Main duties of the job
This role offers the opportunity to be at the heart of Primary Care Network development, supporting projects that make a real difference to patients, practices and the wider community. With a varied workload and no two days the same, it is ideal for someone who enjoys building relationships and contributing to innovative healthcare services.
As a Care Coordinator, you will work with PCN leaders, practice teams and partner organisations to improve patient care and access to services. You will support patients with personalised care needs, helping them navigate health and community services and access the right support at the right time.
You will help promote the work of the PCN through social media, patient engagement activities and health campaigns, raising awareness of local services, screening programmes, immunisations and wellbeing initiatives. You will also support the Patient Participation Group, gather patient feedback and contribute to service improvements.
Working closely with the PCN Manager, Clinical Director and partner organisations, you will support quality improvement projects, audits and healthcare programmes while developing partnerships that enhance integrated patient care.

An exciting opportunity to join the specialist palliative care team in the community mainly based in the West area. The applicant will be someone who enjoys working with people and has some experience of physiotherapy, occupational therapy or similar discipline as a support worker/assistant.
Whilst experience is desirable it is not essential and the successful applicant will be supported to develop in the role, we have clear structured competencies for the new post holder to work towards during their probationary period.
The role involves working with people at the end of life or approaching the end of life usually in their own homes; we strive to help people to live well for as long as possible and help them to achieve life goals.
The successful applicant will have a positive, caring approach and be able to work well in a multi-disciplinary team.
Main duties of the job
The post holder will be expected to:
To be responsible for own caseload.
To assist the qualified Therapists with providing ward and home-based assessments and treatments to patients with complex and highly complex needs with cancer and other life threatening illnesses of limited prognosis.
To provide administrative support to the Therapists to obtain resources and maintain information on resources.
To undertake appropriate training as indicated by the Therapist.
To perform any other reasonable duty as directed by the Therapist.

We are seeking experienced healthcare support workers as part of our urgent care team. Successful applicants will have significant experience of working in urgent, emergency or acute care settings. The post holder work in the community visiting patients who are under the care of our urgent community response service or virtual ward. Supported by registered clinicians and our senior clinician, you will work using your own initiative. The role includes visiting patients in their own homes to undertake observations, venupuncture, wound reviews and other assessments. We are delighted to offer an opportunity for an Advanced Clinical Practitioner (ACP) to join medical Same Day Emergency Care (SDEC) within the Directorate of Urgent and Emergency Care at Oldham Care Organisation. This is a permanent, full time position 37.5 hours per week). An MSc in Advanced Clinical Practice is essential, however we welcome applications from newly qualified, almost qualified and experienced ACP's who have a real passion in admission avoidance.
SDEC boasts a diverse team of ACP's who are supported by medical and nursing staff. Autonomously assessing, investigating and managing same day emergency medical patients will be your primary role; also, you will in-reach into the Emergency Department (ED) as well as supporting referrals from GP's and NWAS. The ACP team together with the consultant Clinical Lead, the nursing team and clerical staff are committed to the ongoing success of SDEC through innovative pathway development and collaborative working, both in secondary and primary care. Essentially, we strive to ensure the right person receives access to the right care at the right time, by the right person.

Main duties of the job
