We are looking to recruit a new Care Navigator.
The working hours are 37 per week split over 4 days plus participation in the Saturday morning rota and the 'extended hours' initiative on some weekday evenings.

Our Community Transfer of Care Hub (CToCH) is looking for an organised Administrator to support our clinical and administrative teams in delivering Discharge to Assess pathways, which helps people leave hospital safely and access the right care in the community.

Working with colleagues across hospitals, social care, primary care, community services and the Integrated Care Board (ICB), you'll support both patients and their families throughout their discharge journey. This is a collaborative multi-disciplinary team, where you will help todevelop new ways of working that ensure people leave hospital safely, promptly and with the right support in place.

Main duties of the job
This role supports the delivery of the admin component within the Community Transfer of Care Hub (CToCH), this service supports the Discharge to Pathways from Acute settings.
As a CToCH Administrator, you will work alongside the Admin lead to provide comprehensive admin support to health and social care professionals, hospitals, GPs, service-users, their relatives and carers and other stakeholders both within Sirona and in the wider health community.
You will be required to answer telephone enquiries in a friendly and professional manner, ensuring effective and accessible communication with staff, service users, the general public and any others as appropriate.

- Establish and maintain filing and administrative systems so that written or digital information is easily accessible and stored securely.

An exciting opportunity has arisen for a Band 3 Frailty Support Worker on Ward 3 in Prince Philip Hospital, Llanelli.
The post offers the opportunity to develop your skills and experience in caring for frail patients. Dementia and frailty are a growing challenge within healthcare, and we, as healthcare professionals are at the forefront of providing the dignity, support and care that people deserve. The aim of the role would be to improve the patient experience. You would be expected to promote the patients' independence by focusing on their mobility, nutrition and hydration in order to maintain and maximize their functional ability.
Main duties of the job
The post holder should hold an NVQ level 3 in Health or equivalent. They will need to demonstrate that they have good communication skills and that they are an excellent team player as well as being able to work alone. They will be expected to work alongside both the nursing and the therapies team.

The Dementia Support Service is a well-respected, supportive and hardworking team of support workers, nurses and occupational therapists who are dedicated to optimising the care of in-patients with dementia and enabling best outcomes for them at the point of discharge.

Job summary
The post is suitable for intensivists wishing to enhance their specialist experience in all aspects of adult cardiothoracic critical care, including transplantation, mechanical assist devices, extracorporeal membrane oxygenation, focused ultrasound examinations and transoesophageal echocardiography (TOE). Applicants will be expected to have completed or be within two years of completing specialist training in their parent specialty.
Specialist Registrars currently enrolled in a UK training programme who wish to retain their National Training Number (NTN) whilst at Royal Papworth, should discuss this 'out-of-programme-experience' (OOP) with their Programme Director. Those not currently in a UK training programme will be appointed as Senior Clinical Fellows, and will be Locally Employed doctors within the Trust. UK applicants should be aware that time spent in a Trust appointed post may not be counted towards the award of the Certificate of Completion of Training (CCT). Prospective approval for up to one year can normally be sought from the relevant Royal College when a numbered post has been obtained following such an appointment. Overseas candidates are advised to check the latest GMC requirements for registration, including English language skills.
This post is available for a period of 6 to 12 months starting February 2027.Main duties of the job
*To participate fully in the delivery of critical care services, research, and training and educational programmes;
*To maintain a logbook and appropriate records of progress;
*To participate in regular performance reviews;
*To provide an out of hours' service;
o The ICU Specialist Registrars (Resident) or ICU Fellow will provide immediate support for the critical care areas and emergency resuscitation support for the entire hospital;
o The daytime ICU cover is provided by 3 ICU consultants along with a minimum of four doctors, including ICU fellows, ICU higher specialty trainees, Internal Medicine trainees and Foundation Programme Doctors. Job summary
The Technology Enabled Care Service - assesses, installs, and monitors assistive technology equipment across predominantly B&NES, Bristol and South Glos. The service promotes the use of assistive technology across the organisation, encouraging individuals to consider assistive technology as a preventative measure to reduce reliance on care & support services. This is a self-funding paid for service.
The Assistive technology service is based at Warmley Business Park, Warmley, Bristol.
Core business hours are between 6am and 8pm across 5 days out of 7 days per week. Current normal working hours for this role are generally Monday to Friday 8.30am - 4.30pm. Not all services operate within the core hours but there may be future requirements for services to move to this arrangement.
Main duties of the job
- To be responsible for all the administration duties within the service. This includes all aspects of office work pertaining to the service.
- To manage all aspects of invoicing for the Assistive technology Service, setting up, managing and checking direct debits
- To liaise with all healthcare professionals and members of the public.
- To prioritise work on a daily basis ensuring effective and efficient running of the Assistive technology Service in accordance with auditors guidelines
- To act in the absence of the Assistive technology lead in keeping all aspects of the service running which maybe for significant periods of time, at short notice throughout the week.

- Experience of providing comprehensive administrative support within a health, social care or customer service environment.
