, or be a final year nursing student studying in the UK
Please note that Interviews for this post will be held face-to-face only to enable participation in a scenario-based assessment therefore applicants must be able to attend in-person.

An exciting opportunity has arisen to join our established and highly respected Critical Care Outreach Team. The team delivers a 24/7 service across Stoke Mandeville and Wycombe General Hospitals at Buckinghamshire Healthcare Trust.
We are looking for a highly motivated, innovative and critical care practitioner to deliver patient-focused care to acutely ill, deteriorating and critically ill patients across the trust. The successful candidate will need to display excellent communication, interpersonal and leadership skills, be able to respond to rapidly changing, high pressure situations and priorities. You will be able to work autonomously and as part of a team and have good critical thinking and decision making skills. You will be instrumental in the early recognition of deterioration and deliver an effective response, liaising with the patient's own team. You will be able to facilitate smooth transitions of care for patients stepping down from Intensive and High Dependency Care to the general wards and be involved in the provision of a range of education for the multidisciplinary team.
Main duties of the job
The post holder will be an experienced practitioner providing early intervention to acutely unwell adults utilising appropriate assessment and observation skills to develop treatment plans. Under supervision of the senior Critical Care Outreach (CCO) team, they will assist in promoting the development of expanded practice throughout the Trust whilst ensuring the role of the CCO service evolves to meet changing demands.
The following objectives for Critical Care Outreach Services (DoH, 2000) are key components of the role:
o Avert admissions by identifying patients who are deteriorating and either helping to prevent admission or ensuring admission to a critical care bed happens in a timely manner
o Enable discharges by supporting the continuing recovery of discharged critical care patients and their significant others.
o Share critical care skills with staff in wards, ensuring enhancement of training opportunities and skills practice and to use information gathered from the ward and community to improve critical care services for patients and relatives.

The neonatal CNS is a vital member of our CNS Team, ensuring that babies with life-limiting or life-threatening conditions receive compassionate, specialist, family centred care from the earliest stages of life. This role is essential in delivering equitable, high quality palliative care from diagnosis onwards, ensuring that every baby and family has access to expert support wherever care is delivered.
Working with our Nurse Consultant and Lead Consultant, this is a fantastic opportunity to be part of the clinical team that shapes and delivers a fully integrated 24/7 paediatric palliative care service across Sussex, working across home, hospice, community and hospital settings to improve continuity of care.

An exciting opportunity has come up for the Adult Community Nursing Service, Night Nursing. We are currently looking for a skilled person who can do 21 hours a week (1 overnight shift- 11 hours and 2 twilight shifts- 10 hours) to work across the Hounslow locality based at Clayponds Hospital on a Fixed Term Contract for 6 months to cover maternity. The service operates from 19:00 until 07:00 seven days a week. The service has 2 shifts, 19:00 until 12 midnight (Twilight) and 19:00 until 7:00 AM (Overnight).
Please note that the service is currently undergoing service re-design and the operational hours will change by 7th December 2026 from 7pm-7am to 8pm-8am under phase 1 transformation project. The phase 2 (TBC) will be happening shortly after the implantation of phase 1 with the Night Service re-design involving the twilight and the overnight shifts. We are looking for someone who will be very flexible in supporting the new service structure in the future.
The service also delivers urgent community response between 20:00 and 22:00 as part of the service specification and to prevent hospital admission.
Main duties of the job
To carry out clinical duties and be prepared to undertake clinical skills as per needs of the service and specific patient after training and competence assessment with support from an identified mentor for example, venepuncture, Blood Pressure, pulse and temperature recording, urinalysis, blood glucose monitoring, simple wound dressings and non-complex tasks such as continence reviews.
To undertake basic duties as delegated and overseen by the registered nurse and actively report any changes in a patient's condition in a timely way to the registered nurse.
Liaise with other health care and multi-agency workers to provide a multi-disciplinary approach to care.

Job summary
We are looking for a Band 5 Nurse with at least 12 months post qualification experience to complement our existing nursing team and to work within our dynamic and friendly team.
The successful applicant will join a multidisciplinary team of skilled and highly motivated staff who provide quality care to children with a wide variety of conditions including medical, surgical, trauma, orthopaedics plastics, ENT and the largest Bone Marrow Transplant Centre non-malignant blood cancers .
We provide specialist education at every level, including the opportunity for Band 5 Nurses to undertake the Paediatric Critical Care Course and specialist training, such as Haemafiltration. We have the support of 2 dedicated Family Liaison Nurses who support our families and staff through a culture of caring and compassionate working.
We are looking for a Band 5 nurse who is committed to delivering high quality care and supporting our current and future services. The post holder must be willing to engage in all the development opportunities and be enthusiastic about their own development and growth.

Main duties of the job
- The post holder will work with the ward manager and multi-disciplinary team members inmonitoring and maintaining policies, standards of nursing care and staff development, aspart of the ward team.

We are looking to recruit compassionate, highly motivated team player with excellent communication skills to join our patient centred supportive specialist Stroke and Neuro rehabilitation unit.
Airedale NHS Foundation Trust is committed to delivering a safe, fast and effective acute stroke service with the support of the Bradford Teaching Hospital Foundation Trust who will provide initial hyper acute assessment, treatment and stroke care. Ward 5 is a 24 bedded specialist unit to provide seamless, evidence based, multidisciplinary care within the most appropriate care setting to individuals who have had a stroke and other neurological needs. Support and guidance will include care givers so as to optimise the clinical outcome and care experience for both patient and carers.
Our motivated hardworking nursing team work together with specialist therapy team to provide safe, caring and supportive environment to achieve rehabilitation goals. We provide unit specific induction programme and regular ward based training sessions to maintain our high standards of quality, safety and patient experience.

Job summary
We are delighted to invite applicants to join our Emergency and Acute Care (EAC) directorate and become part of the Emergency Department and Same Day Emergency Care (SDEC) pharmacy team.
This post is patient-facing and involves working alongside our established clinical pharmacy team to deliver a specialised service to our ED and SDEC. You will have a key role in supporting with medicines reconciliation, counselling patients and working with the team to manage the ward automations systems, helping to ensure the effective running of the Pharmacy support with the Emergency Department and SDEC.
You will be fully supported by a team of experienced Pharmacists, Chief Medicines Management Technician, a Medicines Management Technician and Assistants.

Main duties of the job
In this role you will have leadership and clinical responsibilities and will be one of the Senior Medicines Management Technicians (MMT) leading in patient care and ward automations systems management for the Emergency Department and SDEC.
You will report directly to the Chief Medicines Management Technician for ED and SDEC and assist in implementing service improvements within the department.
You will deliver patient-facing clinical pharmacy care to the ED and SDEC areas and will work closely with the EAC Specialist Clinical Pharmacists to ensure the delivery of a safe, efficient, high quality and cost effective technical service.

To be responsible for the local implementation of standards relating to medicines storage and security including:
Ambient and fridge temperature monitoring
Security of medicines storage in treatment rooms, drug trolleys, fridges, emergency trolleys and boxes, controlled drugs cupboards.
To participate and support in project works under the direction of the Lead Directorate Pharmacist.
The post holder will be expected to take full part in the 7 day shifted rota and bank holidays duties.

This is an important role in supporting the effective and efficient operation of the R&D Office through the timely provision of complex research governance advice and services for primary care, community care and wider health and care settings, and in service of NIHR East of England Research Delivery Network and related contractual obligations.
